1.Three-dimensional gelatin microspheres loaded human umbilical cord mesenchymal stem cells for chronic tendinopathy repair

BACKGROUND:The absence of blood vessels in tendon tissue makes tendon repair challenging.Therefore,improving tendon healing and raising the efficacy of stem cell and other therapeutic cell transplantation after tendon damage have become hotspots for research in both clinical and scientific contexts. OBJECTIVE:The stem cells and gelatin microcarrier scaffold were joined to form tissue engineered stem cells.Human umbilical cord mesenchymal stem cells cultured in gelatin microcarriers were used to investigate the therapeutic impact and mode of action on tendinopathy healing in rats in vitro and In vivo. METHODS:(1)In vitro cell experiments:After seeding human umbilical cord mesenchymal stem cells with three-dimensional gelatin microcarriers,the cell vitality and survival were assessed.Human umbilical cord mesenchymal stem cells conventionally cultured were cultured as controls.(2)In vivo experiment:Adult SD rats were randomly assigned to normal group,tendinopathy group,2D group(tendinopathy+conventional culture of human umbilical cord mesenchymal stem cells),and 3D group(tendinopathy+gelatin microcarrier three-dimensional culture of human umbilical cord mesenchymal stem cells),with 6 rats in each group.Four weeks after therapy,animal behavior tests and histopathologic morphology of the Achilles tendon was examined. RESULTS AND CONCLUSION:(1)In vitro cell experiments:the seeded human umbilical cord mesenchymal stem cells on gelatin microcarriers showed high viability and as time went on,the stem cell proliferation level grew.Compared with the control group,3D stem cell culture preserved cell viability.(2)In vivo experiment:Following a 4-week treatment,the 3D stem cell culture group showed a significant improvement in both functional recovery of the lower limbs and histopathological scores when compared to the tendinopathy group.The 2D stem cell culture group also showed improvement in tendinopathy injury,but its effect is not as much as the 3D stem cell culture group.(3)The outcomes demonstrate that human umbilical cord mesenchymal stem cells cultured with three-dimensional gelatin microcarrier can promote the repair and regeneration of tendon injury tissue,and the repair effect is better than that of conventional human umbilical cord mesenchymal stem cells.

3.Xuandi Ziyin Mixture (玄地滋阴合剂) for Central Precocious Puberty in Girls with Syndrome of Yin Deficiency and Fire Exuberance: A Prospective Cohort Study

ObjectiveTo observe the clinical effectiveness and safety of Xuandi Ziyin Mixture (玄地滋阴合剂) for central precocious puberty (CPP) in girls with syndrome of yin deficiency and fire exuberance, and to analyse the effect of body mass index (BMI) on the effectiveness. MethodsA total of 236 girls with CPP of yin deficiency and fire exuberance syndrome were included, and all of them were given Xuandi Ziyin Mixture, 30 ml each time, twice a day, for a total treatment period of 6 months. Before and after treatment, children's weight, height and bone age were measured, BMI and BMI Z-score (BMI Z) and the difference between bone age and actual age were calculated; ultrasound was used to detect uterine and ovarian sizes, and to calculate uterine volume (Vuterus), bilateral ovarian volume (Vleft ovary, Vright ovary), and bilateral maximal follicle diameters (rleft follicle and rright follicle); and serum sex hormones were measured, including follicle-stimulating hormone (FSH), luteinising hormone (LH), prolactin (PRL), estradiol (E2), and testosterone (T), and were scored for traditional Chinese medicine (TCM) syndrome. Multiple linear regression was used to analyse the influence factors of the difference between bone age and actual age, and changes in uterine volume. The children were divided into the normal weight group and the overweight/obesity group according to baseline BMI, and the bone age, the difference between bone age and actual age, Vuterus and BMI Z scores before and after treatment were compared between the two groups. ResultsFinally, 199 children entered the statistical analysis. Compared with pre-treatment, the bone age, BMI and BMI Z scores of the children increased after treatment, and the difference between bone age and actual age, TCM syndrome scores, Vuterus, Vleft ovary, Vright ovary, rleft follicle and rright follicle decreased; and the levels of serum FSH, LH, E2, and T significantly decreased (P<0.05 or P<0.01). The difference between bone age and actual age was negatively correlated with LH and Vuterus (P<0.05), and changes in uterine volume were positively correlated with LH (P<0.01). Comparing between the groups before and after treatment, the bone age, difference between bone age and actual age, and BMI Z scores of children in the normal weight group (100 cases) were significantly smaller than those in the overweight/obesity group (99 cases) (P<0.01). Compared with pre-treatment, the bone age of the children in both groups increased, but the difference between bone age and actual age and Vuterus were significantly smaller (P<0.01). 4.A laboratory model for evaluating the risk of thrombosis in patients with chronic venous insufficiency

