1.Outcomes of Total Ankle Arthroplasty in Rheumatoid Arthritis and Osteoarthritis:A Propensity Score-Matched Cohort Study

Background:
Total ankle arthroplasty (TAA) is increasingly used as an alternative to ankle arthrodesis for patients with rheumatoid arthritis (RA), although its outcomes remain controversial. Using propensity score matching, this study compared clinical and radiographic outcomes of TAA for patients with RA and osteoarthritis (OA).
Methods:
Eighteen patients with RA were selected from 21 eligible cases and matched—using propensity score matching based on 8 baseline variables—to 36 patients with OA from a pool of 386. All patients underwent mobile-bearing TAA and were followed up for a minimum of 2 years. Clinical outcomes were assessed using the Ankle Osteoarthritis Scale, American Orthopaedic Foot and Ankle Society ankle–hindfoot score, Short Form-36 Physical Component Summary, and a visual analog scale for pain. Radiographic outcomes and postoperative complications were also evaluated.
Results:
At a mean follow-up of 7 years, both groups demonstrated improvement in all clinical outcome measures, with no significant differences between the RA and OA groups (p > 0.05). Radiographic outcomes, including postoperative tibiotalar angle, talar tilt angle, as well as the incidence values for periprosthetic osteolysis, implant subsidence, and aseptic loosening, were similar between groups. However, periprosthetic medial malleolar fractures occurred only in the RA group (3 cases, 16.7%; p = 0.033), and 2 cases of incision wound dehiscence were also observed in the RA group. No deep infections were reported in either group. Overall, reoperation rates did not differ significantly between groups.
Conclusions
Patients with RA who underwent TAA achieved clinical and radiographic outcomes comparable to those with OA.However, periprosthetic fractures and wound complications were more common in the RA group. Further studies with larger, matched cohorts and longer follow-up are needed to confirm these findings.
2.Sequential Changes in Range of Motion and Capsular Fibrosis Following Remobilization in a Rat Adhesive Capsulitis Model Using Cast Immobilization

Background:
The objective of this study was to investigate sequential changes in range of motion (ROM) and histopathology of the joint capsule after remobilization in a rat adhesive capsulitis (AC) model using cast immobilization.
Methods:
Thirty-five 6-week-old Sprague-Dawley rats were immobilized with casting of the left shoulder. After 4 weeks, the rats were randomly divided into 7 groups (n = 5 per group) according to the duration of remobilization following cast removal: 0 (D0), 1 (W1), 2 (W2), 3 (W3), 4 (W4), 5 (W5), and 6 (W6) weeks. At each time point, 5 rats were euthanized for measurement of the glenohumeral abduction angle and evaluation of capsular thickness in the axillary recess.
Results:
Abduction angle was sequentially increased at each time point after remobilization with cast removal. Significant differences between immobilized and non-immobilized shoulders were observed at D0 (130.4° vs 161.3°), W1 (144.7° vs 160.7°), and W2 (142.3° vs 161.3°). Significant differences in capsular thickness of the axillary recess were observed between immobilized and non-immobilized shoulders at all time periods including D0 (98.0 μm vs. 12.9 μm), W1 (117.7 μm vs. 12.2 μm), W2 (115.7 μm vs.13.1 μm), W3 (54.1 μm vs. 14.9 μm), W4 (54.5 μm vs. 13.4 μm), W5 (23.8 μm vs. 13.5 μm), and W6 (19.8 μm vs. 12.4 μm). Significant differences in capsular fibrosis scores were observed between immobilized and non-immobilized shoulders at W1 (2.2 vs. 0.2) and W2 (2.2 vs. 0.0).
Conclusions
Our study showed that ROM increased sequentially after remobilization in a rat AC model using cast immobilization, and it was normalized at 6 weeks after cast removal. Capsular thickening decreased sequentially, but remained at 6 weeks after cast removal. These findings may provide useful information for further research into verifying potential therapeutic targets using a rat AC model with cast immobilization.
3.Health Effects of Sugar-Sweetened and Artificially Sweetened Beverages: Umbrella Review and Evidence-Based Consensus Statement of the Korean Diabetes Association and the Korean Nutrition Society

