Gorham-Stout disease(GSD) is a rare osteolytic disorder characterized by spontaneous and progressive osteolysis, along with abnormal angiogenesis and lymphangiogenesis, with no new bone formation. We present a case of a 15-year-old female admitted due to " recurrent right leg pain for 5 years, 11 months after undergoing right femoral fracture surgery". Through comprehensive integration of the patient's clinical phenotype, laboratory tests, imaging findings, pathological examinations, and molecular biological test results, GSD was considered highly likely. A multidisciplinary treatment approach was conducted, including a combination of zoledronic acid and sirolimus to inhibit osteolysis, along with rehabilitation training and orthopedic intervention, providing a personalized and comprehensive treatment strategy.

Background The high work intensity and possible subsequently increased susceptibility to occupational hazards of calcium carbide plants may lead to hypertension in workers, but there are few studies on the relationship between occupational hazard exposure and hypertension in workers involving the production process of calcium carbide. Objective To explore the influence of occupational hazards on the incidence of hypertension in calcium carbide plants. Methods Using historical cohort design, the employees of a calcium carbide factory in the western part of Inner Mongolia Autonomous Region were selected as research subjects. According to the pre-determined inclusion and exclusion criteria, the study population comprised an exposure group of 377 employees (including furnace workers, inspection workers, and maintenance workers) exposed to dust, noise & carbon monoxide, and a control group of 388 employees (including central control workers, electricians, and administrative personnel) without above-mentioned exposure. The total sample size was 765 participants. The follow-up period was from April 2011 to October 2022, and the study endpoint was defined as the conclusion of the follow-up period or diagnosed hypertension in annual occupational health examination. Information on general demographic characteristics, living habits, and work status was collected from all study subjects. Cox proportional hazards regression model was used to analyze the association between occupational hazard exposure and the risk of hypertension among the calcium carbide plant employees. Results The average age, mean systolic and diastolic blood pressure, proportion of males, smoking rate, and alcohol consumption rate in the exposure group were higher than those in the control group (P<0.05). Compared to baseline, both systolic and diastolic blood pressure levels increased in the exposure group and the control group at the end of the follow-up (P<0.05). At the end of the follow-up, the average differences between systolic/ diastolic blood pressure and baseline values in the exposure group were higher than those in the control group (P<0.05). During the follow-up period, a total of 223 cases of hypertension occurred, with a total follow-up of

This study aims to construct a recombinant vaccine containing the tetanus toxin C frag-ment(HC)with a built-in adjuvant of metallothionein 3(MT-3)and evaluate its immune effect in mice. The gene sequences of MT-3 and HC were fused via a linker to create the pET30a-MT-3-HC recombinant plasmid, which was then transformed into E.coli BL21(DE3)competent cells. The re-combinant plasmid was confirmed through double enzyme digestion. The M3C recombinant protein expression was induced, identified by SDS-PAGE and Western blot, and purified using Ni-NTA af-finity chromatography.Five groups of vaccines, including PBS,HC,TT(tetanus toxoid),M3C,and M3C+CpG(composite adjuvant),were administered to mice via intramuscular injection at 7-day intervals for three immunizations.Blood samples were periodically collected from the tail vein.ELISA was used to measure changes in specific IgG antibody titers in the serum, and on day 28,antibody subtypes(IgG1,IgG2a,IgG2b,IgG3,and IgM)and cytokine levels(IL-4,IFN-γ)were also measured. The results demonstrated that the pET30a-MT-3-HC recombinant plasmid was cor-rectly constructed, and the M3C recombinant protein was highly expressed in the supernatant fol-lowing ultrasonic disruption of the induced bacterial culture, with a single band observed post-puri-fication.ELISA results showed that the titer of specific IgG antibodies in the M3C+CpG group peaked at 3.54×105 14 days after the third immunization, which was 141 times,70 times, and 6.6 times higher than that in HC,TT,and TT groups, respectively.Antibody subtype analysis revealed significantly higher specific antibody subtype titers in the M3C and M3C+CpG groups compared to the PBS,HC,and TT groups(P<0.05),with the M3 C group showing an IgG1/IgG2a ratio greater than 1,and the M3C+CpG group having an IgG1/IgG2a ratio of approximately 1.Serum concentrations of IFN-γ and IL-4 in the M3C and M3C+CpG groups were also significantly higher than those in the other experimental groups(P<0.05).These results showed that the recombinant antigen containing MT-3 built-in adjuvant and tetanus toxin C fragment was successfully expressed and showed strong immunogenicity, which laid the experimental foundation for the development of this recombinant vaccine.

