Objective To gain insights into the biological characteristics of different populations of Culex tritaeniorhynchus within and around the Yellow Sea wetland from the perspective of the occurrence of resistance, we investigated the levels of resistance to deltamethrin and kdr gene mutation in the wetland and its peripheral areas. Methods Specimens were collected from Cx. tritaeniorhynchus populations at two monitoring sites in the Rare Bird National Nature Reserve and Tiaozi Ni Wetland Scenic Area, and also from two populations in Yancheng City and the Liuhe District of Nanjing, and the resistance of these mosquitoes to deltamethrin was determined using the CDC biotest bottle method. For each concentration of deltamethrin assessed, a random subset of exposed specimens was selected for amplification of the kdr gene fragment, followed by Sanger sequencing to identify and analyze resistance-associated mutations. Results The LC50 levels of deltamethrin among mosquitoes from the four populations in Luhe, Yancheng, the Rare Bird National Nature Reserve and the Tiaozi Ni Wetland Scenic Area were 2.048 5, 7.798 2, 3.473 3, and 17.695 5 mg/mL, respectively, with corresponding concentrations of deltamethrin ranging from 0.005 to 5.000,0.050 to 50.000,0.050 to 25.000 and 0.050 to 50.000 mg/mL, respectively. Furthermore, the ranges of the KT50 values were 11.76-107.43, 67.05-216.30,29.77-107.43 and 28.40-329.51 min; the 1-h knockdown rates were 34.58%-99.15%, 9.52%-43.80%, 55.09%-73.01%, and 10.09%-68.07%; and the 24-h mortality rates were 12.15%-67.52%,9.52%-79.56%,13.17%-82.21%, and 11.01%-78.99%, respectively. With respect to kdr gene mutation, we assayed a total of 63,70,59, and 57 mosquitoes for the four populations, for which we detected L1014F mutation frequencies of 14.29%, 35.00%, 20.34%, and 31.58%, respectively, with a majority of these mutations being heterozygous for resistance. In addition, five adult mosquitoes were identified has having synonymous mutations at site 1011[i. e. , AAT(asparagine)mutation to AAC(asparagine)]. Conclusions Our findings revealed the clear resistance of Cx. tritaeniorhynchus to deltamethrin in the Yancheng region of the Yellow Sea wetland, and the resistance phenotype and kdr frequency of Cx. tritaeniorhynchus in the wetland environment were comparable to those of Cx. tritaeniorhynchus in the wetland environment, thereby indicating that the resistance of different populations of Cx. tritaeniorhynchus was homogeneous under the pressure of different insecticide selection within and around the wetland. However, the underlying mechanisms need to be further studied.

Objective:To construct a nomogram model for predicting the occurrence of acute kidney injury (AKI) in patients with cardiac arrest (CA) after cardiopulmonary resuscitation (CPR), and to verify its validity for early prediction. Methods:The study retrospectively included patients aged 18 years and older who received CPR for CA and were admitted to the emergency room of Tianjin Medical University General Hospital from February 2016 to September 2023. The general information, underlying diseases, resuscitation related indicators, and first laboratory test results of patients were collected. The patients were randomly divided into training and validation groups at a ratio of 7:3. AKI diagnosis was based on the diagnostic criteria of the Kidney Disease Improving Global Outcomes. Univariate and multivariate logistic regression models were used to identify independent risk factors for AKI in patients with cardiac arrest, and a nomogram was constructed on the basis of the independent risk factors. The predictive performance was evaluated by the area under the curve (AUC) of the receiver operating characteristic. The calibration curve, decision curve and clinical impact curve were used to evaluate the model. Bootstrap and cross validation methods were used for internal validation. Results:A total of 527 patients with cardiac arrest were included in the study, 230 patients developed AKI, with an AKI incidence of 43.6%. There was no statistically significant difference in clinical baseline data between the training and validation groups (all P>0.05), indicating comparability between the two groups of data. Multivariate logistic analysis revealed that age ( OR=1.346, 95% CI: 1.197-1.543, P<0.001), CA to CPR time ( OR=2.214, 95% CI: 1.512-3.409, P=0.016), adrenaline dosage ( OR=1.921, 95% CI: 1.383-2.783, P=0.004), APACHE-Ⅱ score ( OR=1.531, 95% CI: 1.316-1.820, P<0.001), baseline creatinine ( OR=1.137, 95% CI: 1.090-1.196, P<0.001), and lactate ( OR=2.558, 95% CI: 1.680-4.167, P<0.001) were the independent risk factors for AKI in patients with cardiac arrest. Initial defibrillable rhythm ( OR=0.214, 95% CI: 0.051-0.759, P=0.023) was a protective factor for AKI in patients with cardiac arrest. A nomogram prediction model was constructed based on the above variables. The AUC of the training group was 0.943 (95% CI: 0.921-0.965) and that of the validation group was 0.917 (95% CI: 0.874-0.960). This prediction model demonstrated good discrimination, calibration and clinical applicability. Conclusions:A nomogram predictive model was constructed on the basis of age, CA to CPR time, initial defibrillable rhythm, adrenaline dosage, the APACHE-Ⅱ score, and baseline creatinine and lactate levels. This nomogram has good predictive value for the early occurrence of AKI in patients with cardiac arrest after cardiopulmonary resuscitation, which can provide new strategies for the early identification of AKI and precise intervention.

