An assay was established for detection of toxigenic Clostridioides difficile by combining microfluidic chip analysis with immunochromatography, and its performance was evaluated and compared with those of the Xpert C.difficile/Epi and VIDAS CD AB tests.Primer pairs were designed according to the tcdB and tpi genes in C.difficile.The specificity, limit of detection, reproducibility, and stability were evaluated.A total of 215 stool samples from patients with diarrhea were collected and tested in parallel with the Xpert C.difficile/Epi,VIDAS CDAB,and our assay.C.difficile was isolated from samples, and the tcdB gene was identified when discrepant results were obtained from the three above assays. Our assay showed no cross-reaction with other diarrhea-associated pathogens. Its reproducibility was 100%in testing of two standard plasmids containing tcdB and tpi genes at two concentrations(105 and 102 copies/μL).Two standard plasmids were detected after the PCR and immunochromatography reagents had been stored for 3,6,9,and 12 months, and all the results were posi-tive.The limit of detection was 10 copies/μL for toxigenic C.difficile.Testing of 33 samples positive for C.difficile with our assay(33/215,15.3%)yielded findings statistically coherent with those of the Xpert C.difficile/Epi test(kappa value=0.965).The sensitivity, specificity, positive predictive value, and negative predictive value of our assay, with respect to Xpert C.difficile/Epi as the standard, were 94.3%,100.0%,100.0%,and 98.9%;these values were significantly higher than those of VIDAS CDAB(60.0%,98.9%,91.3%,and 92.7%)(Kappa=0.714,OR=157.50,95%CI:62.03-847.28,P=0.013).In conclusion, our newly developed assay is specific, stable, and reproducible, and may be used for rapid and accu-rate detection of toxigenic C.difficile.The assay could be used for C.difficile infection screening in outpatient and emergen-cy,community medical service center, and epidemiological settings.

OBJECTIVE To retrospectively investigate the incidence of surgical site infection(SSI),distribution and drug resistance of pathogens in 53 hospitals of Suzhou area from 2020 to 2023 so as to provide strategies for pre-vention and control of the hospital-associated infections in Suzhou area.METHODS The data regarding the SSI,distribution and drug resistance of pathogens were statistically analyzed through Suzhou City Regional Hospital In-fection Surveillance Platform.RESULTS The incidence of SSI was 0.24%among the 53 hospitals from 2020 to 2023.Staphylococcus aureus,Enterococcus faecalis,Staphylococcus epidermidis,Enterococcus faecium and Staphylococcus haemolyticus ranked the top 5 species of gram-positive bacteria;Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa,Enterobacter cloacae and Acinetobacter baumannii ranked the top 5 spe-cies of gram-negative bacteria. There were significant differences in the distributions of S.aureus,K.pneumoniae and A.baumannii in the superficial, deep and organ cavities(P<0.05).The isolation rate of carbapenem-resistant Acinetobacter baumannii(CRAB)was 69.23%,and the isolation rate of methicillin-resistant Staphylococcus au-reus(MRSA)was 42.14%.The drug resistance rate of the S.aureus strains to oxacillin reached up to 40.24%;the drug resistance rates of the A.baumannii to imipenem and meropenem reached up 63.00%and 65.80%,re-spectively.The drug resistance rates of the K.pneumoniae strains to imipenem and meropenem approximated to 30%,and the drug resistance rate to tigecycline was 19.84%.CONCLUSIONS The total incidence of SSI shows a downward trend in the 53 hospitals of Suzhou area. The S.aureus is the major species of pathogens, and some of the pathogens show high drug resistance rates.

Objective:To analyze the screening results of the Urban Cancer Early Diagnosis and Treatment Project in Qinghai Province from 2019 to 2024.Methods:A summary and statistical analysis were conducted on six years of screening data from the Urban Cancer Early Dia-gnosis and Treatment Program in Qinghai Province, with the high-risk rate, screening rate, and detection rate calculated separately for each type of cancer. Results:From 2019 to 2024,56,882 high-risk individuals were identified. The high-risk rates for lung,colorectal,breast,up-per gastrointestinal, and liver cancer were 22.02%,21.57%,14.23%,13.52%,and 6.10%,respectively.Overall,13,592 individuals com-pleted clinical screening, with detection rates of 0.32%for lung cancer,0.41%for liver cancer,0.08%for precancerous gastric lesions,3.63%for precancerous colorectal lesions,0.08%for esophageal cancer,0.16%for gastric cancer, and 0.14%for colorectal cancer. Conclusions:The implementation of the Urban Cancer Early Diagnosis and Treatment Program in Qinghai Province aids in the early detection of cancer, improves early diagnosis and survival rates, and reduces mortality.Nevertheless, due to low public awareness and limited participation,en-hancements in program management and public outreach are required.

Objective:To construct a framework for evaluating the quality of health economic evaluation methodology based on the pragmatic clinical trial. Methods:An evaluation framework was constructed based on existing quality evaluation tools for health economic evaluation other quality evaluation tools. The weights of each item in the framework were determined by the Delphi method, and the weighted average was calculated using the expert authority coefficient. Results:A total of 23 experts were consulted, and the expert authority coefficients were 0.88 and 0.90,respectively.The results of the Wilcoxon signed-rank test showed no statistically signifi-cant differences among the expert opinions in two rounds(P>0.05).Finally,a framework with 3 dimensions and 8 items was estab-lished.Conclusion:The evaluation framework has high scientificity and reliability.

Objective: To investigate the clinical efficacy and safety of pudendal nerve modulation (PNM) in the treatment of female pudendal neuralgia (PN),so as to promote the clinical application of this technique. Methods: A retrospective analysis was conducted on 20 female PN patients who failed conservative treatment at Gongli Hospital during Nov.2020 and Oct.2023.All patients underwent simultaneous PNM and sacral nerve modulation (SNM) with the assistance of 3D printing navigation.Dual-stage test electrodes for PNM and SNM were implanted, followed by alternate therapeutic trial for each modality.Secondary conversion rates and longitudinal outcomes, including visual analogue score (VAS),patient health questionnaire-9 (PHQ-9),and quality of life (QoL) scores were compared preoperatively,post-stage Ⅰ,and at 3,6,and 12 months post-stage Ⅱ. Results: All operations were successful. After the trial phase, the secondary conversion rate for PNM was significantly higher than that for SNM; 16 patients (16/20,80%) chose the second-phase PNM implantation surgery,3 (3/20,15%) chose second-phase SNM implantation, and 1 (1/20,5%) had electrodes removed due to ineffective results from both trials.Further assessment revealed that the improvements in VAS,PHQ-9,and QoL scores for PNM patients were significantly better than those for SNM patients after the first phase of surgery and at 3,6 and 12 months after the second-phase conversion (P<0.05).No complications such as electrode migration or infection were observed during the follow-up of 12-15 months. Conclusion: PNM provides more effective relief of pain symptoms and improvements in depressive states for female PN patients compared to SNM.With the assistance of 3D printing navigation, the operation is simple and safe, and offers stable therapeutic effects. It is worthy of clinical promotion and application.
