1.Impact of blood transfusion components on thrombosis risk and construction of a prediction model

Objective: To explore the association between transfusion of different allogeneic blood components and the risk of venous thromboembolism (VTE) in hospitalized patients, to clarify the differences in thrombotic risk among various blood components, and to develop a targeted VTE risk prediction model. Methods: A retrospective cohort study was conducted, enrolling 5 406 hospitalized patients who received allogeneic blood transfusion in our hospital from November 2024 to October 2025. Using 1∶1 nearest neighbor propensity score matching (PSM), 5 402 non-transfused patients were selected as controls. The matching variables included sex, age, VTE risk score, VTE prophylaxis, comorbid tumor, use of antibiotics, and invasive mechanical ventilation, with a caliper of 0.01 and sampling without replacement. Standardized mean difference (SMD) was used to assess matching quality, with SMD<0.1 indicating balanced baseline characteristics. The transfusion group was stratified by the type of blood component transfused. The chi-square test with Bonferroni correction and multivariate logistic regression (including forced adjustment for the unbalanced variables VTE prophylaxis and antibiotic use) were used to analyze risk factors. A prediction model was constructed and validated using the area under the receiver operating characteristic (ROC) curve and the Hosmer-Lemeshow test. The optimal cut-off value was determined by the maximum Youden index. Results: All variables showed SMD<0.1 after matching, indicating good baseline balance. The incidence of VTE was significantly higher in the transfusion group than in the control group (8.7% vs 4.3%, RR=2.02, 95% CI: 1.74-2.35, P<0.001). Significant differences in VTE incidence were observed among different transfusion subgroups, with a thrombotic risk gradient of combined transfusion > plasma > red blood cells ≈ platelets. The incidence of VTE in the combined transfusion subgroup was 11.7%, which was significantly higher than that in the red blood cell subgroup (RR=3.20, 95% CI: 2.41-4.24, P<0.001), plasma subgroup (RR=1.71, 95% CI: 1.24-2.35, P<0.001), and platelet subgroup (RR=6.81, 95% CI: 2.56-18.06, P<0.001). The prediction model showed good discrimination and calibration, with an area under the ROC curve of 0.800. The optimal cut-off value determined by the Youden index was 0.12, corresponding to a sensitivity of 62.3% and a negative predictive value of 91.4%. Conclusion: Allogeneic blood transfusion was an independent risk factor for VTE in hospitalized patients, with significant differences in thrombotic risk among different blood components. The prediction model developed in this study can achieve VTE risk stratification in transfused patients, providing evidence for precise transfusion therapy and patient blood management.

3.Exploring the mechanism of Xiaoqinglong decoction on endothelial-mesenchymal transition in pulmonary arterial hypertension based on the principle of resolving the exterior and expanding the collaterals'principle

