1.HIV self-testing application through online platform among men who have sex with men in Tianjin City

Objective:
To understand characteristics of men who have sex with men (MSM) who applied for HIV antibody self-testing reagents through "AIDS self-testing" column of a WeChat official account named "Dark Blue Public Health Center" in Tianjin City, so as to provide insights into exploring online modes of HIV antibody self-testing for MSM.
Methods:
Data of MSM who were 18 years old or above, currently lived in Tianjin City, had sex with men in the past six months and applied for HIV antibody self-testing reagents through "AIDS self-testing" column from May 2018 to December 2022 were collected. Demographic characteristics, results return and positive findings were descriptively analyzed.
Results:
Data of 2 064 MSM were collected, including 1 052 MSM aged 20 to 29 years (50.97%), 1 522 unmarried MSM (73.74%), 545 workers (26.41%), 1 385 MSM with college education or above (67.10%), and 315 MSM without testing for HIV antibody in the past (15.26%). A total of 6 470 self-testing reagents were applied, and 5 942 testing results were returned, with a return rate of 91.84%. There were 33.28% (687/2 064) of the applicants applying for 66.32% (4 291/6 470) reagents multiple times. There were 73 MSM with positive results, accounting for 1.23%.
Conclusions
The MSM applying for HIV antibody self-testing reagents through "AIDS self-testing" are mainly young and highly educated, including some who have never tested for HIV. However, attention should be paid to duplicate applications and the return rate should be increased.
2.Predictive value of D-dimer for futile recanalization after mechanical thrombectomy in patients with acute ischemic stroke

Objective:To investigate the predictive value of preoperative D-dimer level for futile recanalization (FR) after mechanical thrombectomy (MT) in patients with acute ischemic stroke (AIS).Methods:It was a nested case-control study. A total of 116 patients with large vessel occlusion (LVO) stroke, who underwent successful recanalization (modified Thrombolysis in Cerebral Infarction, mTICI≥2b) after MT at the Stroke Unit of Beijing Hospital from August 2018 to January 2022,were consecutively enrolled, including 72 males (62.1%) with the age of (72.8±13.1) years. According to the 3-month modified Rankin Scale (mRS) score after MT, patients were divided into the meaningful recanalization group (mRS 0-2, n=41) and the futile recanalization group (mRS 3-6, n=75). The baseline clinical data of enrolled patients was collected. Logistic regression analysis was used to identify the independent risk factors for FR after MT in patients with AIS. Receiver operating characteristic (ROC) curve was used to evaluate the predictive value of D-dimer for FR. Results:Multivariate logistic regression analysis showed that high baseline systolic blood pressure (SBP) ( OR=1.038, 95% CI: 1.012-1.065, P=0.004), baseline National Institutes of Health Stroke Scale (NIHSS) score≥12 ( OR=10.157, 95% CI: 3.624-28.470, P<0.001) and high preoperative D-dimer level ( OR=4.536, 95% CI: 1.379-14.922, P=0.013) were independent predictors of FR after MT in AIS patients with LVO. ROC curve analysis indicated a good predictive value of preoperative D-dimer for the occurrence of FR ( AUC=0.733, 95% CI: 0.638-0.829, P<0.05), the optimal cut-off value of D-dimer was 2.65 μg/L(Lg), with the Youden index, sensitivity, specificity and accuracy of 0.435, 53.3%, 90.2% and 66.4%, respectively. Conclusion:High preoperative D-dimer level is an independent predictor of futile recanalization after MT in AIS patients with LVO, which shows good predictive ability for futile recanalization.
3.Protective effects of swertiamarin against radiation-induced lung injury

