1.Analysis of CT manifestations of tumor-like proliferation of extramedullary hematopoiesis in thalassemia

Objective To investigate the CT imaging manifestations of tumor-like proliferation of extramedullary hematopoiesis(EMH)in thalassemia(TM).Methods A retrospective analysis was conducted on the clinical data of 41 patients with tumor-like proliferation of EMH in TM.The CT imaging features of the patients were analyzed,including the location,lesion morphology,density,and degree of enhancement of the lesions.Results Lesions of varying sizes were identified on both sides of the thoracic vertebrae in all 41 cases,including 20 cases with lesions in the inner edge of the ribs and 1 case within the spinal epidural space.A total of 23 abdominal scans and 2 lumbar scans,lesions were found in the sacral region in 18 cases,primarily in the presacral area and sacral wings.Lesions adjacent to the thoracic paravertebral and in the sacral region were quasi-circular,while those on the ribs were quasi-circular or flat and mound-shaped,closely adhering to the inner edge of the ribs.Intraspinal epidural lesions were characterized by a longitudinal orientation,positioned adjacent to the posterior part of the thoracic spinal canal.Most lesions showed homogeneous moderate density.Bone erosion was observed in 14 rib lesions and 7 sacral region lesions,resulting in heterogeneous slightly high density shadow of the lesions.Small nodular high density shadow were identified in 4 cases,and small amounts of fatty density shadow were observed in 4 cases.20 cases showed mild-to-moderate enhancement on enhanced scan,and 11 cases showed"vascular traversal sign"in thoracic paravertebral lesions.Conclusion The CT manifestations of tumor-like proliferation of EMH in TM are characteristic.The combination of typical CT manifestations and medical history facilitates an accurate diagnosis.Indicators such as fat replacement,iron deposition nodules,bone erosion,and"vascular traversal sign"are valuable in the diagnosis of tumor-like proliferation of EMH.

3.Analysis of pathological features and distribution characteristics of 1 237 upper gastrointestinal submucosal tumors

Objective To explore the pathologic features and distribution characteristics of upper gastrointestinal submucosal tumors (SMT).Methods From January 2013 to December 2017,at Department of Gastroenterology of Taizhou Hospital Affiliated to Wenzhou Medical University,clinical data of 1 182 hospitalized patients with 1 237 upper gastrointestinal SMT who underwent endoscopic therapy and diagnosed by pathology and immunohistochemistry was retrospectively analyzed including the pathological types,tumor of locations,endoscopic findings,layer of origin and tumor size.Results There were 473 esophageal SMT,including 387(81.8%) leiomyomas,located in the mucosal muscularis or muscularis propria;and 59(12.5%)cysts located in the submucosa or mucosal muscularis.There were 138(29.2%) lesions,159(33.6%) lesions and 176(37.2%) lesions in the upper,middle and lower esophagus respectively,and the most common type was leiomyoma.A total of 723 tumors were gastric SMT,among them 284 (39.3%) lesions were gastrointestinal stromal tumors (GIST) and 273(37.8%) lesions were leiomyomas,and all located in the muscularis propria.A total of 69(9.5%) lesions located at cardia,the common types were leiomyoma (55 lesions,79.7%) and GIST (nine lesions,13.0%).A total of 239 (33.1%) lesions located at gastric fundus,the common types were GIST (152 lesions,63.6%) and leiomyoma (79 lesions,33.1%).A total of 280 (38.7%) lesions located at gastric body,the common types were leiomyoma (138 lesions,49.3%) and GIST (111 lesions,39.6%).A total of 127 (17.6%) lesions located at gastric antrum,the common types were heterotopic pancrease (71 lesions,55.9%) and lipoma (26 lesions,20.5%),and all were located in the submucosa,some involved the muscularis propria.There were six (0.8%) lesions at gastric angle,and two (0.3%) at gastrointestinal anastomosis.Forty-one lesions were duodenal SMT,among them 23(56.1%) located at duodenal bulb,the common types were cyst (10 lesions,43.5%),lipoma (five lesions,21.7%) and heterotopic pancrease (five lesions,21.7%).A total of 18(43.9%) lesions located at descending duodenum,the common types were lipoma (nine lesions,50.0%) and cyst (five lesions,27.8%),and all lesions located in the submucosa.Conclusions The most common type of SMT in the esophagus and cardia is leiomyoma,however the SMT in gastric fundus and body are mostly leiomyomas and GIST,while in gastric antrum,most SMT are heterotopic pancreases and lipomas.In duodenal bulb and descending duodenum,the common types of SMT are cyst and lipoma.
4.Acupuncture and Chinese medicine of artificial cycle therapy for insulin resistance of polycystic ovary syndrome with phlegm damp type and its mechanism.

