1.Study on the evaluation of medical equipment maintenance services in large public hospitals:taking a tertiary hospital in Tianjin as an example

Medical equipment is one of the important factors influencing medical services,and ensuring the integrity of medical equipment is an important part of hospital management,and the introduction of reliable medical equipment maintenance services can improve the quality of hospital medical management as a whole.Taking a tertiary hospital in Tianjin as an example,the study explores the subjective and objective evaluation indexes of clinical satisfaction after the introduction of maintenance serv-ices,builds a refined management system for medical equipment,and constantly adjusts the evaluation program of medical equip-ment maintenance services through the feedback mechanism,aiming to enhance the safety of the use of medical equipment,re-duce the cost of maintenance of medical equipment,and contribute to the high-quality development of the hospital.
2.Establishment of HPLC characteristic chromatograms and content determination of nine constituents for Yixin Fumai Granules

AIM To establish the HPLC characteristic chromatograms for Yixin Fumai Granules,and to determine the contents of sodium danshensu,protocatechualdehyde,chlorogenic acid,calycosin-7-O-β-D-glucoside,ferulic acid,rosalinic acid,salvianolic acid A,salvianolic acid B,schisandrol A.METHODS The analysis was performed on a 35 ℃ thermostatic Acutfex PA-C18 column(4.6 mm ×250 mm,5 μm),with the mobile phase comprising of acetonitrile-0.1%phosphoric acid flowing at 1.0 mL/min in a gradient elution manner,and the detection wavelengths were set at 210,250,280,320 nm.Subsequently,cluster analysis and principal component analysis were performed.RESULTS There were 11 characteristic peaks in the characteristic chromatograms for 15 batches of samples with the similarities of more than 0.980.Nine constituents showed good linear relationships within their own ranges(r≥0.999 6),whose average recoveries were 97.60%-107.02%with the RSDs of 0.78%-1.87%.Various batches of samples were clustered into 4 categories,2 principal components demonstrated the accumulative variance contribution rate of 89.454%.CONCLUSION This sensitive and reproducible method can provide a reference for the quality evaluation and control of Yixin Fumai Granules.
3.Analysis of rehabilitation motivation status and influencing factors of middle-aged and elderly stroke patients based on random forest model

Objective:To investigate the current situation of rehabilitation motivation of middle-aged and elderly stroke patients and analyze its influencing factors, so as to provide reference for formulating targeted rehabilitation plans and intervention strategies.Methods:A convenience sampling method was used to select middle-aged and elderly stroke patients from (the First Affiliated Hospital of Shihezi University, the People's Hospital of Shihezi, and the People's Hospital of Xinjiang Uygur Autonomous Region from November 2023 to April 2024 as the study subjects. A cross-sectional survey was conducted using a general information questionnaire, the Motivation in Stroke Patients for Rehabilitation Scale, Perceived Social Support Scale, and and Apathy Evaluation Scale-Informant Version.Results:The 328 middle-aged and elderly stroke patients were ultimately included, including 223 males and 105 females, aged (63.63 ± 12.00) years. The total score of rehabilitation motivation, perceived social support and apathy symptoms of middle-aged and elderly stroke patients was ( 86.14 ± 11.28 ), (63.23 ± 6.31), (43.66 ± 4.63) points. LASSO regression screened 8 factors. Multiple linear regression analysis showed that age, education level, stroke duration, neurological deficits, activities of daily living, perceived social support and apathy symptoms were the influencing factors of rehabilitation motivation in middle-aged and elderly stroke patients ( t values were -6.52 to 7.56, all P<0.05), accounting for 72.6% of the total variation. The importance of random forest was ranked as follows: perceived social support, Activity of Daily Living, education level, stroke course, age, apathy symptoms, and neurological deficit. Conclusions:The rehabilitation motivation of middle-aged and elderly stroke patients is at a medium level. Clinical workers should focus on the rehabilitation motivation of stroke patients with insufficient social support, poor self-care ability and low education level, and regularly evaluate the level of rehabilitation motivation and the subjective initiative of rehabilitation exercise.

