1.Mechanism of Huayu jiedu formula in alleviating inflammatory injury in chronic kidney disease based on AIM2 pyroptosis pathway

OBJECTIVE To explore the mechanism of Huayu jiedu formula in regulating inflammatory injury in chronic kidney disease (CKD). METHODS Fifty male SD rats were randomly divided into a sham surgery group (10 rats) and a modeling group (40 rats). The CKD model was replicated in the modeling group by unilateral ureteral obstruction surgery. After successful modeling, the rats were randomly divided into model group, esaxerenone group (positive control), and TCM low- and high-dose groups, with 10 rats in each group. The Esaxerenone group was given 1 mg/kg of esaxerenone, while the TCM low- and high-dose groups were given 13.7 and 27.4 g/kg of Huayu jiedu formula respectively, the sham surgery group and model group were given an equal volume of physiological saline, all groups were intervened continuously for 14 days. Hematoxylin eosin and Masson staining were used to observe the pathological changes in rat kidney tissue. Conventional biochemical methods were used to detect serum urea (SUr), serum creatinine (SCr), malondialdehyde (MDA), and superoxide dismutase (SOD) levels; enzyme-linked immunosorbent assay was used to detect the levels of serum interleukin-1β (IL-1β), IL-6, and tumor necrosis factor-α(TNF-α); immunohistochemistry and Western blot were used to detect the protein expression of peroxisome proliferator-activated receptor γ coactivator-1α(PGC-1α) , mitochondrial transcription factor A (TFAM), absent in melanoma 2(AIM2), caspase-1, gasdermin D (GSDMD), IL-1β and IL-18 in renal tissue; real-time fluorescence quantitative PCR was used to detect the mRNA expression of AIM2. RESULTS Compared with the sham surgery group, the renal tissue of the model group showed pathological changes such as glomerular deformation and destruction, severe tubular dilation, and increased deposition of blue fibrin; the levels of SUr, SCr, MDA, IL-1β, IL-6, TNF-α,the protein expression of AIM2, GSDMD, caspase-1, IL-1β, IL-18 , and the mRNA expression of AIM2 were significantly increased or up-regulated (P<0.01); the levels of SOD, the protein expression of PGC-1α, TFAM were significantly reduced or down-regulated (P<0.01). Compared with the model group, all treatment groups showed improvement in the above symptoms and most indicators in rats. CONCLUSIONS Huayu jiedu formula may improve renal function, alleviate renal inflammatory damage and pyroptosis, and exert renal protective effects by regulating the AIM2 pyroptosis pathway.
2.Relationship of lipoprotein(a)with inflammatory biomarkers and cardiac damage in patients with essential hypertension

Objective To explore the effect of lipoprotein a[Lp(a)]on the inflammatory state and cardiovascular system of patients with essential hypertension.Methods The clinical data of 234 patients with essential hypertension admitted from January 2022 to November 2023 were retrospectively collected.According to the serum Lp(a)concentration,they were divided into the normal Lp(a)group(n=185)and the high Lp(a)group(n=49).The differences in serum inflammatory markers C reactive protein(CRP),neutrophil-to-lymphocyte ratio(NLR),and fibrinogen between the two groups were compared.Pearson correlation analysis was used to analyze the correlation between Lp(a)and inflammatory markers,and the incidence of cardiac target organ damage between the two groups of patients was compared.Results Compared with the normal Lp(a)group,the CRP concentration,NLR and fibrinogen concentration of hypertensive patients in the high Lp(a)group increased(P<0.05,P<0.01),and the concentration of Lp(a)was positively correlated with CRP,NLR and fibrinogen(r=0.168,0.165,0.321,P<0.05).The incidence of myocardial infarction was higher in the high Lp(a)group[22.45%(11/49)]than in the normal Lp(a)group[7.57%(14/185)](P<0.01).Conclusion In patients with essential hypertension,high Lp(a)concentration is associated with a stronger inflammatory response,and elevated Lp(a)concentration is related to an increased incidence of myocardial infarction,suggesting that Lp(a)may affect target organ damage in patients with essential hypertension by promoting inflammation.
3.Analysis on Characteristics of Peripheral Blood Recovery in Treatment of Aplastic Anemia with Bushen Shengxue Method and Yiqi Yangxue Method Combined with Western Medicine

