1.Role of class relationship in the influence of perceived discrimination on pro social behavior of left behind junior middle school students

Objective:
To examine the Longitudinal influence of perceived discrimination on pro social behavior of left alone junior middle school students and to explore the longitudinal mediating role of teacher-student relationship and classmate relationship, so as to provide a reference for improving pro social behavior of left behind junior middle school students.
Methods:
The Perceived Discrimination Questionnaire, Teacher-Student Relationship Questionnaire, Classmate Relationship Questionnaire and Prosocial Behavior Questionnaire were used to conduct two follow up surveys on 930 left behind junior middle school students in Guizhou Province at two time points, in mid November 2021(T1) and mid May 2022(T2). Deviation corrected Bootstrap method was used to examine the mediating effect of T2 teacher-student relationship and T2 classmate relationship on the impact of T1 discrimination perception on T2 pro social behavior.
Results:
The perceptions of discrimination, teacher-student relationship, classmate relationship, and pro social behavior scores of the left behind junior high school students were (1.98±0.94) (4.13± 0.77 ) (3.54±0.91) (3.52±0.75) for T1, and (3.98±0.83)(3.42±0.86)(3.48±0.72) for teacher-student relationship, classmate relationships and pro social behavior scrores of T2. After controlling for gender, age and self assessment of family economic status, T1 perceived discrimination negatively predicted T2 pro social behavior ( β =-0.07); and T1 perceived discrimination indirectly influenced T2 pro social behavior through T2 teacher-student relationship, and mediated effect value was -0.02(95% CI = -0.02 to -0.01); and T1 perceived discrimination indirectly influenced T2 pro social behavior through T2 classmate relationship, mediated effect was -0.03(95% CI =-0.05 to -0.02)( P <0.05).
Conclusions
Perceived discrimination not only directly reduces pro social behavior of left behind junior middle school students, but also indirectly and negatively affects pro social behavior of left alone junior middle school students by reducing teacher-student relationship and classmate relationship.

3.Two-dimensional ultrasound combined with HD-live silhouette technology in embryos/fetuses with normal or abnormal central nervous system during early pregnancy

Objective:To study the ultrasonographic characteristics of embryos/fetuses with normal or abnormal central nervous system (CNS) from 7 to 13 +6 weeks of gestation using high resolution two-dimensional ultrasound combined with HD-live silhouette technology and provide a reference for early diagnosis of CNS abnormalities. Methods:Eighty normal embryos/fetuses during 7-13 +6 weeks and 41 fetuses with CNS malformations in early pregnancy during 11-13 +6 weeks were selected to observe the ultrasonographic features of embryos/fetuses with normal or abnormal CNS using transvaginal high resolution two-dimensional ultrasound and HD-live silhouette technology. Descriptive analysis was performed on the results. Results:From seven weeks of gestational age, high resolution two-dimensional ultrasound combined with HD-live silhouette technology can clearly and stereoscopically show the prosencephalon, mesencephalon and rhombencephalon. The rhombencephalon changed the most in the brain development of embryos. At nine weeks of gestation, cleared structures of pons curvature, the fourth ventricle and cisterna magna were observed. The developing cerebellum and the original Blake pouch cyst were seen at 10 weeks of gestation. From 11 to 13 +6 weeks, the most remarkable change was the choroid plexus of the fourth ventricle changed from perpendicular to parallel to the long axis of the neural tube. Of the 41 fetuses with CNS malformation, 16 (39.0%) were exencephaly, 11 (26.8%) were holoprosencephaly, five (12.2%) were encephalocele, four (9.7%) were anencephaly, three (7.3%) were fourth ventricle dilatation, and two (4.9%) were open spina bifida. Conclusions:High resolution two-dimensional ultrasound combined with HD-live silhouette technology can clearly and stereoscopically display the morphological changes in embryonic embryos/fetuses with development of normal CNS at 7-13 +6 weeks, which is helpful to better understand the origin of CNS embryonic abnormalities and provide diagnostic clues for the early detection of CNS abnormalities.

