1.Current Situation, Problems and Countermeasures of Experimental Research on Traditional Chinese Medicine Regulating PI3K/Akt Signaling Pathway in Rats with Polycystic Ovary Syndrome

Polycystic ovary syndrome(PCOS) and its resulting infertility is one of the common diseases of gynecology and reproductive endocrinology. The phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt) signaling pathway is relatively well-studied in the development of intervention in PCOS, and the experiments on PCOS in rats conducted by traditional Chinese medicine through this signaling pathway is also the main direction of mechanistic research. In this paper, 20 articles published in academic journals in the past 5 years were selected through the corresponding criteria, and the objective situation and existing problems of the selected research projects were analyzed from five aspects, namely, baseline data, modeling and treatment, grouping, evaluative indexes, and pharmacodynamic indexes. It is found that there were different degrees of problems in each research project, such as the observation indicators of modeling, criteria for judging the success of the model, the treatment period, the calculation of dosage of prescription/active ingredients and specific dosage were not clearly defined, which could easily lead the bias of the results or reduce the validity of experimental data. Based on this, the list of PCOS rat experimental research operations was formed, involving five categories of experimental rats, model construction, study implementation, outcome measures and analysis and report with a total of 21 operation lists, with a view to provide a reference for the subsequent PCOS experiments related to scientific research and helping to form high-quality results.
2.Relationship between Physical Activity and Sedentary Time Trajectory and All-cause Mortality in Chinese Adults

Objective It has been proved that physical activity and sedentary behavior are closely related to health status.However,individual physical activity levels and sedentary time are not constant over the course of life and often change with age.The aim of this study was to identify the physical activity(PA)and sedentary time(ST)trajectory and explore the association with all-cause mortality in Chinese adults.Methods The data from the China Health and Nutrition Survey from 2004 to 2015 were used to identify the trajectory patterns of physical activity and sedentary time over the course of life by using the deleted normal model in the multivariate baseline trajectory model,and the association between the grouping of physical activity and sedentary time trajectory and the risk of all-cause mortality was evaluated by the multivariate COX proportional hazard regression model.Results A total of 10576 participants with two or more rounds of complete follow-up data were included in the analysis.Four physical activity and sedentary time trajectories were identified.34.9%showed that PA level decreased and ST maintained a moderate level.The rest were low PA low ST(13.5%),high PA high ST(32.8%)and high PA high ST(13.8%).People with high ST in medium PA(HR=0.54,95%CI:0.37~0.78,P=0.001)and people with high ST in high PA(HR=0.27,95%CI:0.11~0.65,P=0.002)the risk of death was significantly lower than those with low PA and low ST,and there was no significant difference in the risk of death in the ST group with reduced PA(HR=0.82,95%CI:0.59~1.16,P=0.257).Conclusion This study identified four PA and ST tracks in Chinese adults,and emphasized the importance of long-term maintenance of high PA levels in reducing mortality risk.

4.A qualitative study to explore the training experience of task-oriented stroke upper limb rehabilitation ro-bot based on the user experience honeycomb model

Objective:To collect the user feedback on task-oriented upper limb rehabilitation robots from multiple perspec-tives,including stroke patients,caregivers and rehabilitation therapists,and to provide insights for further devel-opment and optimization by analyzing the existing deficiencies.Method:A qualitative research method was used to conduct semi-structured interviews and experience ratings with 15 stroke patients,9 caregivers,and 10 rehabilitation therapists in the Department of Occupational Thera-py of the Rehabilitation Hospital Affiliated to Fujian University of Traditional Chinese Medicine,using purpo-sive sampling based on the user experience honeycomb model.The interview data were analyzed and summa-rized to refine the themes according to Braun & Clarke's thematic analysis method and with the assistance of NVivo 12 software.Result:Six themes and 16 sub-themes were presented,including usefulness(physical function,psychological condition and rehabilitation efficiency),usability(operational difficulty and human-machine interaction),satis-faction(training modules,element design and comfort),findability(information architecture,functional search and task prompts),accessibility(personalization,application environment and social support),and credibility(profes-sionalism,accuracy and safety).The scoring results showed that the score of"accessiblilty"was relatively low at 7.76±1.24,while the average score of the other five topics was above 8,and the scores of"usefulness"and"credibility"of the equipment were significantly higher than those of"accessibility"(P=0.029,P<0.001).Conclusion:The task-oriented upper limb rehabilitation robot has a good sense of user experience.However,further improvements are needed in the efficiency,comfort and application environment of human-computer in-teraction machine.