Objective To investigate the characteristics of peripheral blood in the treatment of aplastic anemia(AA)with Bushen Shengxue Method and Yiqi Yangxue Method combined with Western medicine.Methods Totally 492 AA patients who were treated in 19 centers including Xiyuan Hospital China Academy of Chinese Medical Sciences from September 2018 to March 2021 were selected,and were randomized into three groups:161 cases in the Bushen Shengxue group,164 cases in the Yiqi Yangxue group,and 167 cases in the control group.All three groups were orally administered cyclosporine and androgens.Bushen Shengxue group was given Bushen Shengxue Granules,Yiqi Yangxue group was given Yiqi Yangxue Granules,and the control group was given placebo(half dosage of Bushen Shengxue granules).The general data,overall efficacy,and peripheral blood at the 1st month,the 4th month,and 6th month after treatment were analyzed.Results The total effective rate of Bushen Shengxue group was 98.8%(159/161),which was significantly higher than that of Yiqi Yangxue group(79.9%)and the control group(61.7%),with statistical significance(P<0.001);The total effective rate of Yiqi Yangxue group was significantly higher than that of the control group(P<0.001).Compared with before treatment,the hemoglobin(HGB)levels of all three groups of patients significantly increased after treatment(P<0.001,P<0.01),and the platelet levels of Bushen Shengxue group and the control group significantly increased after treatment(P<0.001);after treatment,the HGB levels in the three groups were ranked from high to low as the Bushen Shengxue group,Yiqi Yangxue group and the control group(P<0.01).25%of patients had HGB levels exceeding 20%of baseline values,Bushen Shengxue group took 1 month,while Yiqi Yangxue group and control group took 4 months;25%of patients had an increase in HGB levels exceeding 50%of the baseline value,Bushen Shengxue group took 6 months,while Yiqi Yangxue group and control group took more than 6 months.At 6 months,the number of patients with HGB elevation exceeding 20%and 50%of baseline values in Bushen Shengxue group was higher than that in Yiqi Yangxue group and the control group(P<0.05,P<0.001).25%of patients had white blood cell elevation exceeding 50%of the baseline value,Bushen Shengxue group took 4 months,Yiqi Yangxue group took 6 months,and the control group took more than 6 months.25%of patients showed an increase in platelet levels exceeding 100%of the baseline value,Bushen Shengxue group took 4 months,while Yiqi Yangxue group and control group took 6 months;at 6 months,the number of patients in Bushen Shengxue group and Yiqi Yangxue group with platelet elevation exceeding 20%of the baseline value was significantly higher than that in the control group(P<0.01,P<0.05).Conclusion Bushen Shengxue method combined with Western medicine intreating AA is better than the method of Yiqi Yangxue method combined with Western medicine.In terms of improving HGB,it is reflected in shortening the recovery time and increasing the number of beneficiaries,and the dosage can affect the recovery time and the number of beneficiaries at the same time;in terms of improving white blood cell and platelet,it is reflected in shortening the recovery time,and the dosage can affect the recovery time.
4.Application and progress of nanomaterials in the treatment of radiation injury

The applications of nuclear technology in industries, medicine, and other fields have increased the risk of radiation injury. Although some small-molecule drugs for radiation injury treatment have been applied clinically or are in preclinical research, their therapeutic efficacy is significantly limited by short circulation time and rapid metabolism. Nanomaterials have attracted growing attention with their outstanding bioactivity, chemical stability, tissue compatibility, and targeted delivery capabilities, therefore having the promise of offering the potential solutions to the limitations of current small-molecule drugs. However, their biosafety and clinical efficacy require further investigation. This review summarizes the design strategies and classifications of nanomaterials for radiation injury treatment, analyzes current research progress in their therapeutic applications, and introduces nanomaterial-based approaches for enhancing the elimination of internal radionuclide contamination. Finally, the challenges and future prospects of nanomaterials in radiation injury treatment are discussed. This review aims to provide researchers with a comprehensive understanding of recent advances in nanomaterial-based radiation injury therapeutics, thereby promoting further development in this field.

