Objective:
To investigate the knowledge, attitudes, practices (KAP) and the situation of home blood pressure monitoring (HBPM) among hypertension patients in rural areas of Shanghai Municipality, so as to provide a basis for enhancing blood pressure management in this population.
Methods:
From June to October 2021, hypertension patients under management in Luodian Town and Luojing Town of rural areas of Shanghai Municipality were selected as survey subjects using a multistage random sampling method. Demographic information, KAP related to hypertension, and HBPM status were collected through questionnaire surveys. Patients were categorized into short disease duration (≤5 years), medium disease duration (6-10 years), and long disease duration (≥11 years) groups. KAP and HBPM status across different disease duration groups were analyzed statistically.
Results:
A total of 2 894 hypertensive patients were surveyed, including 1 317 (45.51%) males and 1 577 (54.49%) females. The median age was 69.00 (interquartile range, 10.00) years. There were 610 cases with short disease duration, accounting for 21.08%; 629 cases with medium duration, accounting for 21.73%; and 1 655 cases with long duration, accounting for 57.19%. The awareness of "hypertension control measures" was relatively high (97.89%), while awareness of "hypertension complications" was relatively low (71.70%). The adherence rate of "not perceiving difficulty in remembering to take medication on time and in correct doses" was relatively low (78.89%), and the proportion of participants who "walked ≥10 minutes per session at least once in the past 7 days" was relatively high (80.48%). A total of 1 209 cases (41.78%) engaged in HBPM, including 233 cases with short disease duration (19.27%), 252 cases with medium disease duration (20.84%), and 724 cases with long disease duration (59.88%). Compared with long disease duration hypertension patients, those with short disease duration had a alower awareness of "principles of pharmacological treatment for hypertension", a lower adherence rate of "medication should not be stopped after blood pressure control", and a lower proportion of providing their blood pressure measurement results to doctors (all P<0.017). However, they had higher proportions of alcohol consumption and daily salt intake >6 g/d (both P<0.017).
Conclusions
Short disease duration hypertension patients exhibit poorer performance in hypertension-related KAP and HBPM compared to long disease duration patients in rural areas of Shanghai Municipality. It is recommended to implement tailored health education based on disease duration, promote HBPM, and enhance blood pressure control rate.

Objective To investigate the role of bufalin(BU)in inhibiting M2-type macrophage-mediated colorec-tal cancer metastasis. Methods Human acute leukemia mononuclear cells(THP-1)were differentiated into M0 macrophages using phorbol ester induction(PMA)for 48 hours. The M0 macrophages were then treated with IL-4 and IL-13 medium.Surface markers and morphological changes were observed through ELISA,morphology, and RT-qPCR experiments.RT-PCR and ELISA experiments were conducted to detect the surface markers TGF-β and IL-10 of M2 macrophages. The secretion level of IL-6 in the supernatant of M2 macrophages and colorectal cancer cells HCT116 was compared using ELISA.Additionally, the effect of conditioned medium on colorectal cancer cell HCT116 was assessed through Transwell,Wound healing,RT-qPCR,and Western blot experiments.Subsequent-ly,bufalin was added to the conditioned medium and the changes in AKT/PI3K protein, migration, and epithelial-mesenchymal transition ability in HCT116 were observed using Western blot,Transwell,Wound healing and RT-qPCR experiments. Results THP-1 were successfully differentiated into M2 macrophages. The activation of AKT/PI3K protein in HCT116 cells was induced by the secretion of IL-6 from M2 macrophages, which in turn promoted the migration and epithelial-mesenchymal transition ability of the HCT116 cells. The migration and epithelial-mes-enchymal transition mediated by M2 macrophages in HCT116 cells were effectively inhibited by Bufalin.Conclu-sion The release of IL-6 from M2 macrophages activates the AKT/PI3K signaling pathway in colorectal cancer cells, thereby promoting their migration and epithelial-mesenchymal transition capacity. Moreover, bufalin exhibits inhibitory effects on this effect.

Gut and Liver 2023;17(6):874-883
Background/Aims:
The accuracy of endosonographers in diagnosing gastric subepithelial lesions (SELs) using endoscopic ultrasonography (EUS) is influenced by experience and subjectivity. Artificial intelligence (AI) has achieved remarkable development in this field. This study aimed to develop an AI-based EUS diagnostic model for the diagnosis of SELs, and evaluated its efficacy with external validation.
Methods:
We developed the EUS-AI model with ResNeSt50 using EUS images from two hospitals to predict the histopathology of the gastric SELs originating from muscularis propria. The diagnostic performance of the model was also validated using EUS images obtained from four other hospitals.
Results:
A total of 2,057 images from 367 patients (375 SELs) were chosen to build the models, and 914 images from 106 patients (108 SELs) were chosen for external validation. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the model for differentiating gastrointestinal stromal tumors (GISTs) and non-GISTs in the external validation sets by images were 82.01%, 68.22%, 86.77%, 59.86%, and 78.12%, respectively. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy in the external validation set by tumors were 83.75%, 71.43%, 89.33%, 60.61%, and 80.56%, respectively. The EUS-AI model showed better performance (especially specificity) than some endosonographers. The model helped improve the sensitivity, specificity, and accuracy of certain endosonographers.
Conclusions
We developed an EUS-AI model to classify gastric SELs originating from muscularis propria into GISTs and non-GISTs with good accuracy. The model may help improve the diagnostic performance of endosonographers. Further work is required to develop a multi-modal EUS-AI system.