Background:
Excess intake of added sugars contributes to obesity, type 2 diabetes mellitus (T2DM), cardiovascular disease (CVD), and premature mortality. Sugar-sweetened beverages (SSBs), the main source of added sugars, are consistently linked to adverse outcomes. Artificially sweetened beverages (ASBs) have been suggested as short-term substitutes, but evidence regarding benefits and harms remains inconclusive, and guidance is lacking.
Methods:
This consensus statement draws on a structured evidence review combining two approaches: an updated meta-analysis of randomized controlled trials (RCTs) assessing short- to intermediate-term effects of replacing SSBs with ASBs on weight and metabolic outcomes; and an umbrella review of systematic reviews of cohort studies evaluating long-term associations of SSBs and ASBs with major outcomes, including mortality, CVD, and T2DM.
Results:
In 14 RCTs (3–76 weeks), replacing SSBs with ASBs produced modest reductions in body weight (–0.73 kg) and body fat (–0.72%), with inconsistent effects on glycemic and cardiometabolic markers. Evidence from 20 systematic reviews of cohorts (up to 34 years follow-up) showed that higher intake of both SSBs and ASBs was associated with increased risks of T2DM, CVD, and mortality, with relative risks for ASBs similar to those for SSBs.
Conclusion
ASBs may serve as a short-term substitution for individuals with high SSB intake, particularly those at elevated metabolic risk. However, regular or long-term use is not recommended due to uncertain safety and potential reinforcement of sweet preference. Public health strategies should emphasize reducing both SSBs and ASBs, prioritizing water and unsweetened beverages as the ultimate goal.
4.The Influence of Metabolic Dysfunction-Associated Steatotic Liver Disease and Body Mass Index on the Incidence of Alzheimer Disease: A Nationwide Cohort Study

Background/Aims:
This study aimed to investigate the influence of metabolic dysfunction-associated steatotic liver disease (MASLD) and body mass index (BMI) on the incidence of Alzheimer disease (AD) in the general South Korean population.
Methods:
The National Screening Program for Transitional Ages collected data from 66-year-old dementia-free Koreans in 2010 and 2011. MASLD was diagnosed based on the fatty liver index(≥30) and the presence of metabolic components, and overweight/obese status was defined as a BMI ≥23 kg/m 2 . The primary outcome was the development of AD up to December 2021.Multivariable Cox analyses were performed to evaluate whether the presence of MASLD or overweight/obese status influenced the risk of developing AD.
Results:
A total of 376,902 dementia-free individuals aged 66 years were included in this cohort. The participants were categorized into four groups: overweight/obese non-MASLD (30.4%, n=114,528), overweight/obese MASLD (37.0%, n=139,551), lean non-MASLD (29.9%, n=126,692), and lean MASLD (2.7%, n=10,131). During a mean follow-up period of 10.38±1.90 years, 23,874 individuals (6.3%) were newly diagnosed with AD. Compared to the overweight/ obese non-MASLD group, the adjusted hazard ratios (95% confidence interval) for AD in the lean MASLD, lean non-MASLD, and overweight/obese MASLD groups were 1.34 (1.24 to 1.45), 1.08 (1.04 to 1.13), and 1.13 (1.09 to 1.17), respectively.
Conclusions
A normal/underweight BMI and the presence of MASLD synergistically increased the risk of AD. The lean MASLD group had a higher risk of developing AD than the overweight/ obese MASLD group, suggesting that the clinical relevance of MASLD for incident AD differs based on the BMI.
5.AI-driven Medical Care: Evaluation of Large Language Models in Generating Personalized Stroke Education Materials

