Rotator cuff tears are a common musculoskeletal disorder that significantly impairs shoulder function, especially in the elderly. Traditionally, mechanical wear and age-related degeneration have been considered the leading causes. Recent studies, however, emphasize that systemic metabolic disorders, such as diabetes mellitus, obesity, and dyslipidemia, play a crucial role in the development and healing of rotator cuff disease. This narrative review comprehensively examines the biological mechanisms and clinical outcomes associated with these metabolic comorbidities in rotator cuff degeneration and surgical repair failure. Based on relevant clinical and experimental studies, this study analyzed how each metabolic disease contributes to tendon injury and impaired regeneration. Diabetes mellitus induces chronic inflammation, inhibits collagen synthesis, and impairs microvascular perfusion, hindering tendon healing. Obesity increases the mechanical load on the shoulder and promotes systemic inflammation, exacerbating fatty infiltration of muscles and delaying tissue repair. Dyslipidemia accelerates tendon degeneration through the accumulation of fat, oxidative stress, and fatty infiltration of the rotator cuff muscles. These systemic factors are associated with higher re-tear rates, delayed functional recovery, and poorer surgical outcomes. As the prevalence of metabolic syndrome increases, precise assessment and perioperative management of these metabolic disorders are essential to improve patient outcomes and develop personalized treatment strategies for rotator cuff tears.

Rotator cuff (RC) injuries cause degenerative changes, including muscle atrophy and fatty infiltration, which remain key pathophysiological factors limiting functional recovery and increasing the risk of re-tears even after surgical repair. In particular, these changes tend to progress to fibrofatty replacement over time, highlighting the limitations of surgery-focused treatments alone. Recent research has addressed this by increasingly focusing on biological therapeutic strategies that directly target the degenerative pathology of the RC muscle. This review article examines the current trends in preclinical and clinical research on three main biological approaches: stem cell-based tissue regeneration, modulation of pathological signaling pathways, and metabolic reprogramming strategies that restore the pathological metabolic environment. Building upon these pathophysiological foundations, this review aims to develop recent biological treatment strategies and explore their potential clinical applications.

Background: The objective of this study was to investigate sequential changes in range of motion (ROM) and histopathology of the joint capsule after remobilization in a rat adhesive capsulitis (AC) model using cast immobilization. Methods: Thirty-five 6-week-old Sprague-Dawley rats were immobilized with casting of the left shoulder. After 4 weeks, the rats were randomly divided into 7 groups (n = 5 per group) according to the duration of remobilization following cast removal: 0 (D0), 1 (W1), 2 (W2), 3 (W3), 4 (W4), 5 (W5), and 6 (W6) weeks. At each time point, 5 rats were euthanized for measurement of the glenohumeral abduction angle and evaluation of capsular thickness in the axillary recess. Results: Abduction angle was sequentially increased at each time point after remobilization with cast removal. Significant differences between immobilized and non-immobilized shoulders were observed at D0 (130.4° vs 161.3°), W1 (144.7° vs 160.7°), and W2 (142.3° vs 161.3°). Significant differences in capsular thickness of the axillary recess were observed between immobilized and non-immobilized shoulders at all time periods including D0 (98.0 μm vs. 12.9 μm), W1 (117.7 μm vs. 12.2 μm), W2 (115.7 μm vs.13.1 μm), W3 (54.1 μm vs. 14.9 μm), W4 (54.5 μm vs. 13.4 μm), W5 (23.8 μm vs. 13.5 μm), and W6 (19.8 μm vs. 12.4 μm). Significant differences in capsular fibrosis scores were observed between immobilized and non-immobilized shoulders at W1 (2.2 vs. 0.2) and W2 (2.2 vs. 0.0). Conclusions Our study showed that ROM increased sequentially after remobilization in a rat AC model using cast immobilization, and it was normalized at 6 weeks after cast removal. Capsular thickening decreased sequentially, but remained at 6 weeks after cast removal. These findings may provide useful information for further research into verifying potential therapeutic targets using a rat AC model with cast immobilization.

Background/Aims: Hepatocellular carcinoma (HCC) exhibits significant sex disparities in incidence, yet its molecular mechanisms remain unclear. We explored the role of telomerase reverse transcriptase (TERT) genetic alterations and hepatitis B virus (HBV) integration, both known major contributors to HCC, in sex-specific risk for HBV-related HCC. Methods: We examined 310 HBV-related HCC tissues to investigate sex-specific TERT promoter (TERT-pro) mutations and HBV integration profiles, stratified by sex and age, and validated with single-cell RNA sequencing (scRNA-seq) data. Results: Tumors predominantly exhibited TERT-pro mutations (26.0% vs. 0%) and HBV-TERT integration (37.0% vs. 3.0%) compared to non-tumorous tissues. While TERT-pro mutations increased with age in both sexes, younger males (≤60 years) showed marked predominance compared to younger females. Males had significantly more HBV integrations at younger ages, while females initially had fewer integrations that gradually increased with age. Younger males' integrations showed significantly greater enrichment in the TERT locus compared to younger females, alongside a preference for promoters, PreS/S regions, and CpG islands. Overall, TERT genetic alterations were significantly sex-differential in younger individuals (75.3% in males vs. 23.1% in females) but not in older individuals (76.9% vs. 83.3%, respectively). These alterations were associated with increased TERT expression. The skewed TERT abnormalities in younger males were further corroborated by independent scRNA-seq data. Conclusions Our findings highlight the critical role of TERT alterations and HBV integration patterns in the male predominance of HCC incidence among younger HBV carriers, offering insights for future exploration to optimize sex-specific patient care and HCC surveillance strategies.

