Lithium is commonly used to treat psychiatric disorders, and NDI is one of the common renal side effects of lithium therapy. On the other hand, NDI induced by lithium medication in a patient after a coronavirus disease-19 (COVID-19) infection has not been reported. This paper presents a patient who developed new-onset NDI secondary to a COVID-19 infection despite being on a stable lithium dose for several years. This case highlights the importance of considering a COVID-19 infection as a possible cause of NDI in patients taking lithium medication.

Purpose: We present a dehazing algorithm using dark channel prior (DCP) and bright channel prior (BCP) to enhance the quality of retinal images obtained through conventional fundus photography. Methods: A retrospective analysis was conducted on retinal images from patients who visited Gangnam Sacred Heart Hospital between January 2000 and September 2022. These images were captured using a digital fundus camera (KOWA Nonmyd 8S Fundus Camera, KOWA Company, Nagoya, Japan) without pupil dilation. We used two mathematical algorithms: DCP only and DCP and BCP combined. The original, DCP-processed, and DCP & BCP-processed images were compared. Fisher's exact test was used to identify significant quality improvements. Results: The DCP and the newly proposed DCP plus BCP algorithm effectively eliminated haze and enhanced the contrast of cataract images. Notably, DCP demonstrated limited improvements in fundus photographs from patients with small pupils, whereas the proposed DCP plus BCP method effectively revealed previously obscured retinal details and vessels. However, these methods exhibited limited performance in severe cataracts compared to the clear images obtained after surgery. The quality enhancement with the proposed method was significant in photographs of patients with cataracts (p = 0.032) and small pupils (p < 0.01). Conclusions Our algorithm produced clearer images of blood vessels and optic disc structures, while significantly reducing artifacts in fundus images from patients with small pupils or cataracts. The proposed algorithm can provide visually enhanced images, potentially aiding physicians in the diagnosis of retinal diseases in patients with cataracts.

Preauricular sinus, a congenital malformation resulting from incomplete fusion of the first and second branchial arches, is a relatively common condition with a reported prevalence of 0.1% to 0.9%. While mostly asymptomatic, it can lead to abscess formation when infected. We present a unique case of a neglected preauricular sinus abscess that extended to the scalp, causing significant complications. The report discusses the clinical presentation, diagnostic approach, treatment modalities, and potential complications associated with this unusual and severe presentation. Prompt and appropriate management, combined with conservative treatment and surgical intervention, led to successful outcomes. This case highlights the importance of recognizing potential risks and implementing a systematic approach in managing recurrent preauricular sinus abscesses.

Xanthogranulomatous osteomyelitis (XO) is a rare chronic inflammatory bone disease characterized by the presence of cholesterol-laden foam macrophages, histiocytes, and plasma cells. We report the case of a 41-year-old man with end-stage renal disease who had undergone deceased donor kidney transplantation 4 years earlier. He presented with a chest wall mass that he had first identified 2 weeks prior to admission. Computed tomography revealed a periosseous heterogeneously enhancing soft tissue mass adjacent to the sternal end of the left clavicle, accompanied by irregular and destructive osteolytic lesions on the left side of the sternal manubrium. A total mass resection, which included partial clavicle and sternum removal, was performed. Pathological examination revealed foamy histiocytes along with numerous lymphoplasmacytic cells, confirming the diagnosis of XO. This case underscores the potential for XO to develop following kidney transplantation.
