1.Epidemiological analysis of imported malaria in Shaoxing City from 2012 to 2024

Objective:To analyze the epidemiological characteristics of imported malaria in Shaoxing City and provide scientific basis for formulating prevention and control measures against imported malaria.Methods:A retrospective analysis was conducted. Data on imported malaria cases reported in Shaoxing City from 2012 to 2024 were collected from the infectious disease reporting information system of China Center for Disease Control and Prevention, and the parasitic disease control information management system, including general information, epidemiological investigation reports and diagnosis/treatment status, to analyze their epidemiological characteristics.Results:From 2012 to 2024, there were 110 imported malaria cases in Shaoxing City, including 105 males and 5 females, with a sex ratio of 21 ∶ 1. The youngest patient was 21 years old and the oldest was 73 years old, with 72 cases (65.45%) in the 30-50 age group. The 110 malaria cases included 69 cases of falciparum malaria, 22 cases of vivax malaria, 15 cases of ovale malaria, 3 cases of quartan malaria, and 1 case of mixed infection. The infection sources were Africa (84.55%, 93/110) and Asia (15.45%, 17/110). Among all patients, there were 19 foreign nationals and 91 Chinese citizens. The main purposes of travel for Chinese citizens were labor work (67.03%, 61/91) and business (29.67%, 27/91). Areas with more reported cases were Zhuji City (32 cases), Keqiao District (27 cases), Xinchang County (19 cases) and Yuecheng District (19 cases). The peak months of onset were January, July and September, with 12, 12 and 18 cases respectively. The first medical visits were mainly at county-level hospitals, center for disease control and prevention at all levels and municipal hospitals, with diagnosis rates of 86.67% (52/60), 100% (21/21) and 12/15 respectively. Compared with cases without malaria history, cases with malaria history had shorter time from onset to diagnosis (median, 5.00 d vs. 2.00 d, Z =-2.40, P = 0.004). Conclusions:From 2012 to 2024, imported malaria in Shaoxing City was mainly falciparum malaria, with Africa being the main infection source. The peak incidence occurred in summer. Male young adult overseas workers were the high-risk population. The prevention and control of imported malaria in Shaoxing City should focus on preventing relapse after importation and cure.
2.Visual analysis of research hotspots on antimicrobial therapy for febrile neutropenia based on Web of Science

Objective To conduct a bibliometric and visual analysis of literature related to anti-microbial therapy for febrile neutropenia(FN)in recent years and explore the research trends and hotspots in this field.Methods The Web of Science database was searched to analyze the publica-tion trends,countries/regions,research institutions,journal distributions,author influences,and keyword co-occurrences.VOSviewer,CiteSpace,and Scimago Graphica software were utilized for mapping analysis to present the research hotspots and development trends of antimicrobial therapy for FN.Results A total of 536 articles were included.The average annual number of publications was approximately 43 from 2014 to 2019 and approximately 54 from 2020 to 2024,showing an upward trend.The United States had the highest number of publications(134 articles,accounting for 25.00%).The institution with the most publications was the University of Melbourne(18 articles).The author with the most publications was GUDIOL C(9 articles).Keyword clustering analysis revealed that the research hotspots were mainly concentrated in areas such as"febrile neutropenia","antimicrobial stewardship","immunocompromised hosts","risk assessment","cost-effectiveness analysis","empirical therapy"and"Staphylococcus aureus".Conclusion The number of publications on re-search related to antimicrobial therapy for FN has shown an upward trend in recent years.Antimicro-bial stewardship,treatment of immunocompromised hosts,control of drug resistance,and personalized therapy may become the main research directions in the future.
3.Bioinformatics analysis of exercise rehabilitation in improving the non-alcoholic fatty liver fibrosis

