1.Effect of community comprehensive management model intervention among patients with dyslipidemia

Objective:
To evaluate the effect of community comprehensive management model intervention among patients with dyslipidemia, so as to provide the reference for optimizing community management strategies and improving the target achievement rate for blood lipids among this population.
Methods:
From May to June 2023, a multi-stage stratified random sampling method was employed to select patients with dyslipidemia from primary healthcare institutions in Jiaxing City, Zhejiang Province. Eligible participants were randomly assigned to either a control group or an intervention group. The control group received routine management, while the intervention group was subjected to a community comprehensive management model in addition to the routine care. Both groups were followed up for 24 months. Data on demographic characteristics, lifestyle behaviors, physical examination indices, and blood biochemical indicators were collected at baseline and after the intervention through questionnaires, physical examinations, and laboratory tests. Changes in obesity rate, central obesity rate, target achievement rates for blood lipids, blood pressure, and blood glucose, as well as lifestyle modifications, were analyzed. Differences between the two groups before and after the intervention were assessed using generalized estimating equations (GEE).
Results:
The control group consisted of 560 patients, including 303 females (54.11%) and 430 individuals aged ≥65 years (76.79%). The intervention group also included 560 patients, with 300 females (53.57%) and 431 individuals aged ≥65 years (76.96%). Before the intervention, no statistically significant differences were observed between the two groups in terms of gender, age, educational level, history of chronic diseases, and atherosclerotic cardiovascular disease risk stratification (all P>0.05). After 24 months of intervention, interaction effects between group and time were observed for obesity rate, central obesity rate, target achievement rate for blood lipids, target achievement rate for blood glucose, composite target achievement rate, physical activity rate, and medication adherence (all P<0.05). Specifically, the intervention group demonstrated lower rates of obesity and central obesity, and higher target achievement rate of blood lipids, target achievement rate of blood glucose, composite target achievement rate, physical activity rate, and medication adherence compared to the control group.
Conclusion
The community comprehensive management model contributed to improvements in multiple metabolic parameters (including body weight, waist circumference, blood lipids, and blood glucose) among patients with dyslipidemia, and was associated with increased physical activity rate and medication adherence.

Objective:To analyze the predictive value of residual lipoprotein cholesterol (RLP-C) combined with three-dimensional spot tracking (3D-STE) technology for myocardial ischemia in patients with coronary heart disease (CHD).Methods:A total of 158 patients with suspected CHD myocardial ischemia admitted to the First Affiliated Hospital of Xi′an Jiaotong University from March 2021 to August 2023 were selected and divided into the observation group (patients with myocardial ischemia) and the control group (patients with non-myocardial ischemia) according to the results of coronary angiography (CAG). The levels of low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and total cholesterol (TC) of the patients were detected by the fully automatic biochemical analyzer, and the level of RLP-C was calculated. The 3D-STE technique was adopted to analyze the global long axis strain (GLS), global circumferential strain (GCS), and global radial strain (GRS) of the left ventricle of the patients. The differences of each index between the two groups were compared, the related influencing factors of myocardial ischemia in patients with CHD were analyzed, and the predictive value of RLP-C combined with 3D-STE technology for myocardial ischemia in patients with CHD was evaluated.Results:CAG revealed 39 patients (24.68%) with myocardial ischemia, with a total of 52 myocardial perfusion abnormalities, including 22 right coronary artery supply areas, 12 left coronary artery circumflex supply areas, and 18 anterior descending branch areas. The levels of cardiac function classification, GCS, GRS, GLS and RLP-C in the observation group were all better than those in the control group (all P<0.05). Binary logistic regression analysis showed that cardiac function classification grade Ⅳ, GCS, GRS, GLS, and RLP-C were the influencing factors of myocardial ischemia in patients with CHD (all P<0.05). Analysis of the receiver operating characteristic curve results showed that the sensitivities of cardiac function classification grade Ⅳ, GCS, GRS, RLP-C, GLS and the combination of the five in predicting myocardial ischemia in patients with CHD were 69.23%, 71.79%, 74.36%, 76.92%, 74.36% and 89.74%, respectively. The specificities were 68.91%, 71.43%, 75.63%, 77.31%, 76.47% and 88.24% respectively. The combined diagnosis of myocardial ischemia in patients with CHD with cardiac function classification grade Ⅳ, GCS, GRS, RLP-C and GLS had a relatively high value (all P<0.05). Conclusions:Cardiac function classification grade Ⅳ, GLS, GCS, GRS, and RLP-C are influencing factors of myocardial ischemia in patients with CHD. Combined detection is helpful to improve the diagnostic accuracy of myocardial ischemia.
3.Clinical effects of Huoxue Qingre Formula combined with radiofrequency ablation on patients with thyroid nodules due to Phlegm Stagnation and Blood Stasis

