OBJECTIVE:
To explore the clinical and genetic features of a child with Pontocerebellar hypoplasia type 2B (PCH2B) due to compound heterozygous variants of the TSEN2 gene.
METHODS:
A PCH2B patient presented at Department of Pediatric Neurology, Xiangya Hospital of Central South University in June 2023 was selected as the study subject. Clinical data of the patient were retrospectively analyzed. The patient and her parents were subjected to whole exome sequencing and bioinformatic analysis. Pathogenicity of the candidate variants were classified based on the guidelines from the American College of Medical Genetics and Genomics (ACMG). A literature review was also conducted by searching the China National Knowledge Infrastructure (CNKI), Wanfang Data, and PubMed databases from their establishment to May 2025 using keywords "TSEN2 gene" "PCH2B" and "Pontocerebellar Hypoplasia 2B" to summarize the clinical and genotypic features of patients with PCH2B due to variants of the TSEN2 gene. This study was approved by the Medical Ethics Committee of the Hospital (No.: #202310892).
RESULTS:
The patient, a 6-year-5-month-old girl, had exhibited severe global developmental delay, developmental regression, autism spectrum disorder, myoclonus of eyelids, feeding difficulty, irritability, progressive microcephaly, esotropia, and hypotonia. MRI showed reduced volume of bilateral cerebellar hemispheres and vermis. Genetic testing revealed that she has harbored compound heterozygous variants of the TSEN2 gene (NM_025265.4), namely c.1054A>T (p.Lys352*) and c.899G>T (p.Ser300Ile), which were inherited from her father and mother, respectively. Both variants were classified as likely pathogenic based on the ACMG guidelines and were previously unreported. Literature review has identified six PCH2B patients with missense, nonsense, frameshift, and splice site variants of the TSEN2 gene. Their main clinical manifestations included global developmental delay, progressive microcephaly, feeding difficulties, irritability, and vermis hypoplasia. Cranial MRI and genetic testing are crucial for definite diagnosis.
CONCLUSION
The c.1054A>T (p.Lys352*) and c.899G>T (p.Ser300Ile) compound heterozygous variants of the TSEN2 gene probably underlay the pathogenesis in this patient. Above findings has expanded the genotypic and phenotypic spectra of TSEN2-related PCH2B, and offered guidance for genetic counseling for this family.

Objective:To investigate the roles of simulated participants in the facilitation of medical scenarios simulation-based course and to provide a reference for the standardized construction of medical scenarios simulation-based course. Methods:Thirty scenario simulation-based teaching sessions on perioperative crisis events conducted during the 2022-2023 academic year were observed on-site. The performance of simulated participants in facilitating case execution and the skills used in facilitation were observed and recorded. At the end of the course, surveys regarding facilitation were administered to students, simulated participants, and instructors for analysis. Results:In the facilitation of 30 scenario simulation-based teaching sessions on perioperative crisis events, the roles of simulated participants included surgeon, anesthesia nurse, circuit nurse, and standardized patient. Two simulated participants accounted for 80.00% and three participants for 20.00% of the total sessions. The simulated participants performed excellently in helping the learners quickly integrate into the scenario and controlling the direction of the case, with an excellence rate of 90.00%-100.00%. However, they had some variations in driving the progress of the case and responding to unexpected situations, with an excellence rate of 50.00%-60.00%. The facilitation skills used by simulated participants included hinting, prompting, and instructing. The performance of facilitation was closely related to the use of skills by the simulated participants. Simulated participants with proficient skill usage significantly enhanced teaching effectiveness. Post-session surveys revealed that learners generally perceived simulated participants as having a positive impact on facilitation, and both simulated participants and instructors emphasized the need for strengthened communication prior to simulation-based teaching. Conclusions:The simulated participants played important roles in guiding learners, advancing case progression, and achieving teaching objectives. Instructors should not only consider the scripting of simulated participants during lesson planning but also prioritize their training and communication during course implementation.

Barrett's esophagus(BE),a precancerous state of esophageal adenocarcinoma, poses a major challenge for prevention and treatment owing to its complex mechanism of inflammation-cancer transformation and the lack of effective clinical treatment and torsion strategies.Building upon the"preventing disease progression"theory, this study aimed to address the critical clinical challenge of intercepting the pathological progression during the inflammation-cancer transformation of BE by proposing an innovative"two critical nodes-three stages"pathomechanism framework. The pathogenesis of BE originates from liver depression and qi stagnation. The pathological progression evolves through two critical nodes:liver depression transforming into heat and heat transforming into blood stasis, representing a three-stage evolutionary pattern of qi stagnation, heat transformation, and blood stasis formation.Acidic bile salts, acting as a pathogenic toxin, permeate the entire process and catalyze carcinogenesis.Based on this understanding, the therapeutic principles of"treatment from the liver"and"truncation and torsion"were established, emphasizing stage-specific interventions. For the qi stagnation stage, treatment focuses on soothing the liver and regulating qi,as well as moistening, harmonizing, and descending the qi.This is achieved by combining modified Chaihu Shugan Powder with Xuanfu Daizhe Decoction, while using pungent and drying herbs cautiously and supplementing them with light and floral herbs. In the heat transformation stage, the strategy aims to clear the liver and drain heat while protecting yin and harmonizing the stomach, employing modified Huaganjian combined with Yiguanjian and supplemented with Jinlingzi Powder to clear depressed fire. For the blood stasis formation stage, treatment involves activating blood and resolving stasis, combined with supporting healthy qi and removing toxins. This is achieved using a modified Gexia Zhuyu Decoction, supplemented with Liujunzi Decoction, and additions such as Radix Salviae Miltiorrhizae and turtle carapace to disperse nodules and reduce masses. This theoretical framework establishes a diagnostic and therapeutic model characterized by the integration of disease mechanisms with pathology and the mutual reference of macro-level signs with micro-level indicators. It provides a comprehensive clinical practice pathway, complete with principles, methods, formulas, and herbs, for the stage-specific interception of inflammation-cancer transformation in BE using traditional Chinese medicine.