5.Comparison of validation results and leaf open time before and after upgrading of helix tomotherapy planning system

Objective:To compare the leaf open time (LOT) parameters and γ passing rates between the 4.0.4 and 4.2.3 helix Tomotherapy planning systems and evaluate the improvement.Methods:Retrospective comparison of the treatment plans of 345 cases selected by 4.0.4 and 4.2.3 versions was performed. The Machine Specific Sinogram of each plan was extracted from the archived plan file to calculate the LOT. The evaluated LOT parameters included the projection time, the maximum LOT, the mean non-zeros time, the time difference between the projection time and the maximum LOT, the relative count of leaves with LOT greater than maximum LOT minus 5 ms, the relative count of leaves with LOT lower than 100 ms and the beam on time. The γ passing rate (criteria: 3 mm/3%, 10% threshold and global error) and the LOT parameters between two version systems were evaluated with the independent t-test. The relationship between the LOT parameters and γ passing rate was analyzed by the multiple linear regression method. Results:The γ passing rate of the Ver 4.0.4 system was 97.86%, significantly lower than 98.6% of the Ver 4.2.3 system ( P<0.001). The time gap between the projection time and the maximum LOT of the Ver 4.2.3 system was significantly less than that of the Ver 4.0.4 system (1 ms vs. 11 ms, P<0.001). For the Ver 4.0.4 system, the multiple linear regression method showed that the maximum LOT ratio and the beam on time were negatively correlated with the γ passing rate (both P<0.001). However, for the Ver 4.2.3 system, only the beam on time showed a negative correlation with the γ passing rate ( P<0.001). Conclusion:The γ passing rate of the Ver 4.2.3 system is significantly higher than that of the Ver 4.0.4 system. The decrease of γ passing rate caused by the leaves near the maximum LOT is properly resolved in the new version system.
6.Sensitivity evaluation of ArcCheck in detecting leaf open time errors of helical tomotherapy delivery

Objective:To evaluate the sensitivity of the ArcCheck dosimetry system in detecting the leaf open time errors during the center and off-center helical tomotheray delivery quality assurance (DQA).Methods:Nine nasopharyngeal carcinoma (NPC) patients were selected in this study. Two DQA plans were created for each patient: the" center" plan was created by moving the image of the ArcCheck phantom to place the high dose region on the phantom center and the " off-center" plan was created by offsetting the phantom and putting several diodes through a higher does region. Leaf open time errors of 2, 4, 6, 8 and 10 ms were introduced to the Sinogram which was modified using Matlab. Each intentional error plan and original (no error) plan for each patient were measured using both " center" and " off-center" DQA methods, the γ analysis was performed to evaluate the DQA results. The different dose and distance error criteria of 3%/3 mm, 3%/2 mm and 2%/2 mm were selected, and different thresholds of 5%, 10% and 15% were selected for γ analysis. The gradient and the minimum detectable error approach were taken to quantitatively analyze the sensitivity. The correlation between different dose distance error criteria and different thresholds was also evaluated by Pearson correlation analysis. Results:The absolute value of γ gradient of the " center" DQA plans were larger than those of the " off-center" plans in all different γ criteria (all P<0.05). The stricter the γ criteria were adopted, the more sensitive DQA results of leaf open time error were obtained. The minimum detectable error was 2 ms in all different γ criteria for the " center" DQA plan. The minimum error detectability of the " off-center" DQA plan was weaker than that of the " center" DQA plan. The γ passing rates of three different dose distance error criteria were significantly strongly correlated for the " center" DQA plan ( R2>0.9). For the " off-center " DQA plan, only the 3%/3 mm and 3%/2 mm criteria were significantly strongly correlated ( R2>0.9). Significant strong correlation was observed in the γ passing rate at different thresholds between the " off-center" and " center" DQA plans. Conclusions:The " center" DQA plan method is more sensitive than the " off-center" DQA plan method in all γ criteria, and the γ passing rates in different γ criteria are strongly correlated for the " center" DQA plan. The " center" DQA plan method is recommended.

BACKGROUND:
Pain and cartilage destruction caused by osteoarthritis (OA) is a major challenge in clinical treatment.Traditional intra-articular injection of hyaluronic acid (HA) can relieve the disease, but limited by the difficulty of longterm maintenance of efficacy.
METHODS:
In this study, an injectable and self-healing hydrogel was synthesized by in situ crosslinking of N-carboxyethyl chitosan (N-chitosan), adipic acid dihydrazide (ADH), and hyaluronic acid–aldehyde (HA-ALD).
RESULTS:
This supramolecular hydrogel sustains good biocompatibility for chondrocytes. Intra-articular injection of this novel hydrogel can significantly alleviate the local inflammation microenvironment in knee joints, through inhibiting the inflammatory cytokines (such as TNF-a, IL-1b, IL-6 and IL-17) in the synovial fluid and cartilage at 2- and even 12-weeks post-injection. Histological and behavioral test indicated that hydrogel injection protected cartilage destruction and relieved pain in OA rats, in comparison to HA injection.
CONCLUSION
This kind of novel hydrogel, which is superior to the traditional HA injection, reveals a great potential for the treatment of OA.
8.Cytomegalovirus infection in preterm infants of gestational age <32 weeks or birth weight <1 500 g fed by their own mother's frozen breast milk