5.Potential profiling analysis and influencing factors of apathy among stroke patients

Objective To identify the potential categories and influencing factors of apathy in stroke patients,providing a basis for precise typing and individualized interventions by healthcare professionals.Methods Using the convenience sampling method,stroke patients from 2 tertiary hospitals in Xinjiang Uggur Automous Region were selected from November 2023 to August 2024,and were surveyed using the General Information Questionnaire,Apathy Evaluation Scale-Informant,Perceived Autonomy Scale,and Stroke Self-Efficacy Questionnaire.The latent profile analysis was used to identify potential categories of apathy,and logistic regression analysis was used to explore the differences in the characteristics of patients in different categories.Results The apathy symptoms of 521 stroke patients could be divided into 3 potential categories:mild apathy group(23.6%),moderate apathy-low interest-low behavioral group(46.5%),and heavy apathy-low interest-low cognitive group(29.9%).Logistic regression showed that age 45~<60 years,duration of the disease<6 months,and high rehabilitation self-efficacy were the common protective factors of the moderate apathy-low interest-low behavioral group and the heavy apathy-low interest-low cognitive group.The dependence on activities of daily living was a risk factor for the moderate apathy-low interest-low behavioral group,and high perception of autonomy was a protective factor for the heavy apathy-low interest-low cognitive group.Conclusion Apathy symptoms in stroke patients are characterized by distinct categories,which are influenced by age,stroke duration,ADL,rehabilitation self-efficacy,and perception of autonomy.Healthcare professionals should adopt effective strategies to precisely intervene based on different potential categories of protective factors and risk factors to alleviate their apathy.
6.Design and application of novel protective ventilator circuit component

Objective To develop a novel protective ventilator circuit component and to verify its performance by water seal and anti-splash experiments.Methods A novel protective ventilator circuit component had a design scheme with the multifunctional joint,and consisted of a tee connection tube,an isolation sleeve and a stop sleeve,of which,the tee connection tube was made of polyethylene polymer material and the others were made of silicone material.The tee connection tube had a T-shaped structure with two standard connection ports,which was composed of an adapter,a sealing cap,a plug and a sealing ring;the isolation sleeve was in the shape of a cylinder with a raised bottom,which was inserted into the adapter;the stop sleeve was located in the isolation sleeve,with an inverted frustum of a cone at the bottom and a rounded hole in the middle of the inverted frustum.An open ventilator circuit tube was involved in the performance verification of the circuit component developed.In the water seal experiment,sputum aspiration was simulated and the heights of the liquid level drop in the L-shaped tubes were compared after sputum aspiration.In the anti-splash experiment,the infection rates on the surfaces of the sterile hole towels and gloves were calculated.Results Water seal experiment showed after sputum aspiration the open ventilator circuit tube had the liquid level at the L-shaped tube higher significantly than that of the circuit component;the anti-splash experiment indicated sputum aspiration resulted in the occurance of the splashing out of the secretion and 77.5%infection rate by the open ventilator circuit tube,while no splashing out and 0%infection rate by the circuit component developed.Conclusion The novel protective ventilator circuit component behaves well in sealing and anti-splashing,and thus is worthy of clinical application for sputum aspiration.[Chinese Medical Equipment Journal,2025,46(4):113-117]
7.Analysis on the current status of outpatient benefit policies for patients with hypertension and diabetes in urban and rural residents

