1.High-dose estrogen impairs demethylation of H3K27me3 by decreasing Kdm6b expression during ovarian hyperstimulation in mice.

Given that ovarian stimulation is vital for assisted reproductive technology (ART) and results in elevated serum estrogen levels, exploring the impact of elevated estrogen exposure on oocytes and embryos is necessary. We investigated the effects of various ovarian stimulation treatments on oocyte and embryo morphology and gene expression using a mouse model and estrogen-treated mouse embryonic stem cells (mESCs). Female C57BL/6J mice were subjected to two types of conventional ovarian stimulation and ovarian hyperstimulation; mice treated with only normal saline served as controls. Hyperstimulation resulted in high serum estrogen levels, enlarged ovaries, an increased number of aberrant oocytes, and decreased embryo formation. The messenger RNA (mRNA)-sequencing of oocytes revealed the dysregulated expression of lysine-specific demethylase 6b (Kdm6b), which may be a key factor indicating hyperstimulation-induced aberrant oocytes and embryos. In vitro, Kdm6b expression was downregulated in mESCs treated with high-dose estrogen; treatment with an estrogen receptor antagonist could reverse this downregulated expression level. Furthermore, treatment with high-dose estrogen resulted in the upregulated expression of histone H3 lysine 27 trimethylation (H3K27me3) and phosphorylated H2A histone family member X (γ-H2AX). Notably, knockdown of Kdm6b and high estrogen levels hindered the formation of embryoid bodies, with a concomitant increase in the expression of H3K27me3 and γ-H2AX. Collectively, our findings revealed that hyperstimulation-induced high-dose estrogen could impair the demethylation of H3K27me3 by reducing Kdm6b expression. Accordingly, Kdm6b could be a promising marker for clinically predicting ART outcomes in patients with ovarian hyperstimulation syndrome.

2.Research hotspots and trends of medical humanities in China since the new era

ObjectiveTo analyze the development trajectory, research hotspots, and trends in medical humanities research in China since the new era. MethodsA search was conducted on the CNKI (China National Knowledge Infrastructure) advanced search page using the themes“medical humanities”or “humanistic medicine,”retrieving a total of 5,758 articles. After applying specific screening criteria, 5,095 articles were included in the analysis. Citespace6.1.R6 was used to visualize and analyze the authors, institutions, and keywords of the 5,095 articles. ResultsSince the new era, the volume of publications on medical humanities in China has shown an overall upward trend, with limited collaboration between core institutions and core authors. The research content of medical humanities has evolved from broad to specific, from abstract to concrete, and from theoretical to practical. ConclusionThe development of medical humanities research in China has generally gone through three stages: defining related concepts, integrating medical humanistic spirit into clinical practice, and applying empirical methods. Narrative medicine, ideological and political education in curricula, and medical humanities education are potential future research directions.
3.Study of circRNA Screening for Differential Expression in Peripheral Blood and Correlation between CircAKT3 Level Expression and Renal Fibrosis in Patients with Type 2 Diabetic Nephropathy