Objective:To explore the protective effects and mechanism of swertiamarin against ionizing radiation-induced lung injury.Methods:The human bronchial epithelial cells (Beas-2B) and human embryonic lung fibroblasts (HELF) were divided into control group, irradiation group, and irradiation + swertiamarin group, and the effect of swertiamarin on lung cells was detected after X-ray irradiation. Cells exposed to radiation were subjected to detections of lactate dehydrogenase (LDH) release, cell proliferation, and the levels of DNA damage, reactive oxygen species (ROS), lipid peroxides, and iron ions, as well as changes in the contents of ferroptosis-related protein SLC7A11 (xCT) and glutathione peroxidase 4 (GPX4). Moreover, to verify the protective effects of swertiamarin against radiation-induced lung injury in vivo, a mouse model of radiation-induced lung injury was developed. Specifically, C57BL/6J male mice were divided into three groups mentioned above, with five mice in each group. After chest irradiation with 15 Gy X-rays, the lung tissues and serum of the mice were collected at 30 days. The pathological changes in the lung tissues, the level of oxidative stress in these tissues, and changes in the levels of γ-H2AX, GPX4, and inflammatory factors were observed. Results:Compared to the radiation group, the radiation plus swertiamarin group (130 μmol/l) exhibited significant increases in the proliferation rate and clone proliferation rate of cells (Beas-2B: t = 5.50-5.92, P < 0.05; HELF: t = 3.79-5.51, P < 0.05), significant decreases in the LDH release rate, ROS content, lipid peroxide level, and iron ion content of cells (Beas-2B: t=3.00-16.99, P<0.05; HELF: t=4.10-10.97, P<0.05), a significant decrease in the level of DNA damage to cells (Beas-2B: t = 5.69-8.27, P < 0.05; HELF: t = 3.44-14.77, P < 0.05), and increased expression of xCT and GPX4 proteins in cells (Beas-2B: t = 2.90-3.27, P < 0.05; HELF: t = 3.01-7.07, P < 0.05). The in vivo experiments suggested that compared to radiation alone, additional pre-treatment using swertiamarin significantly increased the GSH and GPX4 contents in lung tissues of the mice ( t = 2.31-2.65, P < 0.05), decreased the MDA and γ-H2AX contents in the tissues ( t = 2.71-4.19, P < 0.05), and lowered the levels of IL-6 and IL-1β in the serum of the mice ( t = 3.16-4.56, P < 0.05). Conclusions:Swertiamarin has protective effects against ionizing radiation-induced lung injury by lowering the levels of DNA damage and oxidative stress. The result of this study will provide philosophies for the development of new protective agents against radiation-induced lung injury.
4.Risk factors for unfavorable outcomes after recanalization of large vessel occlusion in patients with acute ischemic stroke

Objective:To analyze risk factors for unfavorable outcomes after recanalization of large vessel occlusion (LVO) in patients with acute ischemic stroke (AIS).Methods:Patients with AIS-LVO who underwent recanalization treatment (including intravenous thrombolysis and endovascular intervention) at the Stroke Unit of Beijing Hospital from August 2018 to January 2022 were consecutively enrolled. According to the modified Rankin Scale (mRS) at 90-day follow-up after recanalization treatment, participants were classified as unfavorable outcomes (mRS>2) and favorable outcomes (mRS≤2). Baseline clinical data of enrolled patients was collected, and step-wise multivariate logistic regression analysis was used to identify independent risk factors for unfavorable outcomes after recanalization in AIS-LVO patients.Results:A total of 212 AIS-LVO patients were enrolled, including 86 females (41.35%), with an average age of 72.9 years. There were 75 patients in the favorable outcome group and 137 patients in the unfavorable outcome group. Compared with the favorable outcome group, the unfavorable outcome group had a higher average age, a higher proportion of females and patients with atrial fibrillation, higher baseline NIHSS, higher systolic blood pressure, and higher blood creatinine and D-dimer levels (all P<0.05). After adjusting for age and atrial fibrillation as confounding factors, multivariate logistic regression analysis showed that female ( OR=2.859, 95% CI: 1.202-6.799, P=0.018), higher baseline NIHSS ( OR=14.417, 95% CI: 6.269-33.158, P<0.001), higher pre-treatment systolic blood pressure ( OR=1.034, 95% CI: 1.015-1.054, P=0.001), higher emergency blood creatinine level ( OR=1.378, 95% CI: 1.105-1.719, P=0.005), and higher D-dimer level ( OR=3.594, 95% CI: 1.290-10.014, P=0.014) were independent risk factors for unfavorable outcomes after recanalization treatment in patients with AIS-LVO. Conclusion:Female, higher NIHSS, higher systolic blood pressure, higher blood creatinine level and D-dimer level are independent risk factors for unfavorable functional outcomes at 90 days after recanalization treatment of large vessel occlusion in patients with acute ischemic stroke.
5.Patterns of skin diseases among outpatients attending Hospital of Dermatology of Chinese Academy of Medical Sciences in 2019