Objective:To analyze the current policy design of outpatient benefit policies for patients with hypertension and diabetes(H&D)in urban and rural residents in China,and to provide references for optimizing the outpatient benefit mechanism for patients with H&D.Methods:A questionnaire survey was conducted to collect data on the basic features of General Outpatient Benefit(GOB),Outpatient Medication Guarantee Mechanisms for Hypertension and Diabetes(OMGM-H&D)and Outpatient Benefit for Patient with Chronic and Special Diseases(OB-C&S),focusing on deductibles,policy reimbursement ratios,and maximum payment limits.Descriptive statistical analysis was performed on the data.Results:A total of 334 regions were surveyed,of which 253 regions(75.34%)had implemented all three policies.Regional analysis revealed significant differences(P<0.001)in reimbursement ratios for GOB and the OMGM-H&D between the eastern,central,and western regions,with the western region having notably higher ratios than the eastern and central regions.However,no significant difference (P>0.05) was observed in the reimbursement ratios for OB-C&S across regions. Regarding deductibles,no significant regional difference (P>0.05) was found for GOB,but significant differences (P<0.05) existed for the OMGM-H&D and OB-C&S. Additionally,the annual maximum payment limits for all three policies showed significant regional variations (P<0.001). Conclusions:The outpatient benefits policy for patients with H&D have achieved full coverage nationwide among urban and rural residents in China,but regional equity in benefit levels requires improvement. It is recommended to strengthen data feedback mechanisms and promote provincial-level pooling of medical insurance.

9.Comparison of clinical characteristics between primary bilateral macronodular adrenal hyperplasia and adrenal cortisol-producing adenoma

Objective To comparatively analyze the clinical characteristics of primary bilateral macronodular adrenal hyperplasia(PBMAH)and adrenal cortisol-producing Adenoma(CPA),and enhance the understanding of two diseases.Methods The clinical data of 85 PBMAH patients(PBMAH group)and 195 CPA patients(CPA group)diagnosed at Department of Endocrinology,the First Medical Center of Chinese PLA General Hospital,from September 2014 to August 2024 were retrospectively analyzed.The demographic characteristics,comorbidities,biochemical indicators,adrenocorticotropic hormone-cortisol(ACTH-F)levels,and adrenal imaging features and treatment conditions were compared between the two groups.Results(1)General characteristics:Compared with CPA group,PBMAH group had older age at diagnosis and a higher proportion of male patients.(2)Clinical characteristics:Compared with CPA group,PBMAH group had a longer disease duration,a higher proportion of subclinical Cushing's syndrome(CS),and a higher proportion of hypertension,impaired glucose tolerance/diabetes,bone mass reduction or osteoporosis,with higher serum potassium levels,and the differences were statistically significant(P<0.01).(3)Hormone levels:Both PBMAH and CPA groups showed ACTH-F rhythm disorder,significantly increased cortisol levels and suppressed ACTH.Compared with PBMAH group,CPA group had stronger autonomous cortisol secretion ability,manifested by increased midnight serum cortisol(F0:00),16:00 serum cortisol(F16:00),24-hour urinary free cortisol(24 h UFC)levels and lower 8:00 serum ACTH(ACTH8:00)and 16:00 serum ACTH(ACTH16:00)(P<0.01).After low-dose dexamethasone suppression test(LDDST),CPA group showed lower suppression rates of ACTH and cortisol,and higher proportions of paradoxical elevation in serum cortisol and 24 h UFC compared with PBMAH(P<0.01).Conclusions PBMAH has a longer disease course and higher proportions of comorbid metabolic disorders than CPA,mostly manifested as subclinical Cushing's syndrome.CPA has stronger autonomous cortisol secretion ability,with cortisol less likely to be suppressed after LDDST and more obvious paradoxical elevation of cortisol and 24 h UFC.

Objective To investigate the status of population dynamics and distribution changes of Aedes aegypti in Guangdong Province.Methods Continuous monitoring was conducted from May 2018 to July 2024 in Wushi Town and Qishui Town,Leizhou City,Zhanjiang City,Guangdong Province.Additionally,a survey of the distribution of Ae.aegypti along the Leizhou Peninsula coast was carried out.Results The density of Ae.aegypti in Zhanjiang showed a gradual decline from 2018 to 2024.The last detection of adult Ae.aegypti in Wushi Town was in September 2021,and the last larva was found in October 2023.No Ae.aegypti was detected in Qishui Town during surveys from 2021 to 2024.A survey of 18 coastal villages in the Leizhou Peninsula revealed no detections of Ae.aegypti.Conclusions This study provides a basis for understanding the distribution and population density fluctuations of Ae.aegypti,assessing its invasion risk,and scientifically conducting relevant prevention and control efforts.
