1.Impact of hemodynamic pattern of non-culprit vessel stenosis on the long-term prognosis in patients with acute ST-segment elevation myocardial infarction

Objective:To investigate the correlation between the hemodynamic pattern of non-culprit vessel stenosis and long-term vessel-oriented composite outcome(VOCO) in patients with acute ST-segment elevation myocardial infarction (STEMI).Methods:From January 2019 to December 2021, 233 consecutive patients with STEMI and non-culprit vessel stenosis were prospectively enrolled at Shanghai East Hospital. The median follow-up duration was 3.9 years. The 367 non-culprit vessels of the 233 patients were divided into the VOCO group (33 vessels, 9.0%) and the non-VOCO group (334 vessels, 91.0%). Parameters pertaining to the hemodynamic pattern of non-culprit vessel stenosis between the two groups were compared. Receiver operating characteristic (ROC) curves were used to assess the correlation between hemodynamic pattern and VOCO, and Cox multivariate regression and logistic multivariate regression analyses were applied to identify independent predictors of VOCO.Results:The 233 enrolled patients were aged (62.5±12.9) years, with 193 males (82.8%). In the VOCO group, the maximum quantitative flow ratio (QFR) decreased within 20 mm of the QFR-assessed segment, the difference in QFR across the entire vessel, the length of functionally significant vessel, and the maximum gradient of QFR decrease (dQFR/dsmax) were significantly greater than those in the non-VOCO group. ROC curve analysis showed that the optimal threshold for predicting VOCO using dQFR/dsmax was 0.009 6 (area under the curve: 0.691, 95% CI: 0.606-0.775, P<0.001). Multivariable Cox regression analysis revealed that dQFR/dsmax was an independent predictor of VOCO ( HR=1.199, 95% CI: 1.070-1.343, P=0.002). When anatomical and functional stenosis severities were included in the model, a high pullback pressure gradient (PPG) index ( HR=1.572, 95% CI: 1.052-2.351, P=0.027) emerged as an independent predictor of VOCO. Multivariable logistic regression analysis revealed that a low PPG index( OR=2.851, 95% CI: 1.945-4.178, P<0.001) was an independent predictor of QFR≤0.80 without long-term VOCO. Conclusion:In patients with STEMI, localized hemodynamic patterns of coronary artery stenosis, characterized by high dQFR/dsmax and high PPG index, are associated with long-term VOCO.
2.Subchondral bone intravoxel incoherent motion diffusion weighted imaging for quantitatively evaluating knee joint cartilage degeneration

Objective To observe the value of subchondral bone(SB)intravoxel incoherent motion diffusion weighted imaging(IVIM-DWI)for quantitatively evaluating knee joint cartilage degeneration.Methods Totally 125 patients with knee cartilage degeneration were prospectively enrolled and divided into mild group(n=65,Outerbridge grades Ⅰ-Ⅱ)and moderate-severe group(n=60,Outerbridge grades Ⅲ-Ⅳ)based on arthroscopic findings.Additionally,30 healthy young subjects were recruited as control group.The apparent diffusion coefficient(ADC),true diffusion coefficient(D),pseudo diffusion coefficient(D*)and perfusion fraction(f)values of medial condyle of SB of femur,lateral condyle of femur,medial tibial plateau and lateral tibial plateau,as well as the mean values of each parameter of the above 4 regions were obtained based on knee joint IVIM-DWI and compared within and among groups.Spearman correlation analysis was used to observe the correlations between quantitative parameters and Outerbridge grading.The area under the receiver operating characteristic curve(AUC)was calculated to evaluate the efficacy of each quantitative parameter alone and their combination for assessing knee cartilage degeneration.Results Significant differences of D*values were found among different parts of knee joint SB in moderate-severe group,also of D value,D* value and f value among different parts of knee joint SB in mild group(all P<0.05).Significant differences of IVIM-DWI parameter were found among different parts of knee joint SB in control group(all P<0.05).ADC value of knee joint SB in moderate-severe group,mild group and control group decreased successively,while D value increased successively(all P<0.001).D* values of knee joint SB in moderate-severe group was higher than that in mild group and control group(both P<0.05).No significant difference of f value of knee joint SB was found between moderate-severe group and mild group(P>0.05),while both the latter two were higher than that in control group(both P<0.05).ADC value,D value and f value of knee joint SB were positively correlated(rs=0.671,0.634,0.416,all P<0.01),while D* value of knee joint SB was negatively correlated with Outerbridge grading(rs=-0.729,P<0.01).AUC of combination of parameters for distinguishing mild group from control group,and mild group from moderate-severe group was 0.859 and 0.808,respectively.Conclusion SB IVIM-DWI could be used for quantitatively evaluation of knee cartilage degeneration.

