1.Colorimetric Sensor for Sensitive Detection of Glutathione Based on Metal-Organic Framework Nanosheets

Glutathione(GSH)is a tripeptide containing sulfhydryl groups,which has the function of antioxidant,detoxifying,and immune enhancing effects,and its expression is closely associated with many diseases such as cancer,etc.Therefore,it is of great significance for clinical diagnosis by developing a highly sensitive,simple,and rapid method for detecting GSH.Herein,two-dimensional(2D)Zn-TCPP(Fe)nanosheets employing Zn2+and Fe-bound tetrakis(4-carboxyphenyl)porphyrin ligands were obtained by a surfactant-assisted synthetic method.The 2D Zn-TCPP(Fe)nanosheets exhibited excellent peroxidase-like activity,which allowed the catalysis of the H2O2-initiated oxidation of colorless 3,3',5,5'-tetramethylbenzidine(TMB)to blue oxidized TMB(ox-TMB).In the presence of GSH,GSH underwent a reduction reaction with oxTMB,resulting in the blue color of the solution fading.The proposed colorimetric sensor exhibited favorable sensitivity for GSH in the linear range of 0.1-200 μmol/L with a limit of detection(S/N=3)of 0.042 μmol/L,indicating excellent selectivity.The developed sensor was successfully applied to detect GSH level in the human serum samples with recoveries of 93.0%-107.7%,showing satisfactory results.The developed colorimetric sensor provided a new approach for detecting GSH.
2.Optimizing 5-aminosalicylate for moderate ulcerative colitis: expert recommendations from the Asia-Pacific, Middle East, and Africa Inflammatory Bowel Disease Coalition

The lack of clear definition and classification for “moderate ulcerative colitis (UC)” creates ambiguity regarding the suitability of step-up versus top-down treatment approaches. In this paper, experts address crucial gaps in assessing and managing moderate UC. The Asia-Pacific, Middle East, and Africa Inflammatory Bowel Disease Coalition comprised 24 experts who convened to share, discuss and vote electronically on management recommendations for moderate UC. Experts emphasized that the goal of treating UC is to attain clinical, biomarker, and endoscopic remission using cost-effective strategies such as 5-aminosalicylates (5-ASAs), well-tolerated therapy that can be optimized to improve outcomes. Experts agreed that 5-ASA therapy could be optimized by maximizing dosage (4 g/day for induction of remission), combining oral and topical administration, extending treatment duration beyond 8 weeks, and enhancing patient adherence through personalized counselling and reduced pill burden. Treatment escalation should ideally be reserved for patients with predictors of aggressive disease or those who do not respond to 5-ASA optimization. Premature treatment escalation to advanced therapies (including biologics and oral small molecules) may have long-term health and financial consequences. This paper provides consensus-based expert recommendations and a treatment algorithm, based on current evidence and practices, to assist decision-making in real-world settings.
3.Summary of best evidence for sleep disordered breathing assessment and intervention in stroke patients

Objective:To systematically retrieve, evaluate, extract, and integrate the best evidence on sleep disordered breathing (SDB) assessment and intervention in stroke patients to inform clinical practice.Methods:Based on the "6S" evidence model, the literature on SDB assessment and intervention in stroke patients was searched top-down on clinical decision-making websites, guideline websites, professional association websites, and databases. The search period was from the establishment of the database to September 1, 2024. The evidence-based team was formed to select the corresponding tool for quality assessment based on the type of literature. The evidence was also summarized using the Joanna Briggs Institute Center for Evidence-Based Health Care quality level of evidence and grade of recommendation.Results:A total of twenty papers were included, including six expert consensus, six guidelines, four clinical decision-making, two evidence summaries, and two systematic reviews. Eighteen pieces of evidence were summarized around four aspects of assessment, treatment, adherence, and health education.Conclusions:The assessment and intervention of SDB in stroke patients is multifaceted. It is recommended that healthcare professionals dynamically observe the actual condition of patients when applying the evidence to scientifically assess and manage SDB in stroke patients.
4.Causal effects and cerebrospinal fluid metabolites mediators between immune cell and risk of breast cancer:a Mendelian randomization study

