1.Research progress on the intervention of sarcopenia with traditional Chinese medicine based on the AMPK signaling pathway

arcopenia is a systemic skeletal muscle disorder characterized by a decrease in skeletal muscle mass and progressive decline in function, with multiple signaling pathways involved in its occurrence and development. Among them, the AMP-activated protein kinase (AMPK) signaling pathway, as a key pathway regulating cellular energy homeostasis, plays an important role in the regulation of skeletal muscle metabolism and functional maintenance by improving abnormalities in glucose and lipid metabolism, balancing skeletal muscle protein synthesis and degradation, improving mitochondrial function, promoting autophagy, and inhibiting inflammatory responses and oxidative stress. This article reviews the research progress on how various traditional Chinese medicine (TCM) monomers, including polyphenols, flavonoids, and terpenoids; various traditional Chinese medicine extracts, such as those from Lycium barbarum , Asini Corii Colla, and Panax quinquefolium , and TCM compounds, such as Guiqi zhuangjin decoction, Jianpi qiangji granules, and Qigu capsules, intervene in sarcopenia by regulating the AMPK signaling pathway to promote muscle protein synthesis, inhibit protein degradation, improve mitochondrial function, and alleviate inflammation and oxidative stress. Additionally, their molecular mechanisms are explored. The aim is to deeply elucidate the basis of TCM in the prevention and treatment of sarcopenia and to provide theoretical support for the development of related innovative drugs.
2.Expression of tyrosine 3-monooxygenase/tryptophan 5-monooxygenase activation protein zeta in breast cancer tissues and its effect on the biological function of breast cancer cells

Objective To investigate the expression level of tyrosine 3-monooxygenase/tryptophan 5-monooxygenase activation protein zeta(YWHAZ)in breast cancer tissues and its effect on the proliferation,migration,and invasion ability of breast cancer MCF-7 cells.Methods Real-time quantitative PCR(qRT-PCR)was used to detect the expression levels of YWHAZ in the cancer tissues of 96 breast cancer patients.The correlations of the expression levels of YWHAZ in cancer tissues with clinical pathological parameters and prognosis of the patients were analyzed by the Pearson method.The effects of down-regulating YWHAZ on the proliferation,migration,and invasion ability of MCF-7 cells were verified by the CCK-8,cell scratch,and Transwell assays.The changes in the expression lev-els of epithelial-mesenchymal transition(EMT)related proteins such as E-cadherin and Vimentin were detected by the Western blot.Results The expression levels of YWHAZ in cancer tissues of patients with triple-negative breast cancer(2.659±0.214,t=2.815,P=0.012)and non-triple-negative breast cancer(1.822±0.165,t=5.079,P=0.001)were significantly higher than that in adjacent tissues.The expression levels of YWHAZ in breast cancer tissues were related to vascular invasion,lymph node metastasis,distant me-tastasis,TNM stage,pathological classification and recurrence(P<0.05).Among the breast cancer patients with TNM stages I toⅣ,the recurrence-free survival rate(χ2=31.10,P<0.000 1)and overall survival rate(χ2=18.99,P<0.000 1)of patients with higher levels of YWHAZ were significantly shorter than those of patients with lower levels of YWHAZ.The expression level of YWHAZ was an independent risk factor for the poor prognosis of breast cancer patients.The down-regulation of YWHAZ could inhibit the proliferation(F=10.73,P=0.000 4),migration(t=20.88,P=0.002 3)and invasion(t=13.72,P=0.000 2)ability of MCF-7 cells.Western blot confirmed that down-regulating the expression of YWHAZ could lead to an increase in the levels of E-cadherin protein(4.478±0.832,t=8.45,P=0.013 7)and an reduce in the levels of Vimentin protein(0.549±0.143,t=6.86,P=0.020 6).Conclusion The abnormal high expression of YWHAZ is closely related to the occurrence,development,and poor prognosis of breast cancer.The down-regulation of YWHAZ can inhibit the proliferation,migration,invasion,and EMT progression of breast cancer MCF-7 cells.
3.Determination of polymyxin E in human plasma by LC-MS/MS and its application in therapeutic drug monitoring

