1.Predictive value of systemic immune-inflammation index combined with albumin for the severity of community-acquired pneumonia in elderly patients

Objective To investigate the predictive value of systemic immune-inflammation index(SII)combined with albumin(ALB)in the severity of community-acquired pneumonia(CAP)in elderly patients.Methods A retrospective analysis was conducted on 184 elderly patients with CAP in the Respiratory and E-mergency Medicine departments of the hospital from January 2023 to May 2024.According to the 2016 Chi-nese Adult CAP Diagnosis and Treatment Guidelines,patients were divided into severe CAP group(n=76)and non-severe CAP group(n=108).Basic information of patients was collected,routine blood tests and blood biochemical parameters were collected within 24 hours after admission,and the differences in SII,SIRI,hyper-sensitive CRP,and ALB were compared between the two groups.The ROC curve was applied to calculate the area under the curve and analyze the value of SII,SIRI,hypersensitive CRP,and ALB in predicting severe CAP.Results In terms of inflammation indicators,SII,SIRI,hs-CRP,and neutrophil count in the severe CAP group were higher than those in the non-severe CAP group(P<0.05),while lymphocyte count was lower than that in the non-severe CAP group(P<0.05).In terms of blood biochemistry,serum creatinine and BUN in the severe CAP group were higher than those in the non-severe CAP group,while ALB was lower than that in the non-severe CAP group(P<0.05).Binary logistic regression analysis showed that SII and ALB were in-dependent influencing factors for the severity of CAP in elderly patients(P<0.05).The ROC curve showed that the AUC of SII and ALB for predicting severe CAP in the elderly was 0.863 and 0.906,respectively,and the combined AUC was 0.937(0.904,0.970),with a sensitivity of 0.842 and a specificity of 0.880.Conclusion SII combined with ALB can serve as a predictor for early disease severity in elderly patients with CAP.
2.Differentiation of insulin-producing cells from human umbilical cord mesenchymal stem cells infected by MAFA-PDX1 overexpressed lentivirus

BACKGROUND:Transplantation of stem cell-derived islet β cells has been considered effective for the treatment of type 1 diabetes.Human umbilical cord mesenchymal stem cell is an ideal cellular source,but with a low differentiation efficiency to islet β cells. OBJECTIVE:To explore the possibility of human umbilical cord mesenchymal stem cells modified by MAFA and PDX1 to differentiate into insulin-producing cells. METHODS:MAFA-PDX1 lentivirus expression vectors were constructed.The efficiency and potentiality of human umbilical cord mesenchymal stem cells differentiated into insulin-producing cells with three methods were compared by cell morphology,RT-qPCR,and dithizone staining[protocol A:Simple lentivirus group;protocol B:Drug(nicotinamide β-mercaptoethanol)induction followed by lentivirus group;protocol C:lentivirus and drug induction group]. RESULTS AND CONCLUSION:(1)Morphological change of cells:Cell morphology was all altered after the induction of three protocols.At day 11,human umbilical cord mesenchymal stem cells induced by protocol B showed the most cell clusters among the three protocols,appearing aggregated islet-like cell clusters.(2)Islet-related gene expression detected by RT-qPCR:Horizontal comparison of the three protocols at the same induction time point showed that the expression levels of MAFA and PDX1 genes were the highest in protocol C on day 5 of induction,and those in protocol B were the highest on day 11 of induction.Human umbilical cord mesenchymal stem cells induced by protocol B had the greatest expression of GCG gene at day 5,INS and GLUT2 genes at day 11.(3)Dithizone staining to identify zinc ions:parts of the post-induced cells were stained brownish red by dithizone on day 11.The partial small island cells were stained brownish red with a darker color(positive expression)in protocol B.(4)It is concluded that the overexpression of MAFA and PDX1 can promote the differentiation of human umbilical cord mesenchymal stem cells into insulin-producing cells.The combination of MAFA-PDX1 gene modification and drug induction is superior to the single gene modification.
3.Clinical observation of tolvaptan in the treatment of heart failure with preserved ejection fraction complicated with hyponatremia

OBJECTIVE To explore the therapeutic effect and safety of tolvaptan in the treatment of heart failure with preserved ejection fraction (HFpEF) complicated with hyponatremia. METHODS Overall 106 patients with HFpEF complicated with hyponatremia were collected from the Department of Cardiology in the First Affiliated Hospital of Nanyang Medical College from January 1, 2020 to June 1, 2023. According to the random number table, the patients were divided into conventional treatment group (n=53) and tolvaptan group (n=53). The conventional treatment group was given conventional treatment. Tolvaptan group additionally received Tolvaptan tablets 15 mg on the basis of conventional treatment group, increasing to 30 mg after 24 h, and then adjusting the dosage according to the levels of serum sodium; the maximum dose should not exceed 60 mg/d, and the medication should be stopped when the serum sodium level was≥150 mmol/L. Both groups of patients were treated for 6 months. The levels and changes of cardiac function indexes [left ventricular ejection fraction (LVEF), left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD), N-terminal pro-brain natriuretic peptide (NT-proBNP)], 24 h urine output, serum sodium, blood creatinine and urea nitrogen were compared between 2 groups before and after treatment. The occurrence of adverse drug reaction (ADR) was recorded. RESULTS Before treatment, there was no statistically significant difference in those indexes between 2 groups (P>0.05). After treatment, the levels of LVEF, 24 h urine output and serum sodium in 2 groups were significantly higher than before treatment, and the tolvaptan group was significantly higher than the conventional treatment group; the levels of LVESD, LVEDD and NT-proBNP were significantly lower than before treatment, and the tolvaptan group was significantly lower than the conventional treatment group (P<0.05 or P<0.01). The changes in cardiac function indexes, 24 h urine output and serum sodium levels in the atorvastatin group were more significant than the conventional treatment group (P<0.05). There was no statistically significant difference in the levels and changes of blood creatinine and urea nitrogen before and after treatment, as well as the incidence of nausea and vomiting, dizziness, hypernatremia and frequent urination between 2 groups (P>0.05). CONCLUSIONS Tolvaptan can improve cardiac function and increase the blood sodium levels in patients with HFpEF complicated with hyponatremia, without affecting their renal function or increasing the risk of ADR.
4.Progress in autoantibody profile for autoimmune cardiomyopathy

The autoimmune cardiomyopathy is a variety of myocardial diseases with significant individual heterogeneity that is mediated by autoimmunity reaction. However, the pathogenesis of the autoimmune cardiomyopathy has not been clearly known. Multiple autoantibodies have been detected in the sera of patients with autoimmune cardiomyopathy, and the studies of related autoantibody profile contributes to understanding the pathogenesis, improving screening and diagnosis, supervising disease progression, estimating prognosis and therapeutic effects. This review will summarize the progress and clinical application of the autoantibody profile in patients with autoimmune cardiomyopathy, providing a reference for clinical practice.
