1.Clinical efficacy of hyperbaric oxygen in the treatment of sudden deafness

Objective To investigate the effects of intervening time and course of hyperbaric oxygen (HBO) therapy on the clinical efficacy of sudden deafness.Methods One hundred and sixty-eight patients (188 ears) with sudden deafness were all treated with hyperbaric oxygen (HBO).In accordance with the intervening time and course of HBO therapy,the therapeutic effects were compared between the groups.Results There was statistical significance in the therapeutic efficacy,when comparisons were made between the first-week HBO intervention group (62.26%)and the second-week intervention group(43.18%)and the third-week intervention group (44.74%) (P < 0.05).However,no statistical significance in therapeutic efficacy couldbe noted,when comparisons were made between the second-week intervention group and the third-week intervention group (P > 0.05).Statistical significance could neither be found between one course of HBO therapy and two courses of HBO therapy (P > 0.05).Conclusion HBO intervention in the first week could obviously produce better therapeutic efficacy.However,there were no significant differences in therapeutic efficacy between one course and 2 courses of HBO therapy.

3.Regional blood perfusion, metabolism and angiogenesis of breast cancer

Objective To evaluate the correlation between regional blood perfusion,metabolism and angiogenesis in breast cancer.Methods The PET/CT functional imaging technique was applied to quantitatively detect the central and marginal blood perfusion parameters including blood flow(BF),blood volume(BV)and permeability of surface(Ps),and metabolism index of standard uptake value(SUV)of the tumor in 33 breast cancer patients.The expression of CD31.CD105 and VEGF in paraffin section of breast cancer were detected by immunohistochemical method,then MVD(CD31)and MVD(CD105)were obtained.The relationship between the regional blood perfusion and metabolism and MVD(CD3 1),MVD(CD105)and the expression of VEGF were analyzed. Results There was significant correlation of MVD (CD31)with BF of marginal region(P<0.05).There were significant correlations of MVD(CD105)with BF.PS and SUV(P<0.05). Conclusions Regional blood perfusion,metabolism is correlated with angiogenesis in breast cancer tissue.PETT/CT regional blood perfusion and metabolic imaging iS a noninvasive method which can be used to estimate angiogenesis status clinically in breast cancer.
4.WWOX gene expression and aberrant CpG island hypermethylation of WWOX gene in breast cancer

Objective To evaluate the expression of WWOX gene in breast cancer and its relation to hypermethylation of WWOX gene CpG island. Methods The expression of WWOX protein was evaluated by immunohistochemical staining in breast cancer cell line and breast cancer tissues.Methylation specific PCR(MSP)was used to check whether it Was methylated in the promoter and exon 1 of WWOX.Results Deletion in the WWOX expression was observed more frequently in invasive breast tumors,(32.2%)than normal breasttissues(5.4%)(P<0.01).20.3% of premenopause eases were completely negative for WWOX expression compared to 57.2% of postmenopause breast carcinomas(P<0.01).Furthermore,23.1% of Stage Ⅰ(6/26),28.6%of Stage Ⅱ(10/35),46.2%of Stage Ⅲ(12/26)tunlors showed negative WWOX protein expression.In breast cancer specimens.methylation rate of promoter and exon 1 CpG island of WWOX gene was 55%and 45%respectively.Whereas.WWOX CpG islands of normal mammary tissues were completely unmethylated in all cases.CpG islands of WWOX promoter and exon 1 were methylated in MDA-MB-231 cell line but not MCF-7 cell.Conclusions Some deletion in WWOX expression is common in breast cancer and methylation of WWOX DNA CpG islands plays a crucial role in the silence of WWOX.WWOX may play a role in the carcinogenesis and development of breast cancer.
5.Gene transfer by a novel nonviral vector polyethylenimine.