Objective To develop and validate an efficient and simple liquid chromatography with tandem mass spectrometry(LC-MS/MS)method for determination of polymyxin E in human plasma,and apply the established method in therapeutic drug monitoring(TDM)of polymyxin E.Methods The LC-MS/MS platform was based on AB SCIEX HPLC-4500MD system.Gradient elution was performed with 0.2%formic acid in water and 0.2%formic acid in acetonitrile.Phenomenex Kinetex XB-C18 column(100 mm × 2.1 mm,2.6 μm)were used.The analytes were detected by electrospray ionization(ESI)positive multiple reaction monitoring mode.The ion pairs for analytes(polymyxins E1,E2)and internal standard(polymyxins B1)were m/z 390.7→101.3,m/z 386.0→101.2,and m/z 402.3→101.2,respectively.Plasma samples were processed with protein precipitation method.Results Polymyxin E1 and E2 showed good linearity in the range of 0.031 2-6.24 mg/L and 0.006 15-1.23 mg/L,respectively.The within-run accuracy of polymyxin E1 and E2 in plasma ranged from 89.4%to 99.8%and 91.5%to 108.2%,respectively,while the between-run accuracy ranged from 91.8%to 104.7%and 95.6%to 105.2%,respectively.The within-run precision of polymyxin E1 and E2 in plasma ranged from 4.9%to 8.9%and 2.8%to 8.5%,respectively,while the between-run precision ranged from 4.1%to 7.6%and 4.2%to 9.8%,respectively.The average internal standard normalized matrix effect factors of polymyxins E1 and E2 were 96.9%-111.2%and 106.1%-112.8%in blank plasma samples from 6 different sources,102.5%-106.8%and 98.8%-105.2%in lipemic plasma,respectively,107.8%-108.9%and 106.9%-1 07.4%in hemolyzed plasma,respectively.The precision of matrix effects was less than 15.0%.The average recovery rate was 102.9%-107.5%for polymyxin E1 and E2,and 107.0%for internal standard polymyxin B1.The precision was less than 3.7%.Conclusions In this study,a simple and efficient LC-MS/MS method was established for determination of polymyxin E1 and E2 in human plasma,which is reliable in the therapeutic drug monitoring and pharmacokinetic study of polymyxin E.
4.Potential role of SUMO-specific proteases 1 in ferroptosis

Objective·To explore the potential role of SUMO-specific protease 1(SENP1)in ferroptosis.Methods·The Cancer Genome Atlas(TCGA)database was used to analyze the correlation between the expression levels of SENP1 and the ferroptosis-related genes,acyl-CoA synthetase long chain family member 4(ACSL4)and glutathione peroxidase 4(GPX4).Ferroptosis in human fibrosarcoma HT1080 cells,murine fibrosarcoma MCA-205 cells,and human embryonic kidney 293T cells was induced by RAS-selective lethal 3(RSL3).Quantitative real-time PCR(RT-qPCR)and Western blotting were used to detect the expression of SENP1.In 293T cells,immunoprecipitation-mass spectrometry was used to investigate the interacting proteins of SENP1 in the process of ferroptosis.The Flag-SENP1 plasmid was transiently transfected into 293T cells,and the overexpression efficiency of SENP1,along with the expression levels of ferroptosis-related genes ACSL4 and GPX4,was assessed by RT-qPCR and Western blotting.Results·TCGA database analysis showed that the expression of SENP1 was positively correlated with ACSL4 and negatively correlated with GPX4 in most tumor tissues.RT-qPCR and Western blotting showed that the expression level of SENP1 was significantly down-regulated in RSL3-treated HT1080,MCA-205,and 293T cells.Immunoprecipitation-mass spectrometry showed that SENP1 enriched SUMO molecules in the process of ferroptosis.Western blotting showed that the level of ACSL4 protein increased after SENP1 overexpression,and there was no significant change in the level of GPX4 protein.RT-qPCR showed that after SENP1 overexpression,there was no significant change in the mRNA levels of ACSL4 and GPX4.Conclusion·SENP1 gene expression is downregulated during ferroptosis,and may regulate the stability of ferroptosis-related protein ACSL4.
5.Corylin inhibits Ang Ⅱ-induced cardiomyocyte hypertrophy by modulating SIRT1-/NF-κB-dependent signaling pathway

Aim To investigate the role of corylin in angiotensin Ⅱ(Ang Ⅱ)-induced cardiomyocyte hy-pertrophy and its underlying mechanisms.Methods An Ang Ⅱ-induced cardiomyocyte hypertrophy model was established and treated with corylin.Real-time PCR was employed to assess hypertrophic gene mRNA expression,and immunofluorescence was used to meas-ure cardiomyocyte surface area.Western blot and en-zyme activity assay kits were used to evaluate SIRT1 expression and activity.Results Corylin markedly mitigated Ang Ⅱ-induced hypertrophic gene expression and cardiomyocyte surface area enlargement.Moreo-ver,it prevented the Ang Ⅱ-mediated decline in SIRT1 protein levels and deacetylase activity.Further investi-gation indicated that corylin inhibited Ang Ⅱ-driven NF-κB transcriptional activity and the expression of its downstream target genes,such as TNF-α,IL-6,and IL-1β.Notably,SIRT1 silencing abolished the protective effects of corylin against cardiomyocyte hypertrophy,as well as its regulation of the SIRT1/NF-κB signaling pathway.Conclusion Corylin suppresses cardiomyo-cyte hypertrophy by modulating the SIRT1-dependent NF-κB signaling pathway.

