Objective To analyze the procurement data of lipid-lowering drugs in hospitals at different levels in Jiangsu Province from October 2019 to September 2023,to evaluate the impact of the volume-based procurement(VBP)policy, and to provide references for clinical rational drug use and healthcare policy optimization. Methods Based on procurement data from the Jiangsu Provincial Health Information Center, statistical analyses of procurement expenditures, defined daily doses(DDDs),and defined daily cost(DDC)were conducted.Mixed-effects models were applied to assess changes in procurement expenditures,DDDs,and DDC before and after VBP implementation. Results From 2019 to 2023,statins dominated the market in Jiangsu Province, with rosuvastatin recording the highest DDDs(748 million).Statins, traditional Chinese medicines, and cholesterol absorption inhibitors ranked highest in procurement expenditures.Tertiary hospitals accounted for the largest share of usage(47.6%)and expenditures(55.8%),while secondary hospitals had the lowest DDC(1.22 yuan)and tertiary hospitals the highest(1.89 yuan).Post-VBP,procurement expenditures and DDC decreased by 53.9%and 35.4%,respectively.Primary hospitals showed the largest expenditure reduction(61.6%),and secondary hospitals exhibited the greatest DDC decline(53.9%).DDDs increased significantly in primary care settings(e.g.,pitavastatin surged by 239.79%in secondary hospitals),while tertiary hospitals saw reduced usage of some drugs(e.g.,amlodipine/atorvastatin decreased by 7.34%).Mixed-effects models confirmed that VBP significantly reduced expenditures(OR=-1.07,P<0.01)and DDC(OR=-2.70,P<0.01)while indirectly lowering prices of non-VBP drugs. After covariate adjustment, expenditure reductions for rosuvastatin and atorvastatin narrowed, ezetimibe expenditures increased(OR=0.13,P<0.01),and pitavastatin usage declined(OR=-0.10,P<0.01).Changes in amlodipine/atorvastatin and ezetimibe lacked statistical significance due to short VBP implementation periods.Tertiary hospitals demonstrated the strictest policy adherence, with the largest expenditure and DDC reductions(P<0.01).Subgroup analysis revealed that the policy did not significantly affect clinical demand(DDDs)in hospitals at different levels, though it was considered to have triggered adjustments in medication structure. Conclusion Jiangsu's lipid-lowering drug structure aligns with guidelines(statin-based,moderate-intensity preference).VBP effectively reduced costs, with tertiary hospitals prioritizing originator-to-generic substitution and primary hospitals reflecting cost-control and demand variations.Confounding factors influenced policy evaluation. The study recommends continuous monitoring and policy optimization to enhance procurement efficiency, ensure rational clinical use,and sustain cost savings, providing insights for further healthcare reform.

Molecular knowledge of human gastric corpus epithelium remains incomplete. Here, by integrated analyses using single-cell RNA sequencing (scRNA-seq), spatial transcriptomics, and single-cell assay for transposase accessible chromatin sequencing (scATAC-seq) techniques, we uncovered the spatially resolved expression landscape and gene-regulatory network of human gastric corpus epithelium. Specifically, we identified a stem/progenitor cell population in the isthmus of human gastric corpus, where EGF and WNT signaling pathways were activated. Meanwhile, LGR4, but not LGR5, was responsible for the activation of WNT signaling pathway. Importantly, FABP5 and NME1 were identified and validated as crucial for both normal gastric stem/progenitor cells and gastric cancer cells. Finally, we explored the epigenetic regulation of critical genes for gastric corpus epithelium at chromatin state level, and identified several important cell-type-specific transcription factors. In summary, our work provides novel insights to systematically understand the cellular diversity and homeostasis of human gastric corpus epithelium in vivo.

