1.Establishment and validation of a model for femoral head necrosis after internal fixation of femoral neck fracture using logistic regression and SHAP analysis

BACKGROUND:The most common complication of traumatic femoral neck fractures after internal fixation is femoral head necrosis.Currently,many studies have reported on the risk factors that affect the occurrence and development of postoperative femoral head necrosis,but there is still a lack of tools to predict the risk of femoral head necrosis after internal fixation of femoral neck fractures.OBJECTIVE:To develop a predictive model that estimates the risk of femoral head necrosis shortly after patients with femoral neck fractures receive cannulated screw internal fixation.METHODS:A retrospective analysis reviewed clinical records of 172 patients who underwent cannulated screw internal fixation for femoral neck fractures at Department of Orthopedics of Mianyang Central Hospital from January 2013 to June 2023.Patients were categorized into two groups based on the presence or absence of femoral head necrosis within one year post-operation:the necrosis group and the non-necrosis group.Univariate analysis,Lasso regression,and multivariate Logistic regression techniques were employed to identify the determinants of femoral head necrosis.A nomogram prediction model was constructed using R language's"rms"package,version 4.0.The receiver operating characteristic curve was used to evaluate the discriminatory ability of the model.The Hosmer-Lemeshow test was used to evaluate the goodness of fit of the model,and the decision curve analysis was used to determine its clinical application benefits.Internal validation of the study was conducted using the Bootstrap method,involving 1 000 repeated samplings.To delve deeper into the primary factors influencing femoral head necrosis post-internal fixation of the femoral neck,this paper employed the SHAP method for data set analysis.RESULTS AND CONCLUSION:(1)The risk factors leading to femoral head necrosis in the short term after cannulated screw fixation of femoral neck fractures include:smoking,diabetes,Garden classification,fracture line location,reduction quality,age,and operation time.(2)The prediction model demonstrated robust performance,evidenced by an area under the curve of 0.940(95%Confidence Interval:0.903 to 0.977),indicating a high level of prediction accuracy.The model achieved a sensitivity of 90.2%and a specificity of 87.6%,indicating that its diagnostic performance was stable.The Hosmer-Lemeshow goodness-of-fit test yielded a chi-square value of 6.593 with a P-value of 0.581,confirming that the model's predictions closely align with the observed outcomes.(3)The calibration curve of the model also performed well,and its overall trend was very close to the ideal curve,further proving the high accuracy of the model.(4)The internal validation was carried out by the Bootstrap method with 1 000 repeated samplings,and the area under the curve of the model internal validation was still as high as 0.939,proving that the model had good stability.(5)Through the decision curve,it is found that within the probability threshold range of 1%to 92%,the model can obtain the maximum net benefit value.(6)The SHAP analysis results show that among the risk factors analyzed in this study,the location of the fracture line serves as the most significant predictor of femoral head necrosis following internal fixation with cannulated screws in femoral neck fractures,and subcapital fractures are extremely prone to femoral head necrosis after surgery.(7)It is concluded that the validated prediction model demonstrates strong discriminative power and reliability,offering practical clinical utility.It serves as a useful reference tool for short-term risk assessment of femoral head necrosis following internal fixation of femoral neck fractures.

3.Analysis of the genetic characteristics of varicella-zoster virus prevalent in Qinghai province from 2020 to 2024

