1.Endovascular Treatment for Vertebral Artery Occlusion with Anterior Spinal Artery Involvement: Contrasting Outcomes in Two Cases

Vertebral artery (VA) occlusion is often managed conservatively, but anterior spinal artery (ASA) involvement may cause rapid neurological deterioration. We report two contrasting cases of VA occlusion with ASA compromise. One patient achieved complete recovery after timely endovascular treatment (EVT) restoring ASA flow, whereas the other developed severe bilateral medial medullary infarctions following unsuccessful EVT. These cases highlight the prognostic importance of ASA involvement and suggest that EVT should be considered when ASA perfusion is threatened.
2.The Impact of Nurses' Knowledge of Cancer Pain Management, Empathy, and Communication Skills on Cancer Pain Management Competence

Purpose:
This study aimed to investigate nurses’ knowledge regarding cancer pain management, empathy, and communication skills, as well as to examine factors influencing their competence regarding cancer pain management.
Methods:
A total of 166 nurses participated in this study, and all participants had more than three months’ clinical experience working in wards and intensive care units where cancer patients were admitted. Data were collected using structured questionnaires and analyzed by an independent t-test, a one-way ANOVA, a Mann-Whitney U test, a Kruskal-Wallis test, a Pearson correlation coefficient, and hierarchical regression analysis using SPSS/WIN 27.0.
Results:
Significant differences were found in competence for cancer pain management among the participants based on gender (z=-5.36, p<.001) and education level (F=3.87, p=.023). The results of the regression analysis revealed that the factors affecting nurses’ competence in cancer pain management included knowledge of cancer pain management (β=.29, p<.001) and empathy (β=.20, p=.031). Communication skills (β=-.03, p=.773) had no significant effect on competence in cancer patient management. The total explanatory power was 31%.
Conclusion
Nurses’ competence in cancer pain management was significantly influenced by knowledge and empathy.Programs focusing on education regarding cancer pain and empathy enhancement are needed to improve nursing competence for cancer pain management.
3.Field investigation of bovine leukemia virus: env-based PCR detection and hematologic predictors in Korean dairy herds with productivity issues

Objective:
To investigate prevalence of BLV and its association with hematological alterations in targeted dairy farms experiencing productivity issues in Korea.
Methods:
This study investigated five dairy farms reporting decreased milk yield, recurrent mastitis, poor conception rates, and delayed postpartum recovery. Eighty lactating cows underwent physical examination, body condition scoring, hematological and biochemical analyses, and polymerase chain reaction testing for BLV.
Results:
Overall BLV detection rate among these herds was 70% (56/80), with intra-herd positivity ranging from 57.1 to 90.0%. Lymphocytosis was observed in 31% of the cows.Multivariable generalized estimating equations analysis confirmed that higher lymphocyte counts were the strongest independent predictor of BLV status (adjusted odds ratio, 2.70;p = 0.005), demonstrating a robust association between hematological alterations and BLV positivity.
Conclusions
and Relevance: BLV infection is highly prevalent in the investigated Korean dairy farms with productivity problems. While these results may not represent the nationwide average due to selection bias, they emphasize the need for nationwide BLV surveillance and implementation of control measures in Korea.

Microglial activation during aging is associated with neuroinflammation and cognitive impairment. Galectin-3 plays a crucial role in microglial activation and phagocytosis. However, the role of galectin-3 in the aged brain is not completely understood. In the present study, we investigated aging-related mechanisms and microglial galectin-3 expression in the mouse hippocampus using female 6-, 12-, and 24-month-old C57BL/6 mice. Western blot analysis revealed neurodegeneration, blood-brain barrier leakage, and increased levels of neuroinflammation-related proteins in 24-month-old mice compared to 6- and 12-month-old mice. Immunohistochemistry revealed an increase in activated microglia in the hippocampus of 24-month-old mice compared to 6- and 12-month-old mice. Furthermore, we found more galectin-3 and triggering receptor expressed on myeloid cells-2-positive microglia in 24-month-old mice compared to 6- and 12-month-old mice. Using primary mouse microglial cells, galectin -3 was also increased by lipopolysaccharide treatment. These findings suggest that galectin-3 may play an important role in microglial activation and neuroinflammation during brain aging.

9.Klebsiella pneumoniae-Induced Emphysematous Prostatic Abscess Accompanied by a Spinal Cord Infarction: Case Report

Various strains can be found in emphysematous prostatic abscesses (EPAs), but the most frequent causative organism is Klebsiella pneumoniae . Hypervirulent K. pneumoniae can disseminate to distant sites by forming a muco-polysaccharide network outside the capsule. Here, we present the first case of K. pneumoniae in an EPA accompanied by a spinal cord infarction. A 65-year-old man was referred to our hospital due to sudden-onset paraplegia after a 5-day history of fever, myalgia, and voiding difficulty. Abdominal computed tomography revealed a collection of air pockets in the prostate, and diffusion-weighted magnetic resonance imaging showed high signal intensity in the thoracic spinal cord. The patient was initially treated with antibiotics and surgical drainage. On the third hospital day, therapeutic heparin was added after discussion with a neurologist. The patient had no inflammatory symptoms, experienced some improvement in paraplegia, and was discharged on the 14th hospital day. This study adhered to the case report guidelines.
10.Comparison of GastroPanel® and GENEDIA® in Diagnosing Helicobacter pylori Infection and Gastric Lesions

Serological tests for Helicobacter pylori needs local validation as the diagnostic accuracy may vary depending on the prevalence of H.pylori. This study examined the diagnostic performance of two ELISA, GastroPanel® (GastroPanel ELISA; Biohit Oyj) and GENE-DIA® (GENEDIA® H. pylori ELISA, Green Cross Co.) in Korean population. One thousand seventy seven patients who visited for esophagogastroduodenoscopy between 2013 and 2023 were prospectively enrolled, and serum samples from the subjects were tested using both GastroPanel® and GENEDIA® . The two tests were compared for their diagnostic accuracy in detecting atrophic gastritis (AG), intestinal metaplasia (IM), gastric adenoma (GA), and gastric cancer (GC), and the positivity rates by age and sexwere observed. There was substantial correlation (Pearson coefficient [r] = 0.512, P < 0.001) and agreement (Cohen’s Kappa coefficient [κ] = 0.723, P < 0.001) between the results obtained using GastroPanel® and GENEDIA® . The test results from the two kits did not match perfectly with a discrepancy observed in approximately 16% of cases, that 67 subjects were positive only on GENE-DIA® while 75 subjects were positive only on GastroPanel® . The area under receiver operating characteristic curve for AG, IM, GA,and GC using GastroPanel® were 0.666, 0.635, 0.540, and 0.575, while the results tested using GENEDIA® were 0.649, 0.604, 0.553, and 0.555, respectively, without significant difference between the two results. GastroPanel® and GENEDIA® showed similar performance in terms of diagnostic accuracy; but the test results did not match perfectly. A large-scale validation study in Koreansis needed.
