OBJECTIVES:
To analyze the efficacy and safety of concomitant left atrial appendage clipping during heart valve surgery for valvular heart disease patients with atrial fibrillation.
METHODS:
Fifty-eight patients who underwent concomitant left atrial appendage clipping during cardiac valve surgery in the Second Affiliated Hospital of Nanchang University from January 2017 to June 2023 were included in the analysis, including 1 case who underwent aortic valve replacement, 49 cases who underwent mitral valve replace-ment (or valvuloplasty)+tricuspid valvuloplasty, and 8 cases who underwent double valve replacement+tricuspid valvuloplasty (3 cases combined with coronary artery bypass grafting). The patients were followed up for 3-36 months [(16.69±6.61) months] after operation, and the changes of cardiac function and the occurrence of serious adverse complications were evaluated.
RESULTS:
The cardiopulmonary bypass time ranged from 75 to 145 min [(102.50±21.03) min], and the aortic cross-clamp time ranged from 35 to 80 min [(58.02±14.63) min]. The length of postoperative intensive care unit stay was 1 to 5 days [(2.47±0.82) d], and the length of postoperative hospital stay was 7 to 22 days [(10.84±2.69) d]. Cardiac ultrasound indicated complete closure of the left atrial appendage in all cases. During the follow-up, New York Heart Association (NYHA) functional classifications were improved in 54 patients. No left atrial appendage-related bleeding events or other perioperative complications were observed; and no cerebral infarction, limb embolism events, or mortality cases occurred during the follow-up.
CONCLUSIONS
For valvular heart disease patients with atrial fibrillation, concomitant left atrial appendage clipping during cardiac valve surgery demonstrates efficacy and safety, with no severe adverse events during a medium-term follow-up.

Journal of Chongqing Medical University 2025;50(7):913-919
Objective:To compare the dosimetric characteristics of three fractionated stereotactic radiotherapy techniques,i.e.,tomo-therapy(TOMO),volumetric-modulated arc therapy(VMAT),and CyberKnife(CK),in the treatment of intracranial oligometastases, and to assess their dose distribution, treatment efficiency, and difference in dose delivered to organs at risk(OARs).Methods:A retro-spective analysis was performed for the clinical data of 54 patients with intracranial oligometastases who underwent fractionated stereo-tactic radiotherapy in The First Affiliated Hospital of Army Medical University in 2021-2023.Varian Eclipse 16.1 Physician Worksta-tion was used to perform tumor target volume delineation, and MANTEIA AccContour 3.2 software was used to perform the delineation of OARs,such as brainstem, spinal cord, and optical nerves. The delineated structures and images were transmitted to TOMO,CK,and Eclipse treatment planning systems to design three different radiotherapy treatment plans.Related key parameters were analyzed using the dose-volume histogram to evaluate the dosimetric characteristics of these three radiotherapy techniques, including conformity index(CI)of the target, dose homogeneity index(HI),beam-on time, the number of monitor units(MU),and the exposure dose of OARs.Results:All three treatment plans(TOMO,VMAT,and CK)met the requirements for prescribed dose.TOMO had a slightly better CI than VMAT and CK(1.05 vs.1.09 and 1.17,P<0.001).VMAT had a better HI than CK and TOMO(1.15 vs.1.28 and 1.46,P<0.001).In terms of execution efficiency,VMAT had a significantly shorter beam-on time than TOMO and CK(5 minutes,1 633 MU vs.10 minutes,8 932 MU and 39 minutes,5 191 MU,P<0.001).In terms of the exposure dose of OARs,CK provided the best protection for the lens, with a maximum dose of 15 cGy for the right lens and 17 cGy for the left lens, and TOMO had an advantage in dose control for the right cochlea, with a mean dose of 88 cGy,while VMAT had the best performance in limiting the dose for the spinal cord, with a maximum dose of 31 cGy(P<0.05).Conclusion:This study shows that TOMO,VMAT,and CK all meet the requirements for the prescribed dose and can effectively protect OARs in the treatment of in-tracranial oligometastases. In clinical practice, the most appropriate technique should be selected based on the features of lesions and treatment goals to achieve individualized treatment.