Objective:To understand the genetic characteristics of varicella-zoster virus(VZV)prevalent in Qinghai province,China since 2020.Methods:A total of 54 pharyngeal swab specimens were collected from sporadic suspected varicella cases in Qinghai province in 2020,2023,and 2024. Real-time fluorescence quantitative polymerase chain reaction was used for etiological screening of the specimens. Sequencing of three genes,namely ORF22,ORF38,and ORF62,and single-nucleotide polymorphism(SNP)analysis were performed on VZV nucleic acid-positive specimens.Results:All 54 suspected varicella cases were diagnosed with VZV infection,and three gene sequences were successfully obtained from 53 specimens. The results of genotype identification showed that all VZV infection case specimens obtained in this study in Qinghai province were wild strains. Among them,4 specimens in 2020 were of clade 2 type;among 14 specimens in 2023,7 were of clade 2 type and the remaining 7 were of clade 5 type;among 35 specimens in 2024,27 were of clade5 type,5 were of clade 2 type,and 3 were of clade 4 type. The SNP results showed that in 2023 and 2024,one specimen each had an A→G base mutation at position 37 990,and in 2024,3 specimens had a T→C base mutation at position 37946. Among them,the sequences containing the former mutation have been prevalent and spread in multiple regions of China,and the latter has not been reported in other regions of China.Conclusion:From 2020 to 2024 in Qinghai province,at least three genotypes of VZV,namely clade 2 type,clade 5 type,and clade 4 type,co-prevailed,and the clade 5 genotype of VZV may become the dominant prevalent strain.
4.Preparation and immunogenicity evaluation of rotavirus VP8-mRNA vaccine

Objective:To construct a VP8-mRNA vaccine using human rotavirus spike protein VP8 domain as the immunogen and analyze its immunogenicity in mice.Methods:The VP8-mRNA sequence was designed, optimized, and synthesized. The VP8 gene of rotavirus G1P[8] type was used to construct the plasmid pUC57-VP8-Kan-SapⅠ, which was then sequenced. The plasmid confirmed by sequencing was subjected to large-scale amplification and extraction, followed by linearization, in vitro transcription, and capping. The purified capped products were encapsulated with lipid nanoparticles using a microfluidic control apparatus. The encapsulated VP8-mRNA vaccine was administered intramuscularly to mice at 10, 15, and 20 μg. Serum samples were collected for antibody detection by ELISA. Cellular immune responses were detected by flow cytometry and ELISPOT. Statistical analysis was performed using one-way or two-way analysis of variance and Tukey-Kramer test. Results:The encapsulated VP8-mRNA vaccine was rounded and spherical, with a particle size of about 100 nm, a polymer dispersion index of 0.088, and an encapsulation rate of 92.3%. Two doses of VP8-mRNA vaccine immunization could induce a good immune response in mice. The level of IgG antibody induced after immunization in the 15 μg group was comparable to that of the 20 μg group, and there was no statistical difference ( P>0.05), but the antibody levels in the two groups were significantly higher than that in the 10 μg group ( P<0.000 1). VP8-mRNA vaccine could induce neutralizing antibodies against rotavirus G1 and G9 types. The highest level of neutralizing antibodies against rotavirus type G1 was observed in the 15 μg group, which was significantly higher than that in the 10 μg group ( P<0.05). All immunization groups exhibited good neutralizing ability against rotavirus G9 type. The results of ELISPOT showed that lymphocytes from mice in each vaccine group were able to secrete IFN-γ when stimulated with VP8 peptide. Flow cytometry showed that the proportions of CD8 + T cell subsets in the vaccine groups were higher than that in the control group. Conclusion:The VP8-mRNA vaccine has good immunogenicity in mice and can induce good humoral and T-cell immune responses.

7.Role of exosomes in intrahepatic cholangiocarcinoma

Intrahepatic cholangiocarcinoma (ICC) is a special type of liver cancer with atypical clinical symptoms in the early stage, and most patients are already in the advanced stage at the time of initial diagnosis. Due to a lack of effective molecular markers and treatment options, ICC patients tend to have an extremely low five-year survival rate. Exosomes are vesicles secreted by cells that contain proteins, RNA, and lipids, and they are important carriers of intercellular communication. Recent studies have shown that exosomes play a crucial role in the development and progression of ICC, and this article reviews the role and mechanism of exosomes in the diagnosis and treatment of ICC and looks into the future treatment prospect and potential clinical application of exosomes.

