1.Current status of eating behaviors and its predictive role in overweight and obese of adolescents

Objective:
To explore the current status and influencing factors of eating behaviors in adolescents, so as to provide a theoretical foundation for health promotion education among adolescents.
Methods:
Based on the database from Survey of Chinese Family Health Index (2021), by a random number table method, 1 065 teenagers were selected from the provincial capitals of 22 provinces and 5 autonomous regions in China, as well as 4 municipalities directly under the central government. A general characteristic questionnaire, Patient Health Questionnaire-9 (PHQ-9), Short Form of the Family Health Scale (FHS-SF), 10-item Short Version of the Big Five Personality(BFP-10), Content-based Media Exposure Scale (CM-E) and Sakata Eating Behavior Scale Short Form(EBS-SF) were used to collect information. Multivariate stepwise linear regression analysis was employed to identify and analyze related factors of eating behaviors among adolescents. Receiver operating characteristic was used to validate the predictive ability of the EBS-SF score for overweight and obesity among adolescents.
Results:
The average scores of BFI-10,C-ME, FHS-SF, PHQ-9 and EBS-SF were (33.08±4.64)(19.20±4.55)(38.48±6.65)(6.09±5.63)(16.75±4.36), respectively. Multivariate linear regression showed that family type (other types), agreeableness, conscientiousness, family health and depression were the main related factors of EBS-SF scores among adolescents( B =2.61,-0.42,0.20,-0.11,0.23, P <0.05).The analysis of receiver operating characteristic curve showed that the EBS-SF scores had a good ability in predicting obesity among male adolescents ( AUC= 0.73, P <0.01).
Conclusions
Family type, big five personality, family health,depression are the related factors of eating behaviors among adolescents. EBS-SF scores are predictive of obesity in adolescents, which would provide a new perspective for promoting healthy eating habits among adolescents.
2.Application value of part-cut jejunal transection in digestive tract reconstruction of totally laparoscopic total gastrectomy

Objective:To explore the application value of part-cut jejunal transection in digestive tract reconstruction of totally laparoscopic total gastrectomy.Methods:The propensity score matching and retrospective cohort study was conducted. The clinicopathological data of 112 patients with gastric cancer who underwent totally laparoscopic total gastrectomy in The Second Affiliated Hospital of Anhui Medical University from June 2018 to September 2022 were collected. There were 81 males and 31 females, aged (70±8)years. Among the 112 patients, 60 patients undergoing diges-tive tract reconstruction by Roux-en-Y anastomosis with part-cut jejunum were set as the part-cut group, and 52 patients undergoing digestive tract reconstruction by traditional Roux-en-Y anasto-mosis were set as the traditional group. Observation indicators: (1) propensity score matching status and comparison of clinical data of patients between the two groups after matching; (2) intraopera-tive and postoperative conditions; (3) follow-up. Comparison of measurement data with normal dis-tribution between groups was conducted using the independent sample t test. Comparison of count data between groups was conducted using the chi-square test or Fisher exact probability. Com-parison of ordinal data was conducted using the nonparametic rank sum test. Propensity score matching was performed using the 1∶1 nearest neighbor matching method, with the caliper value of 0.02. Results:(1) Propensity score matching status and comparison of clinical data of patients between the two groups after matching. Of the 112 patients, 90 patients were successfully matched, with 45 cases in each of the part-cut group and the traditional group. After propensity score matching, the elimination of body mass index, clinical TNM staging confounding bias ensured comparability. (2) Intraoperative and postoperative conditions. After propensity score matching, the total operation time and digestive tract reconstruction time of patients in the part-cut group were (217.0±15.1)minutes and (34.7±1.8)minutes, versus (252.6±21.9)minutes and (52.6±7.4)minutes in the traditional group, respectively, showing significant differences in the above indicators between the two groups ( t=?8.97, ?15.66, P<0.05). (3) Follow-up. After propensity score matching, 90 patients were followed up postoperatively for (47±15)months. During the follow-up, no patient in either group received secondary surgery, and there was no death. There were 3 cases and 10 cases of Roux stasis syndrome in the part-cut group and the traditional group, respectively, showing a significant difference between the two groups ( χ2=4.41, P<0.05). Conclusion:Compared with traditional Roux-en-Y anastomosis, the Roux-en-Y anastomosis with part-cut jejunum in totally laparoscopic total gastrectomy can signifi-cantly shorten the time for digestive tract reconstruction and reduce the incidence of postoperative Roux stasis syndrome.
3.2D SECara-Net and 3D U2-Net for detecting unruptured saccular intracranial aneurysms with MR angiography

