Objective:To analyze the molecular characteristics of prevalent Bordetella pertussis (Bp) isolates in Liaocheng City, Shandong Province in 2024. Methods:From March to August 2024, oropharyngeal swabs were collected from suspected pertussis cases at Liaocheng People′s Hospital in Shandong Province for Bp isolation. A total of 99 Bp isolates were obtained. Whole-genome sequencing was performed on all isolates, followed by Multilocus Sequence Typing (MLST), vaccine antigen-related gene typing (including ptxP, ptxA, ptxB, ptxC, ptxD, ptxE, fhaB, fim2, fim3, and prn), 23S rRNA gene typing, and phylogenetic analysis. To capture the differences between Bp isolates and vaccine strains in Liaocheng City, the international vaccine strain Tohama I and the Chinese vaccine strain CS were included in the analysis. Antimicrobial resistance testing against 11 agents was performed on 52 isolates. Results:The throat swabs of 99 Bp isolates were collected from patients aged 44 days to 42 years, and the median age of the patients was 7 (5, 8) years. All isolates ptxP were ptxP3 type. 74 isolates (74.75%) carried the prn150, while 21 isolates (21.21%) were prn-deficient. The predominant antigenic profile was ptxP3/ ptxA1/ ptxB1/ ptxC4/ ptxD1/ ptxE4/ fhaB1/ fim2-1/ fim3-1/ prn150, found in 72 isolates (72.73%). All 99 isolates carried the A2047G mutation in the 23S rRNA gene. Antimicrobial susceptibility testing showed that the MICs of macrolides and clindamycin for all 52 Bp isolates were all >256 mg/L. However, the isolates showed low MIC for seven other antimicrobials tested, including trimethoprim-sulfamethoxazole, amoxicillin, and levofloxacin. MLST typing revealed that 94 isolates (94.95%) were identified as ST-2, while 5 isolates (5.05%) belonged to a novel sequence type (ST-118). Phylogenetic analysis demonstrated that all 99 Bp isolates were highly homologous but clustered in evolutionary branches distinct from vaccine strains. Conclusion:In 2024, Bordetella pertussis isolates in Liaocheng City exhibit distinct clonal epidemic characteristics, with the predominant antigenic genotype being ptxP3/ ptxA1/ ptxB1/ ptxC4/ ptxD1/ ptxE4/ fhaB1/ fim2-1/ fim3-1/ prn150. All isolates are resistant to macrolide antibiotics.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.

The research on medical image segmentation is of great significance in advancing efficient and accurate automated image processing techniques. To address the problem of inaccurate segmentation results caused by significant variations in organ tissue shapes and blurred boundaries present in medical images,a novel network named MFMANet is proposed.Built upon a"U"-shaped architecture, the network integrates multi-scale information fusion modules and multi-attention modules.Specifically,multi-scale information modules capture multi-scale information in the shallow layers of the network to bridge the semantic gap between encoder and decoder features, thereby enhancing the network's ability to handle large variations in organ sizes.Regarding the issue of blurred boundaries,multi-attention mechanism utilizes Swin Transformer as the deep encoder-decoder network, employing channel and spatial attention instead of traditional skip connections to achieve finer feature extraction.Experimental results on the ACDC and Synapse public datasets show that the proposed method achieves improvements of 1.51%and 1.29%in Dice similarity coefficient as compared with MTUNet,fully demonstrating its effectiveness in enhancing segmentation network accuracy.