l-Heptopyranoses are important components of bacterial polysaccharides and biological active secondary metabolites like septacidin (SEP), which represents a group of nucleoside antibiotics with antitumor, antifungal, and pain-relief activities. However, little is known about the formation mechanisms of those l-heptose moieties. In this study, we deciphered the biosynthetic pathway of the l,l-gluco-heptosamine moiety in SEPs by functional characterizing four genes and proposed that SepI initiates the process by oxidizing the 4'-hydroxyl of l-glycero-α-d-manno-heptose moiety of SEP-328 ( 2) to a keto group. Subsequently, SepJ (C5 epimerase) and SepA (C3 epimerase) shape the 4'-keto-l-heptopyranose moiety by sequential epimerization reactions. At the last step, an aminotransferase SepG installs the 4'-amino group of the l,l-gluco-heptosamine moiety to generate SEP-327 ( 3). An interesting phenomenon is that the SEP intermediates with 4'-keto-l-heptopyranose moieties exist as special bicyclic sugars with hemiacetal-hemiketal structures. Notably, l-pyranose is usually converted from d-pyranose by bifunctional C3/C5 epimerase. SepA is an unprecedented monofunctional l-pyranose C3 epimerase. Further in silico and experimental studies revealed that it represents an overlooked metal dependent-sugar epimerase family bearing vicinal oxygen chelate (VOC) architecture.
9.The Pathological Processing of Endocannabinoid System on Cardiac Hypertrophy in a Experimental Rats With Chronic Intermittent Hypoxia

Objective: To investigate the effect of endocannabinoid system on cardiac hypertrophy in experimental rats with chronic intermittent hypoxia and to study the impact of endocannabinoid antagonist, rimonabant in such pathological processing.
Methods: A total of 48 male Wistar rats were divided into 6 groups. 4 and 6 weeks of Normal control group, 4 and 6 weeks of Hypoxia group, 4 and 6 weeks of Hypoxia with rimonabant intervention group. n=8 in each group. The rats were sacrificed to measure left ventricular mass index (LVMI), the myocardial cell morphological changes were observed by optical microscope, the expression of cardiac calcium/calmodulin-dependent protein kinase II (CaMKII) and cardiotrophin-1 (CT-1) were detected by immunohistochemistry at 4 and 6 weeks respectively.
Results: Compared with 4 and 6 weeks of Normal control group, the LVMI, cardiac hypertrophy condition, CaMKII and CT-1 were increased in 4 and 6 weeks of Hypoxia group, all P<0.001. Compared with Normal control group and Hypoxia group, the above indexes in Hypoxia with rimonabant intervention group were higher than those in Normal control group while lower than those in Hypoxia group, all P<0.001.
Conclusion: The Chronic intermittent hypoxia could induce myocardial hypertrophy via endocannabinoid system disorders, such pathological processing could be reduced by rimonabant intervention.
10.Focal Nodular Hyperplasia and Hepatic Adenomas: Differential Diagnosis with Multiphasic Helical CT

Objective To study the methods of differential diagnosis of focal nodular hyperplasia and hepatic adenoma using multiphasic helical CT.Methods The data triphase helical CT of focal nodular hyperplasia (FNH) in 7 cases and hepatic adenomas in 5 cases proved pathologically were aualysed.The number,morphology,size,central scars and calcifications of lesions were observed,and the CT values of lesions and liver parenchyma on plain scan,arterial phase and portal venous phase were measured respectively.Results In 7 cases of FNH , mulitple lesions were present in one case , single lesion was in other 6 cases , totally 10 lesions in which six lesions were larger than 3 cm and four of the other were smaller than 3 cm in diameter , the central scars were detected in 7 cases . Five cases of hepatic adenoma were single and larger than 3 cm in diameter, no central scars were detected . Both focal nodular hyperplasia and hepatic adenoma no calicification could be seen . The study showed no significant difference between mean density values of focal nodular hyperplasia ( 48.18?7.82 ) HU and hepatic adenoma ( 42.54?2.37 ) HU on plain scan . In aterial phase , CT values were significant higher in focal nodular hyperplasia(124.29?18.69) HU than that in hepatic adenoma(83.29?9.09) HU.In the portal venous phase,no significant difference in values were detected between focal nodular hyperplasia(110.51?22.71) HU and hepatic adenoma(123.75?5.01) HU.Conclusion The differential diagnosis of focal nodular hyperplasia and hepatic adenoma can be done by multiphasic helical CT in combination with the quantitative evaluation of the density of liver lesions.