Journal of Guangzhou University of Traditional Chinese Medicine 2025;42(11):2851-2855
This study summarized Professor Su Peiji's clinical experience in differenting and treating lumbago and leg pain.He contends that the disease involves the liver and kidney, manifesting as a syndrome of deficiency in origin and excess in superficiality, with liver-kidney deficiency and insufficiency of essence and blood as the root cause, and exogenous wind, cold, and dampness invading the meridians and then causing obstruction as the superficial manifestation. The principal pathological factors include wind, cold, dampness, blood stasis, and deficiency. In clinical practice,he follows the therapeutic principle of expelling pathogens and supporting healthy qi in accordance with the abundance or deficiency of pathogens and healthy qi,takes the therapy of tonifying the liver and kidney as the primary approach, and applies the adjuvant therapies of dispelling wind, cold, and dampness as well as relaxing tendons and activating collaterals.By incorporating the theory of collateral diseases,Professor Su Peiji proposes that the deficiency-excess of collateral diseases is closely related to the onset of lumbago and leg pain. His therapeutic approach centers around Shuji Formula,a self-formulated formula which is composed of Taxilli Herba,Achyranthis Bidentatae Radix,Stephaniae Tetrandrae Radix,Angelicae Pubescentis Radix,Clematidis Radix et Rhizoma,Aconiti Radix Cocta,Flemingiae Radix et Caulis,Millettiae Speciosae Radix,Poria,Angelicae Sinensis Radix,Paeoniae Radix Alba,Chuanxiong Rhizoma,Spatholobi Caulis,Tinosporae sinensis Caulis, and Glycyrrhizae Radix et Rhizoma Praeparata cum Melle.Modified use of herbs was performed according to the predominance of wind, cold, dampness, blood stasis,or deficiency,so as to achieve the effects of tonifying the liver and kidney, strengthening bones and tendons, dispelling wind, cold and dampness, and relaxing tendons and activating collaterals.

The metastasis-associated lung adenocarcinoma transcription 1 (MALAT1) is a highly conserved long non-coding RNA (lncRNA) gene. However, little is known about the pathological role of lncRNA MALAT1 in glioma. In the present study, we explored the expression level of lncRNA MALAT1 in primary glioma tissues as well as in U87 and U251 glioma cell lines. Using qRT-PCR, we found that the expression of lncRNA MALAT1 was significantly increased in glioma tissues compared with that of paracancerous tissues. Meanwhile, the expression of MALAT1 was highly expressed in U98 and U251 cells. In order to explore the function of MALAT1, the expression of MALAT1 was greatly reduced in U87 and U251 cells transfected with siRNA specifically targeting MALAT1. Consequently, cell viability of U87 and U251 cells were drastically decreased after the knockdown of MALAT1. Concomitantly, the apoptosis rate of the two cell lines was dramatically increased. Furthermore, the expression levels of some tumor markers were reduced after the knockdown of MALAT1, such as CCND1 and MYC. In summary, the current study indicated a promoting role of MALAT1 in the development of glioma cell.

MicroRNAs (miRNAs) that exert function by posttranscriptional suppression have recently brought insight in our understanding of the role of non-protein-coding RNAs in carcinogenesis and metastasis. In this study, we described the function and molecular mechanism of miR-139-5p in colorectal cancer (CRC) and its potential clinical application in CRC. We found that miR-139-5p was significantly downregulated in 73.8% CRC samples compared with adjacent noncancerous tissues (NCTs), and decreased miR-139-5p was associated with poor prognosis. Functional analyses demonstrated that ectopic expression of miR-139-5p suppressed CRC cell migration and invasion in vitro and metastasis in vivo. Mechanistic investigations revealed that miR-139-5p suppress CRC cell invasion and metastasis by targeting AMFR and NOTCH1. Knockdown of the two genes phenocopied the inhibitory effect of miR-139-5p on CRC metastasis. Furthermore, the protein levels of the two genes were upregulated in CRC samples compared with NCTs, and inversely correlated with the miR-139-5p expression. Increased NOTCH1 protein expression was correlated with poor prognosis of CRC patients. Together, our data indicate that miR-139-5p is a potential tumor suppressor and prognostic factor for CRC, and targeting miR-139-5p may repress the metastasis of CRC and improve survival.
