1.Effects of Modified Guomin Decoction (加味过敏煎) on Traditional Chinese Medicine Syndromes and Quality of Life in Patients with Mild to Moderate Atopic Dermatitis of Heart Fire and Spleen Deficiency Pattern:A Randomized,Double-Blind,Placebo-Controlled Trial

ObjectiveTo observe the clinical efficacy and safety of Modified Guomin Decoction (加味过敏煎, MGD) in patients with mild to moderate atopic dermatitis (AD) of the traditional Chinese medicine (TCM) pattern of heart fire and spleen deficiency, and to explore its possible mechanisms. MethodsIn this randomized, double-blind, placebo-controlled study, 72 patients with mild to moderate AD and the TCM pattern of heart fire and spleen deficiency were randomly divided into a treatment group and a control group, with 36 cases in each group. The treatment group received oral MGD granules combined with topical vitamin E emulsion, while the control group received oral placebo granules combined with topical vitamin E treatment. Both groups were treated twice daily for 4 weeks. Clinical efficacy, TCM syndrome scores, Visual Analogue Scale (VAS) for pruritus, Dermatology Life Quality Index (DLQI) scores, Scoring Atopic Dermatitis (SCORAD) and serum biomarkers, including interleukin-33 (IL-33), interleukin-1β (IL-1β), immunoglobulin E (IgE), and tumor necrosis factor-α (TNF-α) were compared before and after treatment. Safety indexes was also assessed. ResultsThe total clinical effective rates were 77.78% (28/36) in the treatment group and 38.89% (14/36) in the control group, with cure rates of 19.44% (7/36) and 2.78% (1/36), respectively. The treatment group showed significantly better clinical outcomes compared to the control group (P<0.05). The treatment group exhibited significant reductions in total TCM syndrome scores, including erythema, edema, papules, scaling, lichenification, pruritus, irritability, insomnia, abdominal distension, and fatigue scores, as well as reductions in VAS, DLQI, SCORAD, and serum IgE and IL-33 levels (P<0.05 or P<0.01). Compared to the control group, the treatment group had significantly better improvements in all indicators except for insomnia (P<0.05). No adverse events occurred in either group. ConclusionMGD is effective and safe in treating mild to moderate AD patients with heart fire and spleen deficiency pattern. It significantly alleviates pruritus, improves TCM syndromes and quality of life, and enhances clinical efficacy, possibly through modulation of immune responses.
2.Mechanism of Weiliuan Mixture in Regulating Ferroptosis and Inhibiting Progression of Gastric Cancer Based on Transcriptome

ObjectiveTo observe the inhibitory effect of the Weiliuan mixture (WLAHJ) on the subcutaneous xenograft tumor of MKN-74 gastric cancer cells, and explore the potential anti-gastric cancer mechanism of WLAHJ by using transcriptomic sequencing technology to reveal related genes and pathways. Methods30 Balb/c nude mice were randomly divided into model, low-, medium-, and high-dose(15,30,45 g·kg-1) WLAHJ and 5-FU (0.025 g·kg-1) groups to build a subcutaneous xenograft tumor model with MKN-74 human gastric cancer cells. After modeling,each group was continuously treated with the corresponding drugs for 28 days. During the treatment period, the body weight and tumor size of the mice were observed and recorded every 2 days. At the end of the treatment, the mice were sacrificed, and required samples were collected to calculate the tumor inhibition rate of WLAHJ on the subcutaneous xenograft tumor. High-throughput transcriptomic sequencing (RNA-seq) technology was used to analyze the differentially expressed genes in the subcutaneous tumor tissues of the model group and the medium-dose WLAHJ group, thus exploring the potential mechanism of WLAHJ in gastric cancer intervention. Immunofluorescence experiments were conducted to detect