1.Effects of different transcranial magnetic stimulation modes on refractory depression in adults:a network meta-analysis

OBJECTIVE:To assess the efficacy and safety of transcranial magnetic stimulation in the treatment of refractory depression and to compare the differences in efficacy between various transcranial magnetic stimulation treatment protocols in refractory depression,thereby providing a theoretical basis for the clinical selection of transcranial magnetic stimulation treatment protocols.METHODS:A comprehensive search was conducted across multiple databases,including PubMed,Embase,Cochrane Library,Web of Science,CNKI,WanFang Data,CBM and VIP.The search terms were"transcranial magnetic stimulation,treatment-resistant depression,randomized controlled trial"in Chinese,and"depressive disorder,treatment-resistant,transcranial magnetic stimulation,randomized controlled trial"in English.The objective was to identify randomized controlled trials on the treatment of patients with refractory depression published from the establishment of the databases to September 2024.The quality of the included studies was evaluated using the Cochrane risk of bias assessment tool,version 5.1.0,and the Physiotherapy Evidence Database scale.Meta-analysis of the outcome indicators was conducted using the Review Manager 5.4 and Stata 18.0 software.RESULTS:(1)Following a comprehensive review,20 randomized controlled trials were included in the analysis.All of the trials were assessed to be of high or very high quality according to the Physiotherapy Evidence Database scale.(2)Meta-analysis results showed that,compared with the sham stimulation group,high-frequency repetitive transcranial magnetic stimulation could significantly reduce the scores of Hamilton Depression Rating Scale[mean difference(MD)=-3.89,95%confidence interval(CI):-6.14 to-1.65,P<0.05)or the Montgomery Depression Rating Scale(MD=-3.97,95%CI:-6.57 to-1.36,P<0.05).(3)The probability ranking results of the network Meta-analysis showed that,in terms of the Hamilton Depression Rating Scale score,the probability ranking results were as follows:high-frequency repetitive transcranial magnetic stimulation(69.9%)>intermittent theta burst stimulation(62.8%)>bilateral theta pulse stimulation(57.5%)>low-frequency repetitive transcranial magnetic stimulation(54.9%)>bilateral sequential transcranial magnetic stimulation(49.0%)>transcranial pulsed electromagnetic field(37.0%)>sham stimulation(18.9%).And in terms of the Montgomery Depression Rating Scale score,the probability ranking results were as follows:high-frequency repetitive transcranial magnetic stimulation(93.3%)>bilateral theta pulse stimulation(50.3%)>sham stimulation(45.9%)>low-frequency repetitive transcranial magnetic stimulation(32.1%)>bilateral sequential transcranial magnetic stimulation(28.4%).CONCLUSION:Transcranial magnetic stimulations can improve the depressive symptoms of patients with treatment-resistant depression.Among them,the high-frequency repetitive transcranial magnetic stimulation mode has the best effect on improving the depressive symptoms of patients with treatment-resistant depression,followed by the intermittent θ burst stimulation mode.
2.Applications and challenges of DNA barcoding in rapid radiation groups: Rhodiola (Crassulaceae) as a case study.

OBJECTIVE:
Rhodiolae Crenulatae Radix et Rhizoma (Hongjingtian in Chinese, RCRR), the roots and rhizomes of Rhodiola crenulata and its application in the medicinal market is very chaotic. In this study, DNA barcoding database and identification engine of Rhodiola species were established, decoction pieces from the medicinal market were identified, and the application and challenges of DNA barcoding in the rapid radiation of Rhodiola species were analyzed. This study provides reference for the protection, rational development, and utilization of endangered resources within Rhodiola species.
METHODS:
A total of 50 original plant samples from 20 species of the genus Rhodiola from Hebei, Xinjiang, Tibet, Jilin, and other major production areas were collected. Theses samples cover the typical distribution area (Qinghai-Tibetan Platea) of Rhodiola species and other scattered alpine regions (Changbai Mountain, Taibai Mountain, Lushan Mountain, etc.), it encompasses all Rhodiola species with thick rhizomes in China. ITS2 and psbA-trnH barcode of Rhodiola database (BORD) were established and an identification engine named Rhodiola-IDE was developed. The stability and accuracy of the standard DNA barcoding database were evaluated using two datasets. Rhodiola-IDE identified 31 decoction pieces of RCRR from the medicinal material market.
RESULTS:
The BORD containing 1 532 sequences of 88 Rhodiola species has been established, and the identification efficiency results showed good accuracy and stability. According to the Chinese Pharmacopoeia (2020 edition), 23 samples (74.2%) were identified as authentic R. crenulata, while the rest of the marketed varieties were R. kirilowii, R. dumulosa, and R. fastigiata. The product label "Larger flower, Hongjingtian" was identified as R. crenulata. Samples labeled as "Smaller flower, Hongjingtian" were identified as R. crenulata, R. kirilowii, and R. fastigiata.
CONCLUSION
ITS2 and psbA-trnH barcodes can identify monophyletic groups represented by R. crenulata. However, for non-monophyletic species, it is necessary to collect as many samples as possible and combine them with multiple markers for joint identification. This study discussed the application and challenges of DNA barcodes in Rhodiola under rapid radiation conditions, providing a scientific basis for the rational development and utilization of Rhodiola varieties.