This study inquired about the influences on the gene delivery efficiency of polyethylenimine. pSVbeta plasmids were transferred into COS-7 and NIH3T3 cells with polyethylenimine. Influences of plasmids factor, albumin, serum, cell density, operation methods and polyethylenimine/DNA preserve factors on transfection efficiency were investigated. Inhibitors of biological activities in plasmids could be removed by ultrafiltration with cutoff molecular weight of 3000 or 10000. Linear plasmids lowered transfection efficiency. Serum and albumin in the culture medium decreased the transfection efficiency of polyethylenimine/DNA. Cell density was associated with PEI/DNA transfection efficiency. Incubation of PEI/DNA complexes with the cells for 8 h and then aspiration for removal of the complexes could obtain an optimal transfection effect. Freezing of PEI/DNA complexes significantly decreased transfection efficiency. In conclusion, polyethylenimine could obtain optimal and reduplicate transfection results by controlling related factors.

6.The double-ring, single-pedicle reduction plasty for gynecomastia.

OBJECTIVETo investigate a new operation of breast reduction for gynecomastia.
METHODSIn the past 5 years we treated 42 cases (80 sites) of gynecomastia with the double-ring incision, a supra-lateral derma-mammary pedicle of the nipple and areola and the tumescent technique.
RESULTSThe tumescent anesthesia was effective. The operation was easily performed with slight injury and less bleeding. It was safe and the patients recovered quickly. 100 to 500 grams tissue was resected from one breast. There have been no severe postoperative complications, such as nipple or areola necrosis. Follow-up from 3 to 48 months showed satisfactory results except that there was mild scar hypertrophy on the incision around the areola.
CONCLUSIONSThe new operation for gynecomastia with the double-ring incision, a supra-lateral derma-mammary pedicle of the nipple and areola, and the tumescent anesthesia is a good, reliable and convenient technique.

7.The Treatment with Selective Hepatic Arteriography and Embolization in Liver Hemangioma

Objective We searched for optimal methods of embolization according to the images of selective hepatic arteriography to improve the efficiency of intervention.Methods 40 patients were selected to embolic with lipiodol ultra-fluida and PYM and followed by 6~48 months.Results According its shape and sign of stainangtime,liver hemangioma was distincted from liver cancer.The hemangioma sizes of 30 cases reduced beyond 50 percent and 4 cases had no change.All diminished hemongioma signs can been detected by ultrasound.The effective rate reached 85 percent.All patients had no side-effect.Conclusion The treatment of hepatic hemangioma with selectic arteriography and embolization is a good way in diagnosis and handle.
8.Qualitative analysis of FDG uptake and blood perfusion by PET/CT in breast cancer

Objective To evaluate the use of PET/CT in the diagnosis of breast cancer. Methods In this study,33 patients with suspicious breast tumor underwent PET/CT imaging. The images of the breast were analyzed for qualitative assessment of increased tracer uptake and blood perfusion with PET/CT. Results Among 27 cases with pathology proved breast cancer,25 was judged as PET/CT positive,2 was false-negative. Sensitivity, specificity and accuracy of PET/CT in identifying breast cancer were 92.6%,100%,93.9%. Conclusion PET/CT is a reliable and sensitive measure in the diagnosis of breast cancer in vivo.
9.INDICATION OF FNA REPLACING EXCIXION BIOPSY FOR DIAGNOSING BREAST CANCER

objective At present, most operable breast cancers are confirmed by excision biopsy during the operation. If the breast cancer was definitely diagnosed by Fine -Needle Aspiration cytology(FNA) preoperatively, the excision biopsy could be avoid. Methods We diagnosed 118 inpatients of breast mass with physical examination and FNA from September, 1995 to May, 1996. Results The sensitivity and positive predict value of FNA for diagnosing breast cancer are 95.7% and 97.1%. When the palpation and FNA are both positive, the doublepositive predict value for breast cancer is 100% in our study. It is the indication of using FNA that a breast mass was diagnosed as cancer by physical examination. We can treat a breast mass with redical operation directly without excision biopsy, if the breast mass was confirmed as breast cancer by both physical examination and FNA. Conclusion We concluded that the preoperative FNA could partially replace the excision biopsy for confirming breast cancer.
10.Evaluation of PET/CT imaging versus mammography in detecting breast cancer

Conclusions In comparison with mammography,PET/CT has a higher degree of sensitivity and specificily in detecting breast cancer,and higher positive predictive value.PET/CT can provide more aspects of in vivo diagnostic information which may be useful in selecting therapeutic strategy and may supplement the inadequacies of mammography.