the protein expression levels of glutathione peroxidase 4 (GPX4), solute carrier family 7 member 11 (SLC7A11), transferrin receptor protein-1 (TFR-1), and acyl-CoA synthetase long-chain family member 4 (ACSL4) in subcutaneous xenograft tumors of each group. Cell counting kit-8(CCK-8) and colony formation assays were used to detect the viability and anti-proliferative ability of human gastric cancer AGS and MKN-74 cells at different concentrations of WLAHJ. Kits were used to detect the levels of Fe2+, reactive oxygen species (ROS), malondialdehyde (MDA), and superoxide dismutase (SOD) activity in cells. Western blot was used to detect the expression levels of GPX4, SLC7A11, TRF-1, ACSL4, spermidine/spermine N1-acetyltransferase 1 (SAT1), arachidonic acid 15-lipoxygenase (ALOX15), and key proteins in the phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/mammalian target of rapamycin (mTOR) signaling pathway. ResultsThe mechanism of WLAHJ in gastric cancer intervention may be related to ferroptosis and the PI3K/Akt /mTOR signaling pathway. The growth of subcutaneous xenograft tumors in nude mice of the WLAHJ and 5-FU groups(P<0.05,P<0.01), GPX4, and SLC7A11 dropped significantly(P<0.01), while TFR-1, ACSL4, SAT1, and ALOX15(P<0.05,P<0.01)increased significantly compared with those in the model group. The levels of ROS, Fe2+, and MDA increased in the WLAHJ and 5-FU groups and the proliferation of gastric cancer cells, SOD activity, the ratios of phosphorybation (p)-mTOR/mTOR, p-PI3K/PI3K, and p-Akt/Akt protein expressions(P<0.05,P<0.01)decreased compared with those in the blank group. ConclusionThe mechanism of WLAHJ in treating gastric cancer may be related to the regulation of the PI3K/ Akt /mTOR signaling pathway to intervene in ferroptosis.
3.Correlation analysis of central venous oxygen saturation-related indexes at different time points with low cardiac output syndrome in children after congenital heart disease correction surgery

Objective To explore the correlation between central venous oxygen saturation(ScvO2)-re-lated indexes at different time points and the occurrence of low cardiac output syndrome(LCOS)after con-genital heart disease(CHD)correction surgery.Methods A total of 73 children who underwent CHD correc-tion surgery in this hospital from July 1st,2021 to July 1st,2024 were selected as the research subjects.The clinical data,preoperative conditions,and postoperative conditions of the children were collected.The ScvO2 and arterial lactate(Lac)levels of the children at different time points(the 1st,6th,12th,and 24th hours after surgery)were monitored,and the ScvO2/Lac at different time points and the change rate of ScvO2 in different time periods were calculated.The correlation between ScvO2-related indexes and LCOS after CHD correction surgery was analyzed.Results ScvO2 at the 6th hour after surgery,ScvO2 at the 12th hour after surgery,Sc-vO2/Lac at the 12th hour after surgery,the change rate of ScvO2 from the 1st to the 24th hour after surgery,the change rate of ScvO2 from the 6th to the 12th hour after surgery,and the change rate of ScvO2 from the 12th to the 24th hour after surgery were independent influencing factors of LCOS occurrence after CHD cor-rection surgery(P<0.05).There was a negative correlation between ScvO2 at the 12th hour after surgery,ScvO2/Lac and LCOS occurrence after CHD correction surgery(r=-0.543,-0.523,P<0.05).The area under the curve(AUC)of ScvO2 at the 12th hour after surgery for predicting LCOS occurrence after CHD correction surgery was 0.938(95%CI:0.865-1.000);the AUC of ScvO2/Lac at the 12th hour after surgery for predicting LCOS occurrence after CHD correction surgery was 0.922(95%CI:0.851-0.994).Conclusion ScvO2 and ScvO2/Lac at the 12th hour after surgery have good predictive potential for LCOS occurrence af-ter CHD correction surgery.