Objective To explore the predictive value of combined thromboelastography(TEG),coagulation index and platelet parameters in evaluating the risk of thrombosis in patients with chronic venous insufficiency(CVI).Methods A total of 359 patients with CVI were enrolled in our hospital from November 2020 to March 2022,and divided into VTE group and non-VTE group according to the occurrence of venous thromboembolism(VTE).The baseline characteristics of the two groups and the value of combined TEG,coagulation index and platelet parameters in predicting the risk of VTE in patients with CVI were analyzed.The risk factors were screened by univariate logistic regression analysis,and the prediction model was constructed by multivariate logistic regression.The performance of the model was evaluated by area under the curve(AUC)and sensitivity specificity.Results Compared with the non-VTE group,the R value(3.27±0.71 vs 3.87±1.16,P<0.05),the prothrombin time(PT)(11.08±3.02 vs 12.86±3.48,P<0.001)and the platelet distribution width(PDW)(12.01±3.87 vs 13.98±3.20,P<0.001)of the VTE group decreased,while fibrinogen(Fib)(3.46 vs 3.10,P<0.05)and D-dimer(DD)(3.00 vs 1.12,P<0.001)increased.It was found that the area under the ROC curve of the thrombosis pre-diction model based on PT,DD,R value,Fib and PDW was 0.842 8,with the sensitivity and specificity of 36.78%and 95.59%,respectively.The decision curve analysis(DCA)indicated that patients would benefit when the model's predicted probability ranged from 0.1 to 0.7.Conclusion The combination of TEG,coagulation index and platelet parameters is ef-fective in predicting the risk of VTE in patients with CVI.
5.Prediction model of platelet transfusion refractoriness in patients with hematological disorders

Objective To explore the risk factors for platelet transfusion refractoriness(PTR)in patients with hemato-logical disorders,construct a prediction model and validate the model efficacy.Methods Patients with hematological disor-ders who received platelet transfusion therapy in the Chengdu Second People's Hospital from December 2021 to December 2022 were retrospectively included to judge the effectiveness of platelet transfusion and screened for risk factors by univariate and multivariate logistic regression.A prediction model for PTR was constructed using receiver operating characteristic(ROC)curve,calibration curve and decision curve(DCA)to assess the differentiation,calibration and clinical value of the model,respectively.Results A total of 334 hematological patients were included,including 168 males and 176 females,with a PTR incidence of 40.4%.Univariate and multivariate logistic regression analysis showed that platelet transfusion vol-ume,erythrocyte transfusion volume,and neutrophil ratio were risk factors for PTR(P<0.05).A prediction model for PTR in hematological patients was established based on these risk factors.The area under the model's curve was 0.8377(95%CI:0.723-0.772),the sensitivity was 58.52%,and the specificity was 89.95%.The calibration curve showed that the S∶P was 0.964,the maximum absolute difference Emax was 0.032,and the average absolute difference Eavg was 0.009.The DCA a-nalysis showed that the model had clinical application value when the risk threshold ranged from 0.2 to 0.9.Conclusion The PTR prediction model based on platelet transfusion volume,erythrocyte transfusion volume and neutrophil ratio can pro-vide a basis for effective platelet transfusion in hematological patients.

Following the principles of evidence-based medicine,in accordance with the structure and drafting rules of standardized documents,based on literature research,according to the characteristics of chronic cough in children and issues that need to form a consensus,the TCM Guidelines for Diagnosis and Treatment of Chronic Cough in Children was formulated based on the Delphi method,expert discussion meetings,and public solicitation of opinions.The guideline includes scope of application,terms and definitions,eti-ology and diagnosis,auxiliary examination,treatment,prevention and care.The aim is to clarify the optimal treatment plan of Chinese medicine in the diagnosis and treatment of this disease,and to provide guidance for improving the clinical diagnosis and treatment of chronic cough in children with Chinese medicine.