Objective:To clarify patterns of skin diseases among outpatients at first and return visits to Hospital of Dermatology of Chinese Academy of Medical Sciences in 2019.Methods:Data were collected from the outpatient electronic medical record information system in Hospital of Dermatology of Chinese Academy of Medical Sciences from January 1st to December 31st, 2019, and patterns of skin diseases among outpatients at first and return visits were analyzed retrospectively.Results:The total number of outpatient consultations was 1 440 580 in 2019, including 941 755 (65.37%) first visits and 498 825 (34.63%) return visits, and the daily average number of outpatient consultations was 4 332. The top 10 most prevalent skin diseases were eczema, acne, urticaria, psoriasis, seborrheic dermatitis, vitiligo, neurodermatitis, pigmented nevus, tinea pedis and onychomycosis among outpatients at the first visits, with the number of outpatient visits being 739 175 and accounting for 78.49% of the total first visits; the top 10 most prevalent skin diseases among outpatients at the return visits were eczema, acne, psoriasis, urticaria, vitiligo, seborrheic dermatitis, neurodermatitis, pigmented nevus, keloid and rosacea, with the number of outpatient visits being 399 594 and accounting for 80.11% of the total return visits.Conclusion:In 2019, skin diseases predominated by common diseases, such as eczema and acne, among outpatients at Hospital of Dermatology of Chinese Academy of Medical Sciences.
6. Advances on pharmacogenomics of diuretics-related adverse reactions

Diuretics are the first-line drugs for the treatment of hypertension. Long-term use of diuretics often causes elevated blood sugar, electrolyte disturbances (most commonly hypokalemia), abnormal lipid metabolism (such as increased triglycerides, elevated cholesterol), increased serum uric acid levels and so on. In order to clarify the specific mechanism of the adverse reactions related to diuretics which are widely used in the treatment of hypertension, many documents have reported the pharmacogenomics research of diuretics-related adverse reactions. This review discusses the related genes and their variants of adverse reactions caused by commonly used diuretics in the treatment of hypertension in hope of providing a basis for the study of individualized use of diuretics.
7.Spinosin Inhibits Aβ1-42 Production and Aggregation via Activating Nrf2/HO-1 Pathway

The present research work primarily investigated whether spinosin has the potential of improving the pathogenesis of Alzheimer’s disease (AD) driven by β-amyloid (Aβ) overproduction through impacting the procession of amyloid precursor protein (APP). Wild type mouse Neuro-2a cells (N2a/WT) and N2a stably expressing human APP695 (N2a/APP695) cells were treated with spinosin for 24 h. The levels of APP protein and secreted enzymes closely related to APP procession were examined by western blot analysis. Oxidative stress related proteins, such as nuclear factor-erythroid 2-related factor 2 (Nrf2), and heme oxygenase-1 (HO-1) were detected by immunofluorescence assay and western blot analysis, respectively. The intracellular reactive oxygen species (ROS) level was analyzed by flow cytometry, the levels of Aβ1-42 were determined by ELISA kit, and Thioflavin T (ThT) assay was used to detect the effect of spinosin on Aβ1-42 aggregation. The results showed that ROS induced the expression of ADAM10 and reduced the expression of BACE1, while spinosin inhibited ROS production by activating Nrf2 and up-regulating the expression of HO-1. Additionally, spinosin reduced Aβ1-42 production by impacting the procession of APP. In addition, spinosin inhibited the aggregation of Aβ1-42. In conclusion, spinosin reduced Aβ1-42 production by activating the Nrf2/HO-1 pathway in N2a/WT and N2a/ APP695 cells. Therefore, spinosin is expected to be a promising treatment of AD.
8.An analysis on newly reported HIV/AIDS cases in medical institutions in Tianjin, 2005-2017