To develop a novel immunocastration vaccine for animals, researchers designed and syn-thesized the recombinant plasmid pET-30a-SF which could express the recombinant protein SF.This protein was then conjugated in vitro with the synthetic peptide STGP to prepare the SF-STGP nanoparticle vaccine, and its immunocastration effect on mice was studied. The Spy Catcher and ferritin amino acid sequences were connected via GGGGS,and after codon optimization for E.coli, the recombinant plasmid pET-30a-SF was constructed and transformed into E.coli for in-duced expression. The recombinant protein SF was purified using Ni-column affinity chromatogra-phy and characterized. The peptide STGP,composed of Spy Tag,GnRH,and PADRE connected by GGGS,was conjugated with the recombinant protein SF in vitro. The self-assembled nanoparticles were observed using transmission electron microscopy(TEM)and dynamic light scattering(DLS).The prepared SF-STGP nanoparticles were mixed with MONTANIDE ISA 206 VG at a 1∶1 ratio to form the vaccine, which was then subcutaneously injected into male BALB/c mice for immunocastration evaluation. The recombinant protein SF showed the highest soluble expression when induced at 18 ℃ with 0.25 mmol/L IPTG for 14 h,and the maximum conjugation efficiency with STGP was achieved at a 1∶8 molar ratio.TEM and DLS analyses revealed that both the re-combinant protein SF and SF-STGP could self-assemble into nanoparticles with average diameters of 16.2 nm and 17.8 nm,respectively.Mouse immunization results demonstrated that the SF-STGP nanoparticle vaccine generated specific GnRH antibodies after the first immunization, with the spe-cific antibody D45o reaching its peak at the 10 th week. The SF-STGP+ISA 206 immunization group showed a peak D450 value of 2.8,and the specific antibody levels in all immunization groups were significantly higher than those in the control group(P<0.05).Additionally,the SF-STGP nanoparticle vaccine effectively reduced serum testosterone levels in mice, with the testosterone concentration in the immunization groups being significantly lower than that in the control group(P<0.05).Compared to the control group, the immunization group exhibits testicular atrophy. The constructed SF-STGP nanoparticle vaccine proves to be a highly effective immunogen, capable of inducing testicular atrophy and reducing gonadal hormone concentrations, demonstrating excellent castration effects. This study provides new insights into immunocastration vaccines for mammals.

Chinese Journal of Pancreatology 2025;25(3):190-197
Objective:To explore the relationship between visceral adipose tissue (VAT) and the severity and prognosis of obese patients with acute pancreatitis (AP).Methods:The clinical data of 224 obese patients with AP admitted to The Third Affiliated Hospital of Chongqing Medical University and Chongqing Dongnan Hospital from January 2020 to December 2023 were collected, retrospectively. According to the severity of AP, they were divided into mild AP group (MAP group), moderately severe AP group (MSAP group), and severe AP group (SAP group). According to the in-hospital treatment outcomes of the patients, they were divided into the survival group and death group (death within 7-30 days after admission). The basic information of AP patients, the severity score of AP and laboratory indicators were collected. The visceral adiposity index (VAI) was calculated through the values of waist circumference, body mass index, triglycerides and high-density lipoprotein cholesterol; and the VAT area was measured by CT scan. The Spearman rank correlation analysis method was used to evaluate the correlation between the VAT area and the AP severity index as well as other indicators. Multivariate logistic regression analysis was used to analyze the influencing factors of in-hospital mortality in obese patients with AP. The receiver operating characteristic curve (ROC) was drawn, and the area under curve (AUC), cut-off value, sensitivity and specificity were calculated to evaluate the predictive value of VAT area for SAP, as well as the predictive value of VAT area for in-hospital mortality in obese AP patients. Results:The VAI of MAP group, MSAP group and SAP group were 1.63(1.21, 2.74), 2.30(1.28, 3.36) and 3.01(2.17, 3.86), respectively, and the VAT area were 138.90(121.30, 155.10)cm 2, 149.90(135.10, 166.70)cm 2 and 192.30(168.80, 223.80)cm 2, respectively. There were significant differences in VAI and VAT area among the three groups (all P values <0.05). Compared with MAP group, the area of VAI and VAT area in MSAP group and SAP group were significantly increased ( P<0.05), and the area of VAT in SAP group was further increased than that in MSAP group, the differences were statistically significant (all P values <0.05). Spearman analysis showed that the VAT area was significantly positively correlated with acute physiology and chronic health evaluationⅡ scoring system (APACHEⅡ) score, modified CT severity index (MCTSI), and Balthazar CT grading score (all P values <0.05). It was also significantly positively correlated with body mass index, serum glycated hemoglobin, triglycerides, low-density lipoprotein cholesterol and VAI (all P values <0.05). ROC analysis showed that the AUC value of VAI in identifying SAP was 0.641 (95% CI 0.562-0.720), the corresponding cut-off value was 2.75, the sensitivity was 57.6%, and the specificity was 69.0%; The AUC value of VAT area in identifying SAP was 0.869 (95% CI 0.813-0.924), the corresponding cut-off value was 165.85 cm 2, the sensitivity was 78.8%, and the specificity was 84.8%, and the predictive value of VAT area was significantly better than that of VAI ( P<0.001). The APACHEⅡ, MCTSI, Balthazar CT grading score, VAI and VAT area in the death group were significantly higher than those in the survival group, and the differences were statistically significant (all P values <0.05). Multivariate logistic regression analysis showed that VAT area was an independent predictor of in-hospital mortality in obese AP patients ( P<0.05), with an AUC value of 0.831 (95% CI 0.756-0.907), a cutoff value of 173.70 cm 2, a sensitivity of 77.8%, and a specificity of 85.5%. Conclusions:The VAT area is significantly associated with the aggravation of AP and the increased risk of in-hospital mortality in obese patients, and is expected to serve as a reliable indicator for predicting the severity and prognosis of obese patients with AP.