Objective To observe the value of 2D SECara-Net and 3D U2-Net models constructed based on 2D maximal intensity projection(MIP)and 3D time-of-flight MR angiography(3D TOF-MRA)images,respectively,also of their combination for MRA detecting unruptured saccular intracranial aneurysms(USIA).Methods Totally 973 patients with single USIA and 300 subjects who underwent healthy physical examination were retrospectively collected and divided into training set(n=923,containing 723 cases of USIA and 200 healthy subjects)and test set(n=350,containing 250 cases of USIA and 100 healthy subjects)at the ratio of 7:3.Pre-processed 3D TOF-MRA and the obtained 2D-MIP images in training set were imported into 3D U2-Net and 2D SECara-Net models for training and adjusting parameters,respectively.The efficiency of 2 models and their combination for detecting USIA were evaluated.Results The sensitivity,specificity and accuracy of 2D SECara-Net model for detecting USIA in test set was 78.80%(197/250),95.00%(95/100)and 83.43%(292/350),of 3D U2-Net model was 82.80%(207/250),86.00%(86/100)and 83.71%(293/350),respectively.The specificity of 2D SECara-Net model was higher than that of 3D U2-Net model(P=0.030),while no significant difference of sensitivity nor accuracy was found between 2 models(both P>0.05).The specificity of the combination of the 2 models was 99.00%(99/100),higher than that of 3D U2-Net model(P<0.05),and the sensitivity and accuracy of the combination was 91.20%(228/250)and 93.43%(327/350),respectivelty,both higher than those of 2 single models(all P<0.05).Conclusion 2D SECara-Net and 3D U2-Net models had similar,sensitivity and accuracy for MRA detecting USIA.Combination of them could improve the detecting efficacy.
4.Design and practice of critical care humanistic nursing curriculum under the perspective of"great ideological and political education"

Objective To construct a critical care humanistic nursing curriculum under the perspective"great ideological and political education",and evaluate its effects in the training of intensive care unit(ICU)standardized training nurses.Methods A research team was established and organized by department of nursing of Zhongshan Hospital,Fudan University,which consisting of 6 critical care nursing managers and specialists,including 1 chief nurse,3 head nurses,1 master's graduate,and 1 doctoral graduate.The team was responsible for literature review,questionnaire design,expert recruitment,survey distribution and collection,data integration,and analysis.A preliminary questionnaire was developed through literature reviews on critical care humanistic nursing,brainstorming sessions,and clinical surveys.A two-round Delphi method was conducted to finalize the humanistic nursing training curriculum.Ideological and political elements,such as political identity,patriotism,moral cultivation,legal awareness,scientific spirit and professional quality were integrated into the program.From July 2023 to June 2024,newly enrolled ICU standardized training nurses at Zhongshan Hospital,Fudan University participated in the training.Training outcomes were assessed via theoretical examinations and pre-and post-training evaluations of humanistic competency.Results A total of 10 experts participated in the consultation,resulting in a training course consisting of two primaryl indicators and 11 secondary indicators.A total of 60 nurses participated in the training,achieving an average post-training theoretical score of 84.92±7.73.Significant improvements were observed across all dimensions of humanistic competency post-training compared to pre-training(humanistic care practice competency:33.95±4.50 vs.30.82±4.10,communication skills:20.27±2.68 vs.18.98±2.98,self-management:10.07±1.91 vs.9.20±2.02,ethical and legal competence:10.38±1.61 vs.9.68±2.02,psychological adaptability:14.20±2.31 vs.13.03±2.19,and total humanistic practice competency score:88.87±7.65 vs.81.71±8.55),with all differences statistically significant(all P<0.05).Conclusion The critical care humanistic nursing curriculum under the perspective of"great ideological and political education"can effectively improve the humanistic quality of nurses and enhance their ICU practical skills,so as to provide reference for the training of nurses in intensive care.