Tumor 2023;43(11):886-894
Colorectal cancer(CRC)is a common malignant tumor of the digestive system. The main treatments for CRC currently include surgical resection, chemotherapy, radiotherapy, and immunotherapy.Immunogenic cell death(ICD)is an important method ofanti-tumortherapy.ICD is a specific type of cell death that involves the release of damage-associated molecular patterns(DAMPs).These DAMPs can be effectively taken up by dendritic cells(DCs),neutrophils, and macrophages, thereby stimulating adaptive immune responses in the body. However, colorectal cancer is considered a"cold tumor"with relatively low immunogenicity, resulting in a low response rate to immunotherapy.Inducing ICD can potentially enhance the immunogenicity of colorectal cancer, leading to long-term tumor control. This review aims to explore the impact of ICD development in colorectal cancer treatment and investigate the potential of ICD inducers in colorectal cancer immunotherapy.

Objective:To evaluate the effect of goal-directed fluid therapy (GDFT) based on stroke volume variation (SVV) guidance on splanchnic perfusion using gastric mucosal pH (pHi) value in elderly patients undergoing lumbar spine surgery. Methods:One hundred and sixty elderly patients of either sex, aged 65-80 yr, of American Society of Anesthesiologists physical status Ⅱ or Ⅲ, undergoing elective lumbar spine surgery under general anesthesia, were selected and divided into 2 groups ( n=80 each) using a random number table method: SVV-guided GDFT group (group G) and conventional fluid therapy group (group C). GDFT was performed with SVV<15% as the target in group G, while conventional fluid infusion was performed under the guidance of mean arterial pressure, central venous pressure and urine volume in group C. Intraoperative amount of crystalloid solution infused, amount of colloid solution infused, total volume of fluid infused, amount of bleeding, urine volume, use of vasoactive drugs and perioperative blood transfusion were recorded in both groups.Cardiac index, SVV, pHi, gastric intramucosal partial pressure of CO 2 (PgCO 2), PaCO 2 and results of arterial blood gas analysis were also recorded before induction of anesthesia (T 1), at 1 h after the start of surgery (T 2) and at the end of surgery (T 3). The postoperative complications, time to first flatus, time to defecation and duration of hospital stay were also recorded in both groups. Results:Compared with group C, the amount of intraoperative crystalloid solution infused, total volume of fluid infused, urine volume and requirement for vasoactive drugs were significantly decreased, the amount of colloid solution infused was increased, cardiac index and pHi value were increased at T 2, 3, and SVV, Pg-aCO 2 and arterial blood lactic acid concentrations were decreased ( P<0.05), and no significant changes were found in the intraoperative amount of bleeding, arterial blood pH value, PaCO 2 and base excess, incidence of postoperative complications, time to first flatus, time to defecation and duration of hospital stay in group G ( P>0.05). Conclusions:GDFT based on SVV guidance is superior to traditional fluid therapy in improving splanchnic perfusion in elderly patients undergoing lumbar spine surgery.

Objective To investigate the effect of Liu-Shen-Wan on transplanted tumors in mice with colon cancer based on the polarization of M2 macrophages in the tumor microenvironment. Methods We established a subcutaneous transplantation tumor model of mice with CT26 colon cancer. Mice were randomly divided into vehicle, oxaliplatin, and oxaliplatin combined with Liu-Shen-Wan groups. Treatment was administered for three weeks, and tumor volume was measured. All mice were weighed during the administration. After the end of the treatment, the mice were dissected and tumors were photographed and weighed. Spleen index was calculated. The expression levels of IFN-γ and IL-12P40 in serum and related blood biochemical indices were measured. The expression levels of M2 macrophage polarization indices, namely, IL-10 and TGF-β, in serum and tumor tissues were detected. The infiltration degree of M2 macrophages in each group was observed by immunohistochemical experiments. Results The tumor volume and mouse weight in the oxaliplatin combined with Liu-Shen-Wan group significantly decreased compared with those in the vehicle group. The spleen index increased, and the expression levels of IFN-γ and IL-12P40 in serum also significantly increased. The mice had no obvious side effects after the drug treatment. In addition, the expression levels of IL-10 and TGF-β in the serum and tissues of mice in the oxaliplatin combined with Liu-Shen-Wan group significantly decreased. The expression levels of CD68 and CD206 in tumor tissues also decreased. Conclusion The anti-tumor effect of Liu-Shen-Wan on the transplanted tumors of mice with colon cancer is related to the inhibition of M2 macrophage polarization in the tumor microenvironment.
