1.Correlation Between Traditional Chinese Medicine Syndrome Type of Unstable Angina Pectoris and Iron Metabolism Indices

ObjectiveTo investigate the correlation between iron metabolism parameters and various syndrome types of unstable angina pectoris (UAP). MethodsA cross-sectional study was conducted from October 2021 to October 2023, encompassing 213 patients diagnosed with UAP at Xiyuan Hospital of Chinese Academy of Chinese Medical Sciences. Additionally, 30 healthy individuals were selected as control cases. Single-factor analysis was used to investigate the differences in clinical data among different Traditional Chinese Medicine (TCM) syndrome types of UAP and their correlation with iron metabolism indices. The study conducted a comparative analysis of the aforementioned clinical data among patients with and without heat-toxic and blood-stasis syndrome. Logistic regression was used to analyze the correlation between TCM syndrome types and related factors. The receiver operator characteristic (ROC) curve was employed to assess the predictive value of iron metabolism indices, along with their sensitivity and specificity. ResultsCompared to those in the control group, serum iron (SI) and serum ferritin (SF) levels were significantly increased in the UAP group (P<0.01), while transferrin (TRF) and total iron binding capacity (TIBC) levels were decreased (P<0.01). However, there was no significant difference in unsaturated iron binding capacity (UIBC). Multivariate binary Logistic regression analysis identified apolipoprotein A1 (ApoA1), homocysteine (HCY), high-sensitivity C-reactive protein (hs-CRP), and SF as independent influencing factors for the UAP patients (P<0.05, P<0.01). Additionally, statistically significant differences were observed in SI, SF, TRF, and TIBC among 213 patients with different TCM types (P<0.01). Patients with heat-toxic and blood-stasis syndrome had higher SI and SF values than those without the syndrome (P<0.01), while their TIBC and TRF values were lower (P<0.01). Multivariate binary logistic regression analysis showed that SI and LDL-C levels were closely associated with the differentiation of heat-toxic and blood-stasis syndrome. ConclusionUAP patients often experience iron metabolism disorders, and the heat-toxic and blood-stasis syndrome are significantly correlated with iron metabolism parameters. The SI and LDL-C levels have high specificity and sensitivity in diagnosing heat-toxic and blood-stasis syndrome.
2."Tongdu Yisui" acupuncture and moxibustion for 15 cases of Meige syndrome.

OBJECTIVE:
To evaluate the clinical effect of "Tongdu Yisui" (unblocking the governor vessel and benefiting marrow) acupuncture and moxibustion on Meige syndrome.
METHODS:
Fifteen patients with Meige syndrome were treated with "Tongdu Yisui" acupuncture and moxibustion. Acupuncture was applied to Baihui (GV20), Dazhui (GV14), Shenzhu (GV12), Zhiyang (GV9), Jinsuo (GV8), bilateral Taixi (KI3), Zhaohai (KI6) and etc. Moxibustion was delivered at Jinsuo (GV8). After acupuncture and moxibustion at these body points, Jiao's scalp acupuncture was operated at bilateral chorea-tremor control area, and the patients were asked to walk for 20 min during needle retaining. Acupuncture and moxibustion were administered 20 min each time, once every two days, 3 times weekly and for 8 consecutive weeks. Assessments were conducted before treatment, after treatment, and follow-up at three months after treatment completion using the Burke-Fahn-Marsden dystonia rating scale (BFMDRS-M), abnormal involuntary movement scale (AIMS), and blepharospasm disability index (BSDI); and the clinical effect was evaluated after treatment.
RESULTS:
Compared before treatment, the scores of the sub-items of BFMDRS-M for eyes, mouth, speech and swallowing, and neck, as well as the total score of the scale, AIMS score and BSDI score decreased after treatment and during follow-up (P<0.05); the scores of the above indexes were not different statistically in comparison between the follow-up and after treatment (P>0.05). After treatment, 13 cases were effective, 2 cases were failed and the total effective rate was 86.7% (13/15).
CONCLUSION
"Tongdu Yisui" acupuncture and moxibustion can effectively alleviate motor symptoms and dysfunction of Meige syndrome and presents the sustained therapeutic effect.

3.eIF3a function in immunity and protection against severe sepsis by regulating B cell quantity and function through m6A modification.

eIF3a is a N 6-methyladenosine (m6A) reader that regulates mRNA translation by recognizing m6A modifications of these mRNAs. It has been suggested that eIF3a may play an important role in regulating translation initiation via m6A during infection when canonical cap-dependent initiation is inhibited. However, the death of animal model studies impedes our understanding of the functional significance of eIF3a in immunity and regulation in vivo. In this study, we investigated the in vivo function of eIF3a using eIF3a knockout and knockdown mouse models and found that eIF3a deficiency resulted in splenic tissue structural disruption and multi-organ damage, which contributed to severe sepsis induced by Lipopolysaccharide (LPS). Ectopic eIF3a overexpression in the eIF3a knockdown mice rescued mice from LPS-induced severe sepsis. We further showed that eIF3a maintains a functional and healthy immune system by regulating B cell function and quantity through m6A modification of mRNAs. These findings unveil a novel mechanism underlying sepsis, implicating the pivotal role of B cells in this complex disease process regulated by eIF3a. Furthermore, eIF3a may be used to develop a potential strategy for treating sepsis.
4.Atypical elastofibroma of the philtrum mimicking rosacea: a case report and literature review