OBJECTIVETo explore the effect difference between acupuncture combined with Chinese medicine of artificial cycle therapy on the basis of western medication and simple western medication for polycystic ovarian syndrome (PCOS) of phlegm damp type with insulin resistance, and to explore its mechanism.
METHODSSixty patients were randomly assigned into an observation group and a control group, 30 cases in each group. Patients of the control group began to take letrozole and metformin orally on the third menstrual day. Patients of the observation group were treated with acupuncture combined with Chinese medicine of artificial cycle therapy on the basis of the treatment of the control group. The main acupoints were Guanyuan (CV 4), Qihai (CV 6) and bilateral Zigong (EX-CA 1), Luanchao (Extra), Fenglong (ST 40), Yinlingquan (SP 9), Zusanli (ST 36), Sanyinjiao (SP 6). Dahe (KI 12) was matched in the follicular phase, and warm acupuncture was applied at Guanyuan (CV 4) and Qihai (CV 6); Zhongji (CV 3) and Xuehai (SP 10) were matched in the ovulatory phase, and electroacupuncture were used at Qihai (CV 6) and Zhongji (CV 3), Xuehai (SP 10) and Yinlingquan (SP 9), 2 Hz and continuous wave; The acupoints in the luteal phase were the above main acupoints, at the same time, moxibustion was used at Guanyuan (CV 4) and Qihai (CV 6), the treatment was given once every other day, 30 min a time. There was no treatment in the menstrual phase. Chinese medicine was applied for tonifying kidney and nourishing blood, strengthening spleen and eliminating phlegm. The patients of the two groups were treated for continuous three menstrual cycles. The indexes before and after treatment were detected, including serum microRNA-29 (miR-29) expression, TCM symptom score, insulin (INS), blood-sugar content and homeostasis model assessment-insulin resistance (HOMA-IR) index, the endocrine hormone values of follicle stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T), LH/FSH ratio, prolactin (PRL). The pregnancy rates of the two groups were compared.
RESULTSAfter treatment the expressions of miR-29 and TCM symptom scores of the two groups were lower than those before treatment (<0.01,<0.05), with better results in the observation group (<0.01,<0.05). The levels of LH LH/FSH and T of the observation group were lower than those before treatment (<0.01,<0.05), and the T value in the control group decreased after treatment (<0.05). The changes of LH and LH/FSH in the observation group were better than those in the control group (both<0.05). After treatment, The INS, blood-sugar content and HOMA-IR were lower than those before treatment in the two groups (<0.05,<0.01), with better results for INS and HOMA-IR improvements in the observation group (both<0.05). The pregnancy rate in the observation group was 56.7% (17/30), which was significantly higher than 30.0% (9/30) in the control group (<0.05).
CONCLUSIONAcupuncture combined with Chinese medicine of artificial cycle therapy can improve the endocrine levels and insulin resistance of PCOS with phlegm damp type. At the same time, the pregnancy rate is improved. By the view of molecular biology, there may be a pathway trough changing serum miR-29 expression.
5.Clinical Study on Shaoyaozhitong Mixture in Treatment of Stage Ⅲ-Ⅳ Endometriosis

Objective To study the effect of Shaoyaozhitong Mixture on stageⅢ-Ⅳendometriosis, and explore its mechanism. Methods Ninty-five cases of stageⅢ-Ⅳ endometriosis were randomly divided into three groups. The traditional Chinese medicine (TCM) group (31 cases) was treated with Shaoyaozhitong Mixture, the GnRHa group (31 cases) was treated with Triptorelin Acetate for Injection, while the expectant group (33 cases) received no medications. The changes of CA125, TNF-α and VEGF levels, visual pain score and SF-36 score, total effective rate, pregnancy and recurrence were compared. Results There were significant increases in CA125, TNF-α and VEGF levels, visual analogue pain score and SF-36 score in all the three groups after treatment (P<0.05). The decline of CA125, TNF-αand VEGF levels and visual analogue pain score in TCM group and GnRHa group was significantly faster and greater than that of the expectant group (P<0.05). The total effective rates of TCM group was 83.9%(26/31), GnRHa group was 87.1%(27/31), and expectant group was 60.6%(20/33), TCM group and GnRHa group were superior to the expectant group, respectively (P<0.05). The pregnancy rate was higher and recurrence rate was lower in TCM group and GnRHa group than that of expectant group, but there was no statistical difference (P>0.05). Conclusion Shaoyaozhitong Mixture is effective in treating endometriosis by inhibiting the growth of ectopic endometrium.