4.Analysis of related factors and predictive value affecting the efficacy of neoadjuvant chemotherapy for esophageal cancer

Objective To analyze the clinical parameters of esophageal cancer patients before and during neoadjuvant chemotherapy,as well as to explore the related factors and predictive value that affect the efficacy of neoadjuvant chemotherapy for esophageal cancer.Methods A total of 194 patients with esophageal cancer who underwent neoadjuvant chemotherapy at the First Affiliated Hospital of Guangxi Medical University from 2020 to 2023 were selected as the research subjects.The treatment process and outcomes of the patients were followed up,and they were divided into an effective group and an ineffective group according to the effi-cacy.Differences were compared in clinical parameters between two groups of patients before and during treat-ment,screen for factors that may affect efficacy,correlation analysis was conduct to explore the correlation be-tween relevant factors and the efficacy of neoadjuvant chemotherapy,and the predictive value of relevant fac-tors were analyzed using receiver operating characteristic(ROC)curve analysis.Results There was a statisti-cally significant difference(P<0.05)in the average cycle cost,lowest WBC value,lowest PLT value,inci-dence of nausea,transaminase abnormalities,and hair loss between the two groups.Logistic regression analysis showed that average cycle cost,transaminase abnormalities,and hair loss were related factors affecting neoad-juvant chemotherapy for esophageal cancer.Spearman correlation analysis showed that the above indicators had a certain correlation with the efficacy of neoadjuvant chemotherapy for esophageal cancer.ROC curve a-nalysis showed that the area under the curve(AUC)for predicting the efficacy of neoadjuvant chemotherapy in esophageal cancer by combining transaminase abnormalities,average cycle cost,and hair loss was 0.758(95%CI:0.683-0.832)Conclusion There is a certain correlation between average cycle cost,transaminase abnormalities,and hair loss and the effectiveness of neoadjuvant chemotherapy for esophageal cancer,which has certain predictive value for the efficacy of neoadjuvant chemotherapy for esophageal cancer.
5.Application of transvaginal ultrasound,multimodal magnetic resonance imaging and 18F-fluorodeoxyglucose positron emission computed tomography/computed tomography in diagnosis of endometrial carcinoma

Objective To investigate the application of transvaginal ultrasound(TVS),multimo-dal magnetic resonance imaging(MRI)and 18F-fluorodeoxyglucose positron emission computed tomo-graphy/computed tomography(18F-FDG PET/CT)in diagnosis of endometrial carcinoma(EC).Methods A total of 157 patients with endometrial cancer in our hospital were selected.The results of EC staging by multimodal MRI and transvaginal ultrasound and positive 18F-FDG PET/CT were recor-ded,and their results were compared with pathological results.Results The results of TVS showed 87 cases in stage Ⅰ,6 cases were misdiagnosed and 12 cases were missed diagnosis.In 55 cases of stage Ⅱ,12 cases were misdiagnosed and 8 cases were missed.In 15 cases of stage Ⅲ,2 cases were misdiagnosed without missed diagnosis.The results of multimodal MRI showed that 86 cases were mis-diagnosed in stage Ⅰ,3 cases were misdiagnosed and 10 cases were missed.In 58 cases of stage Ⅱ,10 cases were misdiagnosed and 3 cases missed diagnosis.There were 13 cases of stage Ⅲ with no missed diagnosis or misdiagnosis.The results of 18F-FDG PET/CT were positive in 145 cases and missed in 12 cases.Based on the golden standard of postoperative pathological staging,the accuracy of TVS di-agnosis of EC patients in stage Ⅰ was 87.10%(81/93),stage Ⅱ was 84.31%(43/51),and stage Ⅲ was 100%(13/13).Based on the golden standard of postoperative pathological staging,the accuracy of mul-timodal MRI in the diagnosis of EC patients was 89.25%(83/93)in stage Ⅰ,94.12%(48/51)in stageⅡ,and 100%(13/13)in stage Ⅲ.According to the pathological staging,145 EC positive cases were de-tected by 18F-FDG PET/CT,the accuracy rate was 92.36%(145/157).Conclusion 18F-FDG PET/CT has a high diagnostic accuracy in EC patients,TVS and multimodal MRI have a high value in the preoperative staging diagnosis of EC,and are worthy of clinical promotion.
