1.Application of brain-computer interface in the rehabilitation after spinal cord injury: a review

Spinal cord injury (SCI) is a neurological disorder that occurs after a direct or indirect violent injury to the spinal cord, often resulting in sensory and motor dysfunction below the injury level. Patients with SCI are often paralyzed in bed due to impaired nerve function and there has been no effective treatment for limb paralysis after SCI. As a cutting-edge technology with a multidisciplinary integration of neuroscience, computer science, biological engineering, electronic engineering and psychology, brain-computer interface (BCI) provides a new program for the rehabilitation of SCI patients by changing the traditional brain signal output pathways and realizing the direct connection between the brain and external devices. In order to further understand the application of BCI in SCI rehabilitation, the authors reviewed the classification, basic principles of BCI and the research progress of the application of BCI in SCI rehabilitation, which may provide references for the clinical transformation of BCI.
2.Research progress on predictive indicators of a clinical cure for chronic hepatitis B

Nucleotide analogues (NAs) and interferon are still the first-line drugs for the treatment of chronic hepatitis B (CHB), but they still cannot completely eliminate covalently closed circular DNA (cccDNA) within hepatocytes. The clinical cure, or the disappearance of HBsAg, is the ideal goal of antiviral therapy. Although interferon therapy has a significantly greater HBsAg clearance rate and seroconversion rate than NAs, combination or sequential treatment can improve the HBsAg clearance rate and seroconversion rate to a certain extent, and only a small proportion of CHB patients can achieve clinical cure. Therefore, finding indications that predict clinical cure before and during antiviral treatment is crucial for identifying patients who are more likely to achieve HBsAg clearance at an early stage, improving clinical cure rates, and reducing treatment costs. This article reviews the research progress on predictive indicators of clinical cure of chronic hepatitis B in the past five years, explores the value of each indicator in predicting clinical cure, and provides a reference for optimizing CHB treatment strategies.
3.Study on the development trajectory and influencing factors of frailty in patients undergoing cardiac surgery

Objective To explore the potential categories of frailty development trajectories in patients undergoing cardiac surgery from pre-operation to 6 months post-operation,and analyze the influencing factors.Methods By a longitudinal study design,patients undergoing elective heart valve replacement surgery or coronary artery bypass grafting in a tertiary general hospital in Zunyi City from August 2022 to June 2023 were selected by the convenience sampling method.Tilburg frailty scale was used to investigate the frailty level of cardiac surgery patients 1 day before surgery(T0),1 month(T1),3 months(T2)and 6 months after surgery(T3).The growth mixture model was used to identify the trajectory categories,and the influencing factors of different frailty trajectories were analyzed by binary Logistic regression.Results 261 patients were enrolled at T0,with 22,9,and 3 patients lost at T1 to T3,and 227 patients were finally included in the analysis.There were 2 types of trajectories being identified as the low frailty decline group and the high frailty maintenance group.Age,use of sedative and analgesic drugs,pace,and depression were the factors influencing the frailty in cardiac surgery patients(P<0.05).Conclusion There are 2 kinds of frailty development trajectories in patients undergoing cardiac surgery.Medical staff should fonnulate precise interventions and nursing measures according to the factors influencing frailty,so as to improve frailty degree and quality of life of patients undergoing cardiac surgery.

8.Improve effects of aromatase inhibitors on the height of children with short stature

The primary approach for clinical treatment of short stature(SS)in children is the use of recombinant human growth hormone(rhGH).However,the high cost of treatment and the daily injection frequency place a significant economic and psychological burden on the families of affected children.For SS adolescents who have missed the peak growth velocity period of puberty,the use of rhGH alone is also difficult to significantly increase their final adult height.Numerous studies have shown that aromatase inhibitors(AIs)are effective in improving the height of SS boys,especially the combination therapy of AIs and rhGH,which has a better effect.Although the use of AIs may affect the children's bones,cognitive function,reproductive system and hormone levels,most of these effects are temporary.Since existing research has not fully elucidated its impact on the pubertal changes of girls,it is necessary to fully weigh the benefits and risks when using AIs to treat SS girls in clinical practice.In the future,it is also necessary to carry out pharmacoeconomic research based on the medical environment of China to evaluate the cost-effectiveness of AIs for SS in children.
9.Multisystem inflammatory syndrome in children related with COVID-19 infection

Multisystem inflammatory syndrome in children is a disease related with severe acute respiratory syndrome corona-virus 2 infection,which can involve multiple system damage.Most cases included features of shock,cardiac dysfunction,gastrointestinal symptoms,significantly elevated markers of inflammation and cardiac damage,and positive test results for SARS-CoV-2 by serology.Although its clinical features overlap with Kawasaki disease,it is more likely to occur in older children and adolescents,and most of them need to be treated with intensive care and a variety of immunomodulators.At present,the pathogenesis and long-term prognosis of the disease need to be further studied.
10.Genetic analysis of a Chinese pedigree with 18q21.2-q22.3 duplication and deletion in two offspring respectively resulting from a maternal intrachromosomal insertion.

OBJECTIVE:
To provide prenatal diagnosis, pedigree analysis and genetic counseling for a pregnant woman who had given birth to a child featuring global developmental delay.
METHODS:
A pregnant woman who underwent prenatal diagnosis at the Affiliated Hospital of Southwest Medical University in August 2021 was selected as the study subject. Peripheral blood samples were collected from the woman, her husband and child, in addition with amniotic fluid sample during mid-pregnancy. Genetic variants were detected by G-banded karyotyping analysis and copy number variation sequencing (CNV-seq). Pathogenicity of the variant was predicted based on the guidelines from the American College of Medical Genetics and Genomics (ACMG). Candidate variant was traced in the pedigree to assess the recurrence risk.
RESULTS:
The karyotypes of the pregnant woman, her fetus, and affected child were 46,XX,ins(18)(p11.2q21q22), 46,X?,rec(18)dup(18)(q21q22)ins(18)(p11.2q21q22)mat and 46,XY,rec(18)del(18)(q21q22)ins(18)(p11.2q21q22)mat, respectively. Her husband was found to have a normal karyotype. CNV-seq has revealed a 19.73 Mb duplication at 18q21.2-q22.3 in the fetus and a 19.77 Mb deletion at 18q21.2-q22.3 in her child. The duplication and deletion fragments were identical to the insertional fragment in the pregnant woman. Based on the ACMG guidelines, the duplication and deletion fragments were both predicted to be pathogenic.
CONCLUSION
The intrachromosomal insertion of 18q21.2-q22.3 carried by the pregnant woman had probably given rise to the 18q21.2-q22.3 duplication and deletion in the two offspring. Above finding has provided a basis for genetic counseling for this pedigree.