Objective This study aimed to investigate the antimicrobial resistance profiles of bacterial strains isolated from pediatric intensive care units(PICU)in China for better antimicrobial therapy.Methods Clinical isolates were collected from 17 institutions,including tertiary care children's hospitals and pediatric department of tertiary general hospitals in China from January 1,2020 to December 31,2022.Antimicrobial susceptibility testing was carried out according to a unified protocol using Kirby-Bauer method or automated systems.Results were interpreted according to the breakpoints released by the Clinical and Laboratory Standards Institute(CLSI)in 2020.Results A total of 10 688 isolates were collected,including gram-positive organisms(39.2%)and gram-negative organisms(60.8%).The top three organisms were S.aureus(13.6%,1 453/10 688),A.baumannii(10.0%,1 067/10 688),and coagulase-negative Staphylococcus(9.9%,1 058/10 688).Multi-drug resistant organisms(MDROs)were very common in children.The prevalence of methicillin-resistant Staphylococcus aureus(MRSA),carbapenem-resistant Enterobacterales(CRE),carbapenem-resistant E.coli,carbapenem-resistant K.pneumoniae(CRKP),carbapenem-resistant A.baumannii(CRAB),and carbapenem-resistant P.aeruginosa(CRPA)was 41.1%,19.4%,8.8%,30.9%,67.4%,and 28.8%,respectively.Overall,more than 50%of Enterobacteriales isolates were resistant to cephalosporins,while nearly 25%of Enterobacteriales isolates were resistant to carbapenems.MDROs were highly resistant to commonly used antibiotics.More than 80%of CRE and CRAB strains were resistant to all beta-lactam antibiotics.CRE and CRAB showed low resistance rates to tigecycline and polymyxin.CRPA showed lower resistance rates to piperacillin,beta-lactamase inhibitor combinations than the resistance rates to third and fourth generation cephalosporins.All of the Staphylococcus and Enterococcus isolates were susceptible to vancomycin and tigecycline.None of PRSP strains isolated from meningitis and nonmeningitis samples were resistant to rifampicin,vancomycin,or linezolid.The prevalence of β-lactamase-negative ampicillin-resistant(BLNAR)strains was 43.3%in Haemophilus influenzae.Conclusions MDROs were prevalent in PICU.It is necessary to establish an effective multidisciplinary team(MDT)to control the antimicrobial resistance.
7.Whole exome sequencing identified new candidate genes for prostate cancer

Purpose Discover new prostate cancer-related single nucleotide variants.Methods Tissue wax blocks from 21 prostate cancer patients who underwent radical surgery and had relatively complete clinical data were collected for somatic mutation detection to analyze new mutated genes associated with prostate cancer.The levels of cor-responding proteins in the urine of prostate cancer patients were tested according to the results of the selected genes.Results All 21 prostate cancer patients showed obvious somatic mutations,and the mutation types were dominated by C>T and G>A.The number of somatic mutations was 521,of which 27 genes had high mutation proportions(≥2 ca-ses),including ZSWIM6(5/21),FOXA1(4/21),SPTA1(2/21),FAM47C(2/21),FLG2(2/21),PRSS3(2/21),TP53(2/21),FLG(2/21),UBR4(2/21),and the mutations occurring in ZSWIM6 were all deletion muta-tions,and the mutations occurring in FOXA1 were missense mutations,deletion mutations,and deletion insertion mu-tations.The urinary levels of UPF1,SPTA1,and IDH1 proteins of the 10 prostate cancer patients were significantly different than those of the healthy controls.Correlation analysis showed that FOXA1 was positively correlated with UBR4(r=0.669,P=0.001),SPTA1 was positively correlated with FLG2(r=1.000,P<0.001),FAM47C was positively correlated with PRSS3(r=1.000,P<0.001),and there was a significant positive correlation between TP53 and FLG(r=1.000,P<0.001).ZSWIM6 and FOXA1 were not correlated with biochemical recurrence.SP-TA1 mutation affected progression-free survival(PFS)[(66.0±0)months vs(30.0±7.8)months,P=0.008].FAM47C was positively correlated with PFS[(66.0±0)months vs(19.0±0)months,P<0.001].ZNF676 was correlated with PFS[(66.0±0)months vs(26.0±5.0)months,P=0.008].FLG2 was correlated with PFS[(66.0±0)months vs(30.0±7.8)months,P=0.008].PRSS3 was correlated with PFS[(66.0±0)months vs(19.0±0)months,P<0.001].Conclusion All 21 prostate cancer patients harbored somatic mutations,including ZSWIM6(5/21)and FOXA1(4/21)mutations.SPTA1,FAM47C,ZNF676,FLG2,and PRSS3 may be associated with prognosis.
8.Comparison of Two Methods of LPS-Induced Two-Hit Acute Lung Injury Models and Analysis of Clinical Syndrome Characteristics in Chinese and Western Medicine