7.Establishment and application of a precise management model for the centralized preparation of cytotoxic drugs in PIVAS

OBJECTIVE To establish the precise management model for the centralized preparation of cytotoxic drugs in pharmacy intravenous admixture services(PIVAS),and evaluate the effects of its application.METHODS Pharmacists in PIVAS established the precise management model by soliciting clinical opinions and consulting literature on the centralized preparation of cytotoxic drugs and continuously refining every step of the preparation of cytotoxic drugs,based on data feedback from the information closed-loop management system and the limit of stability time of finished solutions.The indicators such as the preparation time,delivery time,the storage time of finished infusion solutions after preparation,and the completion rate of infusion within the stability time limit were analyzed before the implementation(January to December 2023)and after the implementation(January to December 2024)of this model,to evaluate its application effectiveness.RESULTS The overall framework for the precise management model included upgrading the functions of the prescription review system,improving the prescription review database,providing specialized training for PIVAS pharmacists,managing dynamic batch decision for drug preparation,managing special drugs,managing finished infusion distribution,and establishing a continuous improvement mechanism.Compared with before implementation,the average preparation time of the second and third batches of cytotoxic drugs with more concentrated morning preparation tasks in this model was significantly shorter than before implementation(P<0.05);the delivery time of finished infusion after implementation[(11.49±2.92)min]was significantly shorter than the delivery time before implementation[(22.11±5.03)min](P<0.001);the storage time of some drugs with shorter stable time limit and carboplatin in combination regimens(paclitaxel or docetaxel+carboplatin)was significantly shortened compared to before implementation(P<0.05),and the completion rate of infusion within the stability time limit was significantly improved compared to before implementation(P<0.05).CONCLUSIONS Our hospital has successfully established a precise management model for the centralized preparation of cytotoxic drugs in PIVAS.This mode can significantly shorten the preparation time of each batch of PIVAS in the morning,make batch decisions more reasonable and improve the infusion completion rate within the stable time limit of the finished product.

9.Basic concepts, recent advances, and future perspectives in the diagnosis of bovine mastitis

Mastitis is one of the most widespread infectious diseases that adversely affects the profitability of the dairy industry worldwide. Accurate diagnosis and identification of pathogens early to cull infected animals and minimize the spread of infection in herds is critical for improving treatment effects and dairy farm welfare. The major pathogens causing mastitis and pathogenesis are assessed first. The most recent and advanced strategies for detecting mastitis, including genomics and proteomics approaches, are then evaluated .Finally, the advantages and disadvantages of each technique, potential research directions, and future perspectives are reported. This review provides a theoretical basis to help veterinarians select the most sensitive, specific, and cost-effective approach for detecting bovine mastitis early.
10.Antibiofilm activity of polyethylene glycol-quercetin nanoparticlesloaded gelatin-N,O-carboxymethyl chitosan composite nanogels against Staphylococcus epidermidis

Background:
Biofilms, such as those from Staphylococcus epidermidis, are generally insensitive to traditional antimicrobial agents, making it difficult to inhibit their formation. Although quercetin has excellent antibiofilm effects, its clinical applications are limited by the lack of sustained and targeted release at the site of S. epidermidis infection.
Objectives:
Polyethylene glycol-quercetin nanoparticles (PQ-NPs)-loaded gelatin-N,Ocarboxymethyl chitosan (N,O-CMCS) composite nanogels were prepared and assessed for the on-demand release potential for reducing S. epidermidis biofilm formation.
Methods:
The formation mechanism, physicochemical characterization, and antibiofilm activity of PQ-nanogels against S. epidermidis were studied.
Results:
Physicochemical characterization confirmed that PQ-nanogels had been prepared by the electrostatic interactions between gelatin and N,O-CMCS with sodium tripolyphosphate. The PQ-nanogels exhibited obvious pH and gelatinase-responsive to achieve on-demand release in the micro-environment (pH 5.5 and gelatinase) of S. epidermidis.In addition, PQ-nanogels had excellent antibiofilm activity, and the potential antibiofilm mechanism may enhance its antibiofilm activity by reducing its relative biofilm formation, surface hydrophobicity, exopolysaccharides production, and eDNA production.
Conclusions
This study will guide the development of the dual responsiveness (pH and gelatinase) of nanogels to achieve on-demand release for reducing S. epidermidis biofilm formation.
