1.Correlation between residual cholesterol and hearing loss in noise-exposed workers

Objective To analyze the effect of residual cholesterol (RC) on hearing loss in noise-exposed workers. Methods A total of 3 412 workers engaged in noise operation work in an underground railway enterprise were selected as the research subjects using the judgment sampling method. Their occupational health examination data were collected to analyze the relationship between RC and hearing loss. Results The noise intensity of workplace in the underground rail enterprise was 80.0-85.0 (81.4±3.2) dB(A). The detection rate of hearing loss was 20.2% (691/3 412). The rates of abnormal total cholesterol, triacylglycerol, high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol were 35.6%, 25.7%, 9.5% and 42.4%, respectively. The median and the 25th and 75th percentiles [M(P25,P75)] of RC level were 0.24 (0.15, 0.37) mmol/L. The levels of total cholesterol, triacylglycerol and RC of workers in hearing loss group were higher than those in normal hearing group [M(P25,P75): 4.91(4.37, 5.58) vs 4.84(4.30, 5.46) mmol/L, 1.29(0.91, 1.93) vs 1.16(0.82, 1.67) mmol/L, 0.26(0.16, 0.41) vs 0.24(0.14, 0.37) mmol/L, all P<0.05]. The detection rate of hearing loss in abnormal triglyceride group was higher than that in normal triglyceride group (24.8% vs 18.7%, P<0.01), and the detection rate of hearing loss in abnormal HDL-C group was higher than that in normal HDL-C group (25.0% vs 19.8%, P<0.05). The higher the serum RC level, the higher the detection rate of hearing loss (P<0.01). Multivariate logistic regression result showed that individual with older age, longer work time and higher serum RC level had higher risk of hearing abnormality (all P<0.05), and the risk of hearing abnormality was higher in patients with abnormal fasting blood glucose than patients with normal faseing blood glucose (P<0.05) after controlling for confounding factors such as gender, alcohol consumption, body mass index, and elevated blood pressure. However, abnormal triacylglycerol and HDL-C levels were not significantly related to the risk of hearing abnormality (both P>0.05). Conclusion Serum RC levels are an independent risk factor for hearing loss among noise-exposed workers exposed to noise level of 80.0-85.0 dB(A) in the workplace.
2.Clinical study on acupuncture combined with hemoperfusion for the treatment of hemodialysis patients with intractable pruritus

Objective:To explore the clinical efficacy of acupuncture combined with hemoperfusion in hemodialysis patients with intractable pruritus.Methods:A retrospective clinical study was conducted. A total of 126 hemodialysis patients with intractable pruritus were enrolled from June 2023 to June 2024 and divided into two groups (63 cases each) based on treatment modalities. The control group received hemoperfusion alone, while the observation group received acupuncture combined with hemoperfusion. Both groups were treated for 3 months. The Modified Pruritus Duo's Score was used to evaluate pruritus severity, the Pittsburgh Sleep Quality Index (PSQI) for sleep quality, and the 36-Item Short Form Health Survey (SF-36) for quality of life before and after treatment. Serum β 2-microglobulin (β 2-MG) and parathyroid hormone (PTH) levels were measured using immunoturbidimetry and chemiluminescence, respectively. Clinical efficacy was assessed. Results:The total effective rate was 98.42% (62/63) in the observation group and 78.77% (49/63) in the control group, with a statistically significant difference ( χ2=16.90, P<0.001). Post-treatment scores for pruritus severity, affected areas, frequency, sleep interference, and total pruritus score in the observation group were significantly lower than those in the control group ( t values were 16.94, 12.23, 12.67, 12.71, 23.91, respectively, P<0.001). The observation group also exhibited lower post-treatment serum β 2-MG [(13.24±3.11) mg/L vs. (17.61±4.58) mg/L, t=10.46] and PTH [(198.11±96.12) ng/L vs. (249.27±88.45) ng/L, t=6.72] levels were lower than those in the control group ( P<0.001). Additionally, the observation group demonstrated significantly lower scores in subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, daytime dysfunction, and total PSQI score ( t values were 14.97, 6.98, 2.06, 23.63, 2.68, 3.56, 14.89, respectively, P<0.001), as well as higher scores in physical health, role-physical, bodily pain, general health, vitality, social functioning, role-emotional, and mental health domains of SF-36 ( t values were -3.13, -4.35, -6.34, -7.90, -4.49, -5.30, -5.00, -3.95, respectively, P<0.05). Conclusion:Acupuncture combined with hemoperfusion effectively alleviates pruritus, reduces serum β 2-MG and PTH levels, improves sleep quality, and enhances overall quality of life in hemodialysis patients with intractable pruritus, demonstrating superior efficacy compared to hemoperfusion alone.