Objective:Hepatic fibrosis is usually the result of chronic liver disease damage,which may develop into cir-rhosis and liver cancer.Exercise can improve the progression of liver fibrosis,but its related mechanisms are still unclear.This study combined transcriptome,gene ontology(GO),Kyoto Encyclopedia of Genes and Ge-nomes(KEGG)analysis,Cytoscape,and other bioinformatics technologies to screen key genes and related pathways in the exercise intervention regulating liver fibrosis.Method:The liver of nonalcoholic fatty liver disease(NAFLD)mice fed a high-fat diet was sequenced by transcriptome,and the differentially expressed genes(DEGs)between the model group and different exercise groups(aerobic exercise group and high-intensity intermittent exercise group)were selected for intersection.The screening conditions were qValue<0.05 and|FoldChange|>2,and the differentially expressed genes(DEGs)were determined.To further determine the hub genes and key enrichment pathways involved in motor regula-tion of fibrosis.Result:Compared with the model group,the aerobic exercise group(Group A)had 144 upregulated genes and 930 downregulated genes.The high-intensity intermittent exercise group(H group)had 122 upregulated genes,559 downregulated genes,and 23 Hub genes.GO and KEGG results showed that the functions and pathways of DEG enrichment included cell adhesion,collagen fiber tissue,protein binding,ECM receptor in-teraction,and the PI3K/Akt signaling pathway,and 23 key genes were screened.Conclusion:Exercise can promote the rehabilitation of nonalcoholic fatty liver-related liver fibrosis.The key pathway and Hub genes are screened through bioinformatics,which is expected to be biomarkers in exercise-re-lated nondrug rehabilitation strategies for liver fibrosis.
4.Clinical Observation of Chu-Needle Therapy Combined with Kang'ai Injection for Cancer-Related Fatigue in Colorectal Cancer of Qi-Blood Deficiency Syndrome Type

Objective To evaluate the clinical efficacy of Chu-Needle Therapy combined with Kang'ai Injection in treating cancer-related fatigue(CRF)in colorectal cancer patients with qi-blood deficiency syndrome.Methods Eighty patients diagnosed with colorectal cancer accompanied by CRF and qi-blood deficiency syndrome were enrolled from the inpatient and outpatient departments of Guangdong Second Traditional Chinese Medicine Hospital between January 2023 and March 2024.Participants were randomly divided into an observation group(n=40)and a control group(n=40)using a random number table.The control group received Kang'ai Injection alone,while the observation group received additional Chu-Needle Therapy.Treatment courses lasted 7 days each,with two consecutive courses administered.After two weeks,clinical efficacy was evaluated by comparing Revised Piper Fatigue Scale scores,traditional Chinese medicine(TCM)syndrome scores,as well as red blood cell count(RBC),hemoglobin(HGB),and albumin(ALB)levels,and immune markers(CD3+,CD4+,CD8+,NK cells)before and after treatment.Safety and adverse reactions were also assessed.Results(1)After treatment,both groups showed significant improvements in all Piper Fatigue Scale(PFS)scores,including behavioral,sensory,affective,and cognitive dimensions,as well as total scores(P<0.05).The observation group demonstrated significantly greater improvements in all PFS subscales and total scores compared to the control group,with statistically significant differences(P<0.05).(2)After treatment,both groups exhibited marked reductions in TCM syndrome scores,including fatigue,palpitations,shortness of breath,pale complexion,dizziness,spontaneous or night sweating,limb numbness,poor appetite,pale nails,and total scores(P<0.05).The observation group showed significantly better improvement in all TCM syndrome scores than the control group,with statistically significant differences(P<0.05).(3)The total effective rate was 82.5%(33/40)in the observation group versus 40.00%(16/40)in the control group,indicating significantly superior therapeutic efficacy in the observation group,with statistically significant differences(P<0.05).(4)After treatment,both groups demonstrated significant improvements in RBC,HGB,ALB levels(P<0.05),with the observation group showing significantly greater improvements in these hematological parameters compared to the control group,with statistically significant differences(P<0.05).(5)After-treatment,both groups showed significant changes in immune markers CD3+,CD4+,CD8+,or NK cells compared to baseline(P<0.05).The observation group exhibited significantly better modulation of CD3+,CD4+,CD8+,and NK cell levels than the control group,with statistically significant differences(P<0.05).(6)The incidence of adverse reactions was 5.00%(2/40)in the observation group versus 22.50%(9/40)in the control group,indicating significantly better safety profile in the observation group,with statistically significant differences(P<0.05).Conclusion Chu-Needle Therapy combined with Kang'ai Injection significantly alleviates fatigue,improves clinical symptoms,enhances nutritional status and immune function,and elevates quality of life in colorectal cancer patients with CRF and qi-blood deficiency syndrome,demonstrating notable clinical efficacy.

