1.The relationship between size distribution of circulating extracellular vesicles and coagulation function after cardiac surgery

Objective:To investigate the relationship between the changes in extracellular vesicles (EVs) size distribution before and after cardiopulmonary bypass (CPB) cardiac surgery and postoperative coagulation function.Methods:A total of 103 patients undergoing cardiac surgery with CPB were enrolled. Venous blood samples were collected at preoperation, postoperative 12 h and 3 days. Additionally, 50 age- and gender-matched healthy volunteers served as a control group. EVs were isolated using gradient centrifugation, and their size distribution was assessed by dynamic light scattering (DLS). The relationship between EV size characteristics, including peak diameter, peak height, and interquartile range( IQR), and postoperative coagulation function was analyzed. Results:Compared to patients with normal postoperative coagulation function, those with postoperative coagulation dysfunction had lower size at peak and IQR, and significantly higher peak intensity. Logistic regression analysis indicated that elevated peak intensity and lower size at peak and IQR were risk factors for coagulation dysfunction. The area under the curve ( AUC) for diagnosing coagulation dysfunction with 12 h postoperative EVs peak intensity was 0.76, with a positive predictive value of 85% at the optimal cutoff of 8.2; the AUC for IQR was 0.84, with a sensitivity of 83%, specificity of 82%, and negative predictive value of 86% at the optimal cutoff of 125.05 nm. Conclusion:The size distribution of circulating EVs show a correlation with coagulation function after cardiac surgery with CPB and may serve as a novel biomarker to predict postoperative coagulation dysfunction.
2.Clinical observation of improving pain,shoulder function,and shoulder muscle strength with DING's Tuina combined with Western medication in patients with rotator cuff injury

Objective:To observe the clinical efficacy of DING's Tuina(Chinese therapeutic massage)plus Western medication in treating rotator cuff injury(RCI)and its impact on pain,shoulder function,and shoulder muscle strength in the patients.Methods:Sixty-two RCI patients were divided into an observation group and a control group using a simple randomization method,each consisting of 31 cases.Both groups of patients were prescribed oral celecoxib capsules.In addition,DING's Tuina treatment was offered to the observation group.After 6 weeks of treatment,the two groups were observed for changes in the Constant-Murley score(CMS)for shoulder joint function,the short-form McGill pain questionnaire(SF-MPQ)score,and peak torque(PT)and peak torque to body mass ratio(PT/BM)at angular velocities 120 °/s and 90 °/s as well as the peak torque ratio(PTR)of agonist muscles to antagonist muscles[including adductors to abductors(ADD/ABD)and flexors to extensors(F/E)].Results:After treatment,both groups presented decreased SF-MPQ scores and increased CMS,and the intra-group differences were statistically significant(P<0.05);the observation group had a lower SF-MPQ score and a higher CMS than the control group,showing statistical significance(P<0.05).After the intervention,the ADD/ABD decreased in the control group at the angular velocity 120 °/s in abduction,and the intra-group change was statistically significant(P<0.05);in the observation group,the PT and PT/BM increased compared to the baseline at angular velocities 120 °/s and 90 °/s in abduction,adduction,forward flexion,and backward extension,showing statistical significance(P<0.05).After treatment,the PT and PT/BM at angular velocities 120 °/s and 90 °/s and ADD/ABD at 120 °/s were higher in the observation group than in the control group,and the between-group differences were statistically significant(P<0.05).Conclusion:DING's Tuina combined with oral celecoxib capsules can reduce pain,improve the joint's motor function,and enhance the strength of shoulder muscle groups in RCI patients,and this treatment has a positive impact on the stability when performing shoulder joint abduction and adduction.

5.Study on characteristic chromatogram of Chaenomeles sinensis and content determination of 3 flavones

OBJECTIVE To establish the characteristic chromatogram of Chaenomeles sinensis, determine the contents of rutin, hyperin and quercitrin, and to identify C. sinensis and C. speciosa. METHODS HPLC method was performed on Agilent 5 TC-C18 column, with acetonitrile-0.2% formic acid solution as the mobile phase for gradient elution, at the flow rate of 1.0 mL/min. The column temperature was 30 ℃ . The detection wavelength was 330 nm in characteristic chromatogram and 350 nm in content determination. The characteristic chromatogram of C. sinensis was established and similarity was evaluated by the Similarity Evaluation System for Chromatographic Fingerprint of TCM (2012 edition). Hierarchical cluster analysis of 15 batches of C. sinensis (S1-S15) was performed by using SPSS 23.0 software. The contents of 3 flavones in 15 batches of C. sinensis and 7 batches of C. speciosa (S16-S22) were determined, while their characteristic chromatograms were compared. RESULTS The similarities of the characteristic chromatogram for 15 batches of C. sinensis ranged from 0.783 to 0.969, and 11 characteristic peaks were confirmed. Four constituents were identified as chlorogenic acid, rutin, hyperin and quercitrin. The medicinal materials in 15 batches of C. sinensis could be divided into 2 categories: S5-S8 were one category, and the others belonged to one category. The characteristic chromatogram of C. sinensis was obviously different from C. speciosa. The contents of rutin, hyperin and quercitrin in 15 batches of C. sinensis were 48.99-294.45, 3.49-102.55, 31.98-149.49 μg/g, respectively. The content of rutin in C. speciosa was lower than that in C. sinensis. None of hyperin (except for S20) and quercitrin were detected in C. speciosa. CONCLUSIONS The characteristic chromatogram and the method for content determination of 3 flavones in C. sinensis are established successfully and can be used for the quality control of C. sinensis and its identification from C. speciosa.
6.Optimization of the processing of vinegar-Corni Fructus through Box-Behnken design combined with entropy-weight method

