Familial adenomatous polyposis (FAP) is an autosomal dominant genetic disorder caused by mutations in the adenomatous polyposis coli gene. FAP is characterized by the development of hundreds to thousands of adenomatous polyps in the colon. While initially the primary concern was colorectal cancer, recent advancements in surveillance and prophylactic colectomy have shifted the focus toward the management of upper gastrointestinal (GI) neoplasms, particularly duodenal and gastric lesions. This review focuses on the epidemiology, clinical features, treatment options, and surveillance strategies for treating upper GI neoplasms in patients suffering from FAP. A review of the risk assessment through the Spigelman classification system and current guidelines for both nonampullary and ampullary lesions was conducted for duodenal neoplasms. Regarding gastric neoplasms, the characteristics and management strategies for fundic gland polyps, gastric adenomas, and cancers were discussed. Overall, early screening and timely removal of suspicious lesions are crucial for the management of upper GI neoplasms in patients with FAP for effective cancer prevention.

Background: This study aimed to investigate the size and characteristics of pediatric clinic closure during the coronavirus disease 2019 (COVID-19) period and what career paths pediatricians chose after closure. Methods: This study utilized database of the Health Insurance Review and Assessment Service from 2013 to 2022. We examined the trend of the number of pediatric clinics in operation over the past 10 years. Additionally, the study identified factors associated with the closure of pediatric clinics during the COVID-19 pandemic. Furthermore, the affiliations of representatives who closed their clinics during the pandemic were tracked as of December 2022. Results: In 2019, there were 2,229 pediatric clinics. During the COVID-19 pandemic, 364 (16.3%) of these clinics closed. Factors associated with the closure of pediatric clinics included pediatricians over the age of 65, operational periods of less than 5 years, and lower levels of medical expenses. As of 2022, among the 364 clinics that closed, 108 pediatricians (29.7%) retired or ceased working, and 127 pediatricians (34.9%) still employed in pediatricrelated healthcare institutions. A concerning phenomenon is that the remaining 129 pediatricians (35.4%) transitioned to unrelated healthcare institutions. Conclusion We have identified the magnitude and factors contributing to pediatric clinic closures. A more pressing issue is that over one-third of the pediatricians have transitioned to non-specialty fields following the closure of their clinics. Pediatrics represents a critical and essential medical field. Health authorities must develop strategies to prevent the avoidable collapse and subsequent exodus of pediatricians.

South Korea’s current vaccination policies leave a surveillance gap for non-National Immunization Program (NIP) vaccines. In this study, we proposed a sentinel surveillance approach for monitoring the safety of non-NIP vaccines. Vaccination data were collected retrospectively among patients hospitalized with pre-defined adverse events of special interest (AESI) by reviewing electronic medical records in five university hospitals. This approach incorporates expert assessment to determine the causal relationship. We confirmed that 16 patients had received non-NIP vaccines among 860 patients diagnosed with AESI.We concluded one case of preeclampsia was possibly related to tetanus-diphtheria-pertussis vaccination. We propose a multi-hospital-based, retrospective assessment system for predefined AESIs as an alternative to active vaccine safety monitoring method. These efforts are expected to enhance both the accuracy and timeliness of safety monitoring in South Korea.

Purpose: This study aimed to identify the types of human rights violations and the associated psychological trauma experienced by North Korean defectors. It also examined the impact of trauma on the defectors’ interpersonal relationships, employment, and overall quality of life, while evaluating existing psychological support policies to suggest potential improvements. Methods: A multidisciplinary research team conducted an observational survey and in-depth interviews with approximately 300 North Korean defectors residing in South Korea from June to September 2017. Standardized measurement tools, including the Post-Traumatic Stress Disorder (PTSD) Checklist (PCL-5), Patient Health Questionnaire-9 (PHQ-9), Generalized Anxiety Disorder Scale-7 (GAD-7), and Short Form-8 Health Survey (SF-8), were employed. Statistical analyses consisted of frequency analysis, cross-tabulation, factor analysis, and logistic regression. Results: The findings revealed a high prevalence of human rights violations, such as public executions (82%), forced self-criticism (82.3%), and severe starvation or illness (62.7%). Additionally, there were elevated rates of PTSD (56%), severe depression (28.3%), anxiety (25%), and insomnia (23.3%). Defectors who resided in China before entering South Korea reported significantly worse mental health outcomes and a lower quality of life. Moreover, trauma was strongly and negatively correlated with social adjustment, interpersonal relationships, employment stability, and overall well-being. Conclusion An urgent revision of existing policies is needed to incorporate specialized, trauma-informed care infrastructures within medical institutions. Furthermore, broad societal education to reduce stigma and enhance integration efforts is essential to effectively support the psychological well-being and social integration of North Korean defectors.

Purpose: . This study evaluated the accuracy of implant impression techniques using scan healing abutments compared to conventional techniques. Materials and methods: . A mandibular model with implants at #34, 36, and 46 was used. Experiments were conducted for single implant and 3-unit bridge scenarios. Groups were as follows: Group IOS - Scan healing abutments were scanned with an intraoral scanner. Group Imp - Scan healing abutments were used, and impressions were taken with silicone material, followed by model scanning. Group Sb - Scan bodies were scanned with an intraoral scanner. Group Trans - Impressions were taken with transfer copings, and models were scanned. Each group was scanned 10 times, and deviations in central distance and angulation of analogs were measured. Statistical analyses were performed using ANOVA and post-hoc tests at a 95% confidence level. Results: . For single implants, there were no significant differences in distance deviation (P > .05). Angular deviation values were: IOS (0.24 ± 0.10°), Imp (0.19 ± 0.06°), Sb (0.38 ± 0.05°), and Trans (0.29 ± 0.06°), with Sb showing significantly larger values than IOS and Imp (P < .05). For bridges, distance deviation did not significantly differ (P > .05). Angular deviations were: IOS (0.21 ± 0.08°), Imp (0.21 ± 0.09°), Sb (0.25 ± 0.10°), and Trans (0.46 ± 0.19°), with Trans showing significantly larger values than the others (P < .05). Conclusion . Scan healing abutment techniques showed similar accuracy to traditional methods. They could be a viable alternative for single implant crowns or short-span fixed dental prostheses.

Objective: Extraction of the mandibular second molar (L7) and substitution by the mandibular third molar (L8) is an effective treatment option. This study aimed to evaluate spontaneously erupted L8 occlusion after L7 extraction, and identify the influencing factors. Methods: This study assessed 46 L8 from 28 patients using dental study models, panoramic radiographs, and lateral cephalograms obtained during L7 extraction (T1) and completion of L8 eruption (T2). At T2, samples were categorized as acceptable (A-group) or unacceptable (U-group) based on the American Board of Orthodontics index.L8 angulation and position, retromolar space, distance between the Xi point and mandibular first molar (L6), and Nolla stage were compared between the groups to identify the predictive factors for successful eruption. Results: At T2, 58.7% of L8 exhibited acceptable occlusion. Age at T1 was significantly higher in the U-group than that in the A-group. Angles ∠6-MnP and ∠8-MnP differed significantly between the groups at T2. Xi-L6 distance was considerably longer in the A-group than that in the U-group at T1 and T2. Younger age at extraction and Xi-L6 distance at T1 affected the acceptable occlusion. Conclusions Younger age at L7 extraction and adequate eruption space (Xi-L6 distance) appear to be the key factors for achieving acceptable L8 occlusion.