5.Effect of frozen-thawed blastocyst transfer of different pronuclear zygotes origins on clinical pregnancy outcome and neonatal outcome

Objective:To explore the clinical application value of frozen-thawed single blastocyst derived from nonpronuclear (0PN) zygotes, monopronuclear (1PN) zygotes and two-pronuclei (2PN) discarded embryos.Methods:A retrospective cohort analysis of the data of patients who underwent frozen-thawed single blastocyst transfer at the Reproduction Center of the Third Affiliated Hospital of Zhengzhou University from March 2014 to November 2020. According to the different sources of transplanted blastocysts, they were divided into 4 groups including group A derived from 2PN available embryos (2PN embryos with morphological rating Ⅰ-Ⅲ), group B derived from 0PN embryos, group C derived from 1PN embryos, group D derived from 2PN discarded embryos (2PN embryos with morphological rating Ⅳ). The basic data of the four groups were analyzed,and group A was used as the reference group to compare the clinical pregnancy outcomes of groups B, C and D, respectively. The neonatal situation was compared at the cycle of singleton live birth. Logistic regression was used to correct for confounding factors and calculate the adjusted odds ratio (a OR) and 95% confidence interval ( CI). Results:After correcting for confounding factors by logistic regression, the live birth rate in group B was significantly lower than that in group A (a OR=0.701, 95% CI: 0.534-0.920, P=0.011). The clinical pregnancy rate and the live birth rate in group D were significantly lower than those in group A (a OR=0.595, 95% CI: 0.456-0.777, P<0.001; a OR=0.600, 95% CI: 0.449-0.800, P=0.001). The differences in miscarriage rate, pregnancy complications and multiple pregnancy rate were not statistically significant compared with group A (all P>0.05). The differences in all the above indicators in group C were not statistically significant compared with group A ( P>0.05). The risk of occurrence of macrosomia (group B: a OR=2.367, 95% CI: 1.299-4.315, P=0.005; group D: a OR=2.711, 95% CI: 1.463-5.026, P=0.002), large for gestational age (group B: a OR=1.930, 95% CI: 1.158-3.217, P=0.012; group D: a OR=2.039, 95% CI: 1.174-3.543, P=0.011) were significantly higher in groups B and D than in group A. The differences in the risk of occurrence of low birth weight, small for gestational age and preterm birth were not statistically significant in groups B and D compared with group A ( P>0.05), and the difference in the risk of occurrence of the above indicators was not statistically significant in group C compared with group A ( P>0.05). Conclusion:When 2PN embryos are not available, abandoned 0PN, 1PN, and 2PN embryos can be transferred after blastocyst culture, but attention should be paid to the increased risk of macrosomia and large for gestational age in offspring.