4.Evaluation of current status in implementation of WS 582-2017 Specifications for testing of quality control in X and γ ray stereotactic radiotherapy system

Objective:To investigate WS 582-2017 Specifications for testing of quality control in X and γ ray stereotactic radiotherapy system (hereinafter referred to as WS 582-2017) with respect to its current status, existing problems and evaluation since its implementation in Beijing, Chongqing, Jiangsu and Sichuan. Methods:The evaluation of this standard was carried out by means of field survey, questionnaire survey and expert seminar. In accordance with the WS 582-2017 implementation assessment programme, a total of 153 professionals involved in the implementation of WS 582-2017 from 80 radiological health technical service institutions, health supervision institutions and medical institutions of two municipalities and two provinces were surveyed in 2023. The investigation was carried out by using a combination of cluster sampling and stratified sampling and through sampling professionals with of different institution characteristics and different technical titles. The general survey method was used for Jiangsu province, while the cluster sampling method was used for others, with 153 individulas from 80 institutions being sampled for the investigation.Results:The awareness rate of WS 582-2017 was 80.39%, of which 90.85% considered it to be scientific and rigorous, 81.05% considered it to be operational and 97.39% considered it to be in consistency with the relevant regulatory standards, 90.20% considerd it to have played an important role in promoting radiological protection optimization for radiotherapy, 90.85% considered the implementation of it to have played a significant role in the quality control of standarded testing of radiotherapy equipment, and 86.93% considered the implementation of it to have played a significant role in enhancing the professional competence of relevant technicians.Conclusions:The overall implementation of WS 582-2017 in two municipalities and two provinces is good, with scientific and operable technical contents and the remarkable effect achieved in its implementation. However, the WS 582-2017 needs to be further improved in some areas, and training on the standard needs to be strengthened to improve the awareness rate of it.
5.Efficiency comparison of the Dem with axial length to corneal radius ratio in predicting refractive state of 3-14 years old children

AIM: To assess the association between the Dem, axial length/corneal radius ratio(AL/CR ratio), and the refractive status of children aged 3 to 14, compare the efficiency of Dem with AL/CR ratio in predicting myopia, and to explore a more effective method for quantitatively predicting the refractive state of children.METHODS: This cross-sectional study involved 519 children aged 3 to 14, including 519 eyes(data from the right eyes were used). All participants were out-patients at the ophthalmic clinic of Xi'an No.1 Hospital from March 2021 to December 2022. The axial length(AL), keratometry(K), Dem, and spherical equivalent refraction(SER)were recorded before ciliary paralysis, and myopia was defined when the SER ≤-0.50 D.RESULTS: There was a positive correlation between SER and Dem(r=0.88, P<0.01), and a negative correlation between SER and the AL/CR ratio(r=-0.87, P<0.01)in children aged 3-14. The optimal cutoff points for the Dem and AL/CR ratio were 20.40 D, and 3.02, respectively. Myopia could be diagnosed when the Dem ≤20.40 D or the AL/CR ratio ≥3.02. SER decreased by 0.57 D for every 1 D decrease in Dem and by 1.00 D for every 0.06 increase in the AL/CR ratio. The area under the ROC curve(AUC)for Dem(0.958)was significantly greater than that for the AL/CR ratio(0.940).CONCLUSION: There is a positive correlation between SER and Dem, and a negative correlation between SER and the AL/CR ratio in children aged 3-14. Dem is more efficient than the AL/CR ratio in myopia assessment. The eye biometrics directly provide Dem values, which is more convenient for calculating the dose-effect relationship with SER, thus making it more efficient for predicting SER in children.

7.Influence of Inflammation on Tumor and Intervention of Chinese Medicine: Based on "Inflammation-tumor Transformation"

