1.Construction of a COPD risk prediction model based on machine learning and the COPD-SQ questionnaire

ObjectiveTo construct and evaluate various machine learning models for predicting the risk of chronic obstructive pulmonary disease (COPD) in individuals, thereby providing data support for early screening and intervention. MethodsA total of 823 subjects were selected for this study, comprising 142 individuals in the high-risk group for COPD and 681 individuals in the low-risk group. Data collected included demographic characteristics, smoking history, symptoms (such as cough and shortness of breath), and scores from the Chronic obstructive pulmonary disease screening questionnaire. Four machine learning algorithms—Logistic regression, random forest, support vector machine, and XGBoost—were utilized to construct risk prediction models. The performance of these models was assessed using 5-fold cross-validation, with evaluation metrics including accuracy, precision, recall, F1-score, area under the receiver operating characteristic curve (AUC), and average precision (AP). Furthermore, a feature importance analysis was performed. ResultsThe Logistic Regression model exhibited superior performance, achieving an AUC of 0.982 and an AP of 0.939. This was closely followed by the Random Forest model, which recorded an AUC of 0.975 and an AP of 0.890. Feature importance analysis revealed that smoking history, symptoms of shortness of breath, and body weight were significant predictors. All models demonstrated robust performance in identifying low-risk populations; however, variations were observed in their efficacy in identifying high-risk populations. ConclusionMachine learning models have proven effective in identifying individuals at high risk for COPD. Among these, the Logistic regression model exhibits the best overall performance, efficiently identifying high-risk populations and serving as a valuable clinical auxiliary screening tool. Various models, each with distinct performance characteristics, are suited to different clinical screening scenarios, thereby offering targeted decision-making support for the establishment of a hierarchical and intelligent COPD screening pathway.

3.Correlation between remnant cholesterol and contrast-induced acute kidney injury in elderly diabetes patients with ACS after PCI

Objective To explore the correlation between remnant cholesterol level and CI-AKI in elderly patients with ACS and concomitant T2DM following PCI.Methods A retrospective case-control study was conducted on 759 elderly T2DM patients with ACS undergoing PCI in the Affi-liated Hospital of Xuzhou Medical University between January 2020 and December 2023.The pa-tients were randomly allocated into a training group(n=608)and a validation group(n=151)in a ratio of 8∶2.Based on CI-AKI diagnostic criteria,the training group was further stratified into a CI-AKI subgroup(n=95)and a non-CI-AKI subgroup(n=513).Multivariate logistic regression analysis was used to identify the independent risk factors for CI-AKI.Results The remnant cho-lesterol level was significantly higher in the CI-AKI subgroup than the non-CI-AKI subgroup[0.78(0.51,1.07)mmol/L vs 0.57(0.40,0.81)mmol/L,P<0.01].Red blood cell count,remnant cholesterol,age,uric acid,log-transformed systemic immune-inflammatory(SII)index,and cysta-tin C were identified as independent predictors for CI-AKI in T2DM patients with ACS after PCI(P<0.01).ROC curve analysis demonstrated that the model combining remnant cholesterol with other independent predictors exhibited superior discriminative performance in both the training group(AUC=0.818,95%CI:0.773-0.863)and validation group(AUC=0.787,95%CI:0.686-0.889)when compared to the models excluding remnant cholesterol(P<0.01).Conclusion Rem-nant cholesterol is an independent predictor of CI-AKI in elderly patients with ACS and T2DM af-ter PCI.A risk prediction model based on remnant cholesterol and other independent risk factors demonstrates enhanced predictive performance for postoperative CI-AKI in the patients.
4.Msx2 regulates differentiation of outer enamel epithelial cells by modu-lating cytoskeleton and cell-cell interactions

AIM:To investigate the mechanism by which muscle segment homeobox 2(Msx2)regulates the differentiation of outer enamel epithelial cells in the enamel organ.METHODS:Tissue paraffin sections were prepared and subjected to hematoxylin-eosin(HE)staining to analyze the effect of Msx2 deficiency on the differentiation status of epithelial cells in the enamel organ at the morphological level.At the ultrastructural level,alterations in cell structure were analyzed.The intermediate steps mediating cell differentiation were identified.Transcriptome sequencing analysis was performed to validate the molecular mechanisms underlying the observed phenomena.RESULTS:Msx2 deficiency was innovatively found to induce severe squamous epithelial hyperplasia in outer enamel epithelial cells of enamel organ,accompanied by dynamic restructuring of the cell cytoskeleton and alterations in cell adhesion at the ultrastructure level.As a transcriptional repressor,the loss of Msx2 expression results in significant increases(P<0.05 or P<0.01)in the mRNA expression levels of integrin β2(Itgβ2),ItgαM,Itgα4,Rac family small GTPase 2(Rac2),Rac/Cdc42 guanine nucleo-tide exchange factor 6(Arhgef6)and protein tyrosine phosphatase receptor type C(Ptprc).CONCLUSION:Msx2 regu-lates cytoskeleton structure and cell-cell interaction through the Rho GTPases signaling pathway,thereby influencing the differentiation state of outer enamel epithelial cells.This study reveals the mechanism through which Msx2 regulates the differentiation of outer enamel epithelial cells,providing a theoretical foundation for the prevention and treatment of enam-el-related clinical dental diseases.
5.Mechanism of different sources of mesenchymal stem cells in treatment of premature ovarian failure