Digestive tract diseases, especially digestive tract tumors, including liver cancer, pancreatic cancer, and colorectal cancer, have high incidence in China. Digestive tract tumor is one of the top 10 cancers in terms of the number of new cases and deaths in the world, and the incidence and mortality of tumor diseases have been increasing year by year. Therefore, the prevention and treatment of tumors is particularly important. With the application and promotion of traditional Chinese medicine in the medical field and the rapid development of molecular biology and pharmacology, more and more potential active components of Chinese medicinal materials have been extracted and studied. These active components can inhibit tumor cells in a multi-target and multi-pathway manner. Cinobufotalin is an effective component extracted from the skin of Bufo bufo gargarizans. It has been prepared into a variety of agents with anti-tumor, immunomodulatory, cardiac boosting, pain-easing, anti-inflammatory, and swelling-relieving activities. In clinical practice, cinobufotalin is mainly used to assist the treatment of liver cancer, lung cancer, colorectal cancer, gastric cancer and other malignant tumors, which can reduce the adverse reactions of patients in the middle and late stages and improve the quality of life and five-year survival rate of patients. The available studies of molecular mechanism have demonstrated that cinobufotalin can play a therapeutic role by inducing cell apoptosis, regulating cell cycle, inhibiting cell proliferation and angiogenesis, modulating immune response, reversing multidrug resistance, enhancing radiochemotherapy sensitivity, inhibiting tumor inflammation, invasion, and metastasis, etc. This review focuses on the clinical application and mechanism of cinobufotalin against digestive tract tumors in recent years, aiming to provide a theoretical basis for the anti-tumor research of cinobufotalin, promote the application of cinobufotalin in tumor treatment, and facilitate the further research and development of this compound.

TAM(tumor-associated macrophage)is a kind of immune cell in tumor microenvironment, which mainly exists in tumor matrix to mediate inflammatory reaction.Generally,TAM can be stimulated by different cytokines to polarize into M1 macrophage or M2 macrophage with different phenotypes. In the early stage of tumor,M1 macrophages in TAM exhibit pro-inflammatory and anti-tumor activities, while as tumor progresses,TAMs tend to polarize into M2 macrophages to play anti-inflammatory and tumor-promoting roles.A large number of studies have shown that TAM is closely related to tumor growth, invasion, metastasis and poor prognosis. Therefore, targeting TAM has become the focus of anti-tumor immunotherapy. This paper has summarized the origin of TAM,and introduced the specific roles of TAM in promoting tumor proliferation, angiogenesis, migration and invasion and the formation of immunosuppressive environment. At the same time, the research progress of TAM-targeted anti-tumor immu-notherapy in recent years was discussed from 3 aspects:inhibiting monocyte recruitment, promoting TAM apoptosis and reshaping TAM phenotype.

The current document is based on a consensus reached by a panel of experts from the Chinese Society of Allergy and the Chinese Society of Otorhinolaryngology-Head and Neck Surgery, Rhinology Group. Chronic rhinosinusitis (CRS) affects approximately 8% of Chinese adults. The inflammatory and remodeling mechanisms of CRS in the Chinese population differ from those observed in the populations of European descent. Recently, precision medicine has been used to treat inflammation by targeting key biomarkers that are involved in the process. However, there are no CRS guidelines or a consensus available from China that can be shared with the international academia. The guidelines presented in this paper cover the epidemiology, economic burden, genetics and epigenetics, mechanisms, phenotypes and endotypes, diagnosis and differential diagnosis, management, and the current status of CRS in China. These guidelines—with a focus on China—will improve the abilities of clinical and medical staff during the treatment of CRS. Additionally, they will help international agencies in improving the verification of CRS endotypes, mapping of eosinophilic shifts, the identification of suitable biomarkers for endotyping, and predicting responses to therapies. In conclusion, these guidelines will help select therapies, such as pharmacotherapy, surgical approaches and innovative biotherapeutics, which are tailored to each of the individual CRS endotypes.