With high incidence and lethality rate and certain disability rate, tumor has become a major global public health threat. It has been verified that the occurrence and development of tumor are resulted from the synergy of environment, heredity, and gene mutation, involving the abnormal activation or inhibition of a variety of related pathways such as oxidative stress, inflammation, autophagy, and mesenchymal transition of cells. Among them, the excessive activation of inflammatory signaling pathway is one of the main mechanisms of carcinogenesis and tumor progression, which enhances the proliferation, chemoradiotherapy resistance, invasion, and metastasis of cancer cells. At the moment, the correlation between long-term chronic uncontrollable inflammation and "inflammation-cancer transformation" has been widely recognized. Therefore, it is of great significance for the prevention and treatment, diagnosis, and prognosis evaluation of tumor to clarify the role of inflammation in the incidence of tumor. Blockers or activators have been developed to target the corresponding inflammatory pathways. However, tumor is accompanied by the abnormality of multiple inflammatory pathways, especially the advanced tumor with metastasis of cancer cells, and thus the efficacy of single pathway-targeting agents is non-ideal. Chinese medicine, featuring multiple components and multiple targets, can remarkably control the inflammatory response, delay tumor progression, enhance the sensitivity of tumor cells to radiotherapy and chemotherapy, and reduce postoperative infection and the adverse reactions caused by radiotherapy and chemotherapy, thereby exerting the anti-cancer effect. Nevertheless, a few reports on the anti-tumor effect of Chinese medicine from the perspective of inflammation are available. Therefore, this paper mainly expounds the influence of inflammation on the occurrence and development of tumor and summarizes the research on the intervention of tumor by Chinese medicine through inflammatory pathway, which is expected to provide a new mindset for the prevention and treatment of tumor.
8.Transcription and regulation of hepatitis B virus genes in host sperm cells.

To investigate whether transcription of hepatitis B virus (HBV) gene occurs in human sperm, total RNA was extracted from sperm of patients with chronic HBV infection (test-1), from donor sperm transfected with a plasmid containing the full-length HBV genome (test-2), and from nontransfected donor sperm (control), used as the template for reverse transcription-polymerase chain reaction (RT-PCR). Positive bands for HBV DNA were observed in the test groups but not in the control. Next, to identify the role of host genes in regulating viral gene transcription in sperm, total RNA was extracted from 2-cell embryos derived from hamster oocytes fertilized in vitro by HBV-transfected (test) or nontransfected (control) human sperm and successively subjected to SMART-PCR, suppression subtractive hybridization, T/A cloning, bacterial amplification, microarray hybridization, sequencing and the Basic Local Alignment Search Tool (BLAST) search to isolate differentially expressed genes. Twenty-nine sequences showing significant identity to five human gene families were identified, with chorionic somatomammotropin hormone 2 (CSH2), eukaryotic translation initiation factor 4 gamma 2 (EIF4G2), pterin-4 alpha-carbinolamine dehydratase 2 (PCBD2), pregnancy-specific beta-1-glycoprotein 4 (PSG4) and titin (TTN) selected to represent target genes. Using real-time quantitative RT-PCR (qRT-PCR), when CSH2 and PCBD2 (or EIF4G2, PSG4 and TTN) were silenced by RNA interference, transcriptional levels of HBV s and x genes significantly decreased (or increased) (P < 0.05). Silencing of a control gene in sperm did not significantly change transcription of HBV s and x genes (P > 0.05). This study provides the first experimental evidence that transcription of HBV genes occurs in human sperm and is regulated by host genes.

9.Analysis of safety difference of 10 kinds of commonly used anti-infective agents with same generic drugs name based on the adverse drug reaction data

Objective To explore the possible safety difference of anti-infective agents with the same generic drug name produced by different manufacturers. Methods Spontaneous Reporting System for Adverse Drug Reactions of Beijing was searched. The reports of adverse drug reactions(ADR)of levofloxacin,azithromycin,moxifloxacin,clindamycin,cefuroxime,etimicin,cefoperazone sulbactam sodium,cefoxitin,ceftriaxone and ornidazole from January 2009 to December 2013 were collected. The number of ADR reports,types of ADR,suspicious drugs which induced ADR,and the manufacturers were recorded. The number of ADR signals was detected by reporting odds ratio(ROR)method. The safety of drugs was evaluated according to the number of signal and the type of ADR. Results A total of 18 366 ADR reports of 10 kinds of anti-infective drugs were collected. The top five ADR′s types were skin and subcutaneous tissue diseases(7 644),gastrointestinal diseases(2 505),systemic diseases and various parts of the site of treatment(1 761),nervous system disease(1 592)and immune system diseases (1 254)successively. The sum of the digits above accounted for 80.3% in total number of 10 kinds of anti-infective drugs ADR reports(14 756/18 366). The drugs which caused the five kinds of ADR were levofloxacin,azithromycin and clindamycin/moxifloxacin mainly. The numbers of ADR signals of evofloxacin,azithromycin,moxifloxacin,clindamycin,cefuroxime,etimicin,cefoperazone sulbactam sodium,cefoxitin,ceftriaxone,and ornidazole which were detected by ROR method were 17,20,1,23,9, 9,20,5,1 and 9,respectively. The ADR types related to ADR signals were 11,12,1,12,6,8,10,5 and 8,respectively. The numbers of manufacturers related to ADR signals were 9,13,1,14,6,9,11,5, 1 and 9,respectively. Conclusions There are some differences in ADR signals and ADR types among the 10 kinds of anti-infective agents with the same generic drug name produced by different manufacturers. It is suggested that there are some differences in safety of the above-mentioned drugs.