Objective To analyze the role and mechanism of Xiaoqinglong decoction in alleviating the pathological process of pulmonary arterial hypertension(PAH),and to observe the effect of Xiaoqinglong decoction on endothelial-mesenchymal transition(EMT)in human pulmonary arterial endothelial cell(HPAEC)and the involvement of the Toll-like receptor/nuclear factor-κB/hypoxia-inducible factor-1α(TLR/NF-κB/HIF-1α)pathway in this mechanism.Methods Thirty-six male Sprague Dawley(SD)rats and HPAEC were randomly divided into control group,model group,Xiaoqinglong decoction plus Earthworm group,Bosentan tablet group,dimethyl sulfoxide(DMSO)group,and monophosphoryl lipid A(MPL)group.PAH rat models and HPAEC models were established by hypoxic exposure.The Xiaoqinglong decoction plus Earthworm group received intragastric administration Xiaoqinglong decoction plus Earthworm(5 g·kg-1·d-1)or cultured with 10%corresponding drug serum,the Bosentan group received Bosentan(100 mg·kg-1·d-1)by gavage or cultured with 10%corresponding drug serum,the MPL group received 1 μg MPL,and the DMSO group received an equivalent volume of the DMSO and corn oil mixed solvent as a negative control for the MPL group.The hemodynamic parameters,including mean pulmonary arterial pressure(mPAP),right ventricular systolic pressure(RVSP),and the maximum rate of right ventricular pressure(+dp/dt max),were measured via right heart catheterization.After euthanasia,lung and heart tissues were collected to assess the right ventricular hypertrophy index(RVHI);hematoxylin-eosin(HE)staining was used to observe the degree of right ventricular cardiomyocyte hypertrophy and to calculate the average intima-media thickness(IMT)in small pulmonary arteries;Western blotting was used to detect the protein expression levels of proliferating cell nuclear antigen(PCNA),CD68,TLR4,NF-κB,HIF-1α,vascular endothelial cadherin,and vimentin;cell counting kit-8(CCK-8),Transwell,and scratch assays were used to observe cell proliferation and migration;Enzyme-linked immunosorbent assay(ELISA)was used to measure the levels of interleukins(IL-8,IL-6),tumor necrosis factor-α(TNF-α),endothelin-1(ET-1),and nitric oxide(NO).Results Compared with the model group,the Xiaoqinglong decoction plus Earthworm group showed significant reductions in mPAP,RVSP,RVHI,and IMT in PAH rats[mPAP(mmHg,1 mmHg≈0.133 kPa):22.17±1.94 vs.42.00±4.90,RVSP(mmHg):34.67±3.20 vs.52.83±3.76,RVHI:0.402±0.057 vs.0.822±0.101,IMT:(37.85±2.49)%vs.(62.06±4.52)%,all P<0.05],and a significant increase in+dP/dT max(mmHg/s:2 730.83±137.89 vs.1 718.33±148.36,P<0.05).Western blotting and ELISA results showed that compared with the model group,the Xiaoqinglong decoction plus Earthworm group had significantly lower protein expression of PCNA and CD68 in lung tissue,and levels of inflammatory factors(IL-6,IL-8,TNF-α)in rat serum[lung tissue:PCNA protein expression(PCNA/GAPDH)was 1.56±0.08 vs.2.20±0.26,CD68 protein expression(CD68/GAPDH):1.46±0.09 vs.2.60±0.23;serum:IL-8(ng/L)was 39.67±6.28 vs.149.17±7.49,IL-6(ng/L):81.00±6.63 vs.211.00±25.31,TNF-α(ng/L):213.17±24.86 vs.799.50±43.51,all P<0.05].In vitro experiments,compared with the model group,Xiaoqinglong decoction plus Earthworm inhibited abnormal proliferation(A value:2.052±0.087 vs.2.242±0.057,P<0.05)and migration[number of migrating cells(per field):101.33±12.01 vs.226.67±17.56,P<0.05]of HPAEC,and reversed the EMT process,manifested as upregulation of vascular endothelial cadherin protein expression levels(vascular endothelial cadherin/GAPDH:0.39±0.06 vs.0.12±0.03,P<0.05)and downregulation of vimentin protein expression(vimentin/GAPDH:4.96±0.33 vs.7.89±0.44,P<0.05).Western blotting results indicated that compared with the model group,the protein expression levels of TLR4,the ratio of phosphorylated p65 to total p65,and HIF-1α in both lung tissue and HPAEC were significantly reduced in the Xiaoqinglong decoction plus Earthworm group[lung tissue:TLR4 protein expression(TLR4/GAPDH)was 3.13±0.20 vs.4.38±0.30,p-p65/p65 ratio:7.11±0.81 vs.12.73±1.80,HIF-1α protein expression(HIF-1α/GAPDH):2.37±0.32 vs.4.45±0.34;HPAEC:TLR4 protein expression(TLR4/GAPDH)was 1.42±0.03 vs.2.43±0.05,p-p65/p65 ratio:6.01±1.84 vs.11.28±1.06,HIF-1α protein expression(HIF-1α/GAPDH)was 3.24±0.17 vs.5.50±0.44,all P<0.05],accompanied by upregulated vascular endothelial cadherin protein expression(vascular endothelial cadherin/GAPDH:0.66±0.03 vs.0.49±0.03,P<0.05)and downregulated vimentin protein expression(vimentin/GAPDH:1.81±0.12 vs.2.47±0.10,P<0.05),indicating that Xiaoqinglong decoction plus Earthworm inhibits the EMT process in endothelial cells by suppressing the activation of the TLR/NF-κB/HIF-1α pathway.Experiments with a TLR agonist further confirmed that activation of the TLR pathway reverses the protective effects of Xiaoqinglong decoction plus Earthworm,as shown by the MPL group compared to the DMSO group having significantly increased protein expression of the p-p65/p65 ratio and HIF-1α[p-p65/p65 ratio:2.17±0.35 vs.1.08±0.14,HIF-1α/GAPDH:3.96±0.25 vs.1.03±0.10,both P<0.05],further decreased vascular endothelial cadherin protein expression(vascular endothelial cadherin/GAPDH:0.66±0.04 vs.0.99±0.02,P<0.05),further increased vimentin protein expression(vimentin/GAPDH:1.53±0.12 vs.0.93±0.07,P<0.05),along with enhanced cell migration capacity[number of migrating cells(per field):176.67±17.50 vs.107.00±11.14;cell migration rate in scratch assay:(34.32±2.82)%vs.(22.71±2.49)%,both P<0.05]and increased proliferation activity(48 hours A value:2.156±0.044 vs.1.810±0.088,P<0.05).Conclusions Xiaoqinglong decoction combined with Pheretima not only significantly reduces pulmonary artery pressure,improves cardiac function and mitigates pulmonary vascular fibrosis in PAH rats,but also alleviates pulmonary vascular remodeling by inhibiting inflammatory responses and EMT.It can further decrease the content of ET-1,increase the level of NO,and ameliorate vascular stenosis.This result further indicates that exterior-relieving medicines exert a significant dilating and supporting effect on the narrowed meridians and collaterals.
4.Hyssopus cuspidatus extract inhibited OVA-sensitized allergic asthma through PI3K/JNK/P38 signaling pathway and lipid homeostasis regulation.