Elastofibroma is a rare soft tissue tumor that most commonly occurs in the subscapular region. It has also been reported in several other anatomical locations. To our knowledge, no previous reports have described elastofibromas in the anterior aspect of the face. A 36-year-old man with intellectual disability was referred to our department for the evaluation of a painless multinodular lesion on the philtrum. The lesion was initially misdiagnosed as a dermatologic condition by the department of dermatology and was treated accordingly, however, it worsened, prompting referral to our department. We performed a surgical excision of the lesion and reconstructed the resulting skin defect with a skin graft. Histopathological examination confirmed the diagnosis of elastofibroma. This case underscores the importance of including elastofibroma in the differential diagnosis of facial neoplasms, particularly when the presentation is phymatous.

A significant portion of newly diagnosed lung cancer cases occurs in populations exposed to air pollution. The World Health Organization has identified air pollution as a human carcinogen, prompting many countries to implement monitoring systems for ambient particulate matter (PM). PM is composed of a complex mixture of organic and inorganic particles, both solid and liquid, that are found in the air. Given the carcinogenic properties of PM and the high prevalence of lung cancer among exposed populations, exploring their connection and clinical implications is critical for effectively preventing lung cancer in this group. This review explores the relationship between ambient PM and lung cancer. Epidemiological studies have demonstrated a dose-response relationship between PM exposure and lung cancer risk. PM exposure induces oxidative stress, disrupts the body’s redox balance, and causes DNA damage, which is a crucial factor in cancer development. Recent findings on the strong correlation between ambient PM and adenocarcinoma highlight the importance of understanding the specific molecular and pathological mechanisms underlying pollution-related lung cancer. In addition to efforts to control emission sources at the international level, a more individualized approach is essential for preventing PM-related lung cancer.

8.Adenylate kinase 5 deficiency impairs epididymal white adipose tissue homeostasis and decreases fat mass

Objective:
We aimed to elucidate the metabolic function and phenotype of AK5, a gene with an unknown function in metabolism.
Methods:
We generated AK5 knockout (KO) mice and administered high-fat and fasting diets to the mice to confirm their physiological phenotype and metabolic differences.
Results:
In the high-fat diet (HFD) test, no differences in body weight, epididymal white adipose tissue (eWAT), or fat mass of AK5 KO mice were observed. In indirect calorimetry, the respiratory exchange ratio and activity counts increased in normal chow diet mice, but there was no difference between the HFD groups. At the mRNA level, there were no significant differences in the expression of lipid metabolism markers between wild-type and AK5 KO mice. Still, the glucose signal of adipocytes increased in AK5 KO mice. In the fasting test, the eWAT of AK5 KO mice was significantly reduced, as was the expression of lipid metabolism and inflammation-related immune markers. Thus, the apoptosis of adipocytes increased, and an imbalance in energy metabolism occurred.
Conclusions
and Relevance: When AK5 KO mice are energy deficient, it becomes difficult to use eWAT for energy storage, imbalance occurs, and apoptosis of adipocytes in eWAT increases.
9.Therapeutic mechanism of of Erlong Zuoci Pills on oxidative stress in HEI-OC1 cells

To the present study aimed to investigate the protective effects of Erlong Zuoci Pills on oxidative stress induced by hydrogen peroxide (H2O2) in House Ear Institute-Organ of Corti 1 (HEI-OC1) and to explore the mechanism by cellular metabolomics. There were 6 groups in the experiment: the control group, model group, three dose groups of ELZC (low, medium, and high), and positive control ascorbic acid group. The oxidative stress injury model was established in the HEI-OC1 by inducing 0.9 mmol/L H2O2 for 12 h. The proliferation of HEI-OC1 cells was observed by CCK-8 assay; the contents and activity of lactate hydrogenase (LDH), reactive oxygen species (ROS), and superoxide dismutase (SOD) in HEI-OC1 cells were detected by corresponding kits. Finally, the endogenous substances of cells were analyzed from the perspective of metabolomics. Compared with the model group, ELZC groups could significantly increase the cell proliferation rate after administration. Moreover, they could also ameliorate the increase of ROS and LDH content and the decrease of antioxidant enzyme SOD caused by H2O2. Metabolomic results revealed significant differences among multiple groups in the scores of partial least squares discriminant analysis. The ELZC group could relocate the model group back to the control group. The metabolic regulation of ELZC on oxidative stress in HEI-OC1 cells mainly affects nucleotide metabolism and amino acid metabolism. In summary, the results indicate that ELZC exhibits protective effects on H2O2-induced oxidative stress in HEI-OC1 cells. Additionally, this protective effect may be produced by increasing the content of amino acids such as uridine and phenylalanine, thereby regulating pathways such as pyrimidine metabolism, phenylalanine metabolism, biosynthesis of phenylalanine, tyrosine, and tryptophan, and histidine metabolism.