Objectives:
Large language models (LLMs) demonstrate remarkable potential in healthcare communication. However, whether they can process complex, high-volume medical information, such as stroke-related content, remains insufficiently validated. This study aimed to evaluate the natural language processing capabilities of LLMs in handling such content and to develop an evaluation instrument.
Methods:
A survey compared educational materials generated by two LLMs (ChatGPT 4.0 and Claude 3) with neurologist-authored content on stroke. The materials were based on two clinical scenarios representing distinct stroke etiologies: cardioembolism and large-artery atherosclerosis. They were evaluated in terms of accuracy, legality, ethics, comprehensiveness, and information delivery. Scores for comprehensiveness and information delivery were compared according to participants’ agreement with the use of LLMs in healthcare.
Results:
ChatGPT received the highest scores across all domains, except for legality in Scenario 2. In Scenario 1, the ranking for accuracy and summarization of clinical information was, from highest to lowest, ChatGPT, Claude, and the neurologist (η2 = 0.140, p < 0.001; η2 = 0.175, p < 0.001). The same hierarchy was observed in Scenario 2 for accuracy (η2 = 0.077, p < 0.001) and summarization (η2 = 0.194, p < 0.001). Participants who agreed with the use of LLMs in healthcare assigned higher scores for the comprehensiveness (Scenario 1, p = 0.005; Scenario 2, p = 0.007) and information delivery (Scenario 1, p = 0.003; Scenario 2, p = 0.026) of ChatGPT-generated materials than participants who did not agree.
Conclusions
LLMs demonstrated adequate capability to convey complex content, such as stroke-related information, in an accessible and understandable manner for non-experts.
6.Prevalence and genotype distribution of human papillomavirus among Korean males: Implications for vaccination strategies

Purpose:
We aimed to investigate the prevalence and genotype distribution of human papillomavirus (HPV) among Korean males and explore implications for targeted vaccination strategies.
Materials and Methods:
A total of 44,065 males underwent HPV testing between March 2014 and February 2022 using the Anyplex™ II HPV 28 system, detecting 19 high-risk (HR) and 9 low-risk (LR) HPV types. Additionally, data from 507 male patients at local clinic (2017–2022) were analyzed to compare genotype prevalence between those with (WAT group) and without (WAT X group) genital warts.
Results:
Overall HPV positivity was 59.1%. HPV 6 (33.3%) and HPV 11 (11.0%) were the most prevalent LR genotypes, while HPV 16 (5.2%) dominated HR infection. Multiple HPV genotype co-infection occurred in 49.3% of positive cases, with 11.3% involving multiple HR types. Younger males (teens, 20s) exhibited higher HR-HPV positivity, although total HPV positivity peaked in the 40s (60.1%). Non–9-valent HR genotypes (HPV 53, 51, 39, 66) accounted for 27.6%–35.0% of infections annually. At local clinic, HPV 43 was significantly associated with genital warts (p=0.017).
Conclusions
These data support including males in national HPV vaccination strategies using the current 9-valent vaccine and underscore the need for ongoing genotype surveillance to monitor non‑vaccine high‑risk types and inform public health policy, and support inclusion of males in national HPV vaccination strategies using the current 9‑valent vaccine.
7.Mild but Not Minor: Reconsidering Clinical Practice and Tinnitus-Related Aspects of Mild Hearing Loss

Mild hearing loss, though often overlooked, can substantially affect daily communication and quality of life. Conventional hearing aids remain the mainstay of hearing loss management, yet their high cost has limited their widespread adoption. Recently, alternative amplification devices, such as personal sound amplification products (PSAPs) and over-the-counter (OTC) hearing aids, have gained attention. Emerging evidence suggests that these devices may provide meaningful benefits to individuals with mild hearing loss and could help expand accessibility. Patients with tinnitus may particularly benefit from amplification, because hearing aids improve tinnitus-related outcomes. However, professionals are not usually involved in the purchase and use of PSAPs and OTC hearing aids. This may lead to suboptimal or even harmful results, especially in tinnitus care, for which counselling and cognitive behavioral therapy are essential components of comprehensive management. Amplification alone should not be considered a sufficient treatment strategy. Ultimately, while PSAPs and OTC hearing aids offer promising and cost-effective options for managing mild hearing loss, their integration into clinical practice requires careful consideration. Clear guidelines and professional consensus are needed to ensure that these devices are used appropriately and effectively, maximizing benefits while minimizing potential risks.
8.Early Onset, High Comorbidity Burden, and Regional Disparities of CADASIL:A Nationwide Cohort Study in South Korea

