1.Clinical Characteristics of Autoimmune Gastritis Patients Complicated With Colorectal Polyps

Background:The association between atrophic gastritis and colorectal polyps has been reported.Autoimmune gastritis(AIG)leads to parietal cell destruction,reduced gastric acid secretion and gastric mucosal atrophy,which may increase the risk of colorectal polyps.However,there are few studies on AIG and colorectal polyps.Aims:To analyze the clinical characteristics of AIG patients complicated with colorectal polyps.Methods:AIG patients complicated with colorectal polyps admitted to Renji Hospital,School of Medicine,Shanghai Jiao Tong University from June 2019 to July 2023 were retrospectively recruited.The medical history was reviewed,data on age at diagnosis,gender,anemia,anti-parietal cell antibody(PCA),anti-intrinsic factor antibody(IFA),pepsinogens(PGs),gastrin-17,vitamin B12,and the presence and pathological features of polyps were collected and analyzed.Results:Seventy-seven cases of AIG patients complicated with colorectal polyps,predominantly females(66.2%),were enrolled.The median age at diagnosis was 63 years old.The positivity rate for PCA was 84.4%.The most common pathological type of colorectal polyps was adenomatous polyps(46.3%),followed by hyperplastic polyps(29.9%).The most common location of colorectal polyps was sigmoid colon(29.9%),followed by transverse colon(20.9%)and ascending colon(19.4%).Thirty-three patients(42.9%)also complicated with gastric polyps,mainly the hyperplastic polyps located at corpus and fundus.Patients with gastric polyps presented higher positivity rate for PCA and higher serum PGⅡand gastrin-17 levels as compared with those without gastric polyps(all P<0.05).Conclusions:The most common pathological types of colorectal polyps in AIG patients are adenomatous and hyperplastic polyps,and the most common sites are sigmoid colon,transverse colon and ascending colon.Nearly 40%of the patients complicated with gastric polyps simultaneously,which suggests that gastric polyps and colorectal polyps might have a symbiotic relationship under the background of gastric mucosal atrophy in AIG patients.
2.Clinical Characteristics of Autoimmune Gastritis Patients Complicated With Pernicious Anemia

Background:Autoimmune gastritis(AIG),used to be called pernicious anemia(PA),is actually a disease with different characteristics at different stages leading to conceptual confusion.In fact,only a proportion of patients with AIG develop PA.In the last decade,there have been few studies on AIG and PA.Aims:To investigate the clinical characteristics of AIG complicated with PA,so as to improve the understanding and diagnosis of this disease.Methods:A total of 33 AIG patients complicated with PA admitted to Renji Hospital,Shanghai Jiao Tong University School of Medicine from June 2019 to July 2023 were retrospectively collected.The clinical data,serological indicators,endoscopic results and histopathological findings of all patients were analyzed.The data included age at diagnosis,sex,anti-parietal cell antibody(PCA),anti-intrinsic factor antibody(IFA),pepsinogen Ⅰ,Ⅱ and their ratio,gastrin 17,OLGA and OLGIM stages,red blood cell count,hemoglobin,mean corpuscular volume,vitamin B12 level,and the degree of anemia.Results:The average age of AIG patients with PA was 62.42 years old,including 11 males and 22 females.Twenty-four patients(72.7%)had mild anemia.PCA was found in 26 patients(78.8%).Iron deficiency anemia was found in 11 patients(33.3%).The mean vitamin B12 level was(92.7±42.0)pg/mL.The difference between the groups was not obvious and not statistically significant for patients over and under 65 years of age.There were significant differences in red blood cell count,mean corpusular volume,and OLGIM staging between patients with and without iron deficiency anemia(all P<0.05).Conclusions:The average age of AIG patients with PA was over 60 years,one third of the patients also had iron deficiency anemia,and the average vitamin B12 level was less than 100 pg/mL.Early diagnosis and intervention are essential for the prognosis of patients.

5. Study on Risk Factors for Gastric Polyp Formation in Autoimmune Gastritis

Background: Autoimmune gastritis (AIG) can lead to hypergastrinemia and enterochromaffin ⁃ like cell hyperplasia, thereby increasing the risk of gastric neuroendocrine neoplasms and gastric adenocarcinoma. However, research on the association between AIG and gastric polyps is limited. Aims: To investigate the risk factors for the development of gastric polyps in AIG patients. Methods: The clinical data of 103 AIG patients visited the Chinese People’s Armed Police Force Sichuan Provincial Corps Hospital from June 2019 to June 2023 was collected retrospectively. The parameters analyzed included gender, age, anti⁃parietal cell antibody, anti⁃intrinsic factor antibody, pepsinogen (PG)I, PGII, ratio for PGI/ II, gastrin ⁃ 17 (G ⁃ 17), OLGA and OLGIM staging, size, location and pathological types of gastric polyps, vitamin B12, Helicobacter pylori infection and presence of anemia at the year AIG diagnosed. Univariate and multivariate Logistic regression analyses were performed to identify the risk factors for gastric polyps in AIG patients. The predictive performance of the variables was assessed by ROC curve analysis. Results: Among the 103 AIG patients enrolled in the study, 67 (65.05%) were females, and the median age was 58 years old. Fourteen (13.59%) patients had gastric polyps. Compared to patients without gastric polyps, those with gastric polyps had significantly higher levels of serum G ⁃ 17 (P< 0.000 1). Multivariate Logistic regression analysis indicated that G⁃ 17 was an independent risk factor for gastric polyps in AIG patients (OR=1.047, 95% CI: 1.017 ⁃ 1.078, P=0.002). The area under the curve (AUC) of G ⁃ 17 and the Logistic regression model in predicting the gastric polyp formation in AIG patients was 0.811 and 0.884, respectively. The optimal cutoff value of G⁃17 was 69.50 pmol/L, with the sensitivity and specificity of 92.9% and 67.4%, respectively. Conclusions: Serum G⁃17 is a potential prognostic factor for AIG. The optimal cutoff value for predicting the development of gastric polyp is 69.50 pmol/L.