6.Construction and implementation of a blood glucose chain management model for critically ill patients after cardiac surgery

Objective:To establish a chain management model for blood glucose in critically ill patients after cardiac surgery and analyze its clinical effectiveness, and to provide a reference for related clinical nursing practices.Methods:A quasi-experimental study design was adopted. Using convenience sampling, 120 critically ill patients after cardiac surgery admitted to Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine were selected as study subjects. They were divided into groups based on the time of admission to the intensive care unit after surgery. Sixty patients admitted from March to May 2024 were assigned to the control group and received conventional blood glucose management. Another 60 patients admitted from June to August 2024 were assigned to the observation group and received the chain management model for blood glucose control in addition to the conventional approach. Blood glucose levels and quality indicators of blood glucose management (including maximum blood glucose fluctuation, time in target glucose range, duration of insulin use, incidence of hypoglycemia, etc.) were compared between the two groups.Results:The control group included 27 males and 33 females, with an age of 63.00(59.00, 69.25) years; the observation group included 28 males and 32 females, with an age of 66.00(60.00, 70.00) years. The blood glucose levels of the observation group on postoperative days 2, 3, 4 were 9.10(8.68, 9.90), 8.90(8.40, 10.00), 8.75(7.38, 9.03) mmol/L, respectively, which were lower than those of the control group [10.30(9.80, 11.00), 9.95(9.40, 11.05), 9.30(8.10, 10.02) mmol/L], with a statistically significant difference ( Z=-5.85, -4.95, -3.50, all P<0.05). The maximum blood glucose fluctuation in the observation group was (4.09 ± 2.45) mmol/L, lower than that of the control group [(5.19 ± 2.47) mmol/L], with a statistically significant difference ( t=2.46, P<0.05). The time in the target glucose range was 67.00(60.00, 75.00)% in the observation group, higher than that of the control group 52.00(45.00, 60.00)%, with a statistically significant difference ( Z=-6.57, P<0.05). The duration of insulin use was 6.00(5.00, 7.00) h in the observation group, shorter than that of the control group [13.00(9.75, 15.32) h], with a statistically significant difference ( Z=-8.68, P<0.05). The incidence of hypoglycemia was 3.33%(2/60) in the observation group and 15.00%(9/60) in the control group, with a statistically significant difference ( χ2=4.90, P<0.05). The mechanical ventilation time, ICU stay, and total hospital stay in the observation group were 42.00(37.00, 89.25) h, 6.00(5.00, 7.00) d, and 12.00(11.75, 13.00) d, respectively, which were shorter than those of the control group [96.00(86.25, 98.00) h, 7.00(7.00, 10.00) d, and 13.00(11.75, 15.00) d], with a statistically significant difference ( Z=8.67, 17.57, 4.73, all P<0.05). Conclusions:The implemented chain management model for blood glucose control meets the comprehensive requirements of blood glucose management. It not only reduces blood glucose fluctuations and decreases the incidence of hypoglycemia but also effectively improves the quality of blood glucose management in critically ill patients after cardiac surgery, enhances the safety of blood glucose control, and promotes patient recovery.
7.Efficacy of nasogastric administration of sesame oil combined with multi-frequency vibrational abdominal massage in stroke patients with constipation undergoing nasogastric feeding

Objective To investigate the clinical efficacy of fasting nasogastric administration of sesame oil combined with multi-frequency vibrational abdominal massage in stroke patients with consti-pation undergoing nasogastric feeding.Methods A total of 50 stroke patients with nasogastric feeding were selected as study subjects and randomly divided into control group and study group,with 25 pa-tients in each group.The control group received routine nursing care plus multi-frequency vibrational abdominal massage,while the study group received fasting nasogastric administration of 20 mL of sesa-me oil on the basis of the control group's treatment.After 15 days of intervention,the defecation condi-tions[constipation clinical symptom score(CSS),stool consistency(Bristol stool scale),and stool volume]were compared between the two groups.Results After treatment,the CSS score in the study group was lower than that in the control group[(6.52±2.52)versus(12.64±3.32),P<0.05].The normal stool consistency(types Ⅳ and Ⅴ on the Bristol stool scale)in the study group was higher than that in the control group(76.0%versus 8.0%,P<0.05).The stool volume in the study group was higher than that in the control group[(303.00±93.79)g versus(196.40±60.27)g,P<0.05].The total effective rate of defecation in the study group was 96.0%,which was higher than 32.0%in the control group(P<0.05).The Patient Assessment of Constipation-Quality of Life(PAC-QOL)score in the study group was lower than that in the control group[(40.07±5.67)versus(63.07±7.46),P<0.05].Conclusion Fasting nasogastric administration of sesame oil com-bined with multi-frequency vibrational abdominal massage can promote the recovery of intestinal function and improve constipation symptoms in stroke patients with nasogastric feeding.
8.Efficacy of toripalimab combined with bronchial arterial chemoembolization and intensity-modulated radiotherapy in advanced lung cancer