ObjectiveThis study aims to develop and validate a novel multimodal predictive model, termed criss-cross network(CCNet)-directed graph neural network(DGNN)(CGN), for accurate assessment of pulmonary nodule progression in high-risk individuals for lung cancer, by integrating longitudinal chest computed tomography(CT) imaging with both traditional Chinese and western clinical evaluation data. MethodsA cohort of 4 432 patients with pulmonary nodules was retrospectively analyzed. A twin CCNet was employed to extract spatiotemporal representations from paired sequential CT scans. Structured clinical assessment and imaging-derived features were encoded via a multilayer perceptron, and a similarity-based alignment strategy was adopted to harmonize multimodal imaging features across temporal dimensions. Subsequently, a DGNN was constructed to integrate heterogeneous features, where nodes represented modality-specific embeddings and edges denoted inter-modal information flow. Finally, model optimization was performed using a joint loss function combining cross-entropy and cosine similarity loss, facilitating robust classification of nodule progression status. ResultsThe proposed CGN model demonstrated superior predictive performance on the held-out test set, achieving an area under the receiver operating characteristic curve(AUC) of 0.830, accuracy of 0.843, sensitivity of 0.657, specificity of 0.712, Cohen's Kappa of 0.417, and F1 score of 0.544. Compared with unimodal baselines, the CGN model yielded a 36%-48% relative improvement in AUC. Ablation studies revealed a 2%-22% increase in AUC when compared to simplified architectures lacking key components, substantiating the efficacy of the proposed multimodal fusion strategy and modular design. Incorporation of traditional Chinese medicine (TCM)-specific symptomatology led to an additional 5% improvement in AUC, underscoring the complementary value of integrating TCM and western clinical data. Through gradient-weighted activation mapping visualization analysis, it was found that the model's attention predominantly focused on nodule regions and effectively captured dynamic associations between clinical data and imaging-derived features. ConclusionThe CGN model, by synergistically combining cross-attention encoding with directed graph-based feature integration, enables effective alignment and fusion of heterogeneous multimodal data. The incorporation of both TCM and western clinical information facilitates complementary feature enrichment, thereby enhancing predictive accuracy for pulmonary nodule progression. This approach holds significant potential for supporting intelligent risk stratification and personalized surveillance strategies in lung cancer prevention.

Objective To evaluate the application value of four-dimensional CT(4D-CT)in the preoperative localization of primary hyperparathyroidism(PHPT). Methods A retrospective analysis was conducted on the clinical data and parathyroid 4D-CT images of 63 patients who underwent PHPT surgery at Peking Union Medical College Hospital between April 2020 and April 2023.Based on the clinical experience of the hospital's surgeons, parathyroid lesions were categorized into six anatomical regions:around the upper pole of the thyroid, posterior to the mid-thyroid,posterior to the lower pole of the thyroid and the tracheoesophageal groove, below the lower pole of the thyroid and the suprasternal fossa, retrosternal anterior mediastinum, and other rare locations. All images were independently analyzed by two experienced radiologists, with discrepancies resolved through discussion led by a senior radiologist.Using pathological results as the gold standard, the accuracy, sensitivity, specificity, positive predictive value(PPV),negative predictive value(NPV),Youden index, positive likelihood ratio(PLR),and negative likelihood ratio(NLR)of preoperative 4D-CT in diagnosing PHPT were calculated. Results There were no statistically significant differences between preoperative 4D-CT and surgical localization in the following regions:around the upper pole of the thyroid(χ2=0.500,P=0.480),posterior to the mid-thyroid(χ2<0.001,P>0.999),posterior to the lower pole of the thyroid and the tracheoesophageal groove(χ2=0.571,P=0.450),below the lower pole of the thyroid and the suprasternal fossa(χ2<0.001,P>0.999),retrosternal anterior mediastinum(χ2<0.001,P>0.999),and other rare locations(χ2<0.001,P>0.999).The preoperative 4D-CT diagnosis of PHPT lesions demonstrated a sensitivity of 82.09%,specificity of 97.43%,PPV of 87.30%,NPV of 96.19%,accuracy of 94.71%,Youden index of 79.52%,PLR of 31.94,and NLR of 0.18. Conclusion Parathyroid 4D-CT demonstrates good diagnostic efficacy in the preoperative localization of PHPT.

