2.Effect of miR-132-3p on epithelial-mesenchymal transition of trophoblast cells by targeting the SIRT1/NF-κB pathway

Objective To investigate the effect of miR-132-3p on epithelial-mesenchymal transition(EMT)of trophoblast cells through targeted regulation of the SIRT1/NF-κB pathway.Methods Placental tissues from 37 pre-eclampsia(PE)patients who underwent cesarean section in Zhengzhou Women&Infants Hospital and 37 normal pregnant women who underwent cesarean section were collected as the research group and control group,respectively.RT-qPCR was performed to detect the expression of miR-132-3p and SIRT1 in tissues.During the logarithmic pro-liferation phase,HTR-8/SVneo cells were grouped into inhibitor negative group,miR-132-3p inhibitor group,mimic negative group,miR-132-3p overexpression group,miR-132-3p inhibitor+interference negative group,and miR-132-3p inhibitor+interference SIRT1 group,with untransfected HTR-8/SVneo cells as the blank group.CCK-8,Transwell assay,scratch assay,and RT-qPCR were used to detect the proliferation,invasion,migration,and changes in miR-132-3p and SIRT1 mRNA expression of HTR-8/SVneo cells.Western blot was used to detect the expression levels of EMT related proteins and SIRT1,NF-κB proteins.Dual luciferase was used to validate the targeting relationship between miR-132-3p and SIRT1.Results The level of miR-132-3p in the research group was significantly higher than that in the control group,while the SIRT1 mRNA was lower(P<0.05).The level of miR-132-3p,E-cadherin,p-NF-κBp65/NF-κBp65 in the miR-132-3p inhibitor group was lower than those in the inhib-itor negative group and blank group.The proliferation rate,invasion cell number,migration rate,level of vimentin,N-cadherin,and SIRT1 were all higher(P<0.05).The miR-132-3p,E-cadherin,p-NF-κBp65/NF-κBp65 in the miR-132-3p overexpression group were higher than those in the mimic negative group,the proliferation rate,inva-sion number,migration rate,vimentin,N-cadherin,and SIRT1 were lower(P<0.05).The level of E-cadherin and p-NF-κBp65/NF-κBp65 in the miR-132-3p inhibitor+interference SIRT1 group was higher than those in the miR-132-3p inhibitor+interference negative group(P<0.05),the proliferation rate,invasion number,migration rate,vimentin,N-cadherin,and SIRT1 expression were lower(P<0.05).MiR-132-3p targeted regulation of SIRT1 expression(P<0.05).Conclusions Inhibition of miR-132-3p promotes the EMT,proliferation,invasion,and migration of trophoblast cells by targeting and activating the SIRT1/NF-κB pathway.
3.Effect of continuous renal replacement therapy on plasma concentration, clinical efficacy and safety of colistin sulfate

Objective:To investigate the effects of continuous renal replacement therapy (CRRT) on plasma concentration, clinical efficacy and safety of colistin sulfate.Methods:Clinical data of patients received with colistin sulfate were retrospectively analyzed from our group's previous clinical registration study, which was a prospective, multicenter observation study on the efficacy and pharmacokinetic characteristics of colistin sulfate in patients with severe infection in intensive care unit (ICU). According to whether patients received blood purification treatment, they were divided into CRRT group and non-CRRT group. Baseline data (gender, age, whether complicated with diabetes, chronic nervous system disease, etc), general data (infection of pathogens and sites, steady-state trough concentration, steady-state peak concentration, clinical efficacy, 28-day all-cause mortality, etc) and adverse event (renal injury, nervous system, skin pigmentation, etc) were collected from the two groups.Results:A total of 90 patients were enrolled, including 22 patients in the CRRT group and 68 patients in the non-CRRT group. ① There was no significant difference in gender, age, basic diseases, liver function, infection of pathogens and sites, colistin sulfate dose between the two groups. Compared with the non-CRRT group, the acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) and sequential organ failure assessment (SOFA) were higher in the CRRT group [APACHE Ⅱ: 21.77±8.26 vs. 18.01±6.34, P < 0.05; SOFA: 8.5 (7.8, 11.0) vs. 6.0 (4.0, 9.0), P < 0.01], serum creatinine level was higher [μmol/L: 162.0 (119.5, 210.5) vs. 72.0 (52.0, 117.0), P < 0.01]. ② Plasma concentration: there was no significant difference in steady-state trough concentration between CRRT group and non-CRRT group (mg/L: 0.58±0.30 vs. 0.64±0.25, P = 0.328), nor was there significant difference in steady-state peak concentration (mg/L: 1.02±0.37 vs. 1.18±0.45, P = 0.133). ③ Clinical efficacy: there was no significant difference in clinical response rate between CRRT group and non-CRRT group [68.2% (15/22) vs. 80.9% (55/68), P = 0.213]. ④ Safety: acute kidney injury occurred in 2 patients (2.9%) in the non-CRRT group. No obvious neurological symptoms and skin pigmentation were found in the two groups. Conclusions:CRRT had little effect on the elimination of colistin sulfate. Routine blood concentration monitoring (TDM) is warranted in patients received with CRRT.
4.Controversy of vitrectomy in the treatment of intraocular malignancy

