The diagnosis of occupational radiation diseases, with the diagnostic criteria as main technical basis, has always been one of the emphases of China's occupational health work. This study compared the diagnostic standards for occupational radiation diseases between China and the International Labour Organization (ILO) across three aspects—types, classifications/grades, and diagnostic dose requirements—categorizing the diseases into tissue reactions and stochastic effects. ① For tissue reactions, 8 types are included both in China and ILO standards, 13 unique to China's standards, and 4 unique to the ILO standards; significant differences exist in classifaton/grading; China's standards are either consistent with or stricter (i.e., requiring higher diagnostic dose values) than the ILO standards. ② For stochastic effects (radiation-induced cancer), 12 types were included in both standards; China's standards provide more comprehensive coverage for certain malignancies (e.g., bone cancer); while 5 types were unique to the ILO standards have, some (e.g. salivary gland cancer) are primarily observed in non-occupational populations. Overall, China’s standards include more disease types and maintain more stringent diagnostic dose requirements. However, specific discrepancies require further deliberation during future revisions. Finally, based on these results, recommendations were provided for basic research and the evaluation of Chinese diagnostic criteria.

Long-term spaceflight, characterized by drastic changes in spatial position, disrupts the Earth's inherent environmental periodic signals, such as the day-night light-dark cycle. This leads to a misalignment between the external environmental cycles and the body's endogenous biological rhythms. The cumulative effect of this persistent"time difference"severely disrupts the core biological pathways that regulate the circadian rhythm. It triggers a series of neuroendocrine and behavioral stress responses, exemplified by sleep-wake disorders, which have become a core medical problem constraining the success of long-term space missions and astronaut health.Traditional Chinese Medicine(TCM)posits that aligning with the rhythmic changes of heaven, earth, day, night, and the four seasons, and maintaining the dynamic balance and harmonious functioning of the body's Yin and Yang vital energies, are fundamental principles for safeguarding vitality and resisting external pathogens and internal injuries. However,in the prolonged weightlessness, confinement, and abnormal lighting environment of deep space flight, the bond between the human body and natural rhythms is forcibly severed.Yin and Yang Qi become difficult to sustain, the mechanisms of ascent, descent, exit, and entry are obstructed, the visceral Qi transformation functions are impaired, and the circulation of Qi and Blood becomes disordered, ultimately leading to the onset of various disorders. This paper, grounded in the TCM temporal medicine perspective of"correspondence between heaven and human"(tian ren xiang ying)and the theories of Yin-Yang and Qi-Blood,meticulously reviews the historical classics and modern research on acupoint intervention strategies for addressing rhythm disorders and harmonizing Yin and Yang.Building upon this foundation,it innovatively proposes and argues for an integrated acupoint prescription strategy:the"Bei Xin Wu Xue"combined with the classic paired points Zhaohai(KI6)-Shenmai(BL62)on the lower limbs to regulate the dynamic balance of the Yin and Yang Heel Vessels(Yin/Yang Qiao Mai),and the classic paired points Taichong(LR3)-Yongquan(KI1)to guide Qi downward. This combination aims to work at multiple levels-"Heart-Brain,Yin-Yang,Liver-Kidney"-to calm the Heart Spirit, clear the Brain Mansion, and tonify the Liver and Kidney. The goal is to restore Yin and Yang Qi to an orderly and coordinated state, thereby assisting the disordered biological rhythm system in regaining homeostasis. This provides a novel TCM-inspired approach of"rhythm regulation and equilibrium restoration"(tiao shi fu heng)to safeguard the long-term in-orbit health of astronauts.

Background::Acute respiratory distress syndrome (ARDS) is a common cause of respiratory failure in many critically ill patients. Although inflammasome activation plays an important role in the induction of acute lung injury (ALI) and ARDS, the regulatory mechanism of this process is still unclear. When cells are stimulated by inflammation, the integrity and physiological function of mitochondria play a crucial part in pyroptosis. However, the underlying mechanisms and function of mitochondrial proteins in the process of pyroptosis are largely not yet known. Here, we identified the 18-kDa translocator protein (TSPO), a mitochondrial outer membrane protein, as an important mediator regulating nucleotide-binding domain, leucine-rich repeat, and pyrin domain-containing protein 3 (NLRP3) inflammasome activation in macrophages during ALI.Methods::TSPO gene knockout (KO) and lipopolysaccharide (LPS)-induced ALI/ARDS mouse models were employed to investigate the biological role of TSPO in the pathogenesis of ARDS. Murine macrophages were used to further characterize the effect of TSPO on the NLRP3 inflammasome pathway. Activation of NLRP3 inflammasome was preformed through LPS + adenosine triphosphate (ATP) co-stimulation, followed by detection of mitochondrial membrane potential, reactive oxygen species (ROS) production, and cell death to evaluate the potential biological function of TSPO. Comparisons between two groups were performed with a two-sided unpaired t-test. Results::TSPO-KO mice exhibited more severe pulmonary inflammation in response to LPS-induced ALI. TSPO deficiency resulted in enhanced activation of the NLRP3 inflammasome pathway, promoting more proinflammatory cytokine production of macrophages in LPS-injured lung tissue, including interleukin (IL)-1β, IL-18, and macrophage inflammatory protein (MIP)-2. Mitochondria in TSPO-KO macrophages tended to depolarize in response to cellular stress. The increased production of mitochondrial damage-associated molecular pattern led to enhanced mitochondrial membrane depolarization and pyroptosis in TSPO-KO cells. Conclusion::TSPO may be the key regulator of cellular pyroptosis, and it plays a vital protective role in ARDS occurrence and development.