Objective To screen the differentially expressed circular RNA(circRNA)in peripheral blood of patients with type 2 diabetic nephropathy(T2DN)and study the correlation between the expression of circAKT3 level and renal fibrosis.Methods A retrospectively study was conducted on 176 T2DM patients admitted to Mianyang 404 Hospital from December 2022 to December 2023 based on whether the patients developed DN,they were divided into a simple T2DM group(n=60)and DN group(n=116).At the same time,30 healthy subjects who underwent physical examination in Mianyang 404 Hospital during the same period were selected as the control group.Clinical data of all subjects were collected.DN patients were divided into microalbuminuria group(M-DN group,n=66)and macroalbuminuria group(MS-DN group,n=50)according to Mogensen classification criteria.Twenty samples were randomly selected from the control group,M-DN group,T2DM group and MS-DN group,and 2 ml peripheral venous blood was extracted for RNA extraction and circRNA microarray scanning analysis to screen the differentially expressed circRNA.The correlation between the most significant differential circRNA expression level and renal fibrosis markers and renal function indicators was statistically analyzed.Receiver operator characteristic(ROC)curve was used to analyze the diagnostic efficacy of circAKT3 for DN.Results Compared with the control group,the level of circAKT3 in MS-DN patients was down-regulated by 9.72 times,which was the most significantly down-regulated circRNA.qRT-PCR showed that compared with the control group,the relative expression of circAKT3 in the T2DM group,M-DN group and MS-DN group was 0.85±0.23,0.54±0.13 and 0.12±0.09,respectively,with the progression of DN,the relative expression level of circAKT3 in peripheral blood of patients gradually decreased,and the differences were statistically significant(t=5.100,8.568,10.960,all P<0.001).Spearman correlation test showed that the level of circAKT3 in peripheral blood of DN patients was correlated with the duration of diabetes,serum Creatinine,BUN,cystatin C,TGF-β,CIV,PCIII and Alb were negatively,correlated(r=-0.756~-0.621,all P<0.05).Glomerular filtration rate was positively correlated(r=0.695,P=0.010).ROC curve analysis showed that the sensitivity and specificity of circAKT3 in the diagnosis of DM were 75.3%and 71.1%,respectively,and the AUC was 0.792.The sensitivity,specificity and AUC for M-DN were 90.3%,85.7%and 0.875,respectively,and the sensitivity,specificity and the AUC for MS-DN were 88.2%,87.6%and 0.916,respectively.Conclusion The level of serum circAKT3 is significantly down-regulated in DN patients,which can be used as a potential auxiliary diagnostic marker of DN.

5.Obesity related pancreatic cancer:potential therapeutic transformation opportunities

Pancreatic cancer is a highly malignant digestive system tumor with poor prognosis.Its incidence is closely related to the level of regional development.Epidemiology shows that with the progress of urbanization,the proportion of obese people in the world is increasing over the years.Meanwhile,obesity has also been found to be one of the risk factors for pancreatic cancer.However,the mechanism of obesity regulating pancreatic cancer is still unclear at present.Researches show that obese people with pancreatic cancers have different tumor pathological mechanisms and higher risk of surgical complications.Therefore,this article will review the potential transformation mechanism between microscopic pathological changes and clinical manifestations of obesity related pancreatic cancer.
6.Research on the current situation of the social application field of medical humanities in China

Objective:To clarify the current situation and trend of the social application field of medical humanities in China,explore contributions and characteristics of interdisciplinary fields,and provide a reference for improving the disciplinary research system and forming disciplinary synergy.Methods:CiteSpace,a visual software,was used to cluster the keywords of the social application of medical humanities journals from 2010 to 2022,which were included in three major databases of CNKI,Wanfang Data,and VIP.The clusters were classified according to the functions of contemporary universities.This paper summarized the current social application fields of medical humanities in China and conducted a holistic analysis.Results:The current social application field of medical humanities in China included eight fields:education and teaching of medical humanities,construction of discipline professional,academic research on discipline ideology,application of medical scientific research work,medical health service and safety management,resolution of doctor-patient relationship and dispute,traditional Chinese medicine culture and humanistic spirit,and adaptation to the national strategic development concept respectively.Conclusion:There was an uneven tendency in the development of the social application field of medical humanities in China.The overall distribution of its social application fields showed a trend of"strong humanities education,strong academic research,strong medical safety,and weak cultural concepts".Medical humanities education was the common focus of the social application of medical humanities in China.
7.Comparative study on the impact of experiential communication and GLTC communication in outpatient settings on patients'emotions