OBJECTIVE:
To investigate the effect and mechanism of Hyssopus cuspidatus Boriss. extract (HCE) in ovalbumin (OVA)-induced allergic asthma.
METHODS:
Components identification of HCE was conducted using ultra performance liquid chromatography-quadrupole-time of flight-mass spectrometry. Mice were sensitized with OVA to establish asthmatic model, and dexamethasone was used as positive control. Respiratory reactivity, white cells counting in bronchoalveolar lavage fluid and peripheral blood, cytokine level measurement in serum and lung tissue, and histologic examination were performed to evaluate the therapeutic effect of HCE on asthma. Network pharmacology approach was used for mechanism prediction. Western blotting and untargeted lipidomics method were applied for mechanism validation.
RESULTS:
Fifty-two compounds were identified in HCE, predominantly terpenoids and flavonoids. HCE markedly reduced airway resistance, the eosinophil infiltration in lung tissues, and the levels of immunoglobulin E, interleukin-4, interleukin-5, and interleukin-13. Network pharmacology analysis suggested phosphatidylinositol 3-kinases (PI3K), c-Jun N-terminal kinase (JNK), and p38 Mitogen-activated protein kinase (p38 MAPK) may be key proteins of HCE in the treatment of allergic asthma. Western blot results indicated that the levels of phosphorylated PI3K, JNK, and P38 were downregulated in HCE-treated group. Moreover, HCE significantly upregulated the levels of ceramide and sphingomyelin and downregulated the level of phosphatidylcholine.
CONCLUSION
HCE inhibited allergic asthma via PI3K/JNK/P38 signaling pathway and lipid homeostasis regulation.
5.Research progress in radiation-induced salivary gland dysfunction

The global incidence of head and neck cancer (HNC) is rising, with over 60% of patients presenting at a locally advanced stage. Radiotherapy remains a cornerstone of HNC treatment, and advancements in modern techniques and concurrent chemotherapy have improved local control and survival rates of HNC patients. However, these benefits also bring challenges in the management of toxicities. Due to the proximity of salivary glands and tumors, especially the highly radiosensitive parotid and submandibular glands, this condition is among the most common adverse effects of radiotherapy. Radiation damages acinar cells and ducts, causing glandular atrophy, fibrosis, and reduced saliva secretion, thereby leading to xerostomia and related complications. The risk and severity of injury are associated with the radiation dose and volume affecting the glands. Prevention and management strategies emphasize precise radiotherapy planning, target optimization, and supportive care. Emerging multimodal imaging techniques offer potential for non-invasive prediction and early diagnosis and treatment of radiation-induced salivary gland injury. Future research in regenerative medicine, tissue engineering, and molecular biology aims to elucidate molecular mechanisms, such as signaling pathways and genomics, facilitating personalized strategies to mitigate radiotherapy-induced toxicities and enhance the quality of life of patients.