7.Preliminarily study of arthrocentesis combined with liquid phase concentrated growth factor injection in the treatment of unilateral temporomandibular joint osteoarthritis

Objective:To observe the clinical effect of arthrocentesis combined with liquid phase con-centrated growth factor(CGF)injection in the treatment of unilateral temporomandibular joint osteoarthri-tis(TMJOA),in order to provide a new treatment option for TMJOA patients.Methods:In this non-randomized controlled study,patients diagnosed with unilateral TMJOA who visited the center for tem-poromandibular joint disorder and orofacial pain of Peking University School and Hospital of Stomatology from June 2021 to January 2023 were selected as research objects.The patients were divided into experi-mental group and control group,which were selected by patients themselves.The experimental group re-ceived arthrocentesis combined with liquid phase CGF injection and the control group received arthrocen-tesis combined with HA injection.Both groups were treated 3 times,once every two weeks.The clinical effect was evaluated by the maximum mouth opening,pain value and the degree of mandibular function limitation 6 months after treatment.The change of condylar bone was evaluated by cone beam CT(CBCT)image fusion technology before and after treatment.Results:A total of 20 patients were included in the experimental group,including 3 males and 17 females,with an average age of(34.40± 8.41)years.A total of 15 patients were included in the control group,including 1 male and 14 females,with an average age of(32.20±12.00)years.There was no statistical difference in general information between the two groups(P>0.05).There were no statistical differences in the mouth opening,pain value and the degree of jaw function limitation between the two groups before treatment(P>0.05),and all of them improved 6 months after treatment compared with before treatment(P<0.05).However,the mouth opening of experimental group was significantly higher than that of control group 6 months after treatment(P<0.05),and the degree of jaw function limitation was significantly lower than that of con-trol group(P<0.05).CBCT 2D images showed that the condylar bone of both groups was smoother after treatment than before treatment,and image fusion results showed that 10 patients(50.0%)in the experimental group and 5 patients(33.3%)in the control group had reparative remodeling area of con-dylar bone,and there was no statistical difference between them(P>0.05).Except for one CGF pa-tient,the other patients in both groups had some absorption areas of condylar bone.Conclusion:The ar-throcentesis combined with liquid phase CGF injection can improve the clinical symptoms and signs of unilateral TMJOA patients in short term,and is better than HA in increasing mouth opening and impro-ving jaw function.CBCT fusion images of both patient groups show some cases of condylar bone repara-tive remodeling and its relevance to treatment plans still requires further study.
8.Analyzing the influencing factors of occupational stress of firefighters in Shenzhen City

Objective To analyze the current status and influencing factors of occupational stress among firefighters in Shenzhen City. Methods A total of 5 434 first-line firefighters in Shenzhen City was selected as the study subjects using the judgment sampling method. The Job Content Questionnaire was used evaluate the occupational stress level. Results The detection rate of occupational stress in the study subjects was 52.5% (2 854/ 5 434). The result of dichotomous logistic regression analysis results showed that the risk of occupational stress was higher in firefighters who had a bachelor's degree or above than in those who had junior high school or below (P<0.01), the risk of occupational stress in those with irregular meals was higher than those with regular meals (P<0.01), and the risk of occupational stress was lower in those who exercised 1-3 times a month or ≥3 times a week than in those who did no daily exercise (all P<0.05). The risk of occupational stress was higher with older the weekly working hours, the more frequent the injury and the more firefighting workload (all P<0.01). The risk of occupational stress in firefighters who stand for a long time was higher than that in workers who not-stand for a long time (P<0.05). Conclusion The detection rate of occupational stress among the firefighters in Shenzhen City is relatively high. Educational level, dietary habits, daily exercise, weekly working hours, injury frequency, long-standing work and firefighting workload are all influencing factors for occupational stress in firefighters.
9.Shear wave viscoelastography for differentiating lung peripheral inflammatory masses and malignant tumors

Objective To observe the value of shear wave viscoelastography(SWV)for differentiating lung peripheral inflammatory masses and malignant tumors.Methods Conventional gray-scale ultrasound and SWV were prospectively performed in 70 patients with lung peripheral inflammatory mass or malignant tumor.The patients were divided into malignant group(n=42)and inflammatory group(n=28)according to pathological results.Clinical and ultrasonic data,including the maximum diameter of lesions,the mean Young's modulus(Emean),mean viscosity(Vmean),and mean dispersion slope(Dmean)were compared between groups.Receiver operating characteristic curves of ultrasonic parameters being significantly different between groups were drawn,and area under the curves(AUCs)were calculated to evaluate the efficacy of each parameter for differentiating lung peripheral inflammatory mass or malignant tumor.Results In malignant group,the maximum diameter and Emean of lesions were both higher,while Vmean and Dmean of lesions were both lower than those in inflammatory group(all P<0.05).Vmean and Dmean of lesions had moderately/good efficacy for differentiating lung peripheral inflammatory mass or malignant tumor(AUC=0.843,0.866),both better than that of conventional ultrasound and Emean(AUC=0.673,0.685)(all P<0.05).The combination of Emean,Vmean and Dmean had good efficacy for differentiating lung peripheral inflammatory masses and malignant tumors,with AUC of 0.874.Conclusion The viscous parameters of SWV could effectively differentiating lung peripheral inflammatory masses and malignant tumors.
10.Efficacy and learning curve of Thulium laser enucleation of the prostate by laser controller anchored at six o'clock position of bladder neck in the treatment of BPH with large gland