Objective:From the perspective of patients'emotions,to compare and study the effectiveness between experiential communication and GLTC communication of outpatient doctors,understand the main communication details that affect patients'emotions,and provide a reference for improving patient emotions and enhancing doctor-patient communication skills in the future.Methods:From July 2021 to January 2022,a pilot study of GLTC communication program in outpatient settings was conducted in a total of 24 departments from 4 tertiary comprehensive hospitals in Jiangsu Province.Descriptive analysis,independent sample t-test,multiple linear regression and other analyses were conducted on the relevant data.Results:After experiential communication in outpatient settings,patients showed improvements in dimensions of emotion such as anxiety,fatigue,confusion,and depression,but there was a decrease in patient vitality(P<0.05).After GLTC communication in outpatient settings,patients'emotions in all six dimensions improved(P<0.05).Meanwhile,the GLTC group had better emotions than the experiential group after communication(P<0.05).After training,the completion rate of relevant communication details of doctors in the GLTC group was mostly significantly higher than that in the experiential within the same unit time(P<0.05).Multiple linear regression showed that smiling,comforting language,and kind gaze were the main factors affecting patients'overall emotions.Conclusion:Experiential communication in outpatient settings has certain communication effects,but there is still a lot of room for improvement compared to GLTC communication in outpatient settings.GLTC communication in outpatient settings can improve patients'emotions,and doctors'smiles,comforting language,and other communication details need to be emphasized in outpatient settings.How to improve the execution of GLTC communication in outpatient settings plans is an important direction for future research.

On August 2-4,2023,the"Third Summit Forum on'Building a Community of Shared Future for Doctors and Patients'"was jointly organized by institutions such as the Chinese Medical Ethics,the Hospital Humanities Management and Talent Training Special Committee of the China Population and Culture Promotion Association,Center for Ethical Studies of Renmin University of China,the Newspaper for China's Physicians,the China Health Law Society,the China Anti-Cancer Association,and the China Association For Ethical Studies in Harbin.The conference arranged a sub-forum for the"Seminar on the Construction of Chinese Medical Humanities",with domestic medical humanities scholars attending the conference.After heated discussions at the seminar,the Scholars'Consensus on the Construction and Development of Chinese Medical Humanities was formed.It was proposed that in the new era,it is urgent to build the medical humanities discipline,as well as lead the academic integration and development of medical humanities under the core socialist values.At the same time,for the construction of the medical humanities discipline,it is necessary to optimize the organizational mechanism,prosper and develop the overall framework of the medical humanities discipline,accelerate the construction of a professional teaching team for the medical humanities discipline,promote the establishment of a new carrier medical humanities education and teaching in cultivating morality and nurturing talents,as well as focus on solving problems related to the cultivation of medical humanities graduate students.

10.Cognitive Analysis and Countermeasures of Medical Humanistic Service Ability Based on the Perspective of Medical Staff in Public Hospitals

【Objective:】 To explore the cognition of medical staff in public hospitals regarding the medical humanistic service ability, and to discuss, analyze, and put forward targeted suggestions based on statistical data. 【Methods:】 A self-designed questionnaire survey was conducted among 825 medical staff randomly selected from public medical institutions in Nanjing, Central, Northern and Southern Jiangsu Province. SPSS21.0 was used to analyze the data. 【Results:】 The average score of medical staff’s familiarity with medical humanities was 3.64 points. In the cognition of the components of medical humanistic service ability, the doctor-patient communication ability accounted for the highest proportion (90.82%). There were differences in the cognition of medical staff with different demographic characteristics on the dimensions of appearance and behavior ability, doctor-patient communication ability, and patient care ability (P<0.05). The surgeon’s cognition of the importance of medical humanities was lower than that of the other four departments (P<0.05). 【Conclusion:】 Medical staff needed to improve their medical humanistic service abilities, and the cognition of humanistic differences among different categories of personnel needed to be adjusted. Public hospitals were suggested to clarify the evaluation elements and standards of medical humanistic services, and focus on solving cognitive differences to improve medical humanistic service abilities, speeding up the construction of humanistic hospitals, promoting medical humanistic service abilities. In addition, it was also necessary to improve the quality of medical humanistic education in colleges and universities through ideological and political courses.