Objective:To optimize the processing technology of vinegar-Corni Fructus using Box Behnken design and entropy weight method.Methods:The optimal processing parameters of vinegar-Corni Fructus were optimized by Box-Behnken design. 5-Hydroxymethylfurfural, Nuxaside, Monosanthin, and appearance character were used as comprehensive evaluation indexes. The entropy weight method was used to calculate the weights of each index, taking Design Expert 12.0 for data analysis.Results:The optimal process for vinegar-Corni Fructus as follow: The ratio of excipients was 15%, the simmering time was 2 h, and the steaming time was 8 h.Conclusion:The optimal vinegar-Corni Fructus processing is stable and feasible, which can be used for the production of processed prescriptions.
7.The effect of mechanical chest compression device on resuscitation in patients with cardiac arrest: A meta-analysis

Objective:To evaluate the effect of mechanical chest compression device in patients with cardiac arrest.Methods:The relevant literatures about mechanical cardiopulmonary resuscitation and manual cardiopulmonary resuscitation were systematically searched from China Knowledge Network (CNKI), VIP, Wanfang, PubMed, Web of Science and other databases. The effective data were extracted and analyzed by RevMan5.3 software.Results:A total of 20 clinical studies involving 29 727 patients were included, of which 11 104 patients received mechanical cardiopulmonary resuscitation and 18 623 patients received traditional manual cardiopulmonary resuscitation. The results of meta-analysis showed that mechanical cardiopulmonary resuscitation could not effectively improve the restoration of spontaneous circulation (ROSC) rate, admission survival rate, discharge survival rate and neurological prognosis in patients with cardiac arrest compared with manual cardiopulmonary resuscitation. ROSC occurrence rate ( RR=1.10, 95% CI: 0.99-1.23, P<0.01), admission survival rate ( RR=1.01, 95% CI: 0.95-1.08, P=0.67), discharge survival rate ( RR=1.00, 95% CI: 0.86-1.15, P=0.14), and good neurological function rate ( RR=0.81, 95% CI: 0.61-1.06, P=0.69) showed no significant differences between the mechanical cardiopulmonary resuscitation and manual cardiopulmonary resuscitation. Conclusions:Mechanical chest compression device has no advantage compared with manual cardiopulmonary resuscitation. Mechanical cardiopulmonary resuscitation is not recommended to completely replace manual chest compression in cardiopulmonary resuscitation.
8.Alterations of gray matter volume and functional connectivity in parents with chronic posttraumatis stress disorder who lost their only child

Objective:To investigate the alterations of brain gray matter volume (GMV) and functional connectivity (FC) in parents with chronic posttraumatic stress disorder (PTSD) who lost their only child. The correlation of the GMV and FC with the severity of CAPS symptoms was also explored.Methods:A total of 19 parents with posttraumatic stress disorder (PTSD group), 28 parents without PTSD (non-PTSD group), and 27 healthy controls (HCs), admitted in our hospital from October 2017 to May 2019, were recruited in our study. The three groups were matched in age, gender and education level. All the participants were scanned by 3.0T MRI examination, followed by a clinical evaluation with the Clinician Administered PTSD Scale (CAPS) for the PTSD diagnosis and to assess the severity. A whole-brain voxel-based morphometry (VBM) approach was used to characterize the GMV. The brain regions with significant differences in gray matter volume between the PTSD group and the non-PTSD group were used as seed points, and based on the seed points, the whole brain was explored its functional connection. The ANOVA and two sample t-test were used to compare the differences in GMV and FC values among groups. The AlphaSim correction was carried out and P<0.05 was considered statistically significant. The correlations of the GMV and FC values in significant regions with clinical scale scores were also analyzed. Results:Compared to the non-PTSD group, the PTSD patients showed decreased GMV in the left superior frontal gyrus and right precuneus and increased GMV in left fusiform gyrus; compared to the HCs, the non-PTSD group showed increased GMV in right hippocampus, right rolandic operculum and right precuneus. Seed-based FC analyses showed that the PTSD patients had decreased FC in bilateral orbitofrontal gyrus, left cerebellum and left hippocampus, which were positively correlated with the CAPS scores; while increased FC in right insula and right angular gyrus, which were negatively correlated with the CAPS scores (all P<0.01). In the PTSD group, GMV in the left fusiform gyrus was negatively correlated with the avoidance/numbing symptom ( r=-0.555, P=0.014), while positively associated with the time since trauma ( r=0.457, P=0.049). Conclusion:There are alterations of GMV and functional connectivity in multiple brain regions in parents who have lost their only child. Less connectivity in the prefrontal-medial temporal lobe (MTL) circuit and more activation of the key node within fear network may contribute to the development of PTSD, while the structural compensation in brain region associated with visual processing may be an important factor in relieving PTSD symptom and PTSD duration.
