1.Exon Sequencing of HNF1β in Chinese Patients with Early-Onset Diabetes

Background:
Maturity-onset diabetes of the young (MODY) due to variants of hepatocyte nuclear factor 1-beta (HNF1β) (MODY5) has not been well studied in the Chinese population. This study aimed to estimate its prevalence and evaluate the application of a clinical screening method (Faguer score) in Chinese early-onset diabetes (EOD) patients.
Methods:
Among 679 EOD patients clinically diagnosed with type 2 diabetes mellitus (age at diagnosis ≤40 years), the exons of HNF1β were sequenced. Functional impact of rare variants was evaluated using a dual-luciferase reporter system. Faguer scores ≥8 prompted multiplex ligation-dependent probe amplification (MLPA) for large deletions. Pathogenicity of HNF1β variants was assessed following the American College of Medical Genetics and Genomics (ACMG) guidelines.
Results:
Two rare HNF1β missense mutations (E105K and G454R) were identified by sequencing in five patients, showing functional impact in vitro. Another patient was found to have a whole-gene deletion by MLPA in 22 patients with the Faguer score above 8. Following ACMG guidelines, six patients carrying pathogenic or likely pathogenic variant were diagnosed with MODY5. The estimated prevalence of MODY5 in Chinese EOD patients was approximately 0.9% or higher.
Conclusion
MODY5 is not uncommon in China. The Faguer score is helpful in deciding whether to perform MLPA analysis on patients with negative sequencing results.

Geriatric oral health care encounters significant challenges with the increase in the proportion of older individuals. Age-related changes in the dentition, muscles, and joints result in a decline in objective masticatory function, subjective restoration requirements, and acceptability among the elderly population, with individual variations influenced by systemic health. Considering functional requirements, the adaptability of stomatognathic and systemic health conditions, health economics and other factors, the authors believe that it should not be limited to the conventional "one-to-one" strategy for replacing missing teeth in geriatric prosthodontics. There is an urgent need for a precise and adaptable restoration strategy that is more suitable for older individuals. The proposal of a new concept of functional tooth loss updates the minimal restoration standards for elderly patients and establishes the theory of age-friendly functional restoration. Based on the restoration strategy of functional tooth loss, this paper proposes a new concept termed "age-friendly functional restoration of the stomatognathic system", which integrates treatment considerations including endodontics, periodontology, mucosa, muscles, temporomandibular joint, and systemic health. Efforts should be made in four areas as follows. Firstly, the "assessment of accessible function" should be enhanced by considering the interrelationship between stomatognathic and systemic health. Secondly, the "evaluation of appropriate function" is supposed to be optimised in view of subjective needs and objective evaluation of the stomatognathic system. Moreover, the "formulation of treatment plans" needs to be accomplished with the aid of assistive technologies, such as artificial intelligence, to accurately exert appropriate functional restoration. Lastly, the "management and maintenance of health" is likely to be strengthened through follow-ups, propaganda and education, and preventive healthcare, so as to improve quality of life and ultimately achieve healthy ageing among older individuals.

3.Impact of rigid container material type and loading volume on the sterilization of thoracoscopic instruments

ObjectiveTo explore the impacts of material type and loading volume of rigid containers on the hydrogen peroxide low temperature plasma sterilization of thoracoscopic instruments, to identify the best rigid containers and loading volume of thoracoscopic instruments. MethodsThoracoscopic instruments sterilized by STERRAD® 100NX hydrogen peroxide low temperature plasma in Shanghai Pulmonary Hospital affiliated to Tongji University from August to September 2024 were selected as the research items. According to the material of rigid containers, the instruments were divided into polyethylene case group (A), stainless steel case group (B) and silicone resin case group (C). In terms of the loading volume, the rigid containers were divided into (loading capacity <80%) groups of 8, 10 and 12 instruments. The results of physical monitoring, the first type of chemical indicator card monitoring, and the five types of card luminal chemical process challenge device (PCD) monitoring of the 9 groups of A8, A10, A12, B8, B10, B12, C8, C10 and C12 were compared and evaluated. ResultsCompared to A8, A10 A12, C8, C10 or C12 groups, the thoracoscope instruments in the stainless steel containers in B8, B10 or B12 group had higher hydrogen peroxide concentrations and shorter elapsed time in the pressure check phases 1 and phases 2, with the differences statistically significant (P<0.05), followed by the silicone resin case group and the polyethylene case group. The nine groups of physical parameter monitoring, the first type of chemical indicator monitoring, and the five types of chemical PCD monitoring for lumen sterilization achieved 100% qualification rates, and there were no significant differences in the qualified rates of sterilization among the 9 groups (P>0.05). ConclusionWhen using hydrogen peroxide low temperature plasma to sterilize thoracoscopic instruments, it is recommended to use stainless steel or silicone resin rigid containers with a controlled loading capacity (≤12) to ensure optimal sterilization quality.

5.Effects of Rehmanniae Radix Praeparata on striatal neuronal apoptosis in ADHD rats via Bcl-2/Bax/caspase-3 pathway.

