Cerebral small vessel disease （CSVD） is a heterogeneous condition affecting small vessels in the brain and is closely associated with cognitive dysfunction， gait abnormalities， and emotional disorder in the elderly. Based on the collateral disease theory of traditional Chinese medicine （TCM）， a "form-function" pathogenesis framework for elderly CSVD is proposed from the perspective of their relationship. In this framework， the "form" refers to the morphological basis of the brain collaterals and sweat pore， while the "function" refers to the activities of qi-blood circulation， nourishment and regulation， as well as the body fluid transportation and distribution； the two are interdependent and jointly maintain the mental activities of the brain. Under this framework， the dynamic pathogenesis evolution patterns of CSVD， namely collateral fragility， collateral rigidity， collateral stagnation， and collateral damage， are systematically explained， revealing the potential mechanisms underlying the transformation of age-related physiological decline of brain collaterals into pathological impairment. This provides a theoretical foundation for the diagnosis and treatment of elderly CSVD with TCM.

Objective The aim of this study was to explore the mechanism of Naotaifang(NFT)in preventing neuronal pyroptosis in cerebral ischemia-reperfusion injury(CIRI).Methods Firstly,a network Meta-analysis was conducted to compare the clinical efficacy of Naotaifang and dl-3-n-butylphthalide in treating ischemic stroke,and dl-3-n-butylphthalide was identified as the positive control drug in this study.Then,a rat CIRI model was established using the middle cerebral artery occlusion/reperfusion(MCAO/R)method.Sixty adult male SD rats were randomly divided into model group(Model group),low-dose Naotaifang group(NTF-L group),medium-dose Naotaifang group(NTF-M group),ahigh-dose Naotaifang group(NTF-H group),NBP group(NBP group),and a sham surgery group(Sham group)using a random number table method,with 10 rats in each group.After MCAO/R,rats received NTF(4.5 g/kg for NTF-L,9 g/kg for NTF-M,and 18 g/kg for NTF-H)or Nimodipine(60 mg/kg)or distilled water(Sham group and Model group)via gavage for seven consecutive days.Neurological function was evaluated using the Zea Longa method,infarct volume was assessed by TTC staining,and HE and Nissl staining were used to observe changes in neurons in the ischemic cortex.ELISA was used to measure serum IL-1β and IL-18 levels,and Western blot was used to detect caspase-1 and GSDMD expression in the ischemic cortex.Results Network Meta-analysis showed no significant difference in clinical efficacy,neurological function scores,and TXB2 expression between Nimodipine and NTF interventions.Animal experiments revealed that neurological scores of the Model group was significantly increased,the volume of cerebral infarction was significantly enlarged,the structure of nerve cells in the ischemic cortical area was destroyed,and the number of nerve cells and Nissl bodies was significantly reduced,and expressions of IL-1β,IL-18 inflammatory factors and caspase-1,and GSDMD focal proteins were significantly decreased(P<0.01).The NTF-H group significantly reduced neurological function scores and cerebral infarction volume of rats in the Model group,significantly improved morphology of nerve cells and the number of Nissl body,and significantly decreased the expressions of IL-1β,IL-18 inflammatory factors,caspase-1,and GSDMD necroptosis proteins(P<0.01).There was no significant difference between the NTF-H group and the NBP group in terms of neurological scores,volume of cerebral infarction,IL-1β,IL-18 levels,and caspase-1 and GSDMD protein expression(P>0.05).Conclusion Both NTF and Nimodipine have therapeutic effects on ischemic stroke patients,with no significant difference between them,making Nimodipine a suitable positive control drug.NTF may alleviate CIRI by reducing pyroptosis through the caspase-1/GSDMD signaling pathway.

This article systematically analyzes the scientific connotations of the four core elements of traditional Chinese medicine-natural view,holistic view,epistemology,and methodology-from a modern scientific perspective.By integrating literature analysis with modern scientific research,this study explores the compatibility between the concept of"harmony between heaven and man"in traditional Chinese medicine and the interactive relationship between"human-environment-microorganisms"in modern biology.It analyzes the theoretical value of holistic views such as consistency of form and spirit,and consistency of time and space in systems medicine,clarifies the understanding of the essence of dynamic balance of life by the concept of constant movement and the monism of Qi,as well as the model of the Yin Yang Five Elements system in the human body.Finally,modern interpretations of traditional Chinese medicine methodologies such as taking signs and fortune,syndrome differentiation and treatment,and preventing diseases are provided.This cross temporal theoretical resonance not only verifies the scientific connotation of traditional Chinese medicine theory,but also provides integrated and innovative ideas for modern medicine to understand the human body and diseases.It has important theoretical significance and practical value for promoting the modernization and development of traditional Chinese medicine theory.