The proposed underlying mechanisms for these effects include alterations in the extracellular matrix, inflammation, and oxidative stress, which hamper the reparative processes in the affected tendon tissues. Recent therapeutic strategies target these pathways, reflecting a growing body of research dedicated to mitigating these effects and promoting healing. This literature review aims to provide a comprehensive understanding of the pathophysiology underlying rotator cuff tears, examine the interplay between hyperlipidemia and rotator cuff tear healing, synthesize current knowledge on contributing biological mechanisms, and outline potential therapeutic interventions to optimize clinical management and treatment outcomes for patients.

Background: This study aimed to investigate changes after repeated subacromial drug injections in a rat model of normal rotator cuff. Methods: Thirty-nine male Sprague-Dawley rats were divided into groups 1 (no injection, n = 3), 2 (parecoxib, n = 18; 6 subgroups, n = 3 each; 0.5 mg/kg), and 3 (triamcinolone, n = 18; 6 subgroups, n = 3 each; 0.3 mg/kg). Groups 2 and 3 received subacromial injections 1–6 times once weekly for 6 weeks. The supraspinatus and infraspinatus tendons and muscles were used for biomechanical and histological evaluation. The subacromial bursa was used to analyze the prostaglandin E2 (PEG2) level. Results: In the biomechanical test, load-to-failure and ultimate stress decreased in groups 2 and 3 with repeated injections and the values were significantly lower in group 3 than in group 1 only at the sixth injection (p = 0.007 and p = 0.008, respectively). On the Bonar score, the cellularity, ground substance, and total score were significantly different among the 3 groups at the fifth and sixth injections (cellularity: p = 0.028 and p = 0.033, ground substance: p = 0.018 and p = 0.006, and total score: p = 0.029 and p = 0.027, respectively). The myocyte cross-sectional area of the infraspinatus muscle showed a significant difference among the 3 groups at the third and fourth injections (p = 0.031 and p = 0.020, respectively). The PEG2 level in the subacromial bursa was significantly different among the 3 groups at the third, fifth, and sixth injections (p = 0.019, p = 0.004, and p = 0.004, respectively). Conclusions In the rat model of normal rotator cuff, repeated local injections of the cyclooxygenase-2 inhibitor showed fewer negative effects on the biomechanical and histological properties of the normal tendon than triamcinolone.

Background/Aims: This study aimed to evaluate the performance of the Model for End-Stage Liver Disease (MELD) 3.0 for predicting mortality and liver-related complications compared with the Child-Pugh classification, albumin-bilirubin (ALBI) grade, the MELD, and the MELD sodium (MELDNa) score. Methods: We evaluated a multicenter retrospective cohort of incorporated patients with cirrhosis between 2013 and 2019. We conducted comparisons of the area under the receiver operating characteristic curve (AUROC) of the MELD3.0 and other models for predicting 3-month mortality. Additionally, we assessed the risk of cirrhosis-related complications according to the MELD3.0 score. Results: A total of 3,314 patients were included. The mean age was 55.9±11.3 years, and 70.2% of the patients were male. Within the initial 3 months, 220 patients (6.6%) died, and the MELD3.0had the best predictive performance among the tested models, with an AUROC of 0.851, outperforming the Child-Pugh classification, ALBI grade, MELD, and MELDNa. A high MELD3.0score was associated with an increased risk of mortality. Compared with that of the group with a MELD3.0 score <10 points, the adjusted hazard ratio of the group with a score of 10–20 pointswas 2.176, and that for the group with a score of ≥20 points was 4.892. Each 1-point increase inthe MELD3.0 score increased the risk of cirrhosis-related complications by 1.033-fold. The risk of hepatorenal syndrome showed the highest increase, with an adjusted hazard ratio of 1.149, followed by hepatic encephalopathy and ascites. Conclusions The MELD3.0 demonstrated robust prognostic performance in predicting mortality in patients with cirrhosis. Moreover, the MELD3.0 score was linked to cirrhosis-related complications, particularly those involving kidney function, such as hepatorenal syndrome and ascites.

Background/Aims: This study aimed to identify the risk factors for chronic kidney disease (CKD) and end-stage renal disease (ESRD) following liver transplantation (LT), with a specific focus on tacrolimus levels and intrapatient variability (IPV). Methods: Among the 1,076 patients who underwent LT between 2000 and 2018, 952 were included in the analysis. The tacrolimus doses and levels were recorded every 3 months, and the IPV was calculated using the coefficient of variability. The cumulative incidence rates of CKD and ESRD were calculated based on baseline kidney function at the time of LT. The impact of tacrolimus levels and their IPV on the development of CKD and ESRD was evaluated, and the significant risk factors were identified. Results: Within a median follow-up of 97.3 months, the 5-year cumulative incidence rates of CKD (0.58 vs. 0.24) and ESRD (0.07 vs. 0.01) were significantly higher in the acute kidney injury group than in the normal glomerular filtration rate (GFR) group. In the normal GFR group, the tacrolimus levels were identified as a risk factor for CKD, with a level of ≤4.5 ng/mL suggested as optimal for minimizing the risk of CKD. Furthermore, the IPV of tacrolimus levels and doses emerged as a significant risk factor for CKD development in both groups (p<0.05), with tenofovir disoproxil fumarate also being a risk factor in HBV-infected patients. The IPV of tacrolimus levels was also a significant factor in ESRD development (p<0.05). Conclusions This study elucidated the optimal tacrolimus trough level and highlighted the impact of IPV on the CKD and ESRD development post-LT.