6.Effects and Mechanism of Quercetin on Osteogenic Differentiation of BMSCs

Objective To explore the effect and mechanism of quercetin on osteogenic differentiation of bone marrow mesenchymal stem cells(BMSCs).Methods BMSCs were divided into a blank control group(Control)and quercetin(Quercetin)low-dose group(4.8 mL/kg),medium-dose group(9.6 mL/kg),and high-dose group(19.2 mL/kg)intervened with drug-containing serum,while the positive control group was treated with osteogenic differentiation medium,respectively.The cell cycle was analysed by flow cytometry,cell proliferation was detected by MTT assay,cell activity was determined by Alkaline phosphatase(ALP)assay kit,and calcified nodule formation was observed by alizarin red staining.The expression of β-catenin and the key factors of osteogenic differentiation,runt-related transcription factor 2(RUNX2)and osteocalcin(OCN),were detected by qPCR and Western blot,respectively.Results Compared with the control group,quercetin-containing serum significantly promoted the proliferation of BMSCs(P=0.000205,P=0.000063)and enhanced the formation of calcium nodules,and increased osteogenic and ALP activity after osteogenic differentiation.The results of qPCR and Western blot showed that the quercetin group significantly up-regulated the mRNA and protein expression of β-catenin(P<0.0001),RUNX2(P<0.0001)and OCN(P<0.0001)during osteogenic differentiation.Conclusion Quercetin can effectively promote the osteogenic differentiation of BMSCs,and its mechanism is achieved by activating the Wnt/β-catenin signaling pathway and up-regulating the expression of the osteogenesis-related transcription factor RUNX2.
7.Comparative analysis of pediatric macrophage activation syndrome combined with systemic juvenile idiopathic arthritis versus with systemic lupus erythematosus

Objective:To compare the similarities and differences of macrophage activation syndrome(MAS)combined with systemic juvenile idiopathic arthritis(sJIA)versus with juvenile onset systemic lupus erythematosus(JSLE).Methods:The clinical data of 48 children with MAS admitted to the Department of Nephrology and Immunology in Children's Hospital of Hebei Province from May 2015 to January 2023 were retrospectively analyzed. The patients were divided into sJIA-MAS and JSLE-MAS group,and the clinical manifestations,laboratory indicators and treatment of the two groups were compared.Results:Among the 48 children(14 males and 34 females)with MAS,the average age of onset was 9.5(3.0,11.8)years. There were 28 cases(11males and 17 females)of sJIA-MAS and 20 cases(3 males and 17 females)of JSLE- MAS. All the 48 children with MAS had fever and hyperferinemia,and the fever with sJIA-MAS was mostly continued fever or remittent fever. Respiratory tract infection was the most common trigger in sJIA-MAS[15 cases(53.6%)],and disease activity was the most common trigger in JSLE-MAS[13 cases(65.0%)].Additionally,viral infections(EB virus and cytomegalovirus)were also one of the triggers in MAS[sJIA:7 cases(25%),JSLE:4 cases(20%)].Compared with JSLE-MAS,the number of days with fever[15.0(12.0,21.0)days vs. 6.0(4.0,9.5)days, Z=-3.812, P=0.001]and the length of hospital stay[29.0(26.3,39.8)days vs.26.0(19.3,30.8)days, Z=-1.958, P=0.049]were longer in sJIA. Compared with JSLE-MAS,ALT[(685.32±561.67)U/L vs.(139.61±124.44)U/L, t=4.973, P=0.001],AST[784.00(235.25,1 251.25)U/L vs.189.50(53.25,374.08)U/L, Z=-3.283, P=0.001],CRP[11.48(3.56,28.89)mg/L vs.1.91(0.53,8.98)mg/L, Z=-3.200, P=0.001],ferritin[32 167.0(12 384.8,65 963.8)μg/L vs.2 003.5(922.5,11 430.0)μg/L, Z=-4.130, P=0.001],ferritin max/ESR min[1 353.35(355.75,4 342.53)vs.91.92(34.94,291.53), Z=-4.120, P=0.001]were higher in sJIA.The decrease of CRP was greater in sJIA[80.04(45.64,143.71)mg/L vs.10.20(6.27,25.64)mg/L, Z=-4.433, P=0.001].Compared with sJIA-MAS,peripheral white blood cell counting[4.05(2.90,7.73)×10 9/L vs.1.56(1.15,3.47)×10 9/L, Z=-3.577, P=0.001]and platelet counting[(162.68±92.19)×10 9/L vs.(110.10±72.99)×10 9/L, t=2.118, P=0.040]were lower in JSLE-MAS. Kidney involvement was more common in JSLE-MAS[10 cases(50%)vs.0 cases(0%), χ 2=17.684, P=0.001].There was no significant difference in the incidence of sJIA-MAS and JSLE-MAS meeting the criteria of hemophagocytic lymphohistiocytosis[6 cases(21.4%)vs.5 cases(25.0%), χ 2=0.084, P=0.772]. Conclusion:Compared with JSLE-MAS,sJIA-MAS is more dangerous and difficult to control,while JSLE-MAS involves more organs,among which the blood system and kidney are more common.