Objective:To analyze the characteristics of newly reported HIV/AIDS cases in medical institutions in Tianjin from 2005 to 2017 and provide evidence for the improvement of AIDS prevention and treatment in medical institutions.Methods:The data of the newly reported cases of HIV/AIDS in medical institutions in Tianjin from 2005 to 2017 were analyzed. Meanwhile a retrospective survey was conducted on the source department of medical institutions and the main disease for treatment of HIV/AIDS cases.Results:A total of 3 035 HIV/AIDS cases were reported in medical institutions in Tianjin from 2005 to 2017 with an annual increase of 24.8 %, among these cases, 49.2 % (1 343/2 728) were reported from inpatients and 50.8 % (1 385/2 728) were reported from outpatients. There was an increasing trend in the HIV positive detection rate in STD outpatients from 2011 to 2017 ( χ2=18.469, P=0.005). The counts of the first CD 4+ T cell counts (CD 4) ( χ2=17.189, P=0.000) among the cases from inpatients and outpatients and the proportions of deaths in the year when the cases were found differed ( χ2=61.198, P=0.000), the differences were significant. And the statistically significant differences in the counts of the first CD 4 ( χ2=16.751, P=0.000) and the proportions of deaths in the year when the cases were found ( χ2=18.926, P=0.000) existed among the cases in grade Ⅲ hospitals and in grade Ⅱ hospitals; meanwhile the differences in the counts of the first CD 4 ( χ2=37.620, P=0.000) and the proportions of deaths in the year when the cases was found ( χ2=95.489, P=0.000) among the cases from different departments were significant. Conclusion:The timeliness of HIV/AIDS case reporting by outpatient departments was better in medical institutions in Tianjin, and the case reporting in grade Ⅱ hospitals were more timely than in grade Ⅲ hospitals; so it is suggested to provide active HIV test in key departments of medical institutions for the better HIV/AIDS prevention and control in Tianjin.
9.Application value of functional transcranial Doppler ultrasound in the evaluation of neurovascular coupling function

Neurovascular coupling (NVC) is the basis of neuronal activity and metabolism, and its dysfunction may be closely associated with the occurrence and development of the nervous system diseases. Functional transcranial doppler ultrasonography (fTCD) can evaluate the NVC function. FTCD researches have shown that ischemic stroke, vascular cognitive impairment and other patients have NVC dysfunction. This article reviews the methods of fTCD examination of NVC and its clinical application.
10.Bibliometric analysis of papers on case management from Web of Science

Objective To search the Web of Science for the papers related to case management to understand the present situation and direction of case management. Methods Papers on case management were retrieved in the Web of Science from 1999 to 2017.The Web of Science and excel were used to analyze the publication years, research themes, journals, countries and institutes. Results Totally,970 articles were included,with a sum of cited times,12,075.The number of publications on the case management was going up. Most of the authors were from the USA and China was ranked at 7 in authorship. Most of the papers concerned the research in chronic diseases such as chronic heart failure,mental disease and HIV. Conclusions Case management has been widely researched in the developed countries.China still has a lot of room for improvement in the study.The future research should be focused on the establishment of case manager training,multidisciplinary combination,case management mode suitable for chronic disease and aging population and exploration of the mode's effectiveness.