5.Efficacy of continuous venovenous hemodiafiltration combined with hemoperfusion in the treatment of acute kidney injury and its influence on levels of serum nerve guidance factor-1 and kidney injury factor-1

Objective:To explore the efficacy of continuous venovenous hemodiafiltration (CVVHDF) combined with hemoperfusion in the treatment of acute kidney injury (AKI) and its influence on the levels of serum nerve guidance factor-1 (Netrin-1) and kidney injury factor-1 (Kim-1).Methods:A total of 193 patients with AKI diagnosed and treated in the First Affiliated Hospital of Xinjiang Medical University from January 2018 to January 2021 were prospectively selected and divided into the control group (96 cases) and the observation group (97 cases) according to random number table method. The control group was given conventional treatment, and the observation group was given CVVHDF combined with hemopirrigation on the basis of conventional treatment. The levels of Netrin-1, Kim-1, renal function index, critical disease score and inflammatory response index before and after treatment were compared between the two groups.Results:After 72 h of treatment, the serum levels of Netrin-1, Kim-1, creatinine and urea nitrogen in the observation group were lower than those in the control group: (5.43 ± 0.61)ng/L vs. (7.52 ± 0.83) ng/L, (0.97 ± 0.23) ng/L vs. (1.52 ± 0.29) ng/L, (97.58 ± 8.51) μmol/L vs. (109.80 ± 7.56) μmol/L, (5.72 ± 1.19) mmol/L vs. (7.40 ± 1.75) mmol/L, there were statistical differences ( P<0.05). After 72 h of treatment, the scores of acute physiology and chronic health evaluation Ⅱ and multiple organ dysfunction syndrome in the observation group were lower than those in the control group: (14.26 ± 5.62) scores vs. (16.82 ± 3.75) scores, (7.15 ± 0.86) scores vs. (8.23 ± 0.92) scores, there were statistical differences ( P<0.05). After 72 h of treatment, the levels of tumor necrosis factor-α, interleukin-6 and C-reactive protein in the observation group were lower than those in the control group: (13.26 ± 4.06) ng/L vs. (29.30 ± 5.81) ng/L, (14.56 ± 3.29) ng/L vs. (29.88 ± 5.40) ng/L, (12.06 ± 3.43) mg/L vs. (33.82 ± 4.94) mg/L, there were statistical differences ( P<0.05). Conclusions:CVVHDF combined with hemoperfusion can effectively improve the renal function of patients with AKI, reduce the levels of serum inflammatory factors, alleviate the disease and promote recovery.
7.Professional identity and its influence factors of administrators in public hospitals in Shanghai

Objective:This study investigated the professional identity of administrators in 3rd class hospitals in Shanghai, analyzing the professional identity of administrators and associated factors in public hospitals.Methods:In May 2019, Huashan Hospital conducted a questionnaire survey. Through the analysis of the survey data, this study discussed the correlation between the professional identity degree and the external factors. Three main dimensions of occupational identity were extracted by principal component analysis, namely the professional self-identity, organizational identity and social identity. External influencing factors were analyzed by factor analysis, and five main factors were extracted. Results:It was found that the professional self-identity was much higher than the organizational and social identity. The degree of organizational identity was significantly related to external factors ( r=0.212,0.426,0.146,0.580,0.610, P <0.01). Respect factors ( r=0.553,0.580,0.570) and self-realization factors ( r=0.563,0.610,0.433) had the highest correlation with professional identity. Conclusion:Initiating with improving organizational identity, the authorities and hospitals should establish a scientific and reasonable promotion and incentive mechanism to enhance the degree of professional identity.

9.Effect of different extracts from Rheum palmatum on weight and tissue structure of hypothalamusand and pituitary of rats

Objective To compare the effects of different extracts of Rheum palmatum on weight and tissue structure of hypothalamusand and pituitary of adult female rats,and screen the main reproductive toxicity extract.Methods The water,chloroform,ethyl acetate,and n-butanol extracts and water-soluble substance of R.palmatum were prepared by polarity gradient extraction method.Female adult rats were randomly divided into blank control group,rhubarb water extract group,and different extracts groups.The dosage of all the groups was equivalent to 4.00 g/kg crude rhubarb.Rats were administered with extracts by gavage for 60 d.Body mass growth rate of rats were calculated before and after administration.The pathological changes of hypothalamic arcuate nucleus neurons and pituitary gonadal cell were observed with light microscope.Results Compared with the blank control group,the body mass growth rate of rhubarb water extract group was decreased (P < 0.05),while those in the different extracts groups were increased (P < 0.01);The hypothalamic arcuate nucleus neurons of rhubarb water extract group showed chromatin marginalization,nissl substance dissolving,fuzzy boundary of nuclear membrane,as well as hell cells,and the total number of adenohypophysis cells reduced and the cells arranged in irregular.However,there were no apparent pathologic changes in different extracts groups.Conclusion Rhubarb water extract administration by long-term dose can reduce weight growth rate and result in pathologic changes of hypothalamic arcuate nucleus and adenohypophysis,while the different extracts can increase weight growth rate significantly and have little effects on the organizational structure ofhypothalamic arcuate nucleus and adenohypophysis.
10.Effect of TGF-β1on the expression of MMP-9 in mouse endometrial stromal cells

Objective To investigate the effect of transforming growth factor-β1(TGF-β1) on the expression of matrix metal-loproteinase-9(MMP-9) in cultured mouse endometrial stromal cells .Methods After separation and purification ,the mouse endom-etrial stromal cells were cultured with different concentrations of TGF-β1 ,and the final concentrations were 0 .1 ,2 .5 ,5 .0 ,10 .0 , 20 .0 ng/mL ,respectively .ELISA method was used to detect the content of MMP-9 in the the culture medium after 48 h culture . Results Compared with the control group ,the expression of MM P-9 in mouse endometrial stromal cells in the experimental groups increased with the TGF-β1 concentration elevation ,and the difference was statistically significant (P < 0 .01) .Conclusion In the mouse endometrial stromal cell system ,TGF-β1 may be involved in the regulation of extracellular matrix metabolism through regu-lating the expression of MMP-9 .
