1.Hazards and health problems in occupations dominated by aged workers in South Korea.

BACKGROUND: South Korea's population is aging more rapidly than any other country. Aging of the productive population will lead to shortage of labor and the decreasing quality of the labor force in South Korea. South Korea needs health care strategies to support the establishment of work environments that are appropriate for elderly workers who have reduced physical capacity. This paper aims to identify occupations that are dominated by aged workers and assess the exposure to hazards and work-related health problems of aged workers in these occupations. METHODS: We identified the 20 occupations in South Korea that employ the most aged workers (at least 55 years-old), among all 149 occupations that are defined as minor categories (identified by three digits) by the Korean Standard Classification of Occupations (KSCO). Exposure to hazards and work-related health problems of individuals in these occupations were evaluated by analyzing the results of the fourth Working Conditions Survey of 2014. RESULTS: Among the 20 occupations that employ the most aged workers, ‘Elementary Occupations', which the KSCO classifies as major category (9), had the largest proportion of aged workers. After this, there were five occupations of skilled manual workers and six occupations of skilled non-manual workers. Aged workers in elementary and skilled manual occupations reported frequent exposure to job-specific hazards, such as noise, vibrations, high and low temperatures, solvents, and chemicals. Relative to other workers, aged workers in the occupations reported more frequent exposure to ergonomic hazards, such as tiring or painful positions, carrying or moving heavy loads, and repetitive movements, and also reported more work-related musculoskeletal disorders and general fatigue. Injury due to accident was common in machinery-handling occupations. CONCLUSION: Job-specific hazards should be reduced to prevent occupation-related disorders in elementary and skilled manual occupations that are dominated by aged workers.

2.A Case of Endobronchial Aspergilloma Associated with Foreign Body in Immunocompetent Patient without Underlying Lung Disease.

Aspergillus causes a variety of clinical syndromes in the lung including tracheobronchial aspergillosis, invasive aspergillosis, chronic necrotizing pulmonary aspergillosis, allergic bronchopulmonary aspergillosis, and aspergilloma. Aspergilloma usually results from ingrowths of colonized Aspergillus in damaged bronchial tree, pulmonary cyst or cavities of patients with underlying lung diseases. There are a few reports on endobronchial aspergilloma without underlying pulmonary lesion. We have experienced a case of endobronchial aspergilloma associated with foreign body developed in an immunocompetent patient without underlying lung diseases. A 59-year-old man is being hospitalized with recurring hemoptysis for 5 months. X-ray and computed tomography scans of chest showed a nodular opacity in superior segment of left lower lobe. Fiberoptic bronchoscopy revealed an irregular, mass-like, brownish material which totally obstructed the sub-segmental bronchus and a foreign body in superior segmental bronchus of the lower left lobe. Histopathologic examinations of biopsy specimen revealed fungal hyphae, characteristic of Aspergillus species.

3.Application of a Loss of Resistance Syringe for Obtaining the Adequate Cuff Pressures of Endotracheal Intubated Patients in an Emergency Department.

PURPOSE: The endotracheal tube cuff pressure must be kept within the optimal range. This study compared the usefulness of the conventional pilot balloon palpation technique using a 10 cc disposable syringe and passive release technique using a Loss of Resistance (LOR) syringe for obtaining adequate intracuff pressures of endotracheal intubated patients in an emergency department. METHODS: This was a prospective, observational study, conducted at the emergency department of a university teaching hospital. Patients who required endotracheal intubation in an emergency department were enrolled in this study. The patients were divided into two groups: one group who underwent the pilot balloon palpation technique using a conventional syringe (group A, n=40) and the other group who underwent the passive release technique using a LOR syringe technique (group B, n=40). The amount of air that infused into the cuff and the cuff pressure were measured. RESULTS: The mean cuff pressures of groups A and B were 41.0+/-23.7 cmH2O and 23.7+/-16.5 cmH2O, respectively. The mean cuff pressure of group A were significantly higher than group B (p=0.002). The mean air volume that infused into the cuff of groups A and B were 8.6+/-2.6 ml and 7.6+/-2.4 ml, respectively, showing no significant difference (p=0.688). The proportion of the optimal cuff pressure (Normal range: 22-32 cmH2O) of group A (9/40, 22.5%) was higher than that of group B (3/40, 7.5%). CONCLUSION: The range of air volumes and pressures for cuff inflation varied and it may not be possible to obtain the appropriate pressure using the LOR syringe technique. The endotracheal tube cuff pressure must be kept within the optimal range using a pressure monitor control inflator.