8.Peripheral blood cell count and characteristics of dyshaematopoiesis in patients with myelodysplastic syndrome

Objective To investigate peripheral blood cell count and characteristics of dyshaematopoiesis of peripheral blood and bone marrow cells in patients with myelodysplastic syndrome(MDS).Methods A total of 40 patients with MDS newly diagnosed in the hospital from January 2015 to December 2022 were selected as the experimental group,while 17 patients with megaloblastic anemia(MA group),11 patients with aplastic a-nemia(AA group),and 24 patients with autoimmune disease(AID group)were selected as the non-MDS group.Blood routine of patients in each group was detected by whole blood cell analyzer Sysmex XN20,and the white blood cell count(WBC),red blood cell count(RBC),hemoglobin(Hb),platelet count(PLT),mean red blood cell volume(MCV)and coefficient of variation of red blood cell distribution width(RDW-CV)of patients in 4 groups were compared and analyzed.Bone marrow and peripheral blood cell morphology in 4 groups were observed and recorded by artificial microscopy.The positive rate of common dyshaematopoiesis of peripheral in 4 groups was compared and analyzed.Results Compared with MA group,RBC and Hb in MDS group were significantly increased(P<0.05),while MCV was significantly decreased(P<0.05).Compared with AA group,WBC,MCV,RDW-CV and PLT in MDS group were significantly increased(P<0.05).Com-pared with AID group,WBC,RBC,Hb and PLT in MDS group were significantly decreased(P<0.05),while MCV and RDW-CV were significantly increased(P<0.05).The positive rate of peripheral blood primitive cells in MDS group was significantly higher than that in non-MDS group(P<0.05).Large red blood cells and polylobulated granulocyte were the main dysplasia types in the peripheral blood of MA group,and the positive rate of dyshematopoietic was significantly higher than that in MDS group(P<0.05).Compared with the non-MDS group,the positive rates of five types of dyshaematopoiesis in the MDS group were significantly higher than those in the control group(P<0.05),including increased blasts,decreased or absent cytoplasmic gran-ules,pseudo Pelger neutrophils,binucleated granulocytes and micromegakaryocytes.The positive rates of polylobulation and giant change in granulocytes,giant change,large red cells,basophilic stippling erythrocyte and H-J corpuscle in erythrocytes in MA group were significantly higher than those in MDS group(P<0.05).Dyshematopoiesis was absent or rare in the bone marrow smears of AA group and AID group.Conclu-sion Specific dyshaematopoiesis combined with peripheral blood cell count is helpful for the diagnosis and differential diagnosis of MDS,and reduces the possibility of misdiagnosis and missed diagnosis.