6.Experience summary of coagulation rebalancing therapy for heat stroke-induced coagulopathy

This case report describes the treatment process of a severe exertional heat stroke-induced coagulopathy in a 20-year-old firefighter. Through a comprehensive treatment strategy of five early and one prevention, including early cooling, early anticoagulation, early coagulation factor supplementation, early anti-inflammation, early blood purification, and prevention of complications, the patient was successfully treated. For the management of heat stroke-induced coagulopathy, based on the coagulation rebalancing strategy-namely anticoagulation combined with goal-directed replacement therapy, along with plasma diafiltration (PDF) treatment-the patient's coagulation function was effectively improved without any bleeding or thrombotic events. The patient's coagulation function returned to normal by day 4 of hospitalization, the tracheal tube was removed on day 6, transferred to the general ward after 11 days, and was discharged after 21 days of rehabilitation.
7.Application value of intrathecal synthetic related markers in cognitive dysfunction and various dementia diseases

Objective To evaluate the value of intrathecal synthetic related markers in patients with mild cognitive impairment(MCI),Alzheimer's disease(AD),and other types of dementia.Methods Retrospec-tively collect the clinical data of 577 patients diagnosed with MCI(MCI group,178 cases),AD(AD group,131 cases),and other types of dementia(other types group,268 cases)from June 2019 to July 2023 in Beijing Tiantan Hospital,Capital Medical University.Oligoclonal zone electrophoresis(OCB)and 24 h intrathecal pro-tein synthesis rate(ISR)of each group were investigated,and the difference of different indexes among the groups was compared to evaluate the value of related indexes in the differential diagnosis of different diseases.Results Compared with AD group and other groups,AD group had a higher proportion of females,more patients were>50-70 years old,and the incidence of abnormal lipid metabolism was higher,with statistical significance(P<0.05).There were significant differences in OB(S),cerebrospinal fluid albumin,serum albumin and cerebrospinal flu-id IgG in different disease groups(P<0.05).IgG index and ISR in patients with positive SOB(CSF)were higher than those in negative and weakly positive patients,and the differences were statistically significant(P<0.001).IgG index was positively correlated with ISR(r=0.878,P<0.001).Conclusion Intrathecal synthetic mark-ers such as IgG index,SOB(CSF)and 24 h ISR have synergistic effects in the diagnosis of cognitive dysfunction and various dementias,which can be collectively utilized in the diagnosis of diseases.

This study was designed to evaluate the correlation between immune antibodies and pelvic inflammatory disease(PID)using retrospective analysis.Cases were selected from 171 patients who met the diagnosis of PID in Liuzhou People's Hospital of Guangxi Province from January 2022 to March 2023,and the PID patients were further divided into simple PID group(53 cases)and in PID combined with reproductive tract infection group(118 cases)according to the presence or absence of reproductive tract infections,while 83 cases of women who did not meet the specific diagnostic criteria of PID and did not have reproductive tract infections were selected as the control group during the same period.The positive rate of immune antibodies in the three groups were observed and compared to explore the relationship between immune antibodies and PID.Data showed that the positive rates of immune antibodies were significantly higher in the PID alone group and the PID combined with reproductive tract infection group than that in the control group.Furthermore,the positive rate of immune antibody TPOAb was significant difference in the PID combined with reproductive tract infection group and the PID alone group(P<0.05).In conclusion,TPOAb is closely associated with reproductive tract infections.
9.A qualitative study of facilitators and hindrances of smoking cessation in patients with chronic obstructive pulmonary disease

Objective:To investigate the facilitators and hindrances of smoking cessation in patients with chronic obstructive pulmonary disease (COPD), and to provide a basis for developing individualized smoking cessation intervention strategies for COPD patients.Methods:Based on the health ecology theory and using a phenomenological approach in qualitative research, purposive sampling was used to select 15 COPD patients with smoking history who were admitted to Tianjin Fourth Central Hospital from March to May 2023 and face-to-face semi-structured interviews were conducted. Colaizzi seven-step analysis was used to analyze the interview content.Results:A total of 15 COPD patients were interviewed, including 13 males and 2 females, aged 61-75 years old. The facilitators of smoking cessation in COPD patients included complications, sequelae of novel coronavirus infection, fear of death, smoking cessation counseling by medical staff, heavy family financial burden, and smoke-free environment. The hindrances of smoking cessation in COPD patients included milder disease symptoms of COPD, higher levels of nicotine dependence, false disease-related perceptions, family supervision and control, and occupational experience.Conclusions:Smoking cessation in COPD patients is influenced by five factors: personal characteristics, behavioral characteristics, interpersonal network, living and working conditions, and policy environment. Medical staff and relevant national institutions should formulate corresponding smoking cessation strategies according to address the facilitators and hindrances of smoking cessation in COPD patients, so as to further reduce the smoking prevalence of COPD patients, promote the health of patients and reduce the burden of disease.