BACKGROUND:A large number of cell and animal tests have confirmed the effect of mesenchymal stem cells on improving ovarian function,and some clinical trials have been completed and their effectiveness has been preliminarily confirmed,bringing hope to women with premature ovarian failure.OBJECTIVE:To summarize and analyze the mechanism,research progress,and related clinical trials of mesenchymal stem cells from different sources in the treatment of premature ovarian failure in recent years,so as to provide a theoretical basis for further research and clinical application of mesenchymal stem cell therapy for premature ovarian failure.METHODS:Using "mesenchymal stem cells,premature ovarian failure" as keywords in Chinese and English,the relevant literature was searched in CNKI,WanFang Data,Chinese Medical Database,and PubMed database,and finally 72 articles that met the requirements were included for review.RESULTS AND CONCLUSION:Currently,there are seven types of commonly used mesenchymal stem cells for premature ovarian failure,which are umbilical cord mesenchymal stem cells,bone marrow mesenchymal stem cells,placental mesenchymal stem cells,menstrual blood mesenchymal stem cells,amniotic mesenchymal stem cells,amniotic fluid mesenchymal stem cells,and adipose mesenchymal stem cells.The mechanisms include inhibiting apoptosis and promoting proliferation,anti-inflammatory and inhibiting oxidative stress,homing,promoting angiogenesis,anti-fibrosis,parasecretory,immune regulation,autophagy,and improving microenvironment.Cell and animal experiments have proven that different sources of mesenchymal stem cells can have better intervention effect on premature ovarian failure through various mechanisms,and delay the progress of premature ovarian failure to a certain extent.If it can be successfully applied to the clinic in the future,it can alleviate the psychological and physical pain of patients to a great extent.However,due to the lack of comprehensive and accurate evidence in clinical studies such as stem cell source,administration mode and dose,adverse reactions,etc.,further studies are still needed to confirm it in the future,and its long-term safety needs to be further observed.
6.Practical application research on discipline-specific research performance evaluation in a tertiary pub-lic hospital in Ningxia

Objective This study focuses on a tertiary public hospital in Ningxia to explore the practical application of discipline-specific research performance evaluation.To establish a performance evaluation index system tailored to the characteris-tics of tertiary public hospitals in underdeveloped regions and propose strategies for improving research performance evaluation through empirical research,thereby promoting high-quality hospital development.Methods Guided by the performance evalua-tion indicators for tertiary public hospitals and the accreditation standards for tertiary hospitals,and aligned with the hospital's o-verall work plan,a multi-dimensional and multi-level evaluation method was adopted.Following the SMART principles(Specific,Measurable,Achievable,Relevant,Time-bound)and differentiated scoring principles,research performance evaluation indica-tors were summarized,screened,and weighted.An empirical analysis of the research status of 20 disciplines from 2021 to 2024 was conducted to establish and continuously optimize a research performance evaluation index system suited to the hospital's needs.Results A research performance evaluation index system for public hospital disciplines was finalized,comprising 5 first-level indicators,14 second-level indicators,and 40 third-level indicators(22 quantitative and 18 qualitative).This system standardized the management of discipline-specific research and effectively promoted steady growth,structural adjustment,and development in hospital research.Conclusion Constructing a scientific,standardized,and operable research performance eval-uation index system is of significant importance for enhancing the research level of disciplines in public hospitals and strengthening discipline construction.
7.Correlation between AIP and SIRI with Contrast-induced Acute Kidney Injury after PCI in Hypertensive Patients Complicated with Acute STEMI

Objective To investigate the predictive value of the atherogenic index of plasma(AIP)and systemic inflammatory re-sponse index(SIRI)for contrast-induced acute kidney injury(CI-AKI)in patients with hypertension complicated by acute ST-seg-ment elevation myocardial infarction(STEMI)undergoing emergency percutaneous coronary intervention(PCI).Methods A total of 964hypertensive patients with acute STEMI who underwent PCI treatment at the Affiliated Hospital of Xuzhou Medical University from Jan-uary 2021 to April 2025 were retrospectively enrolled.Using a randomized grouping method,the sample was divided into the training group(n=771)and the validation group(n=193)in a ratio of 8∶2.According to the diagnostic criteria for CI-AKI,the training group was further divided into the CI-AKI group(n=151)and the non-CI-AKI group(n=620),with baseline characteristics compared between the two groups.Multivariate Logistic regression models were constructed to identify independent predictors of postoperative CI-AKI,followed by receiver operating characteristic(ROC)curve analysis to evaluate the predictive efficacy of AIP,SIRI,and their combined detection for CI-AKI occurrence after PCI treatment.Results Left ventricular ejection fraction(LVEF),urea metabolism level,high-density lipoprotein cholesterol(HDL-C),diuretics,SIRI,and AIP showed significant correlations with the occurrence of CI-AKI in hypertensive patients with acute STEMI after PCI(P<0.05).Multivariate Logistic regression analysis revealed that SIRI(OR=1.148,95%CI:1.060-1.244)and AIP(OR=5.946,95%CI:3.250-10.879)were independent risk factors for CI-AKI.The results of ROC curve analysis demonstrated that the combined detection of SIRI and AIP yielded an area under the curve of 0.805(95%CI:0.774-0.835),with the sensitivity of 70.8%and the specificity of 75.0%,which was significantly better than that of indi-vidual indicators(P<0.001).The results of restricted cubic spline(RCS)indicated that when SIRI ≥ 2.773 or AIP ≥2.089,the risk of CI-AKI increased with rising levels of SIRI and AIP.Conclusion AIP and SIRI are independent risk factors for CI-AKI in hyperten-sive patients with acute STEMI after PCI,and their combined detection can improve the predictive efficacy of CI-AKI in hypertensive pa-tients with acute STEMI following PCI.