Objective Mendelian randomization(MR)analysis was used to explore the genetic link between immunophenotype and breast cancer(BC)risk and how cerebrospinal fluid(CSF)metabolites play a part in mediating this.Methods We used MR to assess the genetic associations between immune cells and BC risk and their possible mediators.Genetic statistics for immune cells and CSF metabolites were obtained from the Genome-Wide Association Study(GWAS)catalog,whereas those for BC were obtained from the Japan Biobank,the UK Biobank,and FinnGen's cross-ethnic meta-analysis.We performed a two-sample MR analysis using inverse variance weighting(IVW)to investigate the genetic association between immunoepidemiology and BC.We also analyzed CSF metabolites as mediators between them.Heterogeneity was tested using the Cochran's Q statistic,horizontal pleiotropy was tested using the MR Egger intercept,and sensitivity analysis was performed using the"leave-one-out"method.Results MR analysis by the IVW method showed that HLA DR+CD4+T cells were associated with a reduced risk of BC(OR=0.972,95% CI:0.955-0.990,P=0.003),and there was a negative genetic association between HLA DR+CD4+T cells and methylsuccinimidyl carnitine level(OR=0.922,95% CI:0.861-0.986,P=0.018),but there was a positive genetic association between the latter and BC risk(OR=1.029,95% CI:1.012-1.047,P<0.001).Mediation analysis showed that the direct effect remained significant after correction for CSF methylsuccinylcarnitine level(β=-0.026,SE=0.008,P=0.002).And the indirect effect(β=-0.002,Delta Method SE=0.001)suggested that this CSF metabolite might mediate 8.36%of the association in the protective effect of immune cells against BC risk(95% CI:-12.4%-29.1%).Conclusion Genetically predicted HLA DR+CD4+T cells may reduce the risk of BC development by modulating the level of methylsuccinylcarnitine,the CSF metabolite.
5.Construction of a rapid and quantitative platform for loop-mediated isothermal amplification and smart-phone-based detection of Cariogenic microorganisms

Objective:To establish a rapid,quantitative and visualized method for the rapid detection of Streptococcus mutans(S.mutans)based on loop-mediated isothermal amplification(LAMP).Methods:Nucleic acid of S.mutans was extracted for LAMP reaction,the detection experimental conditions were optimized.Finally the positive results of the experiments were quantita-tively analyzed by using the mobile-based detection equipment.Results:The system detected the samples within 23 min and showed good specificity and sensitivity with an accuracy of 1.625×10-3 ng/μL.Positive results were captured,identified and quantified u-sing a mobile application,and the quantified results showed a good linear relationship between the absolute G value and the DNA concentration,with an equation of y=3.2x+57.133(R2=0.988 2).Conclusion:Quantitative detection of S.mutans by LAMP has the characteristics of high sensitivity,high specificity,result visualization,convenience and time-saving,and can quantify bacterial DNA.It is a new method for the early detection of caries susceptibility,and can be widely used in clinic.

As an innovative dosage form, traditional Chinese medicine(TCM) dry powder inhalers have emerged as a focal point in the research and development of new preparations due to its high efficiency, safety, and bioavailability. This paper systematically reviewed the relevant literature and patents associated with TCM dry powder inhalers to analyze the origins and the current research and development status. Furthermore, this paper probed into the research and development ideas of TCM dry powder inhalers regarding clinical positioning, prescription screening, and druggability. Additionally, the paper thoroughly analyzed the technical barriers in druggability studies and elaborated on corresponding research techniques and coping measures. Furthermore, it emphasized the need for improved regulations and policies governing TCM dry powder inhalers, advocated for strengthened oversight, and called for the establishment of a scientific quality evaluation system. Measures such as promoting production-education-research collaboration, enhancing personnel training, and fostering international exchanges were proposed to provide a scientific and systematic reference for the future research, development, and application of TCM dry powder inhalers, thereby facilitating the rapid modernization of TCM.

10.Effect of comorbidity for patients with non-small cell lung cancer on exercise tolerance and cardiopulmonary function: A propensity score matching study

Objective To observe the effect of comorbidity for patients with non-small cell lung cancer (NSCLC) on exercise tolerance and cardiopulmonary function. Methods NSCLC patients who underwent cardiopulmonary exercise testing (CPET) before surgery were retrospectively included. According to the Charlson comorbidity index (CCI) score, patients were divided into two groups: a CCI≥3 group and a CCI<3 group. The patients were matched with a ratio of 1 : 1 by propensity score matching according to the age, body mass index, sex, smoking history, exercise habits, pathological stage and type of surgery. After matching, CPET indexes were compared between the two groups to explore the differences in exercise tolerance and cardiopulmonary function. Results A total of 276 patients were included before matching. After matching, 56 patients were enrolled with 28 patients in each group, including 38 (67.9%) males and 18 (32.1%) females with an average age of (70.7±6.8) years. Compared with the CCI<3 group, work rate at peak (WR peak), WR peak/predicted value (WR peak%), kilogram oxygen uptake at anaerobic threshold (VO2/kg AT), VO2/kg peak, VO2/kg peak%, peak carbon dioxide output, the minute ventilation to carbon dioxide production slope, O2 pulse peak and O2 pulse peak% of CCI≥3 group were statistically different (P<0.05). Among them, the rate of postoperative pulmonary complication in the CCI≥3 group was higher than that in the CCI<3 group (60.7% vs. 32.1%, P=0.032). Conclusion In the NSCLC patients, exercise tolerance and cardiopulmonary function decreased in patients with CCI≥3 compared with those with CCI<3. CPET can provide an objective basis for risk assessment in patients with comorbidity scored by CCI for pulmonary resection.
