1.Ameliorative effects and mechanisms of Buyang huanwu decoction and its active fractions on non-alcoholic fatty liver disease

OBJECTIVE To investigate the effects and mechanisms of Buyang huanwu decoction (BYHWD) and its active fractions in ameliorating non-alcoholic fatty liver disease. METHODS BYHWD and its effective fractions obtained through ethanol precipitation, as well as 30% ethanol, 50% ethanol, and 75% ethanol fractions (namely, the CC effective fraction, 30YC effective fraction, 50YC effective fraction, and 75YC effective fraction), were prepared. These preparations were administered to rats via intragastric administration to prepare corresponding drug-containing serum (blank serum and simvastatin-containing serum were prepared using the same protocol). Human L02 hepatocytes were divided into control group, model group, simvastatin-containing serum group, BYHWD-containing serum group, CC-containing serum group, 30YC-containing serum group, 50YC-containing serum group, and 75YC-containing serum group. Except for the control group, other groups were given 0.2 mol/L oleic acid for 24 h to induce a lipid accumulation model, and then intervened with 20% drug-containing serum/blank serum for 24 h. The lipid deposition in cells was observed, and the proportion of lipid droplet area was calculated; the levels of triglycerides (TG) and indicators of oxidative stress [malondialdehyde (MDA), superoxide dismutase (SOD)] as well as liver function [alanine amino- transferase (ALT), aspartate amino-transferase (AST)] in cells were detected; protein and mRNA expressions of AMP-activated protein kinase (AMPK)/sterol regulatory element-binding protein-1 (SREBP-1)/glycerol-3-phosphate acyltransferase (GPAT) signaling pathway were also measured. RESULTS Compared with the control group, cells in the model group exhibited severe cellular steatosis, with a significantly increased proportion of lipid droplet area, as well as the elevated levels of TG, ALT, AST, and MDA in cells, along with significantly up-regulated mRNA and protein expression levels of SREBP-1 and GPAT (P<0.05). The level of SOD, mRNA expression of AMPK, as well as the protein phosphorylation level of AMPK were decreased significantly (P<0.05). Compared with the model group, cellular steatosis was alleviated in all drug-containing serum groups, and the levels of most of the aforementioned quantitative indicators were significantly reversed (P<0.05). CONCLUSIONS BYHWD and its active fractions can exert a therapeutic effect on improving non-alcoholic fatty liver disease by regulating the AMPK/SREBP-1/GPAT signaling pathway, inhibiting oxidative stress responses, and reducing lipid deposition.
2.WU Mianhua's Experience in the Treatment of Tumor-Related Complications Using Hehuanhua (Flos Albiziae)

This paper summarized professor WU Mianhua's experience in the treatment of tumors using Hehuanhua (Flos Albiziae). He believes that the occurrence and development of tumors are closely related to emotions, and thereafter proposed the concept of "masses usually due to constraint, and should be treated by regulating the spirit".Hehuanhua is a light and aromatic flower that enters the heart, liver and spleen meridians, and is good at relieving constraint and regulating the spirit, harmonizing the center with aroma, which can be used to treat all kinds of tumor-related complications. Hehuanmi is flower buds, picked before the flowers fully blooms, have the effect of clearing and lightening upward, opening up the heart and mind, and unblocking liver qi. Commonly used formulas include self-made Ningxin Anmian Decoction (宁心安眠汤) to treat tumor-related insomnia, self-made Aitongxiao Formula (癌痛消方) to treat tumor-related pain, self-made Erhua Jieyu Formula (二花解郁方) to treat tumor-associated depression, and self-made Shugan Tiaohe Decoction (疏肝调和汤) to deal with peri-menopausal state after the treatment of tumors. Hehuanhua is an inflorescence, which captures the aromatic essence and has the function of removing the filth and turbidity, and can wake up the spleen and harmonize the stomach; therefore, it is often used in self-made Tiaomu Hezhong Decoction (调木和中汤) to treat chemotherapy-related digestive tract adverse reactions or combined with rose flowers to treat chemotherapy-induced oral ulcers.
3.Application Value of Neoadjuvant Targeted Therapy in Patients with EGFR-mutant Resectable Lung Adenocarcinoma.

BACKGROUND:
The proportion of patients with non-small cell lung cancer (NSCLC) harboring epidermal growth factor receptor (EGFR) mutations is relatively high in China. However, these patients currently lack significant benefits from available neoadjuvant treatment options. This study aims to explore the potential application value of neoadjuvant targeted therapy by evaluating its efficacy and safety in patients with EGFR-mutant resectable lung adenocarcinoma.
METHODS:
A multicenter retrospective study was used to analyze the treatment effect of patients with stage IIA-IIIB EGFR-mutant lung adenocarcinoma who underwent surgical resection after receiving neoadjuvant targeted therapy from July 2019 to October 2024.
RESULTS:
A total of 24 patients with EGFR-mutant lung adenocarcinoma from three centers were included in this study. All patients successfully underwent surgery and achieved R0 resection of 100.0%. The objective response rate (ORR) was 83.3% (20/24) . The major pathologic response (MPR) rate was 37.5% (9/24), with 2 patients (8.3%) achieving pathological complete response (pCR). During neoadjuvant therapy, 13 out of 24 patients (54.2%) experienced adverse events of grade 1-2, with no occurrences of ≥ grade 3. The most common treatment-related adverse events were rash (n=4, 16.7%), mouth sores (n=2, 8.3%), and diarrhea (n=2, 8.3%). The median follow-up time was 33.0 months, no deaths occurred in all patients, and the overall survival (OS) rate was 100.0%. The 1-year disease-free survival (DFS) rate was 91.1%, and the 2-year DFS rate remained at 86.2%.
CONCLUSIONS
The application of neoadjuvant targeted therapy in patients with EGFR-mutant resectable lung adenocarcinoma is safe and feasible, and is expected to become a highly promising neoadjuvant treatment option for the patients with EGFR-mutant lung adenocarcinoma.