This study investigated the effects of Rehmanniae Radix Praeparata on striatal neuronal apoptosis in rats with attention deficit hyperactivity disorder(ADHD) based on the B-cell lymphoma-2(Bcl-2)/Bcl-2-associated X protein(Bax)/caspase-3 signaling pathway. Twenty-four 3-week-old male spontaneously hypertensive rats(SHR) were randomly divided into a model group, a methylphenidate group(2 mg·kg~(-1)·d~(-1)), and a Rehmanniae Radix Praeparata group(2.4 mg·kg~(-1)·d~(-1)). Age-matched male Wistar Kyoto(WKY) rats were used as the normal control group, with 8 rats in each group. The rats were administered by gavage for 28 days. Body weight and food intake were recorded for each group. The open field test and elevated plus maze test were used to assess hyperactivity and impulsive behaviors. Nissl staining was used to detect changes in striatal neurons and Nissl bodies. Terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL) fluorescence staining was used to detect striatal cell apoptosis. Western blot was employed to detect the expression levels of Bcl-2, Bax, and caspase-3 proteins in the striatum. The results showed that compared with the model group, Rehmanniae Radix Praeparata significantly reduced the total movement distance, average movement speed, and central area residence time in the open field test, and significantly reduced the ratio of open arm entries, open arm stay time, and head dipping in the elevated plus maze test. Furthermore, it increased the number of Nissl bodies in striatal neurons, significantly downregulated the apoptosis index, significantly increased Bcl-2 protein expression and the Bcl-2/Bax ratio, and reduced Bax and caspase-3 protein expression. In conclusion, Rehmanniae Radix Praeparata can reduce hyperactivity and impulsive behaviors in ADHD rats. Its mechanism may be related to the regulation of the Bcl-2/Bax/caspase-3 signaling pathway in the striatum, enhancing the anti-apoptotic capacity of striatal neurons.

This article summarizes the rapid rehabilitation nursing experience for one pediatric pa-tient with pancreatic divisum who underwent en bloc pancreatoduodenectomy preserving the duodenum,common bile duct,and Oddi's sphincter(DCOPPHTR).The nursing care encompassed preoperative prehabilitation to enhance physical and psychological preparation,postoperative precise volume and dynamic blood glucose management,early mobilization to facilitate recovery,personalized nutritional support to improve nutritional status,psychological nursing to bolster confidence,and family involve-ment to enhance quality of life.The pediatric patient experienced rapid recovery and was discharged from the hospital.A six-month follow-up indicated a significant improvement in quality of life.
9.Association between sarcoma and gut microbiota:a two-sample Mendelian randomization analysis

Objective To assess potential associations between sarcoma and gut microbiota using Mendelian randomization(MR)analysis.Methods Instrumental variables(IV)for gut microbiota were identified from summary data of a genome-wide association studies(GWAS)involving 18 340 participants from 24 cohorts published by the MiBioGen Consortium.The GWAS on sarcoma from Europe served as an outcome,involving 456 276 sarcoma patients.The inverse-variance weighted(IVW)method was used for primary analysis while sensitivity analysis was made to test the reliability of results of MR analysis.Results IVW results showed that Peptostreptococcaceae(OR=0.353,95%CI 0.127 to 0.979,P=0.045)and Faecalibacterium(OR=0.408,95%CI 0.173 to 0.961,P=0.040)had a negative causal correlation with sarcoma,but the Eubacterium fissicatena group(OR=2.216,95%CI 1.101 to 4.462,P=0.026)and Lachnospiraceae UCG008(OR=1.822,95%CI 1.005 to 3.305,P=0.048)exhibited a positive causal correlation with sarcoma.Sensitivity analysis showed no evidence of pleiotropy and heterogeneity.Conclusion Through two-sample MR,this study identifies a potential causal relationship between four gut microbiota and sarcoma.Peptostreptococcaceae and Faecalibacterium can reduce the risk of sarcoma while the Eubacterium fissicatena group and Lachnospiraceae UCG008 are linked to an increased risk of sarcoma.More research is required to find out more about the way gut microbiota influences the development of sarcoma.
10.Intratumoral tertiary lymphoid structure for predicting prognosis of soft tissue sarcoma after cryoablation combined with immunotherapy

Objective To observe the value of intratumoral tertiary lymphoid structure(TLS)for predicting prognosis of soft tissue sarcomas(STS)after cryoablation combined with immunotherapy.Methods Forty-three STS patients who underwent cryoablation were retrospectively enrolled,including 38 cases underwent cryoablation combined with but 5 cases without immunotherapy,among them 6 cases with but 37 cases without intratumoral TLS.The progression free survival(PFS)were compared between those with and without immunotherapy,as well as those with and without intratumoral TLS.The value of intratumoral TLS for predicting prognosis of STS after cryoablation combined with immunotherapy was analyzed with Cox regression and receiver operating characteristic curves.Results No significant difference of PFS was found between STS patients undwerwent cryoablation combined with and without immunotherapy(P<0.05).Among 38 cases underwent cryoablation combined with immunotherapy,6 cases were found with but 32 cases without intratumoral TLS,and the median PFS of the former was longer than that of the latter(6 months vs.3 months,P<0.05).Univariate and multivariate Cox regression analysis revealed that intratumoral TLS and interleukin 2 receptor(IL-R2)were both independent influencing factors of PFS of STS patients after cryoablation combined with immunotherapy(both P<0.05).The area under the curve(AUC)of intratumoral TLS for predicting PFS of STS patients at 3,6 and 9 months was 0.550,0.577 and 0.699,of IL-R2 was 0.658,0.742 and 0.583,respectively,while of combination of the above two was 0.762,0.787 and 0.881,respectively,all higher than of each intratumoral TLS and IL-R2 alone(all P<0.05).Conclusion Intratumoral TLS could be used to predict prognosis of STS after cryoablation combined with immunotherapy.Combining with IL-R2 might further improve its predictive efficacy.