Objective:To screen risk factors for ICU-acquired weakness in patients with mechanical ventilation and construct a predictive model, so as to provide a basis for the health management of patients with mechanical ventilation. Methods:Convenience sampling was used to select 312 ICU patients with mechanical ventilation admitted to the Tenth People's Hospital of Tongji University from October 2019 to August 2020 for the study. Patients were divided into training set ( n=220) and test set ( n=92) in a 7∶3 ratio. Based on machine learning algorithms, decision random forest (DRF), extremely-randomized trees (XRT) and generalized linear model (GLM) were used to construct three ICU-acquired weakness risk prediction models for patients with mechanical ventilation, respectively. The performance of the prediction model was evaluated using the area under the receiver operating characteristic curve ( AUC), the area under the precision-recall curve ( AUPRC), and the root mean square error ( RMSE) . Results:There were 7 predictors of risk of ICU-acquired weakness in patients with mechanical ventilation, including age, gender, braking, duration of mechanical ventilation, blood glucose, lactic acid, and parenteral nutrition. Test set and training set validation showed that AUC and AUPRC of GLM prediction model were greater than those of DRF, XRT prediction model. Test set validation indicated that the RMSE, logarithmic loss of GLM prediction model was less than those of DRF, XRT prediction model. Conclusions:Machine learning algorithm based GLM prediction model has good prediction performance. Healthcare professionals can construct evidence-based decisions for interventions in areas such as braking, duration of mechanical ventilation, and blood glucose management.

Chinese Journal of Epidemiology 2025;46(9):1562-1569
Objective:To analyze the current status and trend of the mortality burden of cardiovascular disease in China from 2010 to 2021.Methods:Data related to cardiovascular disease mortality and disability-adjusted life year (DALY) were extracted from the Global Burden of Disease Study 2021 (GBD2021) database. The age-standardized mortality rate and DALY rate were calculated, using the 2021 world standard population estimated by GBD2021. Joinpoint 5.2.0 software was used to calculate the mortality rate, standardized mortality rate, DALY rate, standardized DALY rate, annual percent change (APC), average annual percent change (AAPC), and 95% CI of cardiovascular disease in China from 2010 to 2021. Results:The mortality rate of cardiovascular disease was 357.44/100 000, and the age-standardized mortality rate was 280.11/100 000 in China in 2021. The DALY rate was 7 043.33/100 000, and the age-standardized DALY rate was 5 120.06/100 000. From 2010 to 2021, the mortality rate and DALY rate of cardiovascular disease in China showed an upward trend (AAPC was 1.58% and 0.83%, respectively, both P<0.05), and the standardized mortality rate and standardized DALY rate showed a downward trend (AAPC was -2.13% and -2.02%, respectively, both P<0.05). The mortality burden of cardiovascular disease was higher in males (mortality rate 392.80/100 000, DALY rate 8 156.19/100 000) than in females (mortality rate 320.38/100 000, DALY rate 5 876.87/100 000). With the increase older in age, the mortality burden of cardiovascular disease in China decreased first and then increased. China ranked high in the mortality burden of cardiovascular disease among G20 member countries. Conclusions:The death burden of cardiovascular disease is serious in China. The mortality rate and DALY rate of cardiovascular disease in China showed an increasing trend from 2010 to 2021, and the standardized mortality rate and standardized DALY rate of cardiovascular disease ranked high in G20 member countries. The death burden of cardiovascular disease was more serious in men and the elderly. It is necessary to develop more comprehensive prevention, treatment, and rehabilitation measures for men and the elderly to reduce mortality and disability rates, decrease the disease burden, and improve the quality of life.