4.Comparison of Airway Ultrasonography and Continuous Waveform Capnography to Confirm Endotracheal Tube Placement in Cardiac Arrest Patients: Prospective Observational Study.

PURPOSE: The aim of this study was to assess the accuracy and timeliness of using tracheal ultrasound for examination of endotracheal tube placement in cardiac arrest patients. METHODS: This was a prospective, observational study, conducted at the emergency department of a university teaching hospital. Patients underwent emergency intubation due to cardiac arrest. Airway ultrasonography was performed during emergency intubation with the transducer placed transversely at the trachea over the suprasternal notch. Quantitative waveform capnography was used as the criterion standard for confirmation of tracheal intubation. The main outcome was the timeliness between airway ultrasonography and capnography. RESULTS: A total of 16 patients and 19 intubations were included in the analysis. The endotracheal tube was placed in the trachea in 16 intubations and in the esophagus in three intubations. The overall sensitivity and specificity of ultrasound for confirmation of tracheal intubation was 100%, respectively. The capnography application time after intubation was 17.5(10.0~32.5) seconds. The capnograpny confirmation time after application was 30(10~120) seconds. The ultrasound confirmation time for endotracheal tube placement after application was 5(4~5) seconds. CONCLUSION: When patients were in a low pulmonary blood flow state, such as cardiac arrest, capnography confirmation of endotracheal tube placement was not rapid and needed a lot of times. Ultrasound confirmation was very rapid and accurate, and was not affected by pulmonary blood flow. Ultrasound confirmation of endotracheal tube placement is more useful in the emergency department.

5.The Use of Extension Tube in Sono-guide FNAC.

PURPOSE: Sono-guided FNAC (fine needle aspiration cytology) is a safe and accurate diagnostic procedure for thyroid nodules. We examined the use of an extension tube in FNAC as useful. METHODS: We reviewed 175 cases of patients with thyroid nodules where an extension tube was used in sono-guide FNAC and 153 cases where a syringe holder was used in Jeonju Jesus hospital, from July 2005 to September 2006. RESULTS: For cases with an extension tube, tumor size was smaller than 1 cm (38%), from 1 cm to 2 cm (27%), or larger than 2 cm (35%), with cytology results of benign (73%), intermediate (8%), malignant (7%), or nondiagnostic (12%). In cases with a syringe holder, tumor size was smaller than 1 cm (40%), from 1 cm to 2 cm (40%), or larger than 2 cm (20%), with cytology results of benign (51%), intermediate (4%), malignant (3%), or nondiagnostic (42%). The nondiagnostic percentage in cases where an extension tube was used was lower than in cases using a syringe holder. However, the number of tumors larger than 2 cm was significantly larger in the syringe group than the extension tube group. CONCLUSION: The use of an extension tube or a syringe holder in FNAC produce similar results.

6.A Case of Anisakiasis Concurrently Invading Esophagus and Stomach, and Another Case of Esophageal Anisakiasis.

Human anisakiasis may occur after ingestion of raw marine fish infected with the nematode larvae of Anisakidae. Clinical symptoms such as cramping abdominal pain, nausea, vomiting, diarrhea and epigastric fullness, usually develop within 12 hours after having eaten infected raw sea fish. Anisakiasis is most commonly found in the stomach, and is rarely identified in the small intestine, large intestine, and esophagus. We report two cases of of anisakiasis, one with concurrent invasion of the stomach and esophagus, and the other case with esophageal anisakiasis. Both were treated by endoscopic extraction of the larvae.

