Objective To explore the correlation between systemic inflammatory response index(SIRI)and the prognosis of elderly heart failure(HF)patients. Methods A retrospective study was conducted on 300 elderly HF patients with complete medical records hospitalized in our de-partment from January to December 2022.During the follow-up period for 1 year in different ways,46 of them were lost, and 254 were finally included.Baseline data, complete blood count at admission, and results of auxiliary examinations, and medicine adherence after discharge were col-lected and recorded.According to the occurrence of major adverse cardiovascular events(MACE),the subjected patients were divided into a MACE group(96 cases)and a non-MACE group(158 cases). The baseline data and relevant examination indicators were compared between the two groups, and the relevant factors for the occurrence were analyzed by using logistic regression.ROC curve analysis was employed to assess the predictive value of SIRI for MACE occurrence. Results The MACE group had significantly advanced age, larger proportion of diabetes mellitus, higher neutrophil and platelet counts, and elevated D-dimer level, but lower standardized medication rate when compared with the non-MACE group(P<0.05,P<0.01).The SIRI level was obviously higher in the MACE group than the non-MACE group[1.70(1.13,2.33)vs 1.29(0.85,2.06),P=0.002].Multivariate logistic regression analysis showed that atrial fibrillation, standardized medi-cation,mononuclear cells, and SIRI were independent risk factors for the occurrence of MACE in elderly HF patients(P<0.05,P<0.01).ROC curve analysis indicated that the AUC value of SIRI in predicting the occurrence of MACE was 0.614(95%CI:0.544-0.683),with a sensitivity of 0.813 and a specificity of 0.437.Conclusion SIRI is significantly correlated with the occurrence of MACE in elderly HF patients, and has a certain predictive value for their prognosis.

Objective To compare the effectiveness of cobalt chloride test papers and borescope in evaluating the drying of endoscopes, providing a reference for clinical selection of appropriate assessment tools. Methods 10 gastroscopes and 10 colonoscopes procedures were selected from November 2023 to March 2024 for circulation experiments. After endoscope reprocessing, they were randomly divided into five groups with different drying times(30 s,3 min,6 min,9 min,and 12 min)with 200 samples.Cobalt chloride test paper and borescope were used to evaluate the drying effectiveness. Results The qualified rate of gastrointestinal endoscope by borescope was significantly higher than that by cobalt chloride test paper, but cobalt chloride test paper was obviously better than that by borescope in convenience and cost-effectiveness.The borescope had the function of visualization, and the quantitative positioning observation of residual droplets showed that there were a large number of droplets left after gastrointestinal endoscope drying for 30 s,and the number of droplets was obviously reduced after drying for 3 min.Among them, the gastroscope was mainly light(1~5 drops/strip)and none(0 drops/strip),and the colonoscope was mainly moderate(6~10 drops/strip)and light droplets.Compared with different drying times, there was a significant statistical difference in the number of residual droplets in the endoscope(P<0.01),and the localized droplets mainly remained in the distal bending part and the proximal bifurcation part of the lumen. Conclusion Both cobalt chloride test papers and borescope are important methods for assessing endoscope drying efficacy, each with its own advantages and limitations.Clinicians can use both methods in combination to comprehensively evaluate the overall dryness of endoscopes, thereby ensuring the safety and effectiveness of the endoscope reprocessing process.

Epilepsy,as the second most common neurological disease, has diverse causes, complex, difficult to treat, and disabling characteristics, which impose a heavy burden on patients' families and social life.Long term recurrent epileptic seizures and poor control lead to neuronal inactivation and cognitive impairment in patients. Therefore,it is becoming increasingly important to find new targeted treatment methods and explore the molecular regulatory mechanisms of epilepsy on neuronal damage.Iron induced cell death is a novel programmed cell death mediated by ferrous ions, characterized by the accumulation of ferrous ions, harmful lipid peroxides, and lethal reactive oxygen species, leading to damage to mitochondrial function and structure. With recent in-depth research on ferroptosis,it has been found that inhibiting ferroptosis related targets and pathways can alleviate seizures and progression of epilepsy and protect neurons. This article provides a brief overview of the molecular mechanisms and related regulation of ferroptosis, its role in epilepsy, and recent advances in related treatments.

Aim To investigate the protective effect of liraglutide(LIRA)on acetaminophen(APAP)-in-duced hepatotoxicity at the in vivo level and to reveal the underlying mechanism. Methods Forty SPF grade male C57BL/6J mice were randomly divided into the Control,LIRA(200 μg·kg-1),APAP(500 mg·kg-1),LIRA+APAP,LIRA+APAP+3-methylade-nine(3-MA,30 mg·kg-1)groups,with eight mice in each group. The mice were administered for three con-secutive days, and the materials were taken after 24 h.The general condition and body weight of mice in each group were recorded, and liver morphology was ob-served.Serum ALT and AST levels,as well as SOD ac-tivity,MDA,and GSH content in liver homogenates, were measured using biochemical assay kits. The levels of inflammatory cytokines IL-6,TNF-α,and IL-1β in serum were detected by ELISA.Liver pathological changes were assessed by HE staining, while mitochon-drial and autophagosome structures in liver tissues were observed using transmission electron microscopy. The number of PCNA-positive cells in liver tissues was e-valuated using immunohistochemical staining. The pro-tein expression levels of LC3Ⅱ,p62,Bax,Bcl-2,PC-NA,and CyclinD1 in liver tissues were determined by Western blot. Results LIRA pretreatment can im-prove the general condition of mice with acetamino-phen-induced liver injury(AILI),reduce serum ALT and AST levels, and effectively ameliorate the appear-ance and morphology of the liver as well as the patho-logical damage to liver tissue.Simultaneously, the lev-els of inflammatory cytokines IL-6,TNF-α,and IL-1βare significantly decreased;SOD activity and GSH con-tent are significantly increased, while MDA content is significantly reduced.Transmission electron microsco-py observations reveal the presence of numerous auto-phagosomes in the cytoplasm of liver tissue.Immuno-histochemical staining results indicate a significant in-crease in the number of PCNA-positive cells.Further-more,the expression of LC3Ⅱ,Bcl-2,PCNA,and Cy-clinD1 proteins in liver tissue is significantly upregulat-ed,while the expression of p62 and Bax proteins is significantly downregulated. However, after interven-tion with the autophagy inhibitor 3-MA,the aforemen-tioned protective effects of LIRA are significantly. Conclusions LIRA pretreatment can significantly im-prove liver injury in AILI mice. Its protective mecha-nism may be related to enhancing autophagy in hepato-cytes,thereby reducing oxidative stress, inflammatory response and apoptosis in liver of AILI mice.

A throat swab sample from a pediatric case in Tongzhou District, Beijing was identified as enterovirus; the patient was a 1-year-and-8-month-old male sporadic case. Whole-genome sequencing revealed a viral genome length of 7 436 bp. BLAST alignment confirmed the serotype as EV-D68. Phylogenetic analysis of the whole genome indicated that this strain belongs to the B3 clade, showing closer genetic proximity to the 2018 Shanghai strain MW697453 with 99.53% whole-genome nucleotide homology. Genetic and amino acid variation analysis demonstrated that the B3 subclade to which this strain belongs exhibits a nucleotide deletion at positions 718–726, differing from deletion sites observed in other B3 clade strains. A key neuropathogenic amino acid site, T650A, was found to have undergone mutation. Recombination analysis confirmed no cross-clade recombination events in this strain. This study conducted genetic characterization of the strain's evolutionary relationship with EV-D68 strains from different regions and years in China, providing data support for formulating prevention and control measures against EV-D68 infection.