Chinese Journal of Emergency Medicine 2019;28(7):851-854
Objective To investigate the effects of Nod-like receptor protein 3 inhibitor MCC950 on cognitive function in a mouse model of sepsis-associated encephalopathy (SAE).Methods Ninety adult male C57BL/6 mice were randomly (random number) divided into three groups:the sham + saline group (n=20,sham group),CLP + saline group (n=35,CLP group),and CLP + MCC950 group (n=35,MCC950 group).SAE mouse model was established by cecal ligation and puncture (CLP) surgery.Saline (10 mL/kg) or MCC950 (10 mg/kg) was intraperitoneally injected 30 min before surgery and on day 1,2,4 and 6 after surgery according the grouping.Seven days after surgery, six mice were taken from each group.Western blot was used to detect the hippocampal content of NLRP3,apoptosis-associated specklike protein (ASC),interleukin-1β (IL-1β) and IL-18.The number of NLRP3-positive cells in CA 1 region were detected by immunohistochemical analysis. The remaining mice in each group were used for open field and fear conditioning tests 14 days after surgery.One-way analysis of variance was used for inter-group comparison, and SNK-q test was used for pairwise comparison.A P value <0.05 was considered statistically significant. Results Compared the MCC950 group with the CLP group, the freezing time of context test was significantly increased [(137±21) s vs (84±15) s,P=0.013],the hippocampal content of NLRP3,IL-1β and IL-18 were significantly reduced (P<0.01),and the number of NLRP3-immunoreactive cells per mm2 in CA region were significantly decreased (23±5 vs 74±13,P<0.01).There was no significant changes in protein level of ASC and results of open field tests (P>0.05).Conclusions MCC950 administration can improve cognitive function in a mouse model of SAE,which is probably due to the inhibition of NLRP3 inflammasome and downstream inflammatory cytokines IL-1β and IL-18.

Chinese Journal of Behavioral Medicine and Brain Science 2019;28(5):437-442
Objective To investigate whether modulating the activation of dorsolateral prefrontal cortex(DLPFC) by transcranial direct current stimulation(tDCS) can influence emotional perception. Meth-ods Seventy-eight undergraduates were randomly divided into four groups by simple random sampling meth-od. TDCS (1. 5 mA) noninvasive technique was used to test group 1 (n=23) for 3 minutes to intervene in DLPFC,group 2 (n=17) for 15 minutes to intervene in DLPFC,group 3 (n=20) for 3 minutes to intervene in primary visual cortex, and group 4 (n=18) for non-emotional picture test. The data were analyzed with re-peated measurement variance analysis. Results (1) The interaction between short-term(3 min) stimula-tion of tDCS and facial expression was statistically significant (F(1,22)=7. 448,P=0. 012). There was signifi- cant difference in positive face perception (before:70. 58%,period:74. 75%,P=0. 036) and no significant difference in negative face perception (before:70. 58%,period:70. 73%,P=0. 569). (2) There was no sig-nificant difference in the correlation between prolonged tDCS stimulation(15 min) and face expression recog-nition ( F (1,16)= 1. 621, P=0. 221). (3) The primary visual cortex was not affected by anodal tDCS (F(1,19)<1,P>0. 05). (4) There was no significant difference in the interaction between tDCS and facial ex-pression (F(1,17)=2. 566,P=0. 128) when visual stimulus was changed to non-expressive faces. Conclu-sions By applying tDCS technique, the present findings suggest that modulating DLPFC can influence emo-tional face perception, and support the valence-specific lateralization of emotional perception.

Allergy, Asthma & Immunology Research 2018;10(4):300-353
Allergic rhinitis (AR) is a global health problem that causes major illnesses and disabilities worldwide. Epidemiologic studies have demonstrated that the prevalence of AR has increased progressively over the last few decades in more developed countries and currently affects up to 40% of the population worldwide. Likewise, a rising trend of AR has also been observed over the last 2–3 decades in developing countries including China, with the prevalence of AR varying widely in these countries. A survey of self-reported AR over a 6-year period in the general Chinese adult population reported that the standardized prevalence of adult AR increased from 11.1% in 2005 to 17.6% in 2011. An increasing number of original articles and imporclinical trials on the epidemiology, pathophysiologic mechanisms, diagnosis, management and comorbidities of AR in Chinese subjects have been published in international peer-reviewed journals over the past 2 decades, and substantially added to our understanding of this disease as a global problem. Although guidelines for the diagnosis and treatment of AR in Chinese subjects have also been published, they have not been translated into English and therefore not generally accessible for reference to non-Chinese speaking international medical communities. Moreover, methods for the diagnosis and treatment of AR in China have not been standardized entirely and some patients are still treated according to regional preferences. Thus, the present guidelines have been developed by the Chinese Society of Allergy to be accessible to both national and international medical communities involved in the management of AR patients. These guidelines have been prepared in line with existing international guidelines to provide evidence-based recommendations for the diagnosis and management of AR in China.