PURPOSE: We prospectively conducted a multi-center, open-label, randomized phase II trial to compare the efficacy and safety of docetaxel plus cisplatin (DC) and etoposide plus cisplatin (EC) for treating advanced stage non-small cell lung cancer (NSCLC). MATERIALS AND METHODS: Seventy-eight previously untreated patients with locally advanced, recurrent or metastatic NSCLC were enrolled in this study. The patients received cisplatin 75 mg/m2 on day 1 and either docetaxel 75 mg/m2 on day 1 or etoposide 100 mg/m2 on days 1 to 3 in the DC or EC arm, respectively, every 3 weeks. RESULTS: The objective response rate was 39.4% (15/38) and 18.4% (7/38) (p=0.023) in the DC and EC arms, respectively. The median time to progression (TTP) was 5.9 and 2.7 months (p=0.119), and the overall survival was 12.1 and 8.7 months (p=0.168) in the DC and EC arms, respectively. The prognostic factors for longer survival were an earlier disease stage (stage III, p=0.0095), the responders to DC (p=0.0174) and the adenocarcinoma histology (p=0.0454). The grades 3 and 4 toxicities were similar in both arms, with more febrile neutropenia (7.9% vs. 0%) and fatigue (7.9% vs. 0%) being noted in the DC arm. CONCLUSION: DC offered a superior overall response rate than does EC, along with tolerable toxicity profiles, although the DC drug combination did not show significantly improved survival and TTP.

Nodular fasciitis is rare and benign fibroblastic proliferative disease which tends to be confused microscopically with spindle cell sarcoma. It is a distinctive lesion and a very important one because of its ability to simulate a malignant process. Histopathologically, nodular fasciitis can be grouped into three subtypes based on their relation with the fascia: subcutaneous, fascial, intramuscular. We report a case of nodular fasciitis misdiagnosed as Baker's cyst in a patient with rheumatoid arthritis.

9.Drinking Problems of Hepatitis B Virus Healthy Carriers in Korean Population.

BACKGROUND: Heavy drinking may be a cause of serious health problems in hepatitis B virus carriers. The aim of this study was to reveal the degree of drinking problems in the Korean hepatitis B virus healthy carriers. METHODS: The subjects were 218 hepatitis B virus healthy carriers (124 males and 94 females) recruited from those who visited the Health Promotion Center of Chungnam National University Hospital for a general health check-up. Drinking problems of the subjects were evaluated by the AUDIT (Alcohol Use Disorders Identification Test) questionnaire. RESULTS: The AUDIT scores (mean+/-SD) of the male subjects (9.6+/-7.9) were significantly (P <0.001) higher than those of the female subjects (2.7+/-5.2). The individuals with heavy drinking (more than 14 drinks in males and 7 drinks in females per week) are 24 (19.4%) in males and 5 (5.3%) in females. Alcohol use disorders were suspected in 36 (29.0%) males and 4 (4.3%) females. CONCLUSION: It is suggested that, in Korea, clinical attention in primary health care should be focused on reducing the alcohol consumption among the hepatitis B virus carriers, considering a large proportion have drinking problems and the possible harmful effect on their health by heavy drinking.

10.A Case of Crohn's Disease Which is Diagnosed through Acute Sigmoid Colon Obstruction.

Crohn's disease is an inflammatory bowel disease like ulcerative colitis. Distinct from ulcerative colitis, Crohn's disease may involve any portion of the alimentary tract from the mouth to the anus. Recently, the incidence of Crohn's disease has been increasing in Korea. The proportion of the colon type is smaller than that of the ileocecal type or the small-bowel type. In the colon, Crohn's disease affects mainly the right side. Relatively, the sigmoid colon is rarely involved. Small-bowel obstruction is the most common complication requiring surgery in Crohn's disease. On the contrary, an obstruction limited to the colon requiring surgery is less common in Crohn's disease. We experienced a case of a severe acute sigmoid colon obstruction with peritonitis. At first, we suspected colon cancer, but after an emergency laparotomy, we diagnosed it as Crohn's disease. Such a situation is rare in Korea, so we hope this case report may provide a good opportunity to reconsider Crohn's disease.