Objective To observe the clinical efficacy of toripalimab combined with bronchial ar-terial chemoembolization(BACE)and intensity-modulated radiotherapy(IMRT)in advanced lung cancer.Methods A prospective single-arm trial was conducted in 104 patients with programmed death-ligand 1(PD-L1)-positive,driver gene-negative non-small cell lung cancer(NSCLC)in stages of Ⅲ B to Ⅳ admitted to the First People's Hospital of Guangyuan City of Sichuan Province.All patients received toripalimab combined with BACE and IMRT.Clinical efficacy,symptom improve-ment time,tumor biomarker levels[carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199),cytokeratin 19 fragment(CYFRA21-1),neuron-specific enolase(NSE)],T-lymphocyte subsets(CD3+,CD4+,CD4+/CD8+),survival outcomes,and adverse events were analyzed.Results Among 102 patients,the objective response rate(ORR)was 75.49%,disease control rate(DCR)was 90.20%,survival rate was 68.63%,and 12-month progression-free survival rate was 62.75%.The overall incidence of adverse events of any grade was 72.55%.Post-BACE,post-IMRT,and post-toripalimab treatment levels of CEA,CYFRA21-1,CA199,and NSE were significantly lower than baseline data(P<0.05),with the lowest levels observed after toripalimab treatment compared to post-BACE and post-IMRT(P<0.05).CD3+,CD4+,and CD4+/CD8+decreased after BACE,IMRT and toripalimab therapy,but they were increased following toripalimab therapy compared with the other two therapies(P<0.05).Conclusion Toripalimab combined with BACE and IMRT demonstrates significant clinical efficacy and acceptable tolerability in PD-L1-positive,driver gene-negative NSCLC in stages of ⅢB to Ⅳ,serving as a preferred consolidation regimen after unresect-able chemoradiotherapy.
9.Analysis of concurrent symptom networks and identification of symptom clusters in kidney transplant recipients

Objective To explore the symptom clusters in renal transplant recipients and con-struct a concurrent symptom network to identify core symptoms.Methods A total of 343 patients with followed up after renal transplantation were selected as the study subjects.A general information questionnaire and the Chinese version of the Modified Transplant Symptom Occurrence and Symptom Distress Scale were employed to analyze the occurrence of symptoms in patients.In this study,only symptoms with an incidence rate greater than 20%and Spearman correlation coefficient greater than 0.40 between symptom severity and total score were retained.Exploratory factor analysis was used to extract symptom clusters with a factor loading of ≥0.45 as the criterion.The R language was utilized to construct symptom network,based on which core symptoms and bridge symptoms were identified.Results A total of 5 symptom clusters were extracted in this study:the neuro-gastrointestinal symp-tom cluster,the mood-related symptom cluster,the hormone-related symptom cluster,the energy de-ficiency symptom cluster and the vision-related symptom cluster.The core symptoms were anxiety(rs=1.75),mood swings(rs=1.50)and muscle weakness(rs=1.27).The top three symptoms in terms of bridge strength were muscle weakness(rb=0.87),lack of vitality(rb=0.66)and fa-tigue(rb=0.65).Conclusion Multiple symptoms are presented in patients after renal transplanta-tion.Based on the results of symptom network analysis,clinicians can strengthen the assessment of core symptoms and bridge symptoms to develop precise intervention strategies and improve the effectiveness of symptom management.
10.Development and application of scoring standard for nasal swab collection technique for internal contamination

Objective:To establish the scoring standard of nasal swab collection technique for internal contamination, and to test its clinical effect, so as to standardize the nasal swab collection process and improve the medical rescue ability against nuclear radiation.Methods:The scoring standard of nasal swab collection technique for internal contamination was developed on the basis of literature research, expert consultation and pre-test. It was applied for training and assessment of 55 nurses, and the difference in scores before and after training was analyzed to test the application effect.Results:The effective response rate of expert inquiry questionnaire was 93.3%, the expert familiarity 0.914, the judgment basis 0.921, and the expert authority coefficient 0.917. Finally, the technical scoring standard for nasal swab collection for internal contamination was developed through three stages of preparation, sampling and arrangement. Through practical application, the score after training (94.98±2.84) was significantly better than that before training (81.37±10.45), with statistically significant difference ( t=8.63, P<0.05). Conclusions:The scoring standard of nasal swab collection technique for internal contamination, developed in this study, is reliable, innovative and practical. It is suitable for clinical practice and training and helpful to standardize the collection process and improve the rescue ability in a nuclear emergency.