The theory of blood stasis originated from"The Yellow Emperor's Inner Canon(huang Di Nei Jing)",and the drugs used for promoting blood stasis originated from"Shennong's Classic of Materia Medica(Shen Nong Ben Cao Jing)".Finally,the medical sage Zhang Zhong-jing first put forward the name of"blood stasis"in"Synopsis of the Golden Chamber(Jin Gui Yao Lue)",and used the method of promoting blood stasis to treat various diseases,which provided ideas for the future study on the mechanism and treatment of blood stasis syndrome.Therefore,Zhang Zhong-jing was the founder of the theory of blood stasis.Moreover,the principles,methods,prescriptions and medicines which he put forward were still appropriate effectively in the clinic.On the basis of comprehensively combing the research achievements of Zhongjing's blood stasis theory,with combination with the current research hotspots,the review gives a novel understanding of the theory,method,prescription and medicine of Zhongjing's blood stasis theory from new aspects,including etiology,pathogenesis,treatment of blood stasis theory,and the active ingredients of anti-blood stasis Chinese medicines.We intend to promote the research progress of the scientific connotation of blood stasis theory,and improve the diagnosis and treatment level of clinicians in preventing and treating difficult diseases related to blood stasis syndrome through integrated summary and novel interpretation.

Parkinson's disease is a neurodegenerative disease that mainly affects dopamine neurons in the substantia nigra region of the brain,and its clinical manifestations include shaking and slowness of movement.The means of clinical medicine treatment are limited,but traditional Chinese medicine,with its advantages of low cost and high safety,shows great potential in the prevention and treatment of Parkinson's disease.Our team proposed that the core pathogenesis of Parkinson's disease was"earth deficiency and wood multiplication",that is,spleen(soil)deficiency and liver(wood)dysfunction.Among them,"earth deficiency"was the basis of the disease,and"wood multiplication"was the core of the disease.The treatment advocated"strengthening spleen and soothing liver",the specific methods included strengthening spleen and resolving phlegm,Tonify qi and cultivate the middle and soothing liver and relieve depression,and subdue wind and unblock the meridians,etc.Representative recipes were Lianmei Siwu decoction,Zhengan Xifeng decoction,Huangya decoction and Shenzhu decoction.In the treatment,we should pay attention to the whole concept and syndrome differentiation and treatment,and combine the three factors to improve the clinical effect.At the same time,the advantages and mechanisms of TCM in the prevention and treatment of Parkinson's disease deserve further research and verification.

Objective To establish a scientific and feasible Chinese medicine(CM)comprehensive control and prevention program for elderly hypertension with early renal damage(EH-ERD)patients through high-level evidence-based medicine(EBM)evidence.Methods On the basis of literature research and evidence evaluation,we construct a database of specific prescriptions and implementation methods of CM comprehensive control and prevention program with EH-ERD.40 senior titled-experts were consulted in two rounds of questionnaires based on Delphi methods.We selected,evaluated,and revised specific CM comprehensive control and prevention program of EH-ERD through the analysis of multiple factors,such as expert's positive activity coefficient,authority coefficient,degree of opinion concentration and degree of coordination.Results In view of the CM appropriate intervention techniques with high-grade evidence(level Ⅰ and Ⅱ)and recommendation(level A and B),we have developed the CM comprehensive control and prevention plan items for EH-ERD.The activity coefficients in two-round consultation were 92.5%and 97.14%respectively,the overall authority coefficient(Cr)was exceeded 0.70,and the coordination coefficient was less than 0.25.With a sound activity coefficient,a relatively high Cr and a rather unified degree of opinion concentration and coordination,the CM comprehensive control and prevention program for EH-ERD was established,which included six primary indicators(CM compound preparation,acupuncture and massage,CM health education,medicinal diet therapy,health care exercise and foot bath)and seventeen secondary indicators.Conclusion The CM comprehensive control and prevention program for EH-ERD established in this study can provide a basis for the further formation of expert consensus or guidelines.Moreover,it can supply the strategy and paradigm for standardization of CM-based health management of EH-ERD.

Objective To assess the therapeutic effect of acupuncture in depressive disorders based on current randomized controlled trials(RCTs).Method RCTs on acupuncture interventions for primary depression by searching databases including CNKI,Wanfang,VIP,Sinomed,PubMed,and Embase.Two researchers independently assessed the quality of the literature,extracted data according to the latest Cochrane Handbook for Systematic Reviews of Interventions,and cross-checked the data.Meta-analysis was performed using RevMan 5.4 software.Result A total of 52 studies were included,comprising 5277 patients.The meta-analysis results showed that acupuncture significantly improved depression symptoms as measured by the Hamilton Depression Rating Scale(HAMD)and reduced clinical inefficacy rates{HAMD-17:WMD=-2.35,95%CI[-3.04,-1.67],P<0.000 01;HAMD-24:WMD=-2.77,95%CI[-3.52,-2.03],P<0.000 01;HAMD(unspecified scale):WMD=-2.77,95%CI[-3.52,-2.03],P<0.000 01;Clinical inefficacy:RR=0.39,95%CI[0.33,0.47],P<0.000 01}.Additionally,acupuncture increased 5-HT levels(SMD=1.11,95%CI[0.67,1.55],P<0.000 01).In terms of safety,acupuncture did not increase the incidence of adverse events and reduced TESS scores{Adverse event incidence:RR=0.65,95%CI[0.47,0.89],P=0.007;TESS scores:WMD=-1.66,95%CI[-3.12,-0.20],P=0.03}.There is potential publication bias regarding clinical inefficacy and adverse events,necessitating cautious interpretation.Conclusion Based on the current evidence,acupuncture is an effective and safe treatment for depressive disorders.However,more high-quality RCTs are needed to further validate or refine these conclusions.