Objective:To investigate the effects of different sperm retrieval methods on clinical pregnancy and perinatal outcomes in patients undergoing intracytoplasmic sperm injection (ICSI) for pregnancy.Methods:A retrospective cohort study was conducted to analyze the clinical data of patients who underwent ICSI for male factors in Reproductive Center of the Third Affiliated Hospital of Zhengzhou University from October 2015 to October 2020. A total of 3096 cycles were included. Patients with oligozoospermia had sperm retrieval by ejaculation, which was recorded as ejaculation group ( n=2558); patients with azoospermia had sperm retrieval by surgical method (PESA/TESA/mirco-TESE), which was recorded as surgical sperm retrieval group ( n=538). The ovulation promotion and embryo culture of the two groups were compared and analyzed, and the clinical pregnancy outcome and perinatal outcome indexes of fresh embryo transfer patients in the two groups were observed. Results:Data from both groups were adjusted for confounding factors by linear/logistic regression, which revealed that the number of high-quality embryos [ MD(95% CI)=0.412(0.153-0.670), P=0.002] and blastocyst formation [ MD(95% CI)=0.265(0.035-0.494), P=0.024] were significantly higher in surgical sperm retrieval group relative to ejaculation group, and the number of two pronuclei (2PN) and the number of available embryos were not statistically different (all P>0.05). The embryo implantation rate [a OR(95% CI)=1.311(1.010-1.701), P=0.042] and the clinical pregnancy rate [a OR(95% CI)=1.307(1.007-1.697), P=0.044] were significantly higher in patients after fresh embryo transfer. The differences in live birth rate, multiple birth rate, miscarriage rate, singleton preterm birth rate, singleton full-term neonatal weight and sex ratio between the two groups were not statistically significant (all P>0.05). Conclusion:Patients with azoospermia can still obtain a live birth rate comparable to that of conventional ICSI through surgical sperm retrieval and do not increase the risk of miscarriage, fetal malformation, singleton preterm birth, or full-term low birth weight babies.

9.MiR-142-3p enhances chemosensitivity of breast cancer cells and inhibits autophagy by targeting HMGB1.

MiR-142-3p has been reported to act as a tumor suppressor in breast cancer. However, the regulatory effect of miR-142-3p on drug resistance of breast cancer cells and its underlying mechanism remain unknown. Here, we found that miR-142-3p was significantly downregulated in the doxorubicin (DOX)-resistant MCF-7 cell line (MCF-7/DOX). MiR-142-3p overexpression increased DOX sensitivity and enhanced DOX-induced apoptosis in breast cancer cells. High-mobility group box 1 (HMGB1) is a direct functional target of miR-142-3p in breast cancer cells and miR-142-3p negatively regulated HMGB1 expression. Moreover, overexpression of HMGB1 dramatically reversed the promotion of apoptosis and inhibition of autophagy mediated by miR-142-3p up-regulation. In conclusion, miR-142-3p overexpression may inhibit autophagy and promote the drug sensitivity of breast cancer cells to DOX by targeting HMGB1. The miR-142-3p/HMGB1 axis might be a novel target to regulate the drug resistance of breast cancer patients.
10. Effects and mechanism of TREM-1 on inflammatory response and lipid metabolism in mice with nonalcoholic fatty liver disease

Objective:
Analysis of the effect of triggering receptor-1 expressed on myeloid cells (TREM-1) in nonalcoholic fatty liver disease (NAFLD) and the mechanism.
Methods:
The oleic acid-treated HepG2 cells were divided into model group, overexpression group, interference group A, interference group B and negative control group. The mouse model of NAFLD was generated and randomly divided into (nuclear factor-κB) NF-κB inhibition group, protein kinase B (AKT) inhibition group, knockout group A, knockout group B and control group. The expression of inflammatory factors and TREM-1 in liver tissue was detected by PCR, and fat accumulation was detected by oil red O staining. Western blotting was used to detect the expression of TREM-1 and signaling pathway proteins, and HE staining was used to detect liver tissue changes.
Results:
TREM-1 was up-regulated in liver tissue of NAFLD mice [(0.936±0.127) vs. (0.432±0.105)] and in oleic acid-treated HepG2 cells. In oleic acid-treated HepG2 cells, overexpression of TREM-1 increased inflammatory factor expression and increased lipid droplets; inhibition of TREM-1 expression decreased inflammatory factor expression, and lipid droplets decreased. Knockout of TREM-1 and inhibition of NF-κB in NAFLD mice reduced hepatocyte inflammatory factor expression and reduced liver damage; knockout of TREM-1 and inhibition of AKT reduced liver tissue lipids and drops accumulate.
Conclusions
The overexpression of TREM-1 in NAFLD mice liver tissue can regulate inflammatory factor expression and lipid droplets through NF-κB and AKT signal pathway. TREM-1 might be a potential therapeutic target of NAFLD.