Background:
and Purpose To compare the epidemiological and clinical features of the rare patients with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) with age- and sex-matched controls in a nationwide cohort from South Korea.
Methods:
This observational cohort study analyzed newly diagnosed CADASIL patients aged at least 20 years and matched controls using data from the National Health Information Database for 2004–2022. The cumulative incidence of CADASIL was assessed by age and sex, and compared between regions. Neurologic and systemic diseases were compared between the CADASIL and control groups.
Results:
The study analyzed 816 CADASIL patients and 816 age- and sex-matched controls aged 56.8±15.2 years (mean±standard deviation), among whom 48.3% were male. The cumulative incidence of CADASIL was 1.86 per 100,000 people (95% confidence interval [CI]=1.85– 1.87 per 100,000), and peaked at 60–69 years of age. In terms of regional distribution, the incidence was highest for Jeju, at 39.67 per 100,000 (95% CI 37.84–41.49 per 100,000). Neurologic diseases were more frequent in CADASIL patients, including Alzheimer’s disease (33.1% vs.20.0%), vascular dementia (84.9% vs. 5.0%), epilepsy (34.6% vs. 15.9%), stroke (70.7% vs. 27.6%), parkinsonism (18.9% vs. 11.0%), and depression (60.8% vs. 44.9%). Systemic diseases such as diabetes mellitus (78.9% vs. 68.9%) were also more common in CADASIL patients, while cancer (27.9% vs. 38.7%) and myocardial infarction (10.0% vs. 13.6%) were less common than in controls. The onset ages of all diseases were lower in CADASIL patients.
Conclusions
This study has provided a precise nationwide estimate of the CADASIL incidence and its regional distribution in South Korea. CADASIL patients showed higher incidence rates and earlier onsets of diverse clinical manifestations.
10.Intraoperative Visual Evoked Potential Monitoring in Endoscopic Endonasal Surgery for Nonpituitary Adenoma Suprasellar Tumors

Background:
and Purpose Intraoperative visual evoked potential (VEP) monitoring has been studied mainly in pituitary adenoma, while its role in nonpituitary suprasellar tumors has remained unclear. This study evaluated the predictive usefulness of intraoperative VEP monitoring during endoscopic endonasal surgery (EES) and aimed to identify optimal alarm criteria for visual outcomes.
Methods:
We retrospectively analyzed a cohort of 87 patients who underwent EES with intraoperative VEP monitoring between April 2021 and September 2023. Visual outcomes were evaluated preoperatively and at short-term (≤3 months) and long-term (12 months) followups, with visual deterioration at these time points defined as worsening of either visual acuity or the visual field. Reductions in the VEP amplitude were quantified using both the maximum intraoperative decrease and the final amplitude after recovery. Receiver operating characteristic (ROC) curve analyses were performed to identify the optimal alarm thresholds, and the sensitivity, specificity, positive predictive value, and negative predictive value were calculated for short-term and long-term visual deteriorations.
Results:
Short-term and long-term visual deteriorations were detected in 12 (9.2%) and 5 (3.8%) of the 130 analyzed eyes, respectively. ROC curve analyses identified ≥40% and ≥30% reductions in the N75–P100 amplitude as optimal alarm criteria for short-term and long-term visual deteriorations, respectively. A 30% reduction without intraoperative recovery demonstrated markedly higher sensitivity than the conventional 50% alarm threshold for short-term (58.3% vs. 33.3%) and long-term (80.0% vs. 20.0%) outcomes, while maintaining acceptable specificity (82.2% and 80.8%, respectively).
Conclusions
A 30% reduction in amplitude represents a more-sensitive and clinically relevant alarm threshold than a 50% reduction for intraoperative VEP monitoring during EES for nonpituitary suprasellar tumors. Incorporating both the magnitude and recovery pattern of VEP amplitude changes may improve the accuracy of predictions of long-term visual deterioration. However, the potential for false positives warrants cautious interpretation, and further studies are needed to validate the impact of intraoperative VEP monitoring on visual outcomes.
