ObjectiveTo evaluate the antipyretic effect of the Qingwen Baiduling prescription in a dry yeast-induced rat fever model and to investigate its antipyretic mechanism, providing a theoretical basis for its clinical application. MethodsSPF-grade male SD rats were randomly assigned to six groups (n=6 per group): control, model, aspirin (20 mg·kg-1), and high-, medium-, and low-dose Qingwen Baiduling prescription groups (14.40, 7.20, 3.60 g·kg-1). The fever model was established by subcutaneous injection of dry yeast suspension on the back. After model induction, body temperature was recorded every 1 hour. Drugs were administered at 6 hours after modeling, and body temperature was continuously recorded hourly for 12 hours. Record the body temperature of rats to observe the trend of changes in body temperature. Serum interleukin-6 (IL-6), interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α) levels, as well as hypothalamic prostaglandin E2 (PGE2) and cyclic adenosine monophosphate (cAMP), were measured using enzyme-linked immunosorbent assay (ELISA). Hypothalamic tissue morphology was examined by hematoxylin and eosin (HE) staining. Western blot was used to detect hypothalamic expression of Toll-like receptor 4 (TLR4), myeloid differentiation factor 88 (MyD88), nuclear factor-κB p65 (NF-κB p65), inhibitor κBα (IκBα), phosphorylated IκBα (p-IκBα), IκB kinase α (IKKα), IKKβ, phosphorylated IKKα/β (p-IKKα/β), and cyclooxygenase-2 (COX2). ResultsCompared with the control group, the model group showed a significant increase in body temperature 6 hours after modeling (P0.01), confirming successful fever induction. Serum IL-6, IL-1β, TNF-α, and hypothalamic cAMP and PGE2 levels were significantly elevated (P0.01). Hypothalamic neurons exhibited irregular morphology and disordered distribution, accompanied by inflammatory cell infiltration and microglial aggregation. Expression levels of TLR4/NF-κB pathway-related proteins and phosphorylated proteins were significantly increased (P0.05, P0.01). Compared with the model group, 2-3 hours after administration, all Qingwen Baiduling prescription dose groups significantly reduced body temperature (P0.01). All dose groups significantly decreased serum IL-6, IL-1β, TNF-α, and hypothalamic cAMP and PGE2 levels (P0.05, P0.01). Neuronal morphology was markedly improved in the high- and medium-dose groups, with narrowed intercellular spaces and reduced inflammatory infiltration. The prescription effectively inhibited hypothalamic expression of TLR4, MyD88, NF-κB p65, p-IκBα, p-IKKα/β, and COX2 proteins (P0.05, P0.01). ConclusionQingwen Baiduling prescription effectively reduces body temperature in rats by mitigating the further effects of inflammatory cytokines on the hypothalamic thermoregulatory center through the blood-brain barrier. Its antipyretic mechanism may be related to inhibition of NF-κB pathway activation.

Objective:Conventional myeloid cell staining methodologies were employed to stain splenocytes and peripheral blood cells by fixation and permeabilization and non-fixation and permeabilization methods, respectively, and effects of staining were compared following flow cytometry detection. Methods:Peripheral blood and spleen of three 8-week-old C57 male mice were divided into fixation and permeabilization group and non-fixation and permeabilization group for staining, and flow cytometry was used to detect mean fluorescence intensity(MFI)of each fluorescent antibody and proportion of positive cells. Results:In mouse peripheral blood samples,MFI of 7-AAD-PerCP-Cyanine5.5 and CD147-PE were lower in fixation and permeabilization group than non-fixation and permeabilization group,MFI of F4/80-FITC,MHC Class Ⅱ(I-A/I-E)-APC-R700,Ly-6c-APC-Cy7,CD206-BV421,CD45-BV510,CD11b-BV605,CD11c-BV650 and CD86-BV786 were higher than non-fixation and permeabilization group.Proportions of each immune cell in fixation and permeabilization group and non-fixation and permeabilization group were highly similar. In mouse spleen samples,MFI of antibodies in fixation and permeabilization group were higher than those in non-fixation and permeabilization group, with exception of CD147-PE,which had a lower MFI than non-fixation and permeabilization group;proportions of dendritic cells and Mon/Ly-6clow cells were lower than non-fixation and permeabilization group, whereas proportions of rest myeloid subpopula-tions were higher than non-fixation and permeabilization group. Conclusion:Fixation and permeabilization in peripheral blood cells can improve antibody MFI,but it has little effect on proportion of positive cells.Fixation and permeabilization can enhance antibody MFI in splenocytes, while effect on proportion of each positive cell is more significant. It is advised that when designing staining strate-gies,researchers attempt to choose alternative cell surface antigens for assay or optimize protocols through required pre-experiments in order to acquire stable and dependable results.

Chinese Journal of Ultrasonography 2024;33(2):112-118
Objective:To investigate the clinical value of deep learning model based on contrast enhanced ultrasound (CEUS) video in the differential diagnosis of benign and malignant liver tumors. Methods:Between May 2010 and June 2022, 1 213 patients who underwent CEUS examination for liver masses in the Affiliated Hospital of Southwest Medical University were retrospectively collected, and the enrolled patients were divided into training and independent test cohorts with December 31, 2021 as the time cut-off. In the training cohort, the TimeSformer algorithm was used as the infrastructure, and multiple fixed-time segments were obtained from CEUS arterial videos by using the sliding window of the video, and the classification results of the entire video were obtained after fusing the features of multiple segments, so as to build a deep learning model based on CEUS videos. In the independent test cohort, ROC curves were used to verify the validity of the model and compared with three radiologists with different CEUS experience (R1, R2, and R3, with 3, 6, and 10 years of CEUS experience, respectively). Results:A total of 1 213 patients with liver masses were included in the study, including 1 066 patients in the training cohort (426 cases of malignancy) and 147 patients in the independent test cohort (50 cases of malignancy). The area under curve (AUC)value of deep learning model was 0.93±0.01 in the training cohort and 0.89±0.01 in the independent test cohort, and the accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were 80.42%, 74.19%, 92.00%, 94.52% and 65.71%, respectively. Among the three radiologists, R1 had the lowest diagnostic performance, with accuracy, sensitivity, specificity, PPV and NPV of 67.83%, 51.61%, 98.00%, 97.96% and 52.13%, respectively, while the above indicators of R3 were 82.52%, 76.36%, 94.00%, 95.95% and 68.12%, respectively. McNemar′s test showed that the difference between R1 and the deep learning model was statistically significant ( P<0.001), while the differences between R2 and R3 and the deep learning model were not statistically significant ( P=0.720, 0.868). In addition, the analysis time of the model for a single case was (340.24±16.32)ms, while the average analysis time of radiologists was 62.9 s. Conclusions:The deep learning model based on CEUS can better identify benign and malignant liver masses, and may reach the diagnostic level of experienced radiologists.