5.Accuracy and safety of a new type of transfrontal lateral ventricular puncture in large hemispheric infarction

Objective:To evaluate the accuracy and safety profile of a novel cuboid orientation-guided frontal horn ventriculostomy technique in patients with large hemispheric infarction (LHI).Methods:It was conducted a retrospective cohort study of 48 consecutive LHI patients who underwent the innovative ventriculostomy procedure between time period. Primary outcomes included procedural accuracy (success rates, catheter positioning) and safety indicators (complication rates).Results:All the punctured ventricles were small or of normal size. The success rate of puncture was 100%, the success rate of one-time puncture was 87% (42/48), and the average number of puncture was 1.13 times per case. The ratio of well-positioned tube heads was 87.5% (42/48). The actual angle of the inward deviation of the puncture ranged from -2o to 5o, with an average of 0o±0.3o. The depth of puncture was 7.0-8.0 cm ( 7.3±0.3) cm. The incidence of bleeding around the puncture path was 1.3% (2/48 ) and no massive bleeding occurred. At the 6-month follow-up, one case (2.94%) among the 34 survivors had epilepsy.Conclusions:The cuboid orientation-guided frontal horn ventriculostomy technique demonstrates exceptional procedural accuracy and an excellent safety profile in LHI patients, with high first-pass success rates (87.5%) and minimal complications (4.2% minor hemorrhage). These findings support its clinical adoption for this patient population.
6.Effectiveness of PDCA cycle in refined management of printers in healthcare institutions

Objective To develop a refined printer management system based on the PDCA cycle,aiming to improve both operational cost efficiency and medical service quality through full-process management.Methods A total of 400 printers in use at the Third Affiliated Hospital of Sun Yat-sen University(Zhaoqing Hospital)were included from June 2024 to May 2025.The period from June to November 2024 was designated as the pre-refinement phase(under conventional management),while December 2024 to May 2025 served as the post-refinement phase(under PDCA-based refined management).A fishbone diagram was used to identify root causes of previous management inefficiencies,and targeted measures were developed and implemented.Outcomes were compared between the two phases.Results After implementing the PDCA cycle,the average equipment utiliza-tion rate increased by 5.41%,indicating optimized resource allocation.The average page yield per drum increased by 3.51%,reflecting improved consumable efficiency and reduced operational costs.The incidence of driver failures decreased by 34.71%,and operational issues dropped by 7.25%,demonstrating enhanced device reliability and system stability.Consultation-related work orders increased by 78.68%,and routine maintenance tasks rose by 30.62%,signaling a shift from reactive repairs to pro-active preventive maintenance.Conclusion The PDCA-based refined management model effectively enhances printer operational efficiency,reduces resource waste,and improves device reliability.This approach reinforces a patient-centered service philosophy by increasing the accuracy and efficiency of medical documentation output,thereby supporting safer and more efficient healthcare delivery.
7.Status of latent tuberculosis infection with Mycobacterium tuberculosis and risk factors for its activation in patients with ANCA-associated vasculitis

Objective To evaluate the status of latent tuberculosis infection(LTBI)with Mycobacterium tubercu-losis and the risk of LTBI activation after glucocorticoid therapy in patients with anti-neutrophil cytoplasmic antibody(ANCA)-associated vasculitis(AAV).Methods Clinical data of AAV patients who visited Xiangya Hospital of Central South University from May 2018 to May 2023 were retrospectively analyzed.According to the results of in-terferon-γ release assay(IGRA),they were divided into a LTBI group and a non-LTBI group.The LTBI group was subdivided into an active TB group(ATB group)and a non-ATB group according to the presence of ATB.General conditions and biochemical indicator characteristics of the two groups were compared.The COX regression model was used to analyze the risk factors for LTBI activation in AAV patients.Results A total of 302 AAV patients were included in this study,with an age of 64.0(53.0,71.0)years old,169 male cases(55.96%),and an IGRA positive rate of 24.17%(n=73).The proportion of patients with renal insufficiency in the LTBI group was higher than that in the non-LTBI group(79.45%vs 60.70%),with statistically significant difference(P<0.05).In the LTBI group,the age of patients was 62.0(53.5,72.0)years old,44 cases were male(60.27%),out of which 11 cases(15.07%)had ATB.In the non-LTBI group,4 male cases(1.75%)had ATB.In the LTBI group,there were statistically significant differences between the ATB group and the non-ATB group in terms of daily average glucocorticoid levels,age,renal dysfunction,neutrophils,and blood creatinine(all P<0.05).COX univariate re-gression analysis showed that statistically significant differences existed among the groups in terms of daily average glucocorticoid levels,daily average glucocorticoid levels>11.75 mg/day,age,age>65 years old,and blood crea-tinine levels ≥150 μmol/L(all P<0.05).COX multivariate regression analysis showed that daily average glucocor-ticoid levels>11.75 mg/day(OR=0.14,95%CI:1.30-79.01)and age>65 years(OR=0.08,95%CI:0.01-0.85)were independent influencing factors for LTBI activation.Conclusion AAV patients have a high rate of LT-BI,and daily average glucocorticoid levels>11.75 mg/day is an independent risk factor for LTBI activation in AAV patients.Age>65 years may benefit from receiving lower doses of immunosuppressive therapy,but it is necessary to expand sample size for further study.