AIM To investigate the clinical effects of Huoxue Qingre Formula combined with radiofrequency ablation on patients with thyroid nodules due to Phlegm Stagnation and Blood Stasis.METHODS Ninety patients were randomly assigned into control group(45 cases)for 3-month intervention of both Huoxue Qingre Formula placebo and radiofrequency ablation,and observation group(45 cases)for 3-month intervention of both Huoxue Qingre Formula and radiofrequency ablation.The changes in clinical effects,VRR,TCM syndrome score,cytokines(TNF-α,IFN-γ,IL-23,IL-1β),biochemical indices[Ca2+,25(OH)D,CRP],thyroid function indices(TSH,FT3,FT4)and safety indices were detected.RESULTS At postoperative 12 months,the observation group demonstrated higher total effective rate than the control group(P<0.05).At postoperative 3-12 months,the two groups displayed increased VRR(P<0.05),especially for the observation group(P<0.05,P<0.01).At the 7th,30th postoperative days,the two groups displayed decreased TCM syndrome score and cytokines(P<0.05,P<0.01),especially for the observation group(P<0.05,P<0.01).At the 30th postoperative day,the two groups displayed decreased Ca2+(P<0.01)and increased 25(OH)D(P<0.01),especially for the observation group(P<0.01).At the 7th postoperative day,the two groups displayed increased CRP(P<0.01),especially for the control group(P<0.05),which were decreased at the 30th postoperative day(P<0.05,P<0.01),especially for the observation group(P<0.01).No significant differences in thyroid function indices and safety indices were found between the two groups(P>0.05).CONCLUSION For the patients with thyroid nodules due to Phlegm Stagnation and Blood Stasis,Huoxue Qingre Formula combined with radiofrequency ablation can safely and effectively enhance nodule VRR,alleviate discomfort symptoms,reduce TCM syndrome score,inflammatory factor levels,and accelerate recovery.
4.Effects of Danzhi Jiangtang capsules on myocardial injury of db/db mice based on NLRP3 inflammasome-mediated pyroptosis

Aim To investigate the possible mechanism of the myocardial protective effect of Danzhi Jiangtang capsules(DJC)on db/db mice based on NLRP3 in-flammasome-mediated pyroptosis.Methods The db/db mice were randomly divided into the model group,DJC low,medium,and high dose groups,and the met-formin group,and the db/m mice were taken as the blank group.The administration lasted for eightweeks.At the end of drug administration,blood glucose,blood lipids,cardiac enzymes and inflammatory factors were detected in each group of mice.HE and Masson stai-ning was performed to observe the morphology and fi-brosis of myocardial tissue.TUNEL staining was per-formed to detect apoptosis.RT-qPCR was performed to detect the mRNA expression of ANP,BNP and β-MHC,and Western blot was performed to detect the protein expression of NLRP3,ASC,caspase-1,cleaved-caspase-1,GSDMD and GSDMD-NT in myocardial tis-sue.Results DJC could alleviate myocardial patho-logical damage,reduce collagen deposition and apopto-sis,reduce the levels of blood glucose,blood lipid,myo-cardial enzyme and inflammatory factors in db/db mice.DJC could reduce the mRNA expressions of ANP,BNP and β-MHC,and the protein expressions of NLRP3,ASC,caspase-1,cleavedcaspase-1,GSDMD and GSDMD-NT in myocardial tissues.Conclusion DJC attenuates myocardial injury in db/db mice,prob-ably by inhibiting the activation of NLRP3 inflamma-somes,attenuating cardiomyocyte pyroptosis,and amel-iorating the inflammatory state.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.
7.Exploring the Acupoint Selection Patterns for Acupuncture-Moxibustion Treatment of Chloasma Based on Data Mining Techniques