Objective:To understand cytomegalovirus (CMV) infection status in hospitalized preterm infants who were fed by their own mother's frozen breast milk.Methods:This retrospective study enrolled breastfed neonates with gestational age less than 32 weeks or birth weight less than 1 500 g who were born and admitted to Gynecology and Obstetrics Hospital of Fudan University from January 2018 to December 2020. Clinical data of the babies and their mothers were collected and analyzed, including CMV DNA results of breast milk and urine samples of the subjects by fluorescence quantitative polymerase chain reaction. Chi-square test (or Fisher's exact probability test), two independent samples t test, and Mann-Whitney U test were used for the statistical analysis. Results:A total of 94 parturients and their 103 premature infants (including nine pairs of twins) were included. CMV DNA of breast milk was noted for positive in 75 cases (including eight pairs of twins) and for negative in 28 cases (including one pair of twins). Out of the 75 preterm infants born to mothers with positive CMV DNA breast milk, 67 (including eight pairs of twins) were switched to frozen breast milk (-20 ℃ for 72 h), and six of them were infected by CMV(9.0%) without any treatment. All of the 103 infants were divided into two groups: the frozen milk fed group ( n=67) or fresh milk fed group ( n=36). In the frozen milk fed group, the CMV DNA was mainly detected during 2-8 weeks postpartum with copy number reaching the peak at 8th week. And those infants in the frozen milk fed group, whose mother's breast milk CMV DNA was positive, was further divided into CMV infected ( n=6) or CMV non-infected groups ( n=61) according to the urine test results. Moreover, compared with the non-infected group, the average [22.7(3.0-95.7)×10 3 copies/ml vs 5.0(0.5-89.5)×10 3 copies/ml, Z=-2.218) and the highest[45.9(5.9-261.0)×10 3 copies/ml vs 9.8(1.2-766.0)×10 3 copies/ml, Z=-2.218] copy number of CMV DNA in the breast milk were higher in the CMV infected group (both P<0.05). The incidence of feeding intolerance[37.3% (25/67) vs 50.0% (18/36), χ2=1.550], neonatal necrotizing enterocolitis [0.7% (1/67) vs 0.0% (0/36)], bronchopulmonary dysplasia [28.4% (19/67) vs 27.8% (10/36), χ2=0.004], retinopathy of prematurity [20.9% (14/67) vs 8.3%(3/36), χ2=2.682], and late-onset sepsis [22.4% (15/67) and 30.6% (11/36), χ2=0.828] did not differ significantly between the frozen or fresh milk fed groups (all P>0.05). Conclusions:The incidence of breast milk-related CMV infection in those fed with frozen breast milk was low and does not increase the without increasing risks of related complications or leading to obvious clinical manifestations after infection. For preterm infants with gestational age <32 weeks or birth weight <1 500 g, frozen breast milk can be an alternative for mothers with CMV DNA positive breast milk.

10.Effect of remote pulmonary rehabilitation management in patients with CPOD

Objective:To explore the effect of remote pulmonary rehabilitation management in chronic obstructive pulmonary disease (COPD) patients.Methods:From May 2017 to May 2019, convenience sampling was used to select 114 COPD outpatients in the Respiratory Department of the First Affiliated Hospital of Zhengzhou University. All patients were randomly divided into control group ( n=57) using conventional pulmonary rehabilitation management and observation group ( n=57) using remote pulmonary rehabilitation management, respectively, and the management period was one year. We compared the differences in COPD self-management ability, body mass index, airflow obstruction, dyspnea, exercise capacity (BODE) index, and average annual hospital admission rate between the two groups. Results:After intervention, the scores and the total score of self-management of observation group were higher than those of control group, and the differences were statistically significant ( P<0.01) . The dimension scores and the total score of self-management of observation group after the intervention were higher than those before intervention, and the differences were statistically significant ( P<0.05) . After intervention, the observation group's percentage of forced vital capacity in the first second to predicted value and the 6 minutes walking distance were higher than those of control group, and the dyspnea score and BODE index were lower than those of control group, and the differences were statistically significant ( P<0.05) . The average annual admission rate of the observation group and the control group were 5.41% and 9.06%, respectively. The average annual hospital admission rate of observation group was lower than that of control group, and the difference was statistically significant (χ 2=6.806, P=0.009) . Conclusions:Remote pulmonary rehabilitation management can improve the self-management ability of COPD patients, improve pulmonary function and reduce the admission rate of patients, which is worthy of clinical promotion.