Objective:To analyze the efficacy, safety and learning curve of Thulium laser enucleation of the prostate by laser controller(LC-THuLEP) anchored at six o'clock position of the bladder neck in the treatment of benign prostatic hyperplasia(BPH) with large gland.Methods:The clinical data of the 1st to 45th BPH cases with large gland(prostate volume> 80 ml) treated by a doctor with LC-THuLEP anchored at six o'clock position of bladder neck in Shanghai East Hospital from January to October 2022 were retrospectively analyzed. The patients were divided into groups A, B and C according to the order of operation time, with 15 cases in each group. There were no significant differences among the three groups( P>0.05) in age[(71.8±9.4)years old vs. (73.5±8.2) years old vs.(71.4±5.5)years old], prostate volume[88.3(84.8, 100.6)ml vs.91.5(86.1, 118.4)ml vs. 94.5(84.7, 101.8)ml], prostate specific antigen(PSA)[4.8(2.9, 8.5)ng/ml vs. 7.2(3.2, 11.2)ng/ml vs. 7.8(4.5, 12.7)ng/ml], postvoid residual volume[44.0(34.0, 67.0)ml vs. 60.0(40.0, 76.0)ml vs. 39.0(0, 59.0)ml], maximum urine flow rate(Q max)[8.4(7.6, 11.1)ml/s vs. 8.6(6.5, 10.6)ml/s vs. 10.4(7.8, 13.2)ml/s], international prostate symptom score(IPSS)[20(18, 21) vs. 20(20, 22) vs. 20(20, 25)]and quality of life(QOL)[4(4, 5) vs. 4(4, 4) vs. 4(3, 5)].The doctor had more than 100 cases of TURP surgery experience. LC-THuLEP anchored at six o'clock position of bladder neck was described as follows. The bladder neck at six o'clock position is reserved 0.5-1.0 cm as an "anchor" to fix the prostatic bladder neck when the gland was pushed directly by the laser controller, preventing the detached prostate gland from turning. Finally the bladder neck was cut off at six o'clock position, and the prostate was en-bloc removed. The effect of surgery and postoperative complications were compared. The enucleation efficiency was equal to the weight of prostate tissue removed divided by the time of enucleation. Results:The differences among the three groups in operation time [100.0(90.0, 110.0)min vs. 80.0(70.0, 90.0)min vs. 75.0(70.0, 90.0)min], enucleation time[89.0(72.0, 97.0)min vs. 67.0(64.0, 77.0)min vs. 64.0(60.0, 77.0)min] and the efficiency of enucleation [0.65(0.62, 0.68)g/min vs. 0.84(0.83, 0.94)g/min vs. 0.93(0.82, 1.00)g/min] were statistically significant( P<0.05). The operation time and enucleation time in groups B and C were significantly lower than those in group A, and the enucleation efficiency was significantly higher than that in group A( P<0.05), while there was no significant difference between group B and C. However, the difference of three groups in hemoglobin decrease [8.0(5.0, 11.0)g/L vs. 7.0(2.0, 10.0)g/L vs. 11.0(4.0, 16.0)g/L] and catheter indwelling duration[4.0(2.0, 6.0)d vs. 6.0(3.0, 7.0)d vs. 4.0(3.0, 6.0)d] were not statistically different( P>0.05). All patients were followed up for 6 months after surgery. In three groups, postoperative Q max were 23.2(21.0, 25.1)ml/s, 22.7(21.1, 26.1)ml/s and 22.9(21.5, 25.7)ml/s, IPSS were 6(5, 8), 7(6, 8) and 7(7, 8), QOL were 2(1, 2), 2(1, 2) and 2(1, 2), postvoid residual volume were 20.0(10.0, 25.0)ml, 22.0(15.0, 25.0)ml and 5.0(0, 25.0)ml, respectively, which were all significantly different from that of pre-operation( P<0.05).However, there were no statistically significant differences in the postoperative indicators among the three groups ( P>0.05). No statistical difference was found in postoperative complications among the three groups[26.7%(4/15) vs. 20.0%(3/15) vs. 20.0%(3/15), P>0.05]. Conclusions:LC-THuLEP anchored at six o'clock position of bladder neck was an effective operation in the treatment of BPH with large gland, and the learning curve could be reached after 15 cases.