ObjectiveThis study aimed to evaluate the clinical efficacy of Ruyi Zhenbaowan(RYZBW)in the treatment of initial and early knee osteoarthritis (KOA) through a prospective multicenter,randomized,double-blind,and placebo-parallel controlled trial. MethodFrom October 13th, 2021 to December 25th, 2021, 240 KOA subjects meeting the acceptance criteria were enrolled in 15 sub-centers including Wangjing Hospital, Chinese Academy of Chinese Medical Sciences, and they were randomly divided into observation group and control group, with 120 cases in each group. The intervention measures for the observation group were RYZBW + health education, and the intervention measures for the control group were RYZBW placebo + health education. The intervention period in both groups was four weeks, and they were followed up for four weeks after the intervention. The primary outcome measure was the total score of Western Ontario and McMaster University Osteoarthritis Index score (WOMAC score), and the secondary outcome measures were the response rate of visual scale (VAS) pain score, WOMAC sub item scores (joint pain, joint stiffness, and joint function), quality of life (SF-12) score, and traditional Chinese medicine (TCM) syndrome score. Result(1) Efficacy evaluation. The marginal model results showed that the observation group was better than the control group in improving the WOMAC total score and WOMAC pain score in the treatment of KOA with RYZBW, and the difference was statistically significant (P<0.05). There was no significant difference between the two groups in improving VAS score response rate, WOMAC function score, WOMAC stiffness score, SF12-PCS (quality of life-physical health) score, SF12-MCS (quality of life-mental health) score, and TCM syndrome score. (2) Subgroup analysis. ① In terms of VAS score response rate, the response rate of the observation group was higher than that of the control group for subjects with baseline VAS score of (4, 5], and the difference was statistically significant (P<0.05). ② In terms of TCM syndrome score, for subjects aged [56, 60] and [61, 65], the decrease in total TCM syndrome score in the observation group was better than that in the control group, and the difference was statistically significant (P<0.05). ConclusionTibetan medicine RYZBW has good clinical efficacy in improving WOMAC total score, VAS score response rate, WOMAC pain score, WOMAC function score, and TCM syndrome score for patients with initial and early KOA, which can fill the lack of Tibetan medicine RYZBW in the treatment of KOA and make a demonstration study for the inheritance and development of ethnic medicine.

Methods: A total of 104 pediatric patients infected by SARS-CoV-2 Omicron variant Tianjin First Central Hospital (designated hospital for SARS-CoV-2 infection in Tianjin) for convalescent treatment from January 22nd, 2022 to February 24th were included for a retrospective study.Clinical data including clinical typing, SARS-CoV-2 IgG and IgM test and 2019-nCoV nucleic acid test were collected.The cases were divided into 2-dose group and zero-dose group based on the doses of inactivated SARS-CoV-2 vaccine. The children were divided into repositive group and negative group, according to the nucleic acid test during hospitalization. Chi-square test was used for the comparison between the groups. Results: The age of these 104 children was 10.0 (0.3, 14.0) years on admission, 53 children were males and 51 were females, 92 cases (88.5%) had mild symptoms, 12 cases (11.5%) had common symptoms.The age and SARS-CoV-2 IgG level of zero-dose group was lower (2.0 (0.3, 10.2) vs. 10.0 (3.2, 14.0) years, 10 (2, 17) vs. 193 (157, 215), χ²=-5.57, Z=-48.76,both P<0.001) than that of 2-dose group. The zero-dose group had a high rate of transmission among family members and a high level of SARS-CoV-2 IgM level (13/14 vs. 62.2% (56/90), 0.4 (0.2, 0.8) vs. 0.4 (0.2, 1.1),χ²=5.09, Z=-48.95, both P<0.05) than the 2-dose group. Repositive group had a high rate of underlying diseases and SARS-CoV-2 IgM level was higher (2/13) vs. 1.1% (1/91), (0.6 (0.2, 1.0) vs. 0.3 (0.2, 0.7), χ²=8.29, Z=2.70, both P<0.05) than negative group. The SARS-CoV-2 IgG level of repositive group was lower than that of negative group (160 (78, 197) vs. 213 (186, 231), χ²=-3.20, P<0.05). Conclusions: Children infected with SARS-CoV-2 Omicron variant in Tianjin were mainly transmitted by family members, and most of them had mild symptoms. Two-dose group had higher IgG levels and lower IgM levels than zero-dose group.The probability of SARS-CoV-2 nucleic acid test repositivity increased in children with underlying diseases and lower IgG levels.
