Heart failure (HF) is a major cause of mortality and morbidity in South Korea, imposing substantial physical, emotional, and financial burdens on patients and society. Despite the high burden of symptom and complex care needs of HF patients, palliative care and hospice services remain underutilized in South Korea due to cultural, institutional, and knowledge-related barriers. This position statement from the Korean Society of Heart Failure emphasizes the need for integrating palliative and hospice care into HF management to improve quality of life and support holistic care for patients and their families. By clarifying the role of palliative care in HF and proposing practical referral criteria, this position statement aims to bridge the gap between HF and palliative care services in South Korea, ultimately improving patient-centered outcomes and aligning treatment with the goals and values of HF patients.

Purpose: The aim of this study was to identify novel vaginal probiotics with the potential to prevent vulvovaginal candidiasis (VVC) and bacterial vaginosis (BV). Materials and Methods: Eighteen strains of Lactiplantibacillus plantarum were isolated from healthy Korean women, and their antimicrobial effects against Candida albicans and Gardnerella vaginalis were assessed. Three strains (L. plantarum LM1203, LM1209, and LM1215) were selected for further investigation, focusing on their growth inhibition, biofilm regulation, and cellular mechanisms against these vaginal pathogens. Additionally, electron microscopy revealed damage to G. vaginalis induced by L.plantarum LM1215, and genomic analysis was conducted on this strain. Results: L. plantarum LM1203, LM1209, and LM1215 showed approximately 1 and 2 Log CFU/mL growth reduction in C. albicans and G. vaginalis, respectively. These L. plantarum strains effectively inhibited biofilm formation and eliminated the mature biofilms formed by C. albicans. Furthermore, L. plantarum LM1215 decreased tricarboxylic acid cycle activity by 51.75 (p<0.001) and respiratory metabolic activity by 52.88% (p<0.001) in G. vaginalis. L. plantarum induced cellular membrane damage, inhibition of protein synthesis, and cell wall collapse in G. vaginalis. Genomic analysis confirmed L. plantarum LM1215 as a safe strain for vaginal probiotics. Conclusion The L. plantarum LM1215 is considered a safe probiotic agent suitable for the prevention of VVC and BV.

Purpose: Early detection of hydroxychloroquine (HCQ)-related retinopathy is important. We aimed to determine whether changes in choroidal thickness (ChT) and outer nuclear layer (ONL) thickness might signal impending HCQ toxicity before development of retinopathy. Methods: We included patients with systemic lupus erythematosus or rheumatoid arthritis who were treated with HCQ and followed for ≥ 12 months. Patients were categorized into groups with no HCQ-related retinopathy (HCQ continued (group A, n = 49) or HCQ discontinued (group B, n = 9) and with HCQ-related retinopathy (group C, n = 9) groups. Using optical coherence tomography (OCT) images, ChT measurements were obtained at the subfovea and 1.5 mm nasal and temporal to the fovea. ONL thickness was obtained from the fovea using inner and outer rings of the Early Treatment of Diabetic Retinopathy Study (ETDRS) grid. Results: At baseline, mean ChT was thinner in all of the HCQ intake groups than in normal eyes (n = 35). Among patients with no retinopathy, ChT at all measured locations decreased in group A (average ChT, p < 0.001), whereas the average ChT significantly improved in group B after HCQ discontinuation (p = 0.038). In all HCQ groups, ONL thicknesses in the outer ETDRS ring area continued to show a significant decrease during follow-up, regardless of HCQ continuity (p ≤ 0.050). The association between an average of the outer ETDRS ONL thickness and ChT changes in group A was also statistically significant after adjusting for daily dose per kg and duration of HCQ use (p = 0.042). Conclusions Among patients without HCQ-related retinopathy, ChT continued to decrease during HCQ treatment but stopped thinning and was partially recovered following HCQ discontinuation. ONL thinning was observed progressively regardless of HCQ continuity.ChT and ONL measurements might be used for early detection of HCQ retinal toxicity.

Methods: In this retrospective chart review, data of patients with hemophilia B from eight university hospitals were collected. Demographic and clinical data, treatment data, such as regimen and number of injections, dose of factor IX concentrate, and bleeding data were reviewed. Descriptive analyses were performed with annual data for 2019, 2020, and 2021, as well as the three years consecutively. Results: The medical records of 150 patients with hemophilia B between January 1, 2019, and December 31, 2021, were collected. Among these, 72 (48.0%) were severe, 47 (31.3%) were moderate, and 28 (18.7%) were mild. The results showed approximately two times more patients receiving prophylaxis as those receiving on-demand therapy, with 66.1% of patients receiving prophylaxis in 2019, 64.9% in 2020, and 72.1% in 2021. Annualized bleeding rates were 2.2% (± 3.1) in 2019, 1.8% (± 3.0) in 2020, and 1.8% (± 2.9) in 2021 among patients receiving prophylaxis.For the doses of factor IX concentrate, patients receiving prophylaxis received an average of 41.6 (± 11.9) IU/Kg/Injection in 2019, 45.7 (± 12.9) IU/Kg/Injection in 2020, and 60.1 (± 24.0) IU/Kg/Injection in 2021. Conclusions Clinically, prophylaxis is more prevalent than reported. Based on insights gained from current clinical evidence, it is expected that the unmet medical needs of patients can be identified, and physicians can evaluate the status of patients and actively manage hemophilia B using more effective treatment strategies.

Polytetrafluoroethylene (PTFE) grafts are artificial vascular grafts commonly utilized for reconstructing the middle hepatic vein during living donor liver transplantation. In this report, we present three cases of expanded PTFE (ePTFE) graft migration into the gastrointestinal tract. These migrations were incidentally discovered and later migrated grafts were successfully removed endoscopically. The first case involved a patient presenting with epigastric discomfort, with a migrated ePTFE graft observed in the duodenal lumen during esophagogastroduodenoscopy (EGD). In the second case, a patient who visited the emergency room with hematochezia was found to have a migrated ePTFE graft in the colonic lumen on colonoscopy. The third case involved a patient undergoing regular EGD after endoscopic submucosal dissection for early gastric cancer; graft migration into the duodenal lumen was documented over time through sequential surveillance EGDs. The graft was endoscopically removed after complete migration. Contrary to previous reports, the three cases presented here did not exhibit serious clinical symptoms, and they were successfully treated through endoscopic foreign body removal without complications. We believe these occasions were possible due to the slow migration of the graft and the concurrent spontaneous closure of the fistula tract.

Objectives: Depression and anxiety are emotional difficulties that are common among adolescents. Adolescents suffering from depression and anxiety can exhibit problems such as antisocial behavior, aggression, and academic slump. This study aimed to explore the factors influencing adolescents’ depression and anxiety during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This study examined the data from an online student health survey conducted by the Ministry of Education in 2022. The survey participants were 131194 students ranging from the first grade of middle school to the third grade of high school. The influence of each variable on depression and anxiety was examined. Results: Factors that predicted depression and anxiety were sex (female), grade level (higher), financial difficulties (existing or unknown), physical activity (higher), sleep time (shorter), media usage time (higher), family/friends relationship (worse), and change in the health status of family, friends, and oneself due to COVID-19 (worse). Conclusion These results may help identify patterns of depression and anxiety among adolescents caused by COVID-19 and maximize the intervention effect.