Objective To analyze and summarize the acupoint selection patterns for acupuncture-moxibustion treatment of chloasma using data mining techniques.Methods Computerized searches were conducted in major databases including the CNKI,Wanfang,VIP,CBM,PubMed,Embase,and the Cochrane Library.Clinical research literature on acupuncture-moxibustion treatment of chloasma published between January 1,2003 and August 31,2024 was retrieved.A database for acupuncture-moxibustion treatment of chloasma was established using Microsoft Excel 2019,and SPSS Modeler 18.0,Cytoscape 3.9.1,and SPSS 26.0 were employed to analyze the frequency of acupoint usage,meridian tropism,regional distribution,and specific acupoint categories,as well as to perform association rule analysis and cluster analysis.Results A total of 122 articles were included,involving 185 acupuncture-moxibustion prescriptions and 131 acupoints,with a total application frequency of 1 532.High-frequency acupoints included Sanyinjiao(SP6),Zusanli(ST36),Xuehai(SP10),and Hegu(LI4).Commonly used meridians were the spleen meridian of foot-taiyin,stomach meridian of foot-yangming,conception vessel(ren mai),bladder meridian of foot-taiyang,and large intestine meridian of hand-yangming.Specific acupoints were predominantly front-mu points,five-shu points,and lower he-sea points.Acupoints were mostly distributed in the chest/abdomen and lower limbs.Association rule analysis revealed that the core acupoint combination was"Sanyinjiao-Zusanli-Xuehai-Hegu".Cluster analysis classified high-frequency acupoints into five major categories.Conclusion The acupoint selection patterns for acupuncture-moxibustion treatment of chloasma reflect the principle of combining holistic concepts with syndrome differentiation.The core acupoint combination is"Sanyinjiao-Zusanli-Xuehai-Hegu".During treatment,acupoints in the chest/abdomen and lower limbs are frequently selected,often combined with acupoints on the head and face.

10.Influence of different disinfectants on ultrasound-guided internal jugular vein puncture and adhesive application

Objective To investigate the influence of different disinfectants on the ultrasound-guided internal jugular vein puncture and adhesive application.Methods A total of 102 emergency patients with traumatic hemorrhage who underwent ultrasound-guided internal jugular vein puncture and catheterization were selected as the study subjects and randomly divided into the iodine group(51 cases)and the chlorhexidine-alcohol group(51 cases).Iodophor(0.5%effective iodine)and 2.0%chlorhexidine gluconate+70.0%ethanol were used for disinfection before puncture,respectively.The disinfection effect of the puncture site,puncture condition,ultrasound imaging,and adhesive force of the application of patients were statistically compared between the two groups.Results There was no significant difference in the bacterial count before disinfection,the bacterial count after disinfection or the qualified rate of disinfection at the puncture site of patients between the two groups(P>0.05).There were significant differences in the success rate of one-time puncture,the number of puncture and the time of puncture and catheterization between the two groups(P<0.05).There were significant differences in the rate of clear ultrasound images,adhesive force and rate of warping or shedding of the application between the two groups(P<0.05).Conclusion For emergency patients with traumatic hemorrhage,using iodophor(0.5%effective iodine)and 2.0%chlorhexidine gluconate+70.0%ethanol disinfection before ultrasound-guided internal jugular vein puncture can achieve good disinfection effects.Iodine disinfection can obtain clearer ultrasound images and a higher success rate of one-time puncture,but the adhesive force of the application is poor,making it prone to warping or shedding.