Panax ginseng(PG)and Panax notoginseng(PN)are highly valuable Chinese medicines(CM).Although both CMs have similar active constituents,their clinical applications are clearly different.Over the past decade,RNA sequencing(RNA-seq)analysis has been employed to investigate the molecular mechanisms of extracts or monomers.However,owing to the limited number of samples in standard RNA-seq,few studies have systematically compared the effects of PG and PN spanning multiple conditions at the transcriptomic level.Here,we developed an approach that simultaneously profiles transcriptome changes for multiplexed samples using RNA-seq(TCM-seq),a high-throughput,low-cost workflow to molecularly evaluate CM perturbations.A species-mixing experiment was conducted to illustrate the accuracy of sample multiplexing in TCM-seq.Transcriptomes from repeated samples were used to verify the robustness of TCM-seq.We then focused on the primary active components,Panax notoginseng sa-ponins(PNS)and Panax ginseng saponins(PGS)extracted from PN and PG,respectively.We also char-acterized the transcriptome changes of 10 cell lines,treated with four different doses of PNS and PGS,using TCM-seq to compare the differences in their perturbing effects on genes,functional pathways,gene modules,and molecular networks.The results of transcriptional data analysis showed that the tran-scriptional patterns of various cell lines were significantly distinct.PGS exhibited a stronger regulatory effect on genes involved in cardiovascular disease,whereas PNS resulted in a greater coagulation effect on vascular endothelial cells.This study proposes a paradigm to comprehensively explore the differences in mechanisms of action between CMs based on transcriptome readouts.
9.Study on Chemotype of Tibetan Medicine"Bangjian"Multi-Origin Species Based on HPLC Fingerprint and UPLC-Q-TOF-MS

Objective The HPLC fingerprints of different varieties of Tibetan medicine"Bangjian"were investigated to compare and analyse the differences in the chemical composition of different varieties and to further classify them,so as to provide reference for their quality control and safe clinical use.Methods HPLC method was used to establish the fingerprints of the different species of"Bangjian",and UPLC-Q-TOF-MS method was used to analyse the similarities and differences of the chemical components of the mainstream species and to identify the characteristic peaks.Chemometrics methods were used for the analysis including cluster analysis(HCA),principal component analysis(PCA)and orthogonal partial least squares discriminant analysis(OPLS-DA).Results A total of 11 chromatographic peaks were identified from 93 batches of"Bangjian"samples.The HCA and PCA methods were used to classify the 93 batches into 2 categories,and then OPLS-DA was used to classify them into 3 categories in more detail,while 4 main components were selected according to the principle of VIP>1.The results of UPLC-Q-TOF-MS and chemometric showed that the Tibetan medicine"Bangjian"could be divided into iridoids with benzoyl fragments represented by Gentiana szechenyii Kanitz and monocyclic iridoids represented by Gentiana veitchiorum Hemsl.according to their chemotypes.The latter could be divided into alpine gentian group subtype and multi-branch group ornate subtype according to the content of components.The results of the chemotypic classification proved that the traditional classification of the"Bangjian"has a material basis in science but were also flawed.Conclusion The present study demonstrates that the established HPLC fingerprint can be used to classify the"Bangjian"of the complex base elements effectively,which is expected to provide an effective reference for the improvement of quality standards of"Bangjian"and clinical safety medication.
10.Research progress in differentiation of male germ cells derived from induced pluripotent stem cells in vitro

Male infertility is an important issue affecting reproductive health, and its etiology and mechanism are complex or not clear. For patients with azoospermia, specific clinical treatments are limited. Therefore, to generate patients' own functional sperms is absolutely necessary. With the development of techniques, the scientists start to think about how to attain functional sperms deriving from human induced pluripotent stem cells (iPSCs) in vitro. Mainly, sperm induction with iPSCs includes two steps, first, the formation of diploid primordial germ cell, then the differentiation of haploid mature sperms. Technically, the difficulties lie in the generation of meiosis from a diploid cell into haploid sperm, and unfortunately, the problem is not solved yet. In order to understand this kind of knowledge well, we try to review the technical progress and principle of male gamete induction from iPSCs. Hopefully, it can help us to understand the mechanism of spermatogenesis in vitro well, and to provide a reference for the clinical application of in vitro fertilization.
