1.Progress in Application of Novel Functional Hemostatic Dressings in Patients with Continuous Bleeding after PICC Catheterization.

The high incidence of bleeding after peripherally inserted central catheter (PICC) catheterization increases the risk of puncture site infection and unplanned extubation. Hemostatic dressings should be used in the early stages of catheterization to reduce blood infiltration. However, new hemostatic dressings have various types and advantages, which makes them difficult to choose dressings for medical staff. This paper introduces the types and hemostatic characteristics of novel functional hemostatic dressings, reviews the hemostatic mechanism and hemostatic effect of chitosan, cyanoacrylate gum, alginate, gelatin sponge and oxycellulose dressings in PICC puncture respectively, and prospects the development of new functional hemostatic dressings. It is expected that future hemostatic dressings will move towards multifunctionality and compositeness.

2.Expression and functional mechanism of PPFIA4 gene in hepatocellular

Objective:
To explore the expression level of PPFIA4 in hepatocellular carcinoma tissues and HCCLM3 cells and its regulation of the biological behavior of hepatocellular carcinoma.
Methods :
Bioinformatics analysis, Western blot, and immunohistochemistry were employed to detect the expression of PPFIA4 in tumor tissues of patients with hepatocellular carcinoma and analyze the prognosis of these patients. An siRNA plasmid was designed to knock down the expression of PPFIA4 in HCCLM3 cells. The effects of PPFIA4 knockdown on the migration and invasion abilities of HCCLM3 cells were then evaluated using scratch healing and Transwell assays. Furthermore, Western blot was utilized to detect the expression levels of epithelial-mesenchymal transition(EMT)-related protein markers in the HCCLM3 cell line after transfection with the siRNA plasmid.
Results:
PPFIA4 was highly expressed in hepatocellular carcinoma tissues and hepatocellular carcinoma cells( HCCLM3, Li-7, MHCC97H); the high expression of PPFIA4 indicated that the clinical stage of patients was late and the overall survival(OS) was short. After knocking down the expression of PPFIA4 in HCCLM3 cell line, the migration and invasion ability of HCCLM3 cells decreased(P<0.001) and the expression of EMT markers changed. The expression of epithelial cell marker E-cadherin increased(P<0.01), while the expression of mesenchymal markers Vimentin and N-cadherin decreased(P<0.05,P<0.01).
Conclusion
PPFIA4 is highly expressed in hepatocellular carcinoma tissues and hepatocellular carcinoma cell lines and is associated with poor prognosis of patients. Silencing PPFIA4 can regulate the biological behavior of hepatocellular carcinoma cells and inhibit the migration and invasion of HCCLM3 cells. The specific mechanism may be related to EMT.
3.Long term efficacy and dynamic characteristics analysis on laparoscopic fundoplication for gastroesophageal reflux cough

Objective:To investigate the safety and efficacy of laparoscopic fundoplication in the treatment of gastroesophageal reflux related cough (GERC).Methods:Clinical data of 138 GERC patients undergoing laparoscopic fundoplication at our center from Jan 2013 to Dec 2017 were analyzed and patients were followed up until Dec 2022.Results:A total of 138 patients underwent surgery successfully.The median follow-up time was 51 (7-130) months.There were no mortality,nor major complications. In 138 GERC patients, GERC was cured in 85 patients,the cure rate of fundoplication was 61.6%, and altogether 112 patients reported effective, the effective rate was 81.2%. Multivariate regression analysis showed that normal esophageal peristalsis was independent predictors of good efficacy of fundoplication[ OR=3.810(1.373,10.573), P=0.01]. Conclusions:Laparoscopic fundoplication has long term efficacy for refractory GERC. Esophageal motility is an important dynamic characteristic that affects the postoperative efficacy of GERC.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.

With technological advancements and the accumulation of follow-up data, controversies surrounding key technical aspects have emerged, significantly affecting clinical decision-making. This article systematically reviews the evolution of surgical anti-reflux techniques, focuses on current critical controversies, and explores future directions by integrating evidence-based medicine with clinical experience.
6.Application of telemedicine in patients with breast cancer-related lymphedema: a scoping review

Objective:To conduct a scoping review on the application of telemedicine in patients with breast cancer-related lymphedema (BCRL), to generalize and summarize the intervention methods, application forms, outcome indicators and effects of telemedicine, so as to provide a reference for healthcare professionals to implement interventions.Methods:Computerized searches of PubMed, Web of Science, Embase, Cochrane Library, CINAHL, China National Knowledge Infrastructure, Wanfang Data, China Biology Medicine disc, and VIP were performed with a search period from their establishment to June 1, 2024. The included articles were screened, summarized and analyzed.Results:A total of 14 papers were included. Telemedicine intervention methods included tele-education, tele-exercise, tele-monitoring, tele-consultation, tele-psychological interventions, tele-visits, and tele-evaluation. Application forms involved web-based cloud platforms, social network service systems, telemedicine APPs, and remote portable monitoring devices. Outcome indicators included physical indicators, psychological indicators, quality of life, incidence of lymphedema, degree of lymphedema symptoms, and feasibility indicators.Conclusions:Telemedicine relies on web-based platforms and social network service software to conduct tele-education, tele-consultation, and tele-exercise for BCRL patients, and its feasibility and effectiveness have been proven. In the future, it is necessary to accelerate the establishment of breast cancer telemedicine centers, optimize the telemedicine service team, and increase the investment in the development of portable lymphedema telemonitoring equipment, so as to build a comprehensive and perfect breast cancer telemedicine system and improve the effectiveness of the prevention and treatment of BCRL.