Objective To analyze the clinical efficacy of Yunnan Baiyao in middle aged and elderly lung cancer patients undergoing the uniportal video-assisted thoracoscopic.Methods A total of 90 patients with non-small cell lung cancer meeting the criterias were screened and divided into two groups according to random number table method,which respectively received conventional treatment(control group)and Yunnan Baiyao treatment(experimental group).The perioperative period related indexes,coagulation function related indexes,and the occurrence rate of complications were observed and compared between the two groups.Results The intraoperative bleeding volume and thoracic drainage volume for the first day of experimental group were significantly lower than control group(P<0.05).However,there were no significant difference in the coagulation function related indexes between two groups before and after surgery(P>0.05).Additionally,there were also no significant difference in the occurrence rate of complication between two groups(P>0.05).Conclusion Yunnan Baiyao can reduce intraoperative bleeding volume and chest tube drainage volume for the first day,promots the recovery of patients,and has a good clinical application value.

Hypertrophic cardiomyopathy (HCM) is the most common inherited heart disease and is characterized by primary left ventricular hypertrophy usually caused by mutations in sarcomere genes. The mechanism underlying cardiac remodeling in HCM remains incompletely understood. An investigation of HCM through integrative analysis at multi-omics levels will be helpful for treating HCM. DNA methylation and chromatin accessibility, as well as gene expression, were assessed by nucleosome occupancy and methylome sequencing (NOMe-seq) and RNA-seq, respectively, using the cardiac tissues of HCM patients. Compared with those of the controls, the transcriptome, DNA methylome, and chromatin accessibility of the HCM myocardium showed multifaceted differences. At the transcriptome level, HCM hearts returned to the fetal gene program through decreased sarcomeric and metabolic gene expression and increased extracellular matrix gene expression. In the DNA methylome, hypermethylated and hypomethylated differentially methylated regions were identified in HCM. At the chromatin accessibility level, HCM hearts showed changes in different genome elements. Several transcription factors, including SP1 and EGR1, exhibited a fetal-like pattern of binding motifs in nucleosome-depleted regions in HCM. In particular, the inhibition of SP1 or EGR1 in an HCM mouse model harboring sarcomere mutations markedly alleviated the HCM phenotype of the mutant mice and reversed fetal gene reprogramming. Overall, this study not only provides a high-precision multi-omics map of HCM heart tissue but also sheds light on the therapeutic strategy by intervening in the fetal gene reprogramming in HCM.

9.Advances in the structure and function of chitosanase.

Chitosanases represent a class of glycoside hydrolases with high catalytic activity on chitosan but nearly no activity on chitin. Chitosanases can convert high molecular weight chitosan into functional chitooligosaccharides with low molecular weight. In recent years, remarkable progress has been made in the research on chitosanases. This review summarizes and discusses its biochemical properties, crystal structures, catalytic mechanisms, and protein engineering, highlighting the preparation of pure chitooligosaccharides by enzymatic hydrolysis. This review may advance the understandings on the mechanism of chitosanases and promote its industrial applications.

10.Five new terpenoids from Viburnum odoratissimum var. sessiliflorum.

Five new terpenoids, including two vibsane-type diterpenoids (1, 2) and three iridoid allosides (3-5), together with eight known ones, were isolated from the leaves and twigs of Viburnum odoratissimum var.sessiliflorum. Their planar structures and relative configurations were determined by spectroscopic methods, especially 2D NMR techniques. The sugar moieties of the iridoids were confirmed as β-D-allose by GC analysis after acid hydrolysis and acetylation. The absolute configurations of neovibsanin Q (1) and dehydrovibsanol B (2) were determined by quantum chemical calculation of their theoretical electronic circular dichroism (ECD) spectra and Rh2(OCOCF3)4-induced ECD analysis. The anti-inflammatory activities of compounds 1, 3, 4, and 5 were evaluated using an LPS-induced RAW264.7 cell model. Compounds 3suppressed the release of NO in a dose-dependent manner, with an IC50 value of 55.64 μmol·L-1. The cytotoxicities of compounds 1-5 on HCT-116 cells were assessed and the results showed that compounds 2 and 3 exhibited moderate inhibitory activities with IC50 values of 13.8 and 12.3 μmol·L-1, respectively.