Chinese Journal of Medical Physics 2024;41(1):32-42
A U-Net incorporating improved Transformer and convolutional channel attention module is designed for biventricular segmentation in MRI image.By replacing the high-level convolution of U-Net with the improved Transformer, the global feature information can be effectively extracted to cope with the challenge of poor segmentation performance due to the complex morphological variation of the right ventricle. The improved Transformer incorporates a fixed window attention for position localization in the self-attention module, and aggregates the output feature map for reducing the feature map size;and the network learning capability is improved by increasing network depth through the adjustment of multilayer perceptron. To solve the problem of unsatisfactory segmentation performance caused by blurred tissue edges,a feature aggregation module is used for the fusion of multi-level underlying features, and a convolutional channel attention module is adopted to rescale the underlying features to achieve adaptive learning of feature weights. In addition,a plug-and-play feature enhancement module is integrated to improve the segmentation performance which is affected by feature loss due to channel decay in the codec structure, which guarantees the spatial information while increasing the proportion of useful channel information. The test on the ACDC dataset shows that the proposed method has higher biventricular segmentation accuracy, especially for the right ventricle segmentation.Compared with other methods, the proposed method improves the DSC coefficient by at least 2.83%,proving its effectiveness in biventricular segmentation.

Chinese Journal of Trauma 2024;40(9):769-780
Hip fracture in the elderly is characterized by high incidence, high disability rate, and high mortality and has been recognized as a public health issue threatening their health. Surgery is the preferred choice for the treatment of elderly patients with hip fracture. However, lower extremity deep venous thrombosis (DVT) has an extremely high incidence rate during the perioperative period, and may significantly increase the risk of patients′ death once it progresses to pulmonary embolism. In response to this issue, the clinical guidelines and expert consensuses all emphasize active application of comprehensive preventive measures, including basic prevention, physical prevention, and pharmacological prevention. In this prevention system, basic prevention is the basis of physical and pharmacological prevention. However, there is a lack of unified and definite recommendations for basic preventive measures in clinical practice. To this end, the Orthopedic Nursing Professional Committee of the Chinese Nursing Association and Nursing Department of the Orthopedic Branch of the China International Exchange and Promotive Association for Medical and Health Care organized relevant nursing experts to formulate Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture ( version 2024) . A total of 10 recommendations were proposed, aiming to standardize the basic preventive measures for lower extremity DVT in elderly patients with hip fractures during the perioperative period and promote their subsequent rehabilitation.

Immunological Journal 2024;40(6):514-519
To investigate the effect of baicalin(BA)on acne-induced dermatitis injury of acne rats through regulating stromal cell-derived factor-1/CXC chemokine receptor 4(SDF-1/CXCR)signaling pathway and its mechanism. The rat model of acne was established, and the rats were divided into control group, model group,BA groups(BA-L,BA-M,BA-H groups, intragastric administration of 25,50,100 mg·kg-1·d-1 BA)and combined groups(intragastric administration of 100 mg·kg-1·d-1 BA+intraperitoneal injection of 7 μg·kg-1·d-1 SDF-1α recombinant protein). The apparent changes of auricle skin tissue were observed and the thickness of auricle was measured.HE and Masson staining were used to observe the pathological changes and fibrosis of auricle skin;ELISA was used to detect levels of IL-1β,IL-6,IL-8 and TNF-α in serum;immunofluorescence was used to detect the expression of NLRP3 in auricle skin;Western blot was used to detect the level of SDF-1 and CXCR4 proteins in auricle skin.Compared with the control group, the auricle of the model group was red and swollen, papules and telangiectasia were obvious, the stratum corneum of the ear was thickened, inflammatory cells infiltrated and the size of sebaceous gland was increased, and the hair follicle sebaceous gland was excessively keratinized.Furthermore, rats in model group demonstrated increases in thickness of auricle skin tissue, fibrotic area, hydroxyproline content, levels of IL-1β,IL-6,IL-8,and TNF-α,positive expression rate of NLRP3,and expression of SDF-1 and CXCR4 proteins(P<0.05).BA could reverse these changes mentioned above in model rat(P<0.05),while SDF-1α recombinant protein could suppress the effect of BA-H on the above indexes(P<0.05).In conclusion,BA may alleviate dermatitis injury in acne rats by inhibiting SDF-1/CXCR4 signaling pathway.