A mounting body of research suggests that circRNAs significantly contribute to the develop-ment of myocardial fibrosis. The microarray results of human circular RNA expression profile indicated that circHERC4_041 expression increased in the myocardium of patients with heart failure,RT-qPCR a-nalysis confirmed that the myocardial expression level of circHERC4_041 in individuals with heart failure were considerably elevated compared to that in healthy organ donors.Fluorescence in situ hybridization(FISH)confirmed that circHERC4_041 was abundant in the cytoplasm of human cardiomyocyte AC16.Overexpression of circHERC4_041 in mouse myocardial fibroblasts(mCFs)mediated by adenovirus in-hibited the expression of fibrosis-related proteins in mCFs.Experiments involving cell proliferation, wound healing, and Transwell assays demonstrated that overexpression of circHERC4_041 suppressed the growth and mobility of mCFs(P<0.001).Sequence analysis results suggested that circHERC4_041 con-tains potential ribosome entry sequence(IRES)and open reading frame(ORF).Western blot confirmed that circHERC4_041 could translate the 516 amino acid HERC4-516aa protein, which was mainly located in the cytoplasm of the cell.Cell functional experiments confirmed that circHERC4_041 inhibited the fi-brotic phenotype of mCFs by specifically translating HERC4-516aa(P<0.05).The specific interaction between HERC4-516aa and transglutaminase 2(TGM2)was confirmed by IP-MS screening and Co-IP i-dentification.Further results found that the degradation of TGM2 was promoted through proteasome path-way.The overexpression of TGM2 in mCFs facilitated by adenoviral vectors could counteract the suppres-sive effects of HERC4-516aa on the fibrotic phenotype of mCFs.Therefore, this study confirmed that the HERC4-516aa protein translated by circHERC4_041 can specifically bind to TGM2 to inhibit the fibrotic phenotype of myocardial fibroblasts.

Objective To investigate the effect of operation mode, thickness, and number of implanted electrode leads on tricuspid regurgitation. Methods This was a single-center retrospective study.Patients who received their first cardiac implantable electronic device at the Second Affiliated Hospital of Harbin Medical University from January 2019 to January 2024 and were followed up within(10±4)months were continuously enrolled.General clinical data, preoperative and outpatient follow-up echocardiography, and other indicators were collected.According to the different operation modes, patients were divided into delivery sheath group, support wire group, and delivery sheath combined support wire group.Under the premise of only one electrode wire crossing the tricuspid valve, according to the different thicknesses of the electrode wire, they were divided into 3830 69 group(4.1 F),5076 58/Solia S60 group(7 F),and 6935M-65cm defibrillator group(9 F).The number of electrode wires was divided into 3830 69 subgroup and 6935M-65cm defibrillator subgroup under the premise of balanced operation mode and electrode material. The changes in tricuspid valve regurgitation were compared in each group. Results A total of 219 patients,aged(65.92±12.04)years,were enrolled. There was no difference in tricuspid valve regurgitation in the 6935M-65cm defibrillator subgroup before implantation(all P>0.05).In the 3830 69 subgroup, there was a higher proportion of severe regurgitation in the two-electrode group before implantation(P=0.013).In comparison with the change in tricuspid valve regurgitation before electrode wire implantation and during follow-up,the regurgitation of the tricuspid valve in the 6935M-65cm defibrillator subgroup with≥2 electrodes was unexpectedly improved compared to that in the group with one electrode, and the regurgitation of the valve in the group with one electrode was more severe than that in the group with ≥ 2 electrodes implantation(P=0.019).Conclusions The operation mode and thickness of the implanted electrode lead did not cause significant differences in tricuspid valve regurgitation.Single implantation of a 6935M-65cm defibrillator electrode will lead to increased tricuspid regurgitation, and the effect brought by combination with physiological pacing will offset the adverse effects of electrode leads on the valve, providing directional evidence for future implantation strategy selection.