Objective To explore the efficacy of two lipopolysaccharide(LPS)two-hit methods in establishing an acute lung injury(ALI)mouse model and analyze the characteristics of this model within the context of both Western and traditional Chinese medicine(TCM).Methods Healthy male C57BL/6 mice were randomly divided into six groups:PBS instillation control group,PBS nebulization control group,instillation two-hit group,nebulization two-hit group I,nebulization two-hit group Ⅱ,and nebulization two-hit group Ⅲ.Using LPS as a stimulant,a"two-hit"approach was employed to establish an ALI mouse model.Body temperature,weight,pulse oximetric oxygen saturation(SpO2),lung tissue wet-to-dry weight ratio(W/D),levels of tumor necrosis factor(TNF-α),interleukin-1β(IL-1β),and myeloperoxidase(MPO)activity in lung tissue,as well as total protein concentration and white blood cell count in bronchoalveolar lavage fluid were measured.Through evaluating the consistency between TCM and Western medical syndromes,the classification,characteristics,modeling methods,advantages,and disadvantages of the two-hit ALI animal model were summarized and analyzed based on the clinical diagnostic criteria and syndrome characteristics of ALI in both TCM and Western medicine.Results Compared with the control group,the body temperature,weight,and SpO2 of model group decreased,W/D increased,levels of TNF-α,IL-1β,and MPO activity in lung tissue increased.The alveolar walls thickened with a large exudate of red blood cells.The total protein concentration and white blood cell count in bronchoalveolar lavage fluid significantly increased.The existing two-hit ALI animal model showed a high degree of consistency with Western medical diagnostic main symptoms.Conclusion Both methods of two-hit can prepare mouse ALI models,among which the nebulization two-hit Group Ⅲ showed more pronounced effects in simulating the pathological changes and clinical symptoms of ALI.
9.Global regulatory requirement and considerations for pharmaceutical co-crystals

As a new solid-state form of drugs,pharmaceutical co-crystals can improve the physicochemical properties of drugs(such as melting point,stability,solubility,hygroscopicity,compressibility,permeability,bioavailability,etc),thereby changing drug performance or enhancing therapeutic efficacy,providing new ideas for drug development.In recent years,pharmaceutical co-crystals has attracted much attention as a hot topic in the research of crystalline drugs,but there is currently no specialized guiding principle for pharmaceutical co-crystals research in China.This article mainly investigates the technical documents on pharmaceutical co-crystals research released by the Food and Drug Administration(FDA)and the European Medicines Agency(EMA),elaborates on the regulatory requirements for pharmaceutical co-crystals in foreign countries,compares and analyzes the regulatory requirements of FDA and EMA,in order to provide references for the research and regulation of pharmaceutical co-crystals in China.
10.Analysis of the therapeutic effect of precise disconnection of pargastric varices guided by endoscopic ultrasound for the treatment of esophagogastric variceal bleeding(20 cases)

Objective To explore the therapeutic effect of precise disconnection of pargastric varices guided by endoscopic ultrasound in the treatment of esophagogastric variceal bleeding.Method A retrospective analysis was conducted on 20 patients with cirrhosis esophagogastric variceal bleeding treated with endoscopic ultrasound-guided precise disconnection of pargastric varices from January 1,2024 to December 31,2024.The efficacy was analyzed.Result All 20 patients successfully completed the precise disconnection of pargastric varices under the guidance of endoscopic ultrasound.The injection of tissue gel combined with the placement of spring coils(14 cases)and the injection of tissue gel alone(4 cases)successfully blocked the pargastric varices.All patients did not experience perforation,esophageal and cardia stenosis,massive bleeding,septicemia,or ectopic embolization.One patient who received tissue gel alone had slight bleeding from the pargastric varices after surgery and improved after 3 days of treatment to reduce portal vein pressure.Another one patient who received tissue gel alone had a low-grade fever and normal body temperature after 3 days of anti-infection treatment.Conclusion Precise disconnection of pargastric varices under the guidance of endoscopic ultrasound has a good therapeutic effect on esophagogastric variceal bleeding,with fewer complications such as ectopic embolization,massive bleeding,infection,and perforation.However,close follow-up observation is still needed to address the issue of pargastric varices.