Retinoblastoma (RB) and choroidal malignant melanoma are the most common primary intraocular malignancy in children and adults, respectively.Due to the advance in the diagnosis and treatment of intraocular tumors, the survival rate and survival time of the patients are significantly improved.The goal of the therapy for the patients of intraocular malignant tumors has gradually changed from simply saving life to trying to preserve eyeball on the basis of saving life.Although there are various treatment options for intraocular malignant tumors, eye salvage is still an great challenge for the patients with refractory tumors.In recent years, vitrectomy has been used again by some doctors to the treatment of refractory RB and choroidal malignant melanoma, which undoubtedly gives the hope of eye salvage to the patients with intraocular malignant tumors.However, this therapy arouse controversy because serious intraoperative or postoperative complications occurred in some patients.Ophthalmologists should select suitable indications for this surgery cautiously, perform the operation carefully, combine other chemotherapy and/or radiotherapy and local treatment and close follow-up if necessary.
5.Comparative study of interventional and conservative treatment of intracranial vertebrobasilar artery trunk large aneurysms

Objective:To evaluate the safety and efficacy of endovascular interventional treatment of the intracranial vertebrobasilar trunk large aneurysms (VBTLAs) compared with conservative treatment.Methods:This is a prospective multi-center cohort study. From October 2012 to October 2018, a total of 69 patients with intracranial vertebrobasilar trunk large aneurysm (diameter>10 mm) from Henan Province People's Hospital and People's Liberation Army Rocket Medical Center were included in this study. Patients themselves chose either endovascular interventional therapy (interventional group) or conservative treatment (conservative group) after discussion with their doctors. The χ 2 test was used to compare the incidence of deaths, stroke, and all other serious adverse events including other site bleeding, myocardial infarction and others between the two groups. Results:A total of 69 patients were enrolled, of whom 51 patients were enrolled in interventional group, 18 patients underwent endovascular reconstructive therapy, 11 patients underwent deconstructive therapy, and 4 patients underwent conjunction interventional treatments. Eighteen patients were enrolled in conservative group, of whom 11 cases received simple risk factor control, 7 cases received antiplatelet and risk factors control. The proportions of hypertensive patients 94.4% (17/18) and giant aneurysms 50.0% (9/18) in the conservative group were higher than those in the surgery group 64.7% (33/51, χ 2=4.500, P=0.034), 19.6% (10/51, χ 2=4.730, P= 0.030).The incidence of all serious adverse events associated with protocol was 15.7% (8/51) in the interventional group and 44.4% (8/18) in the conservative group [risk ratio (RR) =0.353, 95% confidence intervals (CI): 0.156-0.801], and the difference was significant (χ 2=4.668, P=0.031). The incidence of fatal events associated with protocol was 2.0% (1/51) in the interventional group and 38.9% (7/18) in the conservative group (RR=0.050, 95%CI: 0.007-0.382), and the difference was significant (χ 2=14.281, P<0.001). The incidence of hemorrhage events associated with protocol was 2.0% (1/51) in the interventional group and 22.2% (4/18) in the conservative group (RR=0.088, 95%CI: 0.011-0.738), and the difference was significant (χ 2 =5.391, P=0.020). Follow-up imaging showed that the occlusion rate of aneurysms in 44 patients in the interventional group was 56.8% (25/44) after a median follow-up of 6 months. Imaging follow-up was obtained in 9 patients, whose occlusion rate of aneurysms was 0 and the median follow-up time was 12 months, in the conservative group. The difference was significant(χ 2 =7.534, P=0.006). Conclusion:Compared with conservative treatment, endovascular intervention of the intracranial VBTLAs has lower incidences of serious adverse events and death events.

Objective:
To investigate the application value of caesarean section scar or bikini line incisional approach in laparoscopic sleeve gastrectomy.
Methods:
The retrospective cohort study was conducted. The clinical data of 162 patients with obesity and metabolic diseases who were admitted to the First Affiliated Hospital of Jinan University between March 2018 and April 2019 were collected. There were 51 males and 111 females, aged (35±8)years, with a range from 12 to 47 years. Of 162 patients, 72 undergoing laparoscopic sleeve gastrectomy via caesarean section scar or bikini line incisional approach and 90 undergoing laparoscopic sleeve gastrectomy via traditional incisional approach were respectively allocated into concealed incision group and traditional incision group. Observation indicators: (1) surgical situations; (2) intraoperative situations; (3) postoperative situations; (4) follow-up. Follow-up using outpatient examination, telephone interview, and WeChat was performed to detect the postoperative complications at 1, 3, 6 months and 1, 2, 5 years postoperatively up to May 2019. 7.Are you ready? The new era of targeted local chemotherapy for retinoblastoma is coming

Retinoblastoma (RB) therapy has evolved over decades.In the 1970s, enucleation was important for improving life prognosis.In the 1980s, external beam radiotherapy (EBRT) was popular.In the 1990s, systemic intravenous chemotherapy (IVC) was introduced and currently remains prevalent worldwide for intraocular RB control as well as prevention of systemic metastasis.However, RB seeds in vitreous and subretinal space are still the major obstacles for successful treatments.Advanced ophthalmic imaging technology promotes thorough and detailed description and observation of RB seeds.In the 2000s, interests in periocular chemotherapy (POC), intra-arterial chemotherapy (IAC) and intravitreous chemotherapy (IVitC) have been explored to overcome the low drug concentration around the seeds and to reduce the systemic side effects.This leads us into a new era of target local chemotherapy of RB.How to make the treatment decision based on biological behavior of RB seeds is a major task for us now.
