Electroconvulsive therapy (ECT) is performed under general anesthesia, and the choice of anesthetic induction agents critically influences multiple aspects of the treatment, including the seizure quality, cardiovascular stability, and post-procedural recovery. Developing anesthetic strategies that reliably achieve loss of consciousness while preserving adequate seizure expression has been a central challenge in ECT practice because most commonly used induction agents possess anticonvulsant properties. Dexmedetomidine (DEX), recently introduced primarily for sedation in intensive care and various procedural settings, is a selective α2-adrenergic receptor agonist that has minimal impacts on the seizure threshold and seizure duration while providing a complex profile of sedation, analgesia, anxiolysis, and sympatholysis. These properties help reduce the acute increases in blood pressure and heart rate induced by electrical stimulation, and DEX also offers advantages for airway management because it induces virtually no respiratory depression.Importantly, DEX has also been consistently reported to reduce pre-procedural anxiety and postictal agitation or aggression, suggesting that it may allow more stable management throughout the entire ECT course compared with conventional anesthetic agents. This review examines the pharmacological characteristics and clinical implications of DEX in anesthetic induction for ECT and discusses its potential roles in future clinical practice.

Current ABO titration methods lack standardization and harmonization. We analyzed the consistency of ABO antibody titer testing among Korean laboratories and discussed future directions for standardization by analyzing external quality control data collected by the Korean Association of External Quality Assessment Service over 5 yrs (2019–2023). The analysis included the number of participating institutions and methods, as well as the proportion of acceptable results. To compare column agglutination technology (CAT) and tube methods, we created a normalized variable: ([log 2 titer of laboratory test result]–[mean of log 2 titer for the peer group]). The number of participating institutions and methods increased over time. The use of CAT methods expanded, whereas that of tube methods declined. The proportion of acceptable results ranged from 84.0% to 100%, with no significant differences between CAT and tube methods. An F-test revealed no significant variance differences among institutions using these methods. Tube methods demonstrated lower variance in anti-human globulin testing, and room temperature tube methods exhibited lower variance than that of CAT methods. Domestic laboratories demonstrated highquality performance in ABO antibody titer testing, with no significant differences in acceptable result rates or variance across methods. Continuous efforts toward standardization remain essential.

Purpose: This study aimed to evaluate patient-reported experiences with automated breast ultrasound (ABUS) and identify patient characteristics associated with discomfort, to inform screening modality selection and improve resource use. Methods: A survey was prospectively conducted on 140 patients who underwent ABUS for breast cancer screening. The survey assessed patient satisfaction, discomfort, and pain before and after the procedure. Factors such as age, breast density, and body mass index (BMI) were analyzed to determine their impact on patient experiences. Results: The majority of patients expressed satisfaction with ABUS. However, younger patients, those with small or dense breasts, and those with a low BMI reported lower satisfaction and higher pain scores. Conclusion Based on patient-reported outcomes, ABUS could complement handheld ultrasound in breast screening.Selecting modalities by age, BMI, breast size, and density may enhance satisfaction and resource use. Personalized strategies may improve screening experience and efficiency.

Porcine islets, deemed a viable alternative, recently demonstrated successful longterm survival without zoonotic risks in a clinically relevant pig-to-non-human primate islet transplantation model. This success prompted the development of a clinical trial protocol for porcine islet xenotransplantation in humans. Methods: A single-center, open-label clinical trial initiated by the sponsor will assess the safety and efficacy of porcine islet transplantation for diabetes patients at Gachon Hospital. The protocol received approval from the Gachon Hospital Institutional Review Board (IRB) and the Korean Ministry of Food and Drug Safety (MFDS) under the Investigational New Drug (IND) process. Two diabetic patients, experiencing inadequate glycemic control despite intensive insulin treatment and frequent hypoglycemic unawareness, will be enrolled. Participants and their family members will engage in deliberation before xenotransplantation during the screening period. Each patient will receive islets isolated from designated pathogen-free pigs. Immunosuppressants and systemic infection prophylaxis will follow the program schedule. The primary endpoint is to confirm the safety of porcine islets in patients, and the secondary endpoint is to assess whether porcine islets can reduce insulin dose and the frequency of hypoglycemic unawareness. Conclusion A clinical trial protocol adhering to global consensus guidelines for porcine islet xenotransplantation is presented, facilitating streamlined implementation of comparable human trials worldwide.

The Korean Breast Cancer Society (KBCS) has collected nationwide registry data on clinicopathologic characteristics and treatment since 1996. This study aimed to analyze the clinical characteristics of breast cancer in Korea and assess changes in breast cancer statistics for 2021 using data from the KBCS registry and the Korean Central Cancer Registry. In 2021, 34,628 women were newly diagnosed with breast cancer. The median age of women diagnosed with breast cancer was 53.4 years, with the highest incidence occurring in the 40–49 age group. The most common molecular subtype was hormone receptor-positive and human epidermal growth factor receptor 2 (HER2)-negative, accounting for 69.1% of cases, while HER2-positive subtypes comprised 19.3%. During the coronavirus disease 2019 pandemic, the national breast cancer screening rate declined. However, the incidence of early-stage breast cancer (stages 0 and I) continued to increase, accounting for 65.6% of newly diagnosed cases in 2021. Our results showed that the overall survival rate for patients with breast cancer has improved, primarily due to a rise in early-stage diagnoses and advancements in treatment.

Mechanical power (MP) has been reported to be associated with clinical outcomes. Because the original MP equation is derived from paralyzed patients under volume-controlled ventilation, its application in practice could be limited in patients receiving pressure-controlled ventilation (PCV). Recently, a simplified equation for patients under PCV was developed. We investigated the association between MP and intensive care unit (ICU) mortality. Methods: We conducted a retrospective analysis of Korean data from the Fourth International Study of Mechanical Ventilation. We extracted data of patients under PCV on day 1 and calculated MP using the following simplified equation: MPPCV = 0.098 ∙ respiratory rate ∙ tidal volume ∙ (ΔPinsp + positive end-expiratory pressure), where ΔPinsp is the change in airway pressure during inspiration. Patients were divided into survivors and non-survivors and then compared. Multivariable logistic regression was performed to determine association between MPPCV and ICU mortality. The interaction of MPPCV and use of neuromuscular blocking agent (NMBA) was also analyzed. Results: A total of 125 patients was eligible for final analysis, of whom 38 died in the ICU. MPPCV was higher in non-survivors (17.6 vs. 26.3 J/min, P<0.001). In logistic regression analysis, only MPPCV was significantly associated with ICU mortality (odds ratio, 1.090; 95% confidence interval, 1.029–1.155; P=0.003). There was no significant effect of the interaction between MPPCV and use of NMBA on ICU mortality (P=0.579). Conclusions: MPPCV is associated with ICU mortality in patients mechanically ventilated with PCV mode, regardless of NMBA use.