Objective Prepubertal mice are administered subcutaneously with letrozole sustained-release pellets behind the neck and treated with a high-fat diet to establish a mouse model of polycystic ovary syndrome(PCOS).The liver transcriptomes of the model mice are compared with those of the placebo control mice to investigate the underlying mechanisms of liver involvement in the pathogenesis of PCOS.Methods A customized 2 mg dose of letrozole sustained-release pellets with a 40-day release period was used. The control placebo and letrozole pellets were implanted subcutaneously in the dorsal cervical region of 3-4-week-old C57BL/6J mice(8 mice per group)to establish the control group and letrozole-induced PCOS model group. Both groups were treated with a high-fat diet starting the day after administration. The modeling period lasted for 5 weeks, during which body weight and 24-hour food intake were monitored in each group every week. When samples were collected, liver weight was recorded.Pathological changes in ovarian and hepatic tissues were examined by hematoxylin-eosin(HE)staining,while hepatic lipid deposition was observed by Oil Red O staining. The extent of macrophage infiltration in the liver was evaluated via F4/80 immunohistochemical staining, and hepatic fibrosis levels were observed by Masson's trichrome staining.Transcriptomic sequencing was performed to analyze differentially expressed genes(DEGs)in liver tissues between the control and model groups, followed by enrichment analysis of significant DEGs.Quantitative real-time fluorescent quantitative PCR(qPCR)was subsequently used to validate the expression of significant DEGs in liver tissues of both groups. Results Compared with the control group, the model group which received subcutaneous letrozole sustained-release pellets combined with a high-fat diet exhibited significantly increased body weight(P<0.001),prominent polycystic ovarian morphology, and significantly decreased liver-to-body weight ratio(P<0.05).However,no significant changes were observed in absolute liver weight(P>0.05),hepatic histomorphology,or lipid deposition.Transcriptome sequencing identified 119 upregulated and 217 downregulated DEGs in the liver tissues of letrozole-treated mice, which were predominantly enriched in pathways related to cholesterol and steroid biosynthesis, steroid hormone metabolism, and inflammatory responses.qPCR validation demonstrated that mRNA expression of HSD3B2 and HMGCR was significantly upregulated in liver(P<0.01),while mRNA expression of IL4,CCL2 and COL1A1 was downregulated(P<0.05)in the model group compared with the control group. However,Masson's trichrome staining and F4/80 immunohistochemical analysis showed no significant changes in hepatic fibrosis or macrophage infiltration. Conclusion Subcutaneous administration of letrozole sustained-release pellets combined with a high-fat diet successfully establishes a mouse model of PCOS.The model mice exhibited significant changes in hepatic gene expression.Liver may contribute to PCOS pathogenesis through regulating cholesterol and steroid metabolism.

Objective:To summarise the main points of operation during thyroid surgery using bilateral axillo-breast approach (BABA) with a domestic robot and to analyse the learning curve. Methods:From May. 2023 to Oct. 2023, we retrospectively analysed the clinical data of 38 patients who underwent thyroid surgery with domestically manufactured robot via BABA attending the Department of Thyroid and Breast Surgery of the 960th Hospital of PLA Joint Logistics Support Force, proposed and constructed learning curves using cumulative and analytical methods to study the minimum number of learning cases required for the loading time of assistants and surgeon-in-charge with da Vinci surgical experience to transition from the initial learning stage to the proficiency stage. The age, BMI, surgical time (loading time, operating time), postoperative drainage time, and number of lymph nodes dissected were compared among patients at different stages of the learning curve. Results:A total of 38 thyroidectomy patients successfully underwent surgery, with an average surgical time of (176.61 ± 47.98) minutes. After 18 cases, the assistant's loading time shortened and stabilized at (42.20 ± 3.44) minutes. The operator reached peak proficiency at the 18th case (loading time, operating time), which was considered the dividing point between the learning phase and the proficiency phase. During the learning phase, the average surgical time was (209.72 ± 49.28) minutes, operation time: (165.44 ± 49.93) minutes, while in the proficiency phase, the average surgical time decreased to 146.80 ± 18.34 minutes, operation time: (104.60 ± 19.01) minutes. There were no statistically significant differences in baseline characteristics such as age, BMI, postoperative drainage time, or the number of central compartment lymph nodes dissected between the two phases ( P > 0.05) . Conclusion:Over the results of this study, it was shown that for robosurgeons with experience in da Vinci robotic surgery, the transition from the learning stage to the proficiency stage could be considered after crossing the learning curve for 18 cases, and that assistants could become proficient in loading techniques after 18 procedures.
